Objective To construct a lung cancer surgery-oriented disease-specific database covering the entire perioperative care pathway, thereby improving the quality and usability of key surgical data elements. Methods Real-world clinical data were extracted from a single-center thoracic surgery department. A standardized data model was established based on the open electronic health record (openEHR) standard. Large language model (LLM), optical character recognition (OCR), and artificial intelligence (AI)-driven techniques were employed to extract, structure, and perform quality control on unstructured clinical narratives, imaging reports, and radiological data, with a focus on capturing surgically relevant perioperative indicator. Results A multimodal database comprising 19 917 patients was established, including 7 930 males and 11 987 females, with ages ranging from 15 to 97 (61.7±9.7) years. The database includes 582 structured data variables, textual report data corresponding to 69 clinical indicators, 13 000 pulmonary function test PDF reports, and chest CT imaging data from 16 884 patients. This database comprehensively covers major information relevant to surgical diagnosis and treatment of lung cancer, significantly improving the completeness and granularity of surgical detail data. Large language models (LLMs) and optical character recognition (OCR) technologies enhanced the efficiency of converting unstructured data into structured formats, while a multi-level manual verification process ensured data accuracy and traceability. The database supports real-world research including comparisons of surgical procedures, prediction of postoperative complications, prognosis assessment, and multimodal data association analyses.

Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.

Background and aims
The mechanism of cholestatic liver injury (CLI) is unclear, and effective therapies are lacking. While peroxisome proliferator-activated receptor alpha (PPARα) agonists show potential hepatoprotective effect and pyroptosis is implicated in hepatocellular damage, how PPARα activation mitigates lithocholic acid (LCA)-induced pyroptosis remains unknown.
Methods
The hepatoprotective effect of PPARα agonists was evaluated in a mouse model of intrahepatic cholestasis induced by LCA. Liver injury was assessed via serum biochemistry, hematoxylin and eosin and TUNEL staining, and electron microscopy. Pyroptosis pathways were analyzed using real-time quantitative polymerase chain reaction, Western blot, and co-immunoprecipitation.
Results
Combined morphological, histopathological, and biochemical analyses confirmed that PPARα activation protects against CLI. Compared with LCA treatment alone, PPARα activation significantly attenuated the elevation of serum lactate dehydrogenase (LDH), the increased TUNEL-positive cells, and the formation of hepatocyte membrane pores. Mechanistically, PPARα activation suppressed both NOD-like receptor protein 3 (NLRP3) inflammasome-mediated pyroptosis and apoptosis protease-activating factor-1 (APAF-1)/CASPASE-3/GSDME-mediated pyroptosis. Furthermore, PPARα agonist pretreatment inhibited activation of the nuclear factor-kappa B (NF-κB) and forkhead box O1 (FOXO1) signaling pathways.
Conclusions
PPARα protects against LCA-induced CLI by inhibiting both NLRP3 inflammasome-mediated pyroptosis associated with NF-κB and APAF-1/CASPASE-3/GSDME-mediated pyroptosis associated with the FOXO1 signaling pathway.

Objective:To explore the intervention effect of virtual reality technology assisted early cardiac rehabilitation on exercise fear and self-efficacy after coronary artery bypass grafting (CABG), and provide theoretical basis for constructing intelligent and personalized cardiac rehabilitation models. Methods:A prospective non-contemporaneous controlled study was conducted. One hundred and two CABG patients admitted to the First Affiliated Hospital of the Chinese People's Liberation Army Naval Medical University were selected as the research subjects. They were divided into a control group (51 cases, from January to June 2024) and an experimental group (51 cases, from August to December 2024) according to their admission time, with July 2024 as the washout period. The control group received routine postoperative early cardiac rehabilitation intervention, while the experimental group received virtual reality technology intervention on the basis of the control group. The Tampa Scale of Kinesiophobia Heart (TSK-SV-Heart) and General Self-Efficacy Scale (GSES) were used to evaluate exercise fear and self-efficacy on the third day after surgery, one day before discharge, and one month after discharge, and the 6-minute walking distance was recorded at one day before discharge, and one month after discharge. Results:The control group consisted of 49 complete participants, aged (58.50 ± 5.88) years, with 26 males and 23 females, while the experimental group consisted of 48 complete participants, aged (60.31±6.58) years, with 30 males and 18 females. There was no statistically significant difference in the scores of exercise fear and self-efficacy between the two groups on the third day after surgery (both P>0.05). The exercise fear scores of the experimental group at one day before discharge and one month after discharge were (34.16 ± 5.76), (26.86 ± 4.52) points, respectively, which were lower than those of the control group (37.95 ± 6.14), (33.71 ± 5.49) points, and the differences were statistically significant ( t=3.13, 6.70, both P<0.05). The self-efficacy scores of the experimental group at one day before discharge and one month after discharge were (30.78 ± 3.49), (32.42 ± 3.37) points, respectively, which were higher than those of the control group (26.65 ± 4.21) , (26.54 ± 3.68) points, and the differences were statistically significant ( t=4.73, 8.25, both P<0.05). The 6-minute walking distance of the experimental group at one day before discharge and one month after discharge were (422.65 ± 74.32), (485.22 ± 76.84) m, respectively, which were longer than those of the control group (385.43 ± 68.17), (423.76 ± 62.75) m, and the differences were statistically significant ( t=2.57, 4.32, both P<0.05). Conclusions:Virtual reality technology can improve the fear of movement in patients after CABG surgery, enhance their self-efficacy, and simultaneously improve the 6-minute walking distance, providing a theoretical reference for the development of intelligent nursing measures after CABG surgery.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Objective:To investigate the efficacy and safety of high-frequency irreversible electroporation (H-FIRE) in treating benign prostatic hyperplasia (BPH).Methods:This randomized controlled open-label multicenter clinical trial enrolled patients from nine medical centers in China between August 2020 and July 2022. Inclusion criteria: age 50–80 years, International Prostate Symptom Score (IPSS) ≥12, maximum urinary flow rate (Q max) >5 ml/min and ≤15 ml/min. Exclusion criteria: prostate malignancy, contraindications to surgery or anesthesia. Patients were randomized 1∶1 into the H-FIRE group (experimental) or the control group (daily oral 0.2 mg tamsulosin hydrochloride sustained-release capsules). Primary outcomes included Q max, IPSS, prostate volume, and International Index of Erectile Function-5 (IIEF-5) scores, measured at baseline, 1 and 3 months post-treatment. Results:A total of 160 cases were included in this study, including 80 cases in the experimental group and 80 cases in the control group, 30 cases in Renji Hospital, 7 cases in Qilu Hospital of Shandong University, 8 cases in Tongji Hospital, 3 cases in Hunan Provincial Hospital, 13 cases in Shanghai Pudong Hospital, 29 cases in Hwa Mei Hospital, 18 cases in Yiyuan County People's Hospital, and 38 cases in Shanghai East Hospital, and 14 cases in Sun Yat-sen Memorial Hospital. At 3 months of post-treatment, Q max in the experimental group increased by a median of 7.50 (3.55, 14.50) ml/s from the baseline value, whereas in the control group it increased by a median of 1.70 (-1.40, 6.00) ml/s, and the difference between the two groups was statistically significant ( P < 0.01, U = 1 083); and at 3 months of post-treatment, IPSS in the experimental group decreased by a median of 12.00 (7.00, 17.00) points in the test group and 6.00 (2.00, 11.00) points in the control group, and the magnitude of improvement in IPSS scores in the test group was significantly higher than that in the control group ( P < 0.01, U = 1 248); at 3 months of post-treatment, the prostate volume decreased by a median of 12.16 (5.69, 18.27) ml in the experimental group and 0 (-3.94, 6.89) ml in the control group, suggesting that H-FIRE significantly reduced prostate gland volume ( P<0.01, U=1 111). The difference in elevated IIEF-5 scores from baseline at 3 months of treatment between the experimental and control groups was not statistically significant[0(-2.00, 1.00) points vs. 0(-2.00, 1.50) points; P=0.54, U=2 338]. There were no serious adverse events in the two groups. Conclusions:H-FIRE could significantly improve both subjective and objective symptoms of BPH with a low risk of severe complications.

Chinese Journal of Ultrasonography 2025;34(3):239-246
Objective:To analyze transcranial sonography(TCS)imaging characteristics of rats with 6-hydroxydopamine hydrochloride(6-OHDA)and explore the correlations between the imaging characteristics with motor deficits and pathological changes.Methods:Twenty-nine male SD rats were divided into 3 groups:8 in the no-treatment control(NC)group,10 in the Sham group and 11 in the 6-OHDA group. The model for Sham/Parkinson's disease(PD)was established by stereotacticly injecting saline/6-OHDA containing ascorbic acid to bilateral substantia nigra pars compacta(SNpc). After three weeks, the models were stable. At the fourth week and seventh week, the behavioral testing was accomplished. The TCS examination was performed weekly at the same time for four weeks. At the eighth week, the rats were sacrificed for pathology.Results:①Behavioral testing:6-OHDA group showed asymmetric motor deficits and the difference was significant compared with the NC group and Sham group(both P<0.001). ②TCS examination:compared with the NC and Sham group, there were asymmetric substantia nigra hyperechogenicity(SNH)in 6-OHDA group(both P<0.05);meanwhile the area of SNH in the left was significantly larger in the right side( P<0.05).No significant change in SNH area was found during the continuous observation period of weeks 4-7. For 6-OHDA group, the area of SNH was negatively correlated with the number of forelimb wall-touches( r=-0.825, P<0.001). ③Pathological examination:compared with NC group and sham group, the substantia nigra(SN)of 6-OHDA group showed a series of pathological events, including dopaminergic(DA)neurons asymmetrically decreasing, asymmetric ironion deposition and the number of active microglia increasing(all P<0.05). Correlation analysis showed that the area of SNH was negatively correlated with the number of DA neurons survivors( r=-0.689, P=0.013),while the activation of microglial and the deposition of iron were positively correlated with the area of SNH( r=0.915,0.735;all P<0.001). Conclusions:Asymmetric SNH of 6-OHDA PD rats is a representation of asymmetric motor deficits, and the mechanism is related to a catalogue of asymmetric pathological changes in SN,which comprise DA neurons decreasing, asymmetric iron ions deposition, microglial activating.
