2.Epidemiological characteristics of marburg virus disease: a systematic review

Objective:To analyze the epidemiological characteristics and natural focus distribution of marburg virus disease (MVD), and provide evidence for the prevention and control of MVD.Methods:A systematic literature retrieval was conducted, and descriptive epidemiological method was used to analyze the temporal, spatial, and population distributions of MVD as well as its natural hosts. The case fatality rate of MVD was evaluated through Meta-analysis, and the phylogenetic analysis of viral genomes was done by using software IQ-TREE 2.3.6.Results:Marburg virus has endemic spread in Africa and can be detected in various animals such as primates and bats. The positive rate is higher in Egyptian fruit bats, and the disease epidemic areas continue to expand. The human infections occur frequently. As of December 20, 2024, a total of 18 MVD outbreaks had been reported worldwide, resulting in 722 reported cases and 548 deaths. Meta- analysis showed a case fatality rate of about 65.19% (95% CI: 48.07%-80.50%). Populations are generally susceptible, with a higher proportion of the cases in miners and medical workers. Marburg virus specific antibody can be detected in populations and animals in some countries and regions where no cases have been reported in Africa, indicating the potential of virus transmission. Conclusion:The transmission capacity of Marburg virus shows no significant changes, but the areas affected by the virus transmission expands, indicating increased risk for MVD outbreak.

Human amnion (hAM), as a biomaterial, has made significant progress in the field of ophthalmology, particularly in the treatment of retinal diseases. hAM possesses biological properties such as promoting tissue repair, inhibiting inflammation and neovascularization, and reducing fibrosis, which have led to its promising clinical outcomes in treating macular holes, retinal detachment, proliferative vitreoretinopathy, optic disc depression-related macular detachment, and age-related macular degeneration. The application of hAM can improve surgical success rates and promote vision recovery, with no significant rejection reactions observed due to its low immunogenicity. Nevertheless, the use of hAM still faces challenges in optimizing preparation and storage techniques, enhancing therapeutic efficacy, and reducing the risk of infectious disease transmission. Future research should focus on addressing these issues to further promote the application of hAM in retinal disease treatment and enhance its effectiveness.
4.Armadillo repeat containing X-linked 3 regulates the proliferation,migration,and invasion of breast cancer cells

Objective:To investigate the effect of armadillo repeat containing X-linked 3(ARMCX3)on the proliferation,migration,and invasion of breast cancer cells and its potential molecular mechanism.Methods:Immunohistochemistry was used to measure the expression of ARMCX3 in breast cancer tissue and adjacent tissue,and Western blot was used to measure the expression of ARMCX3 in breast cancer cells.Breast cancer cells were transfected with lentivirus to establish a model of breast cancer cells with low expres-sion of ARMCX3,and flow cytometry,colony formation assay,scratch assay,and Transwell assay were used to observe the effect of ARMCX3 on the proliferation,migration,and invasion of breast cancer cells.TCGA and GTEx databases were used to analyze the ex-pression of SRY-box transcription factor 9(SOX9)in breast cancer tissue,and GEO database was used to analyze the correlation be-tween SOX9 and ARMCX3.Western blot was used to measure the expression levels of E-cadherin,N-cadherin,vimentin,and SOX9 in breast cancer cells.Results:Compared with adjacent tissue and human normal breast epithelial cells,there was a significant in-crease in the expression level of ARMCX3 in breast cancer tissue and most breast cancer cell lines(P<0.05).Compared with the con-trol group,the low expression of ARMCX3 inhibited the prolifera-tion,migration,and invasion of MCF-7 and MDA-MB-231 cells,promoted the expression of E-cadherin,and inhibited the expres-sion of N-cadherin and vimentin(P<0.05).Compared with the nor-mal tissue,there was a significant increase in the expression level of SOX9 in breast cancer tissue(P<0.001),which was correlated with ARMCX3,and the low expression of ARMCX3 also significantly in-hibited the expression of SOX9.Conclusion:ARMCX3 is highly ex-pressed in breast cancer tissue and breast cancer cells,and it may regulate the proliferation,migration,and invasion of breast cancer cells through SOX9.
5.Insulin-like growth factor 1 attenuates sepsis-induced acute lung injury in mice by down-regulating the PI3K/AKT pathway

Objective:To investigate the effect of insulin-like growth factor 1 (IGF-1) on acute lung injury in septic mice and its underlying molecular mechanism.Methods:Twenty SPF male C57BL/6J mice aged 6-8 weeks were randomly (random number) divided into the sham-operated group, sham-operated + IGF-1 group, sepsis group and sepsis + IGF-1 group, with 5 mice in each group. IGF-1 [60 μg/(kg·d)] was injected via the tail vein for 3 consecutive days in the sham-operated + IGF-1 group and sepsis + IGF-1 group, and mice in the sham-operated group and sepsis group were injected with an equal volume of saline. The tissue of the upper lobe of the right lung was taken to calculate the wet-to-dry ratio, and the upper lobe of the left lung was subjected to HE staining to analyze pathological changes and evaluate lung injury. The levels of interleukin (IL)-6 and IL-1β in the bronchoalveolar lavage fluid (BALF) and serum of mice were detected by ELISA. The expression of p-PI3K, PI3K, p-AKT and AKT in lung tissues was determined via Western blotting. The quantitative data with a normal distribution and homogeneity of variance were compared between the two groups by two independent sample t test. Results:Lung volume was reduced in the sepsis group than in the sham-operated group, obvious surface congestion, dark red color, large bruises and hemorrhagic foci were observed under the pericardium, and the wet-to-dry ratio was significantly elevated ( P<0.05). Compared with the sepsis group, the sepsis + IGF-1 group had slightly increased lung volume, less congestion, darker red color, fewer bruises and hemorrhagic foci, and a lower wet-to-dry ratio ( P<0.05). There was no significant change in lung tissue morphology in the sham-operated + IGF-1 group compared with the sham-operated group. HE staining and lung histopathological scores showed that lung tissue was significantly damaged in the sepsis group than the sham-operated group ( P<0.001), and the pathological score of lung tissue was less damaged in the sepsis + IGF-1 group compared with the sepsis group ( P<0.01). The ELISA results demonstrated that the serum levels of IL-6 and IL-1β were markedly decreased in the sepsis + IGF-1 group than in the sepsis group [(26.22±1.60) pg/mL vs. (45.61±7.85) pg/mL, P<0.05; (87.99±11.80) pg/mL vs. (181.26±10.11) pg/mL, P<0.001]. Moreover, the IL-6 and IL-1β contents in the BALF of the sepsis + IGF-1 group were notably lower than those in the BALF of the sepsis group [(7.67±0.42) pg/mL vs. (20.25±0.43) pg/mL, P<0.001; (17.00±6.08) pg/mL vs. (108.61±5.18), pg/mL P <0.001]. Western blot analysis revealed that the expression of p-PI3K, PI3K, p-AKT and AKT in the lung tissues of mice in the sepsis+IGF-1 group were markedly lower than that in the sepsis group [(0.71±0.05) vs. (1.21±0.09), P<0.05; (0.57±0.08) vs. (1.24±0.22), P<0.01; (0.29±0.07) vs. (1.10±0.04), P<0.001; (0.65±0.17) vs. (1.19±0.07), P<0.01]. Conclusion:IGF-1 ameliorates sepsis-induced acute lung injury in mice, and its protective effect may be achieved by inhibiting the PI3K/AKT pathway.

7.Exploring the Development and Metastasis of Breast Cancer from the Perspective of Qi-Blood-Collateral Transmission

[Objective]To explore the mechanism of breast cancer occurrence,development and metastasis from the perspective of Qi-blood-colleteral transmission.[Methods]Starting from the classical Chinese medical texts and the etiology and pathogenesis of breast cancer,it analyses and elaborates the correlation between the metastasis of breast cancer and the transmission of Qi,blood and collaterals,and systematically analyses the treatment of breast cancer from the perspective of Qi,blood and collaterals,citing a clinical case to support it.[Results]Integrating antiquarian analysis,traditional meridian content and modern scholarly evidence-based surveys,it is found that breast cancer is located in the liver,Qi depression is the core factor of breast cancer,which produces toxin,caused by Qi and blood,mixed with phlegm and blood stasis,and finally reaches the deepest place of the body-collaterals.Therefore,for the treatment of breast cancer,depending on the stage of the disease,the diagnosis and treatment can be made based on the diagnosis and treatment.At the initial stage,the Qi should be adjusted to resolve the toxicity of depression,the blood should be dispersed to remove the toxicity of turbidity,and then gradually penetrate into the channels to remove the toxicity of stasis,and the medication should also be based on the three steps of Qi,blood,channels,and the treatment should be from shallow to deep.The syndrome differentiation of the attached case belonged to Qi deficiency and blood stasis,toxin and blood stasis blocking collaterals,with the method of supplementing Qi and activating blood circulation,detoxifying and removing blood stasis,searching collaterals and removing pathogens,finally the case achieved good effect and improved the lifequality.[Conclusion]The pathogenesis of breast cancer is preceded by Qi stagnation,the poison starts from Qi,enters the blood from Qi,the blood and the collaterals,and the collaterals'deficiency and toxicity are the important etiological mechanism for the emergence,development and metastasis of breast cancer.The treatment of breast cancer puts emphasis on clearing and resolving the toxicity of Qi,dispersing the turbid toxicity of blood,and searching and eliminating the toxicity of the stasis in the channel,which can provide new ideas for the treatment of breast cancer with traditional Chinese medicine.

10.Early effectiveness of unilateral biportal endoscopy technique for migrated lumbar intervertebral disc herniation.

OBJECTIVE:
To explore early effectiveness of unilateral biportal endoscopy (UBE) technique in the treatment of migrated lumbar intervertebral disc herniation.
METHODS:
A retrospective analysis was conducted on 87 patients with migrated lumbar intervertebral disc herniation, who were treated with UBE technique between May 2021 and December 2022 and met the selection criteria. There were 55 males and 32 females, with an average age of 48.8 years (range, 29-74 years). The disease duration ranged from 2 to 23 months, with an average of 9.1 months. The surgical segments included 17 cases of L 3, 4, 32 cases of L 4, 5, and 38 cases of L 5, S 1. According to Lee's classification criteria, there were 12 cases of type 1, 17 cases of type 2, 37 cases of type 3, and 21 cases of type 4. The operation time, length of hospital stay, and complications were recorded. The visual analogue scale (VAS) score was used to assess the degree of low back and leg pain before operaion and at 3 days, 3 months, 6 months, and 12 months after operation. The Oswestry disability index (ODI) was used to evaluate the lumbar spine function. At last follow-up, the modified MacNab criteria was used to evaluate the effectiveness. According to the preoperative migrated intervertebral disc classification, the patients were allocated into groups Ⅰ to Ⅳ. The differences in VAS score and ODI were compared.
RESULTS:
All 87 patients successfully completed the operations. There was no nerve root injury, dural sac injury, or dural tear during operation. The operation time was (58.6±14.6) minutes and the length of hospital stay was (4.0±0.8) days. All incisions healed by first intention after operation. No symptomatic epidural hematoma occurred. All patients were followed up for 12 months. There were significant differences in VAS scores and ODI at each time point after operation when compared with those before operation ( P<0.05). There were significant differences in VAS score at 3 days after operation when compared with that at 3, 6, and 12 months after operation ( P<0.05). For ODI, except that there was no significant difference between 6 and 12 months after operation ( P>0.05), there were significant differences between other time points after operation ( P<0.05). At last follow-up, the effectiveness was rated as excellent in 66 cases, good in 13 cases, and fair in 8 cases according to the modified MacNab criteria, and the excellent and good rate was 90.8%. There was no intervertebral disc herniation recurred during follow-up period. There was no significant difference in VAS score and ODI among groups Ⅰ -Ⅳ before operation and at each time point after operation ( P>0.05).
CONCLUSION
The UBE technique is safe and effective in the treatment of migrated lumbar intervertebral disc herniation, with a low complication rate and satisfactory early effectiveness.
