1.Exploration of the Dosage Pattern and Mechanism of Chinese Medicines for the Treatment of Alzheimer's Disease Based on Data Mining,Network Pharmacology and Molecular Simulation

Objective Based on the Ancient and Modern Medical Case Cloud Platform V2.3.7,analyze the medication pattern and mechanism of action of Chinese medicines commonly used in famous medical cases for the treatment of Alzheimer's disease(AD).Methods The core Chinese medicines for AD were screened according to the inclusion and exclusion criteria by searching the famous Chinese medicine cases in the platform,then established the disease-core Chinese medicines-active ingredients-common targets network.The protein-protein interaction network(PPI)was established through the common targets.Gene oncology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analyses were performed by David platform.Molecular docking and dynamics simulations were employed to evaluate the binding affinity and stability between key targets and active ingredients.Results Data mining was performed on all the famous medical cases from the time of database construction to October 2023,and the results were summarized as follows.The high-frequency regulating Qi medications and tonifying Qi medications were Citri Reticulatae Pericarpium and Aurantii Fructus Immaturus;Panax ginseng,Ziziphi Jujubae Fructus,Codonopsis pilosula,Astragali Radix,Atractylodis Macrocephalae Rhizoma,Dioscoreae Rhizoma,and Glycyrrhizae Radix et Rhizoma.The core herbs contained 778 active ingredients and 377 targets;1986 disease targets;79 common targets were obtained after intersection with AD targets,and 7 core targets were identified through PPI network topology analysis,including TNF,AKT1,TP53,PPARG,etc.GO and KEGG pathway enrichment analyses confirmed that the targets of the active ingredients of the core herbs involved 138 pathways,and the core herbs could regulate chemical carcinogenesis-receptor activation、serotonergic synapse、chemical carcinogenesis-reactive oxygen species pathways to treat AD.Molecular docking and molecular dynamics simulation results showed good binding ability and stability between the key targets and the core components.Conclusion Through data mining to analyze the core herbs for the treatment of AD,the present investigation showing the pharmacological mechanism of 12-O-Nicotinoylisolineolone and Odoratin in the treatment of Alzheimer's disease at the molecular level,lay a certain theoretical foundation of 12-O-Nicotinoylisolineolone and Odoratin in the future.

Objective:To explore the clinical characteristics and establish a corresponding prognostic scoring model in patients with early-stage clinical features of hepatitis B-induced acute-on-chronic liver failure (HBV-ACLF).Methods:Clinical characteristics of 725 cases with hepatitis B-related acute-on-chronic hepatic dysfunction (HBV-ACHD) were retrospectively analyzed using Chinese group on the study of severe hepatitis B (COSSH). The independent risk factors associated with 90-day prognosis to establish a prognostic scoring model was analyzed by multivariate Cox regression, and was validated by 500 internal and 390 external HBV-ACHD patients.Results:Among 725 cases with HBV-ACHD, 76.8% were male, 96.8% had cirrhosis base,66.5% had complications of ascites, 4.1% had coagulation failure in respect to organ failure, and 9.2% had 90-day mortality rate. Multivariate Cox regression analysis showed that TBil, WBC and ALP were the best predictors of 90-day mortality rate in HBV-ACHD patients. The established scoring model was COSS-HACHADs = 0.75 × ln(WBC) + 0.57 × ln(TBil)-0.94 × ln(ALP) +10. The area under the receiver operating characteristic curve (AUROC) of subjects was significantly higher than MELD, MELD-Na, CTP and CLIF-C ADs( P < 0.05). An analysis of 500 and 390 cases of internal random selection group and external group had similar verified results. Conclusion:HBV-ACHD patients are a group of people with decompensated cirrhosis combined with small number of organ failure, and the 90-day mortality rate is 9.2%. COSSH-ACHDs have a higher predictive effect on HBV-ACHD patients' 90-day prognosis, and thus provide evidence-based medicine for early clinical diagnosis and treatment.
4.Recent advances in metabolic markers related to invasive pituitary adenomas and magnetic resonance evaluation

Pituitary adenomas are common benign intracranial tumors;however,some pituitary adenomas are invasive.Recent studies on the invasive pituitary adenomas have mainly focused on genomics,proteomics,and imaging.Although there are few studies on metabolites and magnetic resonance (MR) signs of pituitary adenomas,these studies can aid in understanding the metabolism of tumor cells and relevant imaging characteristics.This paper reviews the research progress on MR imaging evaluation and metabolites in invasive pituitary adenomas.
5.Fast -track surgery strategy for microscopic resection of pituitary adenomas using a single -nostril transsphenoidal approach

Objective To explore the fast-track surgery (FTS) strategy in microscopic resection of pituitary adenomas using a single-nostril transsphenoidal approach. Methods The clinical data of 88 patients with pituitary adenomas, admitted to and underwent microscopic surgery using a transsphenoidal approach in our hospital from June 2015 to December 2017, were retrospectively analyzed. Only one nostril was used during surgery through a transverse incision in the nasal septum; the middle turbinates and nasal septum were not resected, and only the anterior wall of the sphenoidal sinus was removed; fenestration of the mucosa at the sellar floor was conducted; no hemostatic material was used to fill the sphenoidal sinus cavity, and the sphenoidal sinus drainage tube was retained. Four months after surgery, the modified Chinese version of 22-item Sino-nasal Outcome Test (SNOT-22) was used to evaluate the postoperative nasal complications and quality of life of the patients. The experiences with FTS strategy through microscopic resection of pituitary adenomas using a single-nostril transsphenoidal approach were summarized. Results The complete tumor resection rate was 65.90% (58/88), near total resection rate was 12.50% (11/88), subtotal resection rate was 19.32% (17/88), and partial resection rate was 2.27% (2/88). No such complications, as nasal septum perforation, delayed epistaxis, or internal carotid artery injury, were observed. Of the 88 patients, 32 were lost to follow-up; the 56 patients who reported for follow-up were evaluated in accordance with modified Chinese version of SNOT-22: 8 (14%) had a total scores>10; the mean highest and lowest scores for the various items were 0.02 (purulent nasal discharge) and 0.88 (hyposmia), respectively; most items had scores of 0-0.3. The postoperative discomfort experienced by the patients mainly included hyposmia, dizziness, head and facial pain or pressure, fatigue, and nasal congestion. Among the 56 patients, there were 26 with hyposmia, including 10 with mild hyposmia, 9 with moderate hyposmia and 7 with severe hyposmia. Conclusion Application of FTS reduces the incidence of nasal complications, and increases the postoperative comfort of patients in protecting the normal tissue structure in the nasal cavity for patients with pituitary adenomas.
6.Correlation research of photosynthetic characteristics and medicinal materials production with 4 Uncariae Cum Uncis.

Using four Uncariae Cum Uncis materials including Uncaria sinensis (HGT), U. hirsutea (MGT), Jianhe U. rhynchophylla (JHGT) and U. rhynchophylla(GT) as the research objects, the correlations between medicinal materials' yield and photosynthetic ecophysiology-factors in the plant exuberant growth period were studied. Results showed that the Uncaria plants net photosynthetic rate (Pn) changed by unimodal curve. There was not "midday depression" phenomenon. There was a different relationship among the photosynthetic ecophysiology-factors and between photosynthetic ecophysiology-factors and medicinal materials' yield. Pn,Tl,Gs had a significant correlation with medicinal materials' yield(M)and were the most important factors of growth.
7.Syringaresinol-4---d-glucoside alters lipid and glucose metabolism in HepG2 cells and C2C12 myotubes.

Syringaresinol-4---d-glucoside (SSG), a furofuran-type lignan, was found to modulate lipid and glucose metabolism through an activity screen of lipid accumulation and glucose consumption, and was therefore considered as a promising candidate for the prevention and treatment of metabolic disorder, especially in lipid and glucose metabolic homeostasis. In this study, the effects of SSG on lipogenesis and glucose consumption in HepG2 cells and C2C12 myotubes were further investigated. Treatment with SSG significantly inhibited lipid accumulation by oil red O staining and reduced the intracellular contents of total lipid, cholesterol and triglyceride in HepG2 cells. No effect was observed on cell viability in the MTT assay at concentrations of 0.1-10 μmol/L. SSG also increased glucose consumption by HepG2 cells and glucose uptake by C2C12 myotubes. Furthermore, real-time quantitative PCR revealed that the beneficial effects were associated with the down-regulation of sterol regulatory element-binding proteins-1c, -2 (), fatty acid synthase (), acetyl CoA carboxylase () and hydroxyl methylglutaryl CoA reductase (), and up-regulation of peroxisome proliferator-activated receptors alpha and gamma (and). SSG also significantly elevated transcription activity oftested by luciferase assay. These results suggest that SSG is an effective regulator of lipogenesis and glucose consumption and might be a candidate for further research in the prevention and treatment of lipid and glucose metabolic diseases.
8.Application of CRISPR/Cas9 in plant biology.

The CRISPR/Cas (clustered regularly interspaced short palindromic repeats/CRISPR-associated proteins) system was first identified in bacteria and archaea and can degrade exogenous substrates. It was developed as a gene editing technology in 2013. Over the subsequent years, it has received extensive attention owing to its easy manipulation, high efficiency, and wide application in gene mutation and transcriptional regulation in mammals and plants. The process of CRISPR/Cas is optimized constantly and its application has also expanded dramatically. Therefore, CRISPR/Cas is considered a revolutionary technology in plant biology. Here, we introduce the mechanism of the type II CRISPR/Cas called CRISPR/Cas9, update its recent advances in various applications in plants, and discuss its future prospects to provide an argument for its use in the study of medicinal plants.
9.Clinical study of Guizhi Fuling capsules combined with fosfomycin trometamol power on pelvic inflammatory disease and pelvic effusion

Objective To analyze the clinical value of Guizhi Fuling capsules combined with fosfomycin trometamol power in the treatment of pelvic inflammatory disease and pelvic effusion.Methods A total of 60 patients with pelvic inflammatory disease and pelvic effusion from February 2013 to March 2016 in our hospital were selected and divided into the control group and the observation group , 30 cases in each group.The control group were treated with fosfomycin trometamol power, and the observation group were additionally treated with Guizhi Fuling capsules.The therapeutic effects and changes of TCM syndrome scores were compared between the two groups.The levels of serum C reactive protein ( CRP ) in the two groups were determined before and after treatment and the ultrasound examination was performed and changes of pelvic effusion diameter was determined before and after treatment.Results The total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05).There was no significant difference in TCM syndrome scores between the two groups before treatment .After treatment, the TCM symptom scores of both groups were decreased, the difference was statistically significant ( P<0.05 ) .The scores of lumbosacral swelling pain, abdominal pain, morbid leucorrhoea increase, fatigue, low-grade fever and soreness of waist in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05).Before treatment, there was no statistically significant difference in pelvic effusion and CRP levels between the two groups.After treatment, the pelvic effusion diameter and CRP levels were decreased, the difference was statistically significant (P<0.05), and those in the observation group were lower than the control group, the difference was statistically significant (P<0.05). Conclusion Guizhi Fuling capsules combined with the fosfomycin trometamol power is significantly effective in the treatment of pelvic inflammatory disease and pelvic effusion.It can significantly reduce the inflammatory reactions of patients, and improve the TCM syndrome scores.
10.Therapeutic effect of clarithromycin combined with Cassia Twig-Poria Cocos capsule in chronic pelvic inflammatory disease and its influence on high sensitivity C - reactive protein

Objective To observe the therapeutic effect of clarithromycin combined with Cassia Twig-Poria Cocos capsule on patients with chronic pelvic inflammatory disease and its influence on high sensitivity C - reactive protein (hs-CRP). Methods 106 cases of patients with chronic pelvic inflammatory disease were selected to be divided into the observation group and control group with each group of 53 patients. The patients in control group were only given clarithromycin, on the base of control group, patients in observation group were given Cassia Twig-Poria Cocos. Both group were treated for three courses of 7-14 days. Then the therapeutic effect, adverse reactions, hs-CRP, erythrocyte sedimentation rate and hematocrit of patients were investigated. Results The total effective rate of patients in observation group was 90.57%, and the control group was 75.47%. The total effective rate of observation group was higher than control group with statistically significant difference (P<0.05). The hs-CRP, erythrocyte sedimentation rate and hematocrit of two groups both decreased after treatment (P<0.05) and these indexes of observation group was much less than control group (P<0.05). Besides, there was no difference of incidence of adverse reactions between two groups during the treatment. Conclusion Treatment of clarithromycin combined with Cassia Twig-Poria Cocos capsule on patients with chronic pelvic inflammatory disease not only can lessen inflammatory reaction, but also can improve the therapeutic effect effectively.
