Journal of Clinical Hepatology 2025;41(9):1788-1794
ObjectiveTo investigate the diagnostic accuracy and grading capability of ultrasound-derived fat fraction (UDFF), controlled attenuation parameter (CAP), and hepatic/renal ratio (HRR) in assessing hepatic steatosis in metabolic associated fatty liver disease (MAFLD) with magnetic resonance imaging-proton density fat fraction (MRI-PDFF) as the gold standard. MethodsA total of 150 patients with MAFLD who attended The First Hospital of Hebei Medical University from January 2023 to December 2024 were enrolled, and 148 healthy volunteers were recruited. All subjects underwent MRI-PDFF, UDFF, CAP, and HRR examinations. Hepatic steatosis was graded based on MRI-PDFF (S0:148 cases; S1:92 cases; S2:21 cases; S3:37 cases), and the MAFLD patients with different grades of hepatic steatosis were compared in terms of UDFF, CAP, HRR, and clinical features. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups and the Tukey HSD test was used for further comparision between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Mann-Whitney U test was used for further comparison between two groups; the chi-square test was used for comparison of categorical data between groups. The Spearman correlation analysis was used to investigate the correlation between UDFF, CAP, HRR, and MRI-PDFF in different grades of MAFLD; the receiver operating characteristic (ROC) curve was used to investigate the efficacy of UDFF, CAP, and HRR in the diagnosis of different degrees of hepatic steatosis in MAFLD; the Bland-Altman difference plot was used to analyze the consistency between UDFF and MRI-PDFF in different degrees of hepatic steatosis in MAFLD. ResultsUDFF measurement gradually increased with the increase in the grade of fatty liver (H=201.52,P0.001). The Spearman correlation analysis showed that there was a strong correlation between any two indicators of UDFF, CAP, HRR, and MRI-PDFF in S1, S2, and S3 MAFLD (all P0.001), with the strongest correlation between UDFF and MRI-PDFF (rs1=0.884,rs2=0.962,rs3=0.929, all P0.001). The ROC curve analysis showed that UDFF had a larger area under the ROC curve (AUC) than CAP and HRR in the graded diagnosis of S1 and S3 (all P0.05), while in the diagnosis of S2 MAFLD, UDFF had a significantly larger AUC than HRR (P0.05) and a similar AUC to CAP (P0.05). The Bland-Altman difference plot showed good consistency between UDFF and MRI-PDFF in different degrees of hepatic steatosis in MAFLD. ConclusionCompared with CAP and HRR, UDFF can accurately measure liver fat content and has good efficacy in identifying varying degrees of hepatic steatosis in MAFLD.

ObjectiveTo investigate the menstrual conditions of women infected with COVID-19 in Shanghai and analyze the influencing factors. MethodsFrom December 2022 to March 2023, menstrual data from 281 women infected with COVID-19 in Shanghai were collected with a questionnaire survey, including usual menstrual characteristics, the most recent menstrual period post-infection, symptoms of infection, and medication usage. According to the crossover period between the menstrual period and the infection period of the respondents, the samples were divided into two groups for comparative analysis: those whose menstrual and infection periods overlapped (positive group) and those whose menstruation started after conversion to virus-negative (negative conversion group). ResultsAmong the 281 respondents, 196 (65.8%) experienced menstrual changes. Among them, 145 (51.6%) had changes in menstrual volume, color and texture, and 109 (38.8%) had changes in menstrual duration or cycle. Decreased menstrual volume (22.1%), darker color (23.49%), thicker texture (21.0%), increased blood clots (16.7%), and prolonged duration (21.8%) were observed in both groups. The rate of changes in menstrual volume, color, and texture was higher in the positive group (56.8%, 69/110) than that in negative group (37.3%, 76/171) (P<0.05). Regarding the menstrual cycle changes, the rate of early onset was higher in the positive group (14.5%) compared to the negative conversion group (3.5%)(P<0.05), while the rate of delayed menstruation was higher in the negative conversion group (25.1%) than that in the positive group (5.5%) (P<0.05). Correlation analysis showed a weak association between sore throat and menstrual changes (r=0.154, P<0.05). ConclusionSome women infected with COVID-19 experience short-term changes in their menstrual conditions, characterized by reduced volume, darker color, thick texture, increased clots, and prolonged menstrual duration, reflecting a pathogenesis of blood stasis. Menstruation during the infection period tends to occur earlier, while delayed menstruation is more prevalent at post-conversion.

ObjectiveTo explore the correlation between skeletal muscle mass and metabolic syndrome (MS) disease risk among middle-aged and elderly community residents in Urumqi, and to provide a theoretical basis for understanding the relationship between skeletal muscle mass and MS among middle-aged and elderly community residents in China. MethodsA total of 1 438 community residents ≥ 50 years old were selected as the research subjects from July 2018 to January 2019 in Urumqi. They were selected from a multi-ethnic natural population cohort in Xinjiang. Data were collected through questionnaires, physical examination, bioelectrical impedance analysis (BIA), laboratory tests, etc. Skeletal muscle mass was evaluated using the limb skeletal muscle mass index (SMI) corrected for body weight; MS was defined as it at least includes three of the following: abdominal obesity, hypertension, hyperglycemia, high triglycerides and low high-density lipoprotein cholesterol. SMI was divided into four quantile arrays of Q1‒Q4. Trend χ2 test was applied to explore whether there was a correlation between SMI changes and MS. A multivariate logistic regression model was used to analyze whether there is a difference in the risk of MS between the higher SMI group (Q2, Q3, Q4) and the reference group Q1. ResultA total of 560 MS patients were detected in this study, with a prevalence rate of 38.94%. Among them, the prevalence rate of MS was 39.16% in males and 38.80% in females. The increase in male SMI grading level is not correlated with the prevalence of MS (trend P>0.05); After adjusting for confounding factors (model 4), the increase in SMI was still not related to the prevalence of MS (Ptrend=0.995). There was no statistical difference in the risk of MS between the lowest quartile group Q1 and the highest quartile group Q4 (OR=1.01, 95%CI: 0.69‒1.78). The prevalence of MS in women gradually decreased with the increase of SMI grading level (Ptrend<0.001); After adjusting for confounding factors (model 4), there was still a correlation between the increase of SMI and the prevalence of MS (Ptrend=0.005). With the lowest quartile of SMI Q1 as the reference group, the risk of MS in Q2 (OR=0.63, 95%CI: 0.40‒1.00), Q3 (OR=0.56, 95%CI: 0.34‒0.94), Q4 (OR=0.42, 95%CI: 0.23‒0.76) decreased. ConclusionAn increase in skeletal muscle mass may be beneficial for preventing MS, especially among middle-aged and elderly female residents. Considering the intensification of aging in China and the close relationship between MS and related comorbidities, managing skeletal muscle mass may contribute to potential MS prevention.

【Objective】 To study the correlation between platelet transfusion efficacy and KIR receptor-HLA ligand. 【Methods】 Thirty-three leukemia patients with positive HLA antibody were tested for cross-matching with donor platelets. Platelets from suitable donors were selected for transfusion, and the 24-hour platelet corrected count increment (CCI) was used to determine the transfusion effect. KIR and ligand genotyping were performed on blood samples from patients and donors by PCR-SSP method, and the relationship between platelet transfusion effects and KIR receptor-HLA ligand was analyzed. 【Results】 In 74 occasions of platelet transfusion, 42 were ineffective and 32 were effective. When the donor had C2 gene and HLA-B Bw4-80T gene, the frequency of ineffective platelet transfusion in the recipient was 69.0% (29/42) and 52.4% (22/35), respectively, which was significantly higher than that in the effective group [25.0% (8/32) and 25.0% (8/32)]. When the donor had C1 gene, and the frequency of effective platelet transfusion in the recipient was 100.0%(32/32), which was higher than that in the ineffective group [83.3%(35/42)]. When the recipient-donor matching mode was KIR2DL1-C2 and KIR3DL1-(HLA-B Bw4-80T), the frequency of ineffective platelet transfusion was 69.0%(29/42) and 40.5%(22/42),higher than that of the effective group [25% (8/32) and 18.8% (6/32)]. When the recipient-donor matching model was KIR2DL3-C1, the rate of effective platelet transfusion in 32 patients (100.0%), which was higher than that (35 patients 83. 3%) in the ineffective group. When the mismatch mode of recipient iKIR+donor HLA ligand receptor was KIR2DL1-C2, the frequency of effective platelet transfusion in the recipient was 78.1% (25/32), which was much higher than that in the ineffective group [31.0% (13/42)]. When the mismatch mode was KIR3DL1-(HLA-B Bw4-80T), the rate of effective platelet transfusion in the recipient was 68.8% (22/32), which was higher than that in the ineffective group (42.9%, 18/42). The difference between the above groups was statistically significant(P<0.05). 【Conclusion】 HLA-C1 and HLA-C2 genes are the key factors affecting the efficacy of platelet transfusion. For platelet refractorines, HLA-C1 is the protective gene, while HLA-C2 and HLA-B Bw4-80T are the susceptible genes. The recipient iKIR+donor HLA ligand receptor model may play an important role in platelet refractoriness.

Objective:To compare the efficacy and safety between WangShiBaoChiWan and mosapride in the treatment of functional dyspepsia (FD).Methods:From September 2019 to September 2020, patients with postprandial fullness and early satiation who met the Rome Ⅳ criteria for FD diagnosis were enrolled from 15 hospitals, including the First Affiliated Hospital of Naval Medical University (Shanghai Changhai Hospital), Beijing Traditional Chinese Medicine Hospital Affiliated to Capital Medical College. The subjects were randomly divided into WangShiBaoChiWan (experimental) group and mosapride (control) group in the ratio of 1∶1. The treatment regimens were WangShiBaoChiWan+ mosapride simulator, WangShiBaoChiWan simulator+ mosapride, respectively with a treatment period of 2 weeks. The primary efficacy outcome was the improvement rates of main symptoms before and after treatment, the secondary efficacy primary efficacy outcome was the total clinical effective rate and the change of the single symptom score. And the safety indicator included adverse events. Independent sample t-test, paired t-test and chi-square test were used for statistical analysis. Results:A total of 251 FD patients were enrolled in the full analysis set, including 124 in the experimental group and 127 in the control group; 241 FD patients were in the per-protocol analysis set, including 117 in the experimental group and 124 in the control group. The analysis of per-protocol analysis set showed that the improvement rates of the main symptoms of the experimental group and the control group were (66±29)% and (60±30)%, respectively, and the difference was not statistically significant ( P>0.05). The improvement rate of the main symptoms of the experimental group reached 117% of that of the control group, which exceeded the expected non-inferiority standard of 80%. The total clinical effective rates of the experimental group and the control group were 76.07% (89/117) and 75.81% (94/124), respectively, and the difference was not statistically significant ( P>0.05). The results of full analysis set showed that the incidence of adverse events of the experimental group and the control group was 1.62% (2/124) and 1.57% (2/127), respectively, and the difference was not statistically significant ( P>0.05). There were no serious adverse events in the two groups. Conclusion:The improvement rate of the main symptoms of WangShiBaoChiWan is not inferior to that of mosapride in the treatment of FD, and it has good safety.

Journal of Environmental and Occupational Medicine 2022;39(11):1262-1268
Background There is a lack of evidence on whether exposure to PM2.5 and its constituents would affect the relationship between the dietary approaches to stop hypertension (DASH) and central obesity. Objective To investigate the effect of exposure to PM2.5 and its constituents on the correlation between the DASH dietary pattern and the prevalence of central obesity. Methods The data were obtained from the baseline survey of the "Xinjiang Multi-Ethnic Natural Population Cohort Construction and Health Follow-Up Study" in Urumqi. A DASH score was calculated according to intake frequency of 8 food groups, and summed from intake frequency of recommended food groups scored from 1 to 5 from low to high, and intake frequency of restricted food groups scored from 1 to 5 from high to low. A higher DASH score indicates better compliance with the DASH dietary pattern. We estimated exposure using satellite-derived PM2.5 and a chemical transport model (GEOS-Chem) for its constituents, including organic carbon (OC), black carbon (BC), sulfate (SO42−), nitrate (NO3−), ammonium (NH4+), and soil dust. Central obesity was defined by waist circumference: ≥90 cm for men or ≥85 cm for women according to Criteria of weight for adults (WS/T 428—2013). A logistic regression model was used to analyze the effects of the DASH dietary pattern as well as PM2.5 and its constituents on central obesity, and a stratified analysis was used to explore the effects of PM2.5 and its constituents on the association between the DASH dietary pattern and central obesity. Results The study included 9 565 urban residents, aged (62.30±9.42) years, with a central obesity prevalence rate of 60.75%. After adjusting for selected confounders, the DASH score Q5 group had a 17.5% lower risk of central obesity than the Q1 group (OR=0.825, 95%CI: 0.720-0.947). PM2.5 and its constituents OC, BC, SO42−, NH4+, and soil dust were positively associated with the prevalence of central obesity, but no association was observed between constituent NO3− exposure and central obesity. The stratified analysis revealed that the prevalence of central obesity was reduced in the DASH score Q5 group in participants exposed to low concentrations of PM2.5 and its constituents NO3−, NH4+, and soil dust, while the protective effect of the DASH pattern on central obesity disappeared in subjects exposed to high concentrations of PM2.5 and its constituents NO3−, NH4+, and soil dust. Conclusion Exposure to PM2.5 and its constituents NO3−, NH4+, and soil dust could attenuate the protective effect of the DASH pattern on central obesity.

Objective:To understand the infection status and elimination of anal human papillomavirus (HPV) among men who have sex with men (MSM) in Urumqi, Xinjiang. Methods:The sample size formula in cohort study, a dynamic cohort study method, was used, with the sample size estimated to be 712 according to the new infection rate of HPV16. With the help from non-government organzitions in Urumqi, we recruited 810 MSMs by the snowballing method, and a follow-up program was carried every six months. Anal exfoliated cells were collected to analyze HPV type 16 and 18 infection rates. For statistical analyses, Poisson regression was used to estimate the density of new infections and persistent infections. Cox proportional hazard model was used to explore the influencing factors on both new and persistent infections and the diseases' natural clearing. Results:A total of 810 MSM were recruited, and 482 MSMs with the number of follow-ups more than 2 times were included in the analysis, with the total follow-up numbers as 994.7 person-years. The median number of follow-up and follow-up times was 4 ( P 25, P 75:3,5) times , and 2.2 ( P 25, P 75:1.8, 2.6) years. The baseline infection rates of HPV16 and HPV18 were 8.5% (41/482) and 3.3% (16/482), respectively. And the baseline mixed infection rate of the two types was 0.6% (3/482). The first new infection densities of HPV16 and HPV18 were 10.06 (95% CI:8.12-12.45)/100 person-years and 5.24 (95% CI:3.95-6.96)/100 person-years. The rate of natural infection clearance of HPV16 and HPV18 were 71.2% (89/125) and 71.8% (46/64), respectively. The natural clearance rate of HPV18 after 1.5 years follow-up was higher than HPV16 (97.7% vs. 94.1%). The persistent infection rate of HPV16 and HPV18 were 4.5% (20/441) and 1.7% (8/466), respectively. The risk of persistent HPV16 infection among unmarried subjects was lower than that of married subjects (a HR=0.29,95% CI:0.12-0.71). The natural clearance rate of HPV18 without condom use in anal intercourse in the last six months was 2.63 times higher than that of condom use (95% CI:1.08-6.42). Conclusions:HPV16 and 18 new infections are more common among MSM in Urumqi, and the natural clearance rate is higher. Compared with HPV18, HPV16 has a higher density of new and persistent infections, a low natural clearance rate, and a greater risk of the diseases.
