Objective:To analyze the efficacy of enzyme replacement therapy and anti-drug antibody production in children with Fabry disease. Methods:The clinical data of 7 children with Fabry disease treated with enzyme replacement therapy for more than 1 year at Children′s Hospital of Zhejiang University School of Medicine from July 2021 to June 2024 were retrospectively analyzed. The basic information and the changes of related clinical indicators before and after treatment were collected. Paired sample t test was used to compare renal function, left heart mass index, pain score and other related indexes before and after treatment. The anti-drug antibodies were detected by enzyme-linked immunosorbent assay. Results:A total of 6 boys and 1 girl were included. The age of diagnosis was (12.2±1.8) years. After 1 year of enzyme replacement therapy, the abnormal substrate globotriaosylsphingosine and brief pain inventory scores of all children were significantly lower than those before treatment ((16±11) vs. (63±42) μg/L, 22±19 vs. 45±29, t=3.88, 3.43, both P<0.05). There were no significant differences in glomerular filtration rate, urinary microalbumin to creatinine and left heart mass index before and after treatment ((124±35) vs. (136±26) ml/(min·1.73 m 2), (9.3±8.3) vs. (3.8±2.5) mg/g, (38±9) vs. (33±6) g/m 2.7, t=1.33, 1.74, 1.19, all P>0.05). Patients 4, 5 and 6 developed anti-drug antibodies at 1 month, 4 months and 1 month after medication, respectively. Patient 4 had persistently high anti-drug antibody titers (absorbance 3.65-3.73) accompanied by urticaria, elevated globotriaosylsphingosine and worsening clinical symptoms. Conclusions:The enzyme replacement therapy can effectively improve the clinical symptoms and reduce the level of globotriaosylsphingosine in children with Fabry disease. The anti-drug antibody is common in patients after long-term enzyme replacement therapy and may diminish the efficacy, which needs dynamic monitoring.

ObjectiveTo explore whether the mechanism of Shuangshen Ningxin capsules (SSNX) in alleviating myocardial ischemia-reperfusion injury (MIRI) in rats is related to the regulation of mitochondrial fission and fusion. MethodThis study focused on Sprague Dawley (SD) rats and ligated the left anterior descending branch of the coronary artery to construct a rat model of MIRI. The rats were divided into the sham operation group, model group, SSNX group (90 mg·kg-1) and trimetazidine group (5.4 mg·kg-1). The activity of superoxide dismutase (SOD) and the content of malondialdehyde (MDA) were detected by micro method. Changes in mitochondrial membrane potential (△Ψm) and the degree of mitochondrial permeability transition pore (mPTP) opening were detected by the chemical fluorescence method. The intracellular adenosine triphosphate (ATP) level was detected by the luciferase assay. The messenger ribonucleic acid (mRNA) and protein expression levels of mitochondrial fission and fusion related factors dynamin-related protein 1 (DRP1), mitochondrial fission 1 protein (FIS1), optic atrophy protein 1 (OPA1), mitochondrial outer membrane fusion protein 1 (MFN1), and MFN2 were detected by real-time polymerase chain reaction (real-time PCR) and Western blot. ResultCompared with the sham operation group, the model group showed a decrease in serum SOD activity and an increase in MDA content. The opening level of mPTP, the level of △Ψm and ATP content decreased, the protein expressions of mitochondrial fission factors DRP1 and FIS1 increased, and the protein expressions and mRNA transcription levels of fusion related factors OPA1 and MFN1 decreased. Compared with the model group,SSNX significantly increased serum SOD activity, reduced MDA content, increased intracellular ATP level and △Ψm, reduced the opening level of mPTP, downregulated the protein expressions of mitochondrial fission factors DRP1 and FIS1, and increased the mRNA transcription levels and protein expressions of fusion related factors OPA1 and MFN1. ConclusionSSNX inhibits the expressions of mitochondrial fission factors DRP1 and FIS1, and increases the expressions of fusion related factors OPA1 and MFN1, inhibiting mitochondrial fission and increasing mitochondrial fusion, thereby alleviating MIRI.

Myocardial ischemia-reperfusion injury (MIRI) is a common injury in the treatment of ischemic heart diseases. MIRI can be categorized as chest impediment and palpitation in traditional Chinese medicine, with the pathogenesis related to Qi and blood disharmony. The simultaneous disorders of Qi and blood are the key mechanism of MIRI, and thus the differentiation of Qi and blood syndromes is the prerequisite for the treatment. The theory of Qi and blood interacting in vessels is proposed by our team based on Qi being the commander of blood and blood being the mother of Qi as well as previous pharmacological studies. Specifically, Qi marshals blood by vessels, and the blood carries Qi by vessels. Accordingly, Qi and blood interact in the vessels. MIRI is accompanied by mitochondrial dysfunction, platelet function abnormality, and vascular endothelial damage, which are correlated with Qi deficiency, blood stasis, and vessel damage, respectively. Mitochondrial, platelet, and vascular endothelial structural and functional changes triggered by their interactions are one of the mechanisms by which Qi deficiency, blood stasis, and vessel damage lead to the occurrence and development of MIRI. By exploring the correlations between Qi and mitochondria, between blood and platelets, and between vessels and blood vessels, we can explain the modern scientific content of the theory of Qi and blood interacting in vessels in traditional Chinese medicine. According to the pathogenesis of Qi and blood disharmony in vessels, we discussed the pharmacological mechanisms of Qi-replenishing medicines, blood-activating medicines, and their combinations in the prevention and treatment of MIRI. On the basis of the research achievements in the prevention and treatment of MIRI by Qi-replenishing and blood-activating Chinese medicines based on the theory of Qi and blood interacting in vessels, we analyzed the effects of these medicines on Qi, blood, and vessels. According to the theory of Qi and blood, this article reveals the theoretical basis and scientific connotations of the prevention and treatment of cardiovascular diseases, with the aim of providing new ideas and references for the clinical application of traditional Chinese medicine in the prevention and treatment of cardiovascular diseases.

Chinese Pediatric Emergency Medicine 2024;31(9):678-684
Objective:To study the occurrence and high risk factors of extrapulmonary complications in premature infants with bronchopulmonary dysplasia(BPD),by calculating cumulative supplementary oxygen exposure(CSO)as a predictive value for reference. Methods:The clinical data, laboratory results, incidence and treatment of complications of premature infants with gestational age<32 weeks and birth weight<1 500g in the First Neonatal Internal Medicine Ward at Shengjing Hospital of China Medical University from January 2020 to December 2022 were collected. According to the occurrence of BPD and the severity of BPD, all children were divided into non-BPD group, mild BPD group and moderate to severe BPD group. According to the complications, children with BPD were divided into white matter injury (WMI)/ retinopathy of prematurity (ROP)/ metabolic osteopathy (MBDP) group and non-WMI/ROP/MBDP group. The occurrence and risk factors of these three complications in children with BPD were summarized. And the prediction index was put forward by single factor analysis and Logistic regression analysis. Results:A total of 405 premature infants were included, including 206 cases in non-BPD group, 128 cases in mild BPD group and 71 cases in moderate to severe BPD group. The gestational age and birth weight of BPD preterm infants were smaller than those of non-BPD preterm infants, and the incidences of asphyxia, multi-dose (two or more) pulmonary surfactant use, patent ductus arteriosus and septicemia in moderate to severe BPD group were higher than those in mild BPD group and non-BPD group. The cumulative oxygen therapy time and invasive mechanical ventilation time in moderate to severe BPD group were also longer than those in mild BPD group and non-BPD group. The differences were all statistically significant( P<0.05). Among 199 cases of BPD, there were 68 cases in WMI group and 131 cases in non-WMI group,89 cases in ROP group and 110 cases in non-ROP group,131 cases in MBDP group and 68 cases in non-MBDP group. In multivariate Logistic regression analysis,1-28CSO( OR=2.042,95% CI 1.102-3.783, P=0.023),cumulative oxygen therapy time( OR=1.044,95% CI 1.009-1.080, P=0.012)within 28 days after birth and male( OR=2.711,95% CI 1.123-6.544, P=0.027)were the risk factors of ROP in children with BPD.1-28CSO( OR=2.124,95% CI 1.193-3.784, P=0.011),birth weight( OR=0.040,95% CI 0.005-0.352, P=0.004)and late-onset sepsis( OR=2.165,95% CI 1.027-4.567, P=0.042)were the risk factors of MBDP in children with BPD.The receiver operating characteristic curve analysis showed that the areas under the curve of 1-28CSO for BPD combined with ROP and MBDP were 0.872(95% CI 0.824-0.921)and 0.779(95% CI 0.711-0.846), respectively, the sensitivities were 70.8% and 77.9%, the specificities were 88.2% and 72.0%,respectively, and the critical values was 2.900 and 2.125,respectively. Conclusion:The increase of cumulative oxygen therapy time and male may increase the risk of BPD with ROP, low birth weight and the occurrence of late-onset sepsis may increase the risk of BPD with MBDP, and 1-28CSO is a common risk factor for both.1-28CSO has a high predictive value in early evaluation of BPD with ROP and MBDP.

Objective:To investigate damaging effects of clomifene citrate(CC)on endometrial stromal cells(hEndoSCs),and to study effects of spermidine on autophagy and inflammatory cytokine expression in damaged endometrial stromal cells. Methods:Groups were firstly divided into control group, spermidine group, clomiphene group(CC group),CC+Spermidine group.MTT assay was used to detect cell survival rate of hEndoSCs after co-incubation with different concentrations of CC or Spermidine for 24 h.Con-tent of intracellular reactive oxygen species(ROS)and level of apoptosis in cells of the 4 groups were detected by flow cytometry tech-nique.Western blot was used to detect expressions of autophagy pathway-related proteins ULK1,p-ULK1,LC-3Ⅱ,and apoptosis-re-lated proteins Bax,Bcl-2,Cleaved-caspase 3.RT-qPCR was used to detect mRNA expressions of IL-6,IL-1β and TNF-α.Results:Compared with control group,CC group showed decreased cell survival, increased apoptosis rate,ROS content,Bax,Cleaved-cas-pase 3 expressions, decreased Bcl-2 expression, decreased levels of autophagy-related proteins p-ULK1 and LC-3Ⅱ/Ⅰ,and elevated expressions of inflammatory factors IL-6,IL-1β and TNF-α mRNA(P<0.01).There was no significant changes viability of cells in spermidine group compared with control group(P>0.05).Compared with CC group, cell survival rate in CC+spermidine group was sig-nificantly increased, apoptosis rate,ROS content,Bax and Cleaved-caspase 3 expressions were decreased,Bcl-2 expression was in-creased,expressions of autophagy-related proteins p-ULK1 and LC-3Ⅱ/Ⅰ were elevated, while expressions of inflammatory factors IL-6,IL-1β and TNF-α mRNA were decreased(P<0.01).Conclusion:CC can inhibit endometrial stromal cell proliferation, promote apoptosis, and increase the transcript levels of inflammatory factors IL-6,IL-1β and TNF-α.Spermidine can reduce intracellular ROS in clomiphene-injured endometrial stromal cells by activating cellular autophagy, increase cell survival, and inhibit the expressions of inflammatory factors IL-6,IL-1β and TNF-α.
