1.Umbrella decision-making model for diagnosis and treatment of elderly lung cancer patients: Construction and practice

With the accelerating trend of population aging, the number of elderly patients with lung cancer continues to rise, and the disease burden is becoming increasingly heavy. The clinical management of these patients faces severe challenges due to their decreased physiological reserve, complex comorbidities, and significant individual heterogeneity. Consequently, under traditional diagnosis and treatment models, doctors often struggle to identify the individualized risks of elderly patients in a timely and comprehensive manner, which can easily lead to decision biases such as undertreatment or overtreatment. In view of this, this study advocates for the establishment of an umbrella decision-making model specifically tailored for elderly lung cancer patients. Grounded in a multidisciplinary team (MDT) platform, this model deeply integrates oncological indicators with the comprehensive geriatric assessment (CGA) system. By holistically considering multidimensional variables including tumor burden, organ function, frailty index, cognitive status, and social support, the model establishes an operational mechanism characterized by "single entry, precise stratification, and targeted selection". Accordingly, patients can be scientifically triaged into distinct intervention tiers, such as active surveillance, minimally invasive surgery, drug therapy, radiotherapy, and best supportive care, thereby achieving real-time alignment between treatment intensity and patient fitness. This article elaborates on the construction logic and key operational procedures of this novel decision-making framework, aiming to guide clinical practice beyond the limitations of a tumor-centric perspective toward a holistic, dynamic, whole-course management strategy. This transition seeks to ensure optimal quality of life and clinical net benefit for elderly patients alongside survival prolongation.
2.Aldehyde Dehydrogenase 2 Gene Mutation May Reduce the Risk of Rupture of Intracranial Aneurysm in Chinese Han Population

Background:
and Purpose Ruptured intracranial aneurysms (RIA) are associated with a mortality rate of up to 40% in the Chinese population, highlighting the critical need for targeted treatment interventions for at-risk individuals. Although the impact of aldehyde dehydrogenase 2 (ALDH2) gene mutations on susceptibility to intracranial aneurysms (IA) is well documented, the potential connection between ALDH2 rs671 single-nucleotide polymorphism (SNP) and RIA remains unexplored. Given the increased prevalence of ALDH2 gene mutations among Chinese Han individuals, it is clinically relevant to investigate the link between ALDH2 rs671 SNP and IA rupture.
Methods:
A prospective study was conducted on 546 patients diagnosed with IA to investigate the association between ALDH2 rs671 SNP and the risk of IA rupture.
Results:
The ALDH2 rs671 SNP (ALDH2*2) was significantly more prevalent in patients with unruptured IA (UIA) than in those with RIA (32.56% vs. 18.58%, P=0.004). Multivariate logistic regression analysis revealed that people with the ALDH2 mutation (ALDH2*1/*2 and ALDH2*2/*2 gene type) had a significantly reduced odds ratio (OR=0.49; 95% confidence level [CI] 0.27–0.88; P=0.018) for RIAs. Age-specific subgroup analysis indicated that the ALDH2 mutation provided a stronger protective effect in individuals aged 60 years and above with IA compared to those under 60 years old (OR=0.38 vs. OR=0.52, both P<0.05).
Conclusion
The incidence of RIA was significantly higher in individuals with a normal ALDH2 gene (ALDH2*1/*1) than in those with an ALDH2 rs671 SNP (ALDH2*1/*2 or ALDH2*2/*2). ALDH2 rs671 SNP may serve as a protective factor against RIA in the Chinese Han population.
3.Renal metabolomics study of Huangqi preparations for alleviating nephrotoxicity of cisplatin

Aim To investigate the effect of Huangqi injection(HI)and Huangqi oral solution(HO)on cisplatin-induced nephrotoxicity(CIN)based on un-targeted metabolomics technology and the underlying mechanisms.Methods Sprague Dawley(SD)rats were randomly divided into the blank group,cisplatin(CDDP)model group,HI treatment group,and HO treatment group,then the CIN model was built with low dose multiple intraperitoneal injections of CDDP.Pre-liminary evaluation of the renal protective efficacy of HI and HO was performed by measuring serum creatinine(Scr),blood urea nitrogen(BUN),and organ indi-ces.Further screening and identification of potential biomarkers(PBs)related to CIN and HI/HO pharma-cological effects were attained through metabolomics studies of renal tissues,and pathway enrichment analy-sis was conducted.Results HI and HO significantly restored the abnormal increase in renal function indica-tors and abnormal decrease in organ indices caused by CDDP,as well as significantly improved the abnormal renal metabolic profile induced by CDDP,indicating that both HI and HO had good alleviating effects on CIN.HI significantly reversed 47 out of 54 CIN related PBs,mainly involving metabolic pathways such as glycerophospholipid metabolism,tryptophan metabo-lism,pantothenate and CoA biosynthesis;HO signifi-cantly reversed 18 out of 54 CIN related PBs,mainly involving metabolic pathways such as taurine and hypo-taurine metabolism,ascorbate and aldarate metabo-lism,pentose and glucuronate interconversions.Con-clusions Both HI and HO have significant alleviating effects on CIN.In the short term,HI salleviating effect is superior to that of HO.Overall,the mechanisms by which both alleviate CIN are mainly related to regula-ting lipid metabolism,amino acid metabolism.
4.Protective Effect and Mechanism of MyD88 Inhibitor in Acute Pancreatitis Associated Gastric Injury

Objective To investigate the inhibitory effect of MyD88 inhibitor St-2825 on inflammatory pathways and its protective efficacy on gastric tissue in sodium taurocholate-induced acute pancreatitis-associated gastric injury.Methods An a-cute pancreatitis rat model was established by subcapsular pancreatic injection of sodium taurocholate.Rats were randomly di-vided into normal control group(Group C,received saline+saline treatment),acute pancreatitis group(Group P,received sodium taurocholate+saline treatment),and intervention group(Group T,received sodium taurocholate+MyD88 inhibitor St-2825 treatment),with 16 rats in each group,48 rats in total.Pancreatic and gastric tissue samples were collected at 2 h and 6 h post-modeling.The samples were paraffin-embedded,sectioned,and stained with hematoxylin-eosin(HE)for comparative pathological scoring.Serum levels of tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),and interleukin-6(IL-6)were measured by ELISA.Immunofluorescence staining was performed to assess immune cell infiltration in gastric tissues.Western blot was used to determine the protein expression of MyD88 and the phosphorylation ratio of p65 in the TLR/MyD88/NF-κB p65 signaling pathway in rat gastric tissues.Results In sodium taurocholate-induced acute pancreatitis-associated gastric injury rats,both pancreatic and gastric pathological scores were increased progressively over time.The plasma levels of TNF-α,IL-1 and IL-6 were significantly elevated.During pancreatic injury,gastric tissues exhibited markedly increased infiltration of neutrophils,mac-rophages,dendritic cells and T lymphocytes.The upregulated MyD88 in tissues stimulated NF-κB pathway activation,leading to phosphorylated p65 nuclear translocation and downstream inflammatory factors transcription.MyD88 inhibitor St-2825 inter-vention effectively blocked p65 phosphorylation and nuclear translocation,attenuated downstream inflammatory responses,and reduced immune cell infiltration in gastric tissues.Conclusion Gastric tissues undergo progressive damage during pancreatic in-jury.The MyD88 inhibitor St-2825 attenuates inflammatory responses and mitigates immune cell infiltration in acute pancreati-tis-associated gastric injury,thereby exerting protective effects.
5.Chemical constituents from Tylophora ovata and their antibacterial activities

AIM To study the chemical constituents from Tylophora ovata(Lindl.)Hook.ex Steud.and their antibacterial activities.METHODS Ethanol extract was isolated and purified by MCI,silica gel,Sephadex LH-20 and semi-preparative HPLC,then the structures of obtained compounds were identified by spectral data.The inhibitory activities of each compound against Phomopsis sp.were determined by mycelial growth rate method.RESULTS Twenty-six compounds were identified as paeonol(1),stigmast-4-en-3-one(2),ergosta-4,6,8(14),22-tetraen-3-one(3),2,4-methoxyphenol(4),1,2,4-trimethoxybenzene(5),3-methoxyphenol(6),3,4-dimethoxyacetophenone(7),5α,8α-epidioxy-ergosta-6,22(E)-diene-3β-ol(8),kaempferol 3-O-β-D-galactopyranoside(9),glaucogenind C(10),glaucoge-nin A 3-O-β-D-cymaropyranoside(11),dibutyl phthalate(12),cynatratoside A(13),hirundigoside C(14),sublanceoside B2(15),cynanoside A(16),dipentyl phthalate(17),5-hydroxymethyl-2-furancarboxaldehyde(18),bis-(2-ethyl)hexylphthalate(19),p-hydroxybenzoic acid(20),syringic acid(21),β-hydroxypropiovanillone(22),3-hydroxy-l-(4-hydroxy-3,5-dimethoxyphenyl)-1-propanone(23),(+)-syringare sinol(24),(-)-syringare sinol(25),(+)-medioresinol(26).IC50 value of compound 12 was 37.27 μg/mL.CONCLUSION Compounds 1-26 are isolated from this plant for the first time.Compound 12 has inhibitory activity against Phomopsis sp.
6.Effect of periacetabular osteotomy on pelvic sagittal tilt in developmental dysplasia of hip

BACKGROUND:Developmental dysplasia of hip refers to the deformity of acetabulum and proximal femur,which can lead to hip instability and hip osteoarthritis. Periacetabular osteotomy is the main treatment for non-terminal developmental dysplasia of hip in adolescents and adults. Pelvic inclination plays an important role in biomechanics of hip joint,which determines the stress direction of hip joint. In hip joint diseases,pelvic compensatory tilt can occur to minimize the abnormal stress of hip joint.OBJECTIVE:To observe whether periacetabular osteotomy changes pelvic sagittal tilt in patients with developmental dysplasia of hip,and to investigate the effect of acetabular covering on pelvic sagittal tilt.METHODS:A retrospective analysis was performed on 29 patients who underwent periacetabular osteotomy due to developmental dysplasia of hip in West China Hospital of Sichuan University from August 2019 to June 2022. Anteroposterior X-rays of the standing pelvis were collected before and 6 months after surgery. The pelvic tilt parameters,including the lateral center-edge angle,pubic symphysis to sacroiliac joint distance,pubic symphysis to sacroiliac joint midline distance,sacrofemoral-pubic angle,and pelvic tilt,were measured. Hip functions of all patients were assessed by hip disability and osteoarthritis score and Harris score before and 6 months after surgery.RESULTS AND CONCLUSION:(1) The lateral center-edge angle,pubic symphysis to sacroiliac joint midline distance,and sacrofemoral-pubic angle were significantly increased 6 months postoperatively compared to preoperative values (P<0.05). Tonnis angle and pelvic tilt were significantly decreased (P<0.05). There was no significant difference in pubic symphysis to sacroiliac joint distance between preoperative and postoperative measurements (P>0.05). (2) The hip disability and osteoarthritis scores and Harris scores were significantly increased 6 months after operation (P<0.01). (3) It is indicated that periacetabular osteotomy surgery can significantly improve acetabular coverage and the pelvis showed significant anteversion changes in patients with developmental dysplasia of hip.
7.A qualitative study on digital-intelligent equipment empowering"generalized"development of traditional Chinese medicine inspection

Objective This study investigated feasible cases and their significance in promoting the"generalized"development of inspection through digital-intelligent equipment.Methods A qualitative research approach was used,involving interviews conducted between February 2025 and March 2025 with experts in traditional Chinese medicine diagnostics,clinical research methodology,medical engineering integration,and related disciplines,using both online and offline methods.In accordance with the Consolidated Criteria for Reporting Qualitative Research,feasible cases involving the specific application of digital equipment in various parts of observation were collected through item enrichment.The significance of extending observation capabilities via these cases was analyzed,along with the overall implications of integrating digital technologies with traditional inspection method.Results Interviews were completed with 11 experts from domestic universities and research institutes in the fields of traditional Chinese medicine diagnosis,medical engineering integration,and related disciplines.A total of 78 feasible cases of digital-intelligent inspection were identified,along with 69 insights regarding the significance of enhancing the inspection capabilities.These insights were synthesized into two dimensions and 23 holistic meanings.The first dimension is to expand the scope of inspection,including obtaining internal environmental characteristics,observing external environmental characteristics,expanding thermodynamic characteristic data,and crossing time and space.The second dimension is to improve the quality of observation and diagnosis information collection and analysis,including 19 specific meanings,such as standardized collection environment,objective quantification,and refined observation.Conclusion Digital-intelligent equipment plays a significant role in expanding the scope of inspection content and achieving high-quality acquisition and analysis of extensive inspection information.These advancements extend and enrich the capabilities of traditional inspection method in traditional Chinese medicine.
8.Progress in application of fecal microbiota transplantation in cardiovas-cular diseases

Cardiovascular diseases pose a significant threat to the public health and healthcare systems in Chi-na.Their pathogenesis is complex and not yet fully elucidated.Increasing evidence suggests that the gut microbiota plays a crucial role in maintaining health and modulating disease progression.Dysbiosis of the gut microbiota has emerged as a key contributor to the development of cardiovascular diseases and has become a prominent focus of current biomedical re-search.Fecal microbiota transplantation,a direct and effective method for restoring gut microbial balance,has been ap-plied in clinical practice with continuously expanding indications,demonstrating the potential to become a novel therapeu-tic strategy for the prevention and treatment of cardiovascular diseases.This article reviews the technology of fecal microbi-ota transplantation,with a particular focus on its applications in major cardiovascular diseases,including atherosclerosis,myocardial infarction,heart failure,hypertension,atrial fibrillation,and myocarditis.It also outlines the potential mecha-nisms through which fecal microbiota transplantation may exert its therapeutic effects,aiming to provide a reference for clinical strategies in the prevention and management of cardiovascular diseases.

10.Single nucleotide polymorphism typing of Yersinia pestis in natural plague foci around Qinghai Lake

This study was aimed at determining the molecular characteristics of Yersinia pestis in the natural plague foci around Qinghai Lake through single nucleotide polymorphism technology,to lay a foundation for molecular epidemiological and source-tracing analysis of Y.pestis in this area.Using the whole genome sequencing technology,we obtained the whole genome sequences of 84 representative Y.pestis strains.Using the sequences of Y.pestis and Yersinia pseudotuberculosis IP32953 from the NCBI database as references,we compared and analyzed the 2 298 SNP loci of these strains.From 1957 to 2020,84 representative strains of Y.pestis from the natural plague foci around Qinghai Lake were divided into two clades:1.IN2 and 3.ANT1.The 1.IN2 clade was the characteristic population of Y.pestis throughout all epidemic years in this area.Additionally,analysis of the SNP distribution and hosts in the region indicated that the 1.IN2 clade was located in five counties except Wulan,whereas the 3.ANT1 clade was isolated from Himalayan marmot and dog in two counties.In conclusion,the population structure of SNP of Y.pestis in the natural plague foci around Qinghai Lake is relatively simple,and SNP analysis of Y.pestis provided a scientific basis for tracing plague epidemic sources and formulating plague prevention and control measures in this area.
