1.Rapid health technology assessment of deucravacitinib in the treatment of moderate-to-severe plaque psoriasis

OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of deucravacitinib in the treatment of moderate- to-severe plaque psoriasis. METHODS Rapid health technology assessment (HTA) reports, systematic reviews (SR)/meta- analyses, and pharmacoeconomic studies on deucravacitinib for the treatment of moderate-to-severe plaque psoriasis were identified by searching PubMed, Web of Science, Embase, CNKI, Wanfang data and official HTA websites. The search time frame spanned from database inception to July 2025. After literature screening, data extraction, and quality assessment, the study results were subjected to descriptive analysis and synthesis. RESULTS A total of 14 articles were finally included, consisting of 1 HTA report, 10 SR/meta-analyses, and 3 pharmacoeconomic studies. Regarding efficacy, deucravacitinib demonstrated superior efficacy to both placebo and apremilast, with significantly higher response rates for Psoriasis Area and Severity Index 50/75/90/100, Static Physician’ s Global Assessment 0/1, and Dermatology Life Quality Index 0/1, as well as greater reduction in Psoriasis Symptoms and Signs Diary Score (P<0.05). Regarding safety, deucravacitinib was well-tolerated. Although the overall incidence of adverse events (AEs) was higher than placebo, it was not significantly different from apremilast. Moreover, the incidence of serious AEs and the rate of discontinuation due to AEs did not differ significantly from placebo (P>0.05). Regarding cost-effectiveness, deucravacitinib proved to be more cost-effective than apremilast across multiple healthcare system perspectives, including those of the United States, Japan, and China. CONCLUSIONS Deucravacitinib exhibits favorable efficacy, safety, and cost-effectiveness in the treatment of moderate-to-severe plaque psoriasis. Additional real-world studies are warranted to further refine its evaluation.
2.Expression of circCEMIP in bladder cancer and its regulatory effects on the proliferation, migration and invasion of UMUC-3 cells

[摘 要] 目的:探讨环状RNA CEMIP(circCEMIP)对膀胱癌UMUC-3细胞增殖、迁移和侵袭的影响及其分子机制。方法:通过TCGA数据库分析circCEMIP在膀胱癌组织中的表达水平,分析其表达与膀胱癌患者临床分期及生存期的关系。采用qPCR法检测circCEMIP在膀胱癌细胞5637、UMUC-3、MGH-U3、J82和T24中的表达。利用RNA干扰技术,分别将si-circCEMIP及其阴性对照(si-NC)、anti-miR-335及其阴性对照(anti-miR-NC)转染UMUC-3细胞,记为si-circCEMIP组、si-NC组、si-circCEMIP + anti-miR-335组和si-circCEMIP + anti-miR-NC组。采用克隆形成实验、划痕愈合实验和Transwell实验分别检测circCEMIP和miR-335表达对UMUC-3细胞增殖、迁移和侵袭能力的影响,双萤光素酶报告基因实验验证circCEMIP与miR-335的靶向关系,WB法检测细胞中VEGF-C信号通路相关蛋白的表达。构建UMUC-3细胞裸鼠皮下移植瘤模型,观察敲低circCEMIP对移植瘤生长的影响。结果:膀胱癌组织中circCEMIP呈高表达(P < 0.01),其表达水平与膀胱癌的临床分期正相关(P < 0.01),circCEMIP高表达患者生存率较低(P < 0.01)。circCEMIP在膀胱癌5637、UMUC-3、MGH-U3、J82和T24细胞中呈高表达(均P < 0.01)。敲低circCEMIP显著降低UMUC-3细胞的增殖、迁移和侵袭能力(均P < 0.01)。circCEMIP可靶向结合miR-335(P < 0.01),敲低circCEMIP能显著上调miR-335表达(P < 0.01)。抑制miR-335表达能逆转敲低circCEMIP对UMUC-3细胞增殖、迁移和侵袭的抑制作用(均P < 0.01)。敲低circCEMIP能明显下调VEGF-C信号通路相关蛋白VEGF-C、MMP-2、MMP-9和β-catenin表达(均P < 0.01),抑制miR-335表达能部分逆转敲低circCEMIP对该通路相关蛋白表达的抑制作用(均P < 0.01)。体内实验证实,敲低circCEMIP能够抑制裸鼠膀胱癌移植瘤的生长(P < 0.01)。结论:敲低circCEMIP通过上调miR-335表达抑制膀胱癌UMUC-3细胞的增殖、迁移和侵袭。

Dry eye disease(DED)is a common ocular surface disorder worldwide, primarily characterized by a loss of homeostasis of the tear film, and frequently associated with meibomian gland dysfunction(MGD), decreased tear film stability, ocular discomfort, and visual impairment. In recent years, factors such as the widespread use of digital devices,the aging population, and environmental changes have contributed to a significant increase in its global prevalence, making it a major public health concern. Red light therapy(RLT), also known as low-level laser therapy(LLLT)or photobiomodulation(PBM), is a non-invasive treatment that utilizes low-energy red or near-infrared light to irradiate tissues. It exerts photobiomodulatory effects to promote cellular repair and functional recovery. This therapy has demonstrated considerable potential in treating various ocular conditions. Its broader clinical application could improve therapeutic outcomes, alleviate patient discomfort and financial burden, and reduce the consumption of healthcare resources, thereby yielding significant socio-economic benefits. This paper systematically reviews the multifaceted mechanisms and application prospects of RLT in managing DED, including its anti-inflammatory effects, improvement of meibomian gland function, promotion of conjunctival goblet cell repair, and alleviation of visual fatigue, aiming to provide a theoretical foundation and practical reference for its clinical adoption.
4.Mechanism of Xixintang in Protecting Blood-brain Barrier in Alzheimer's Disease Model Rats Based on AQP4 Polarization

ObjectiveThis paper aims to investigate the effects of Xixintang on aquaporin-4 (AQP4) polarity distribution, blood-brain barrier (BBB) function, and neuroinflammationin rats with Alzheimer's disease (AD), thereby revealing the potential mechanism through which this formula protects the BBB by regulating AQP4 polarization. The aim is to provide a scientific basis for clinical treatment. MethodsSixty Sprague-Dawley (SD) rats were randomly divided into a normal group, a model group, a probiotic group, a donepezil group, and an Xixintang group. The model was established by intraperitoneal injection of D-galactose (D-Gal) combined with bilateral intracerebroventricular injection of amyloid-β25-35 (Aβ25-35). The probiotic group (30.85 mg·kg-1), donepezil group (0.88 mg·kg-1), and Xixintang group (1.174 g·kg-1) received daily gavage administration, while the normal and model groups received intragastric administration with an equal volume of normal saline for one month. Cognitive ability was assessed by using the Morris water maze. BBB permeability was detected via Evans blue extravasation. The contents of interleukin-6 (IL-6), amyloid-β1-42 (Aβ1-42), and tumor necrosis factor-α (TNF-α) in the hippocampal tissues were measured by enzyme-linked immunosorbent assay (ELISA). The protein expressions of zonula occludens-1 (ZO-1), occludin, tissue inhibitor of metalloproteinase-1 (TIMP-1), matrix metalloproteinase-9 (MMP-9), and AQP4 in the hippocampal tissues were detected by western blot. The expression and co-localization levels of Aβ1-42, ionized calcium-binding adapter molecule 1 (IBA1), and AQP4/platelet endothelial cell adhesion molecule 31 (CD31) in the hippocampal region were examined by immunofluorescence. ResultsCompared with the normal group, the model group exhibited a significant decline in cognitive ability (P<0.01) and a marked increase in Evans blue extravasation in the brain (P<0.01). The expressions of ZO-1, occludin, and TIMP-1 were significantly decreased (P<0.01), while the expressions of AQP4 and MMP-9 were significantly increased (P<0.01). The co-localization level of AQP4/CD31 was significantly reduced (P<0.01), and the expressions of Aβ1-42, IL-6, TNF-α, and IBA1 were significantly elevated (P<0.01). Compared with the model group, the Xixintang group showed significant improvement in cognitive ability (P<0.01) and a significant reduction in Evans blue extravasation in the brain (P<0.01). The expressions of occludin, TIMP-1, and ZO-1 were significantly increased (P<0.05, P<0.01), while the expressions of AQP4 and MMP-9 were significantly decreased (P<0.05). The co-localization level of AQP4/CD31 was significantly enhanced (P<0.01), and the expressions of Aβ1-42, IL-6, TNF-α, and IBA1 were significantly reduced (P<0.05, P<0.01). ConclusionXixintang may improve cognitive function and alleviate AD pathology in AD model rats by regulating AQP4 polarity distribution, thereby breaking the vicious cycle of "Aβ deposition-neuroinflammation-BBB damage" and restoring the homeostasis of the microenvironment in the brain.
5.Safety of endoscopic variceal ligation and endoscopic cyanoacrylate injection in treatment of esophagogastric varices in patients with liver cirrhosis and influencing factors for postoperative bleeding

ObjectiveTo investigate the risk factors for bleeding within 5 days and 2 weeks after endoscopic variceal ligation (EVL) or endoscopic cyanoacrylate injection (ECI) for the treatment of esophagogastric varices in patients with liver cirrhosis, as well as the safety of EVL/ECI in patients with thrombocytopenia. MethodsA total of 489 patients with liver cirrhosis and esophagogastric varices who underwent EVL/ECI in Guangzhou Eighth People’s Hospital, Guangzhou Medical University, from January 2018 to December 2023 were enrolled as subjects, and according to the presence or absence of bleeding after surgery, they were divided into bleeding group and non-bleeding group. The risk factors for bleeding within 5 days and 2 weeks after surgery were analyzed. The independent-samples t test or the Mann-Whitney U test was used for comparison of continuous data between groups, and the chi-square test or the continuity-corrected chi-square test was used for comparison of categorical data between groups; the receiver operating characteristic (ROC) curve was plotted to determine the cut-off value of MELD score; a multivariate logistic regression analysis was used to identify the independent risk factors for postoperative bleeding. ResultsThere were no significant differences in the bleeding rates within 5 days and 2 weeks after EVL/ECI between the 386 patients with a platelet count of ≥50×109/L and the 103 patients with a platelet count of (25 — 49)×109/L (5 days: 1.94% vs 2.85%, P=0.870; 2 weeks: 2.91% vs 4.92%, P=0.544). The overall bleeding rate was 2.66% (13/489) and 4.50% (22/489), respectively, within 5 days and 2 weeks after EVL/ECI. The multivariate logistic regression analysis showed that MELD score was an independent risk factor for bleeding within 5 days (odds ratio [OR]=3.726, 95% confidence interval [CI]: 1.214 — 11.429, P=0.021) and 2 weeks (OR=5.760, 95%CI: 1.779 — 18.651, P=0.003) after EVL/ECI, while hemoglobin (Hb) was a protective factor against bleeding within 5 days (OR=0.972, 95%CI: 0.948 — 0.996, P=0.025) and 2 weeks (OR=0.976, 95%CI: 0.957 — 0.995, P=0.016) after surgery; portal vein tumor thrombus (OR=2.667, 95%CI: 1.000 — 7.117, P=0.050) was an independent risk factor for bleeding within 2 weeks after surgery, while platelet count [(25 — 49)×10⁹/L] was not a risk factor for postoperative bleeding (P>0.05). ConclusionBoth EVL and ECI have good safety in patients with liver diseases and grade 3 thrombocytopenia. MELD score is an independent risk factor for bleeding within 5 days and 2 weeks after EVL/ECI, while Hb is a protective factor; portal vein tumor thrombus is an independent risk factor for bleeding within 2 weeks after surgery.
6.Effect of Xixintang on Colonic Mucosal Barrier and TLR4 /NF-κB p65 Signaling Pathway in AD Model Rats Induced by D-galactose Combined with Aβ25-35

ObjectiveThis study aims to investigate whether Xixintang could ameliorate cognitive dysfunction in an Alzheimer's disease (AD) rat model induced by D-galactose and β-amyloid (Aβ25-35), by means of repairing the colonic mucosal barrier, regulating the Toll-like receptor 4 (TLR4)/nuclear factor-κB p65 (NF-κB p65) signaling pathway, and intervening in the pathological process mediated by the gut-brain axis. MethodsSixty specific pathogen-free (SPF) male Sprague-Dawley (SD) rats were randomly divided to five groups (n=12): A control group, a model group, a donepezil group, an Xixintang group, and a probiotic group. Except for those in the control group, rats in all other groups received daily intraperitoneal injections of D-galactose for six consecutive weeks. Subsequently, aggregated Aβ25-35 was injected stereotactically into the bilateral ventricles to establish the AD model. During the intervention periods, the rats in all groups were administered their respective drugs and normal saline by gavage. The Morris water maze test was used to assess the capacity for spatial learning and memory. Hematoxylin-eosin (HE) staining was employed to observe the histopathological changes in the colon tissues. Immunofluorescence was used to detect Aβ1-41 deposition in the hippocampal region and Mucin 2 (MUC2) expression in the colonic mucosa. Western blot was performed to measure the protein expression levels of FFAR2,TLR4, NF-κB p65, occludin (OCLN), zonula occludens-1 (ZO-1), and MUC2 in the colonic tissues. Enzyme-linked immunosorbent assay (ELISA) was used to determine the contents of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), serum amyloid A (SAA), and Aβ1-42 in the hippocampal region from the colonic tissues. The lipopolysaccharide (LPS) concentrations in colon tissues of rats were measured by using a dynamic chromogenic limulus assay. ResultsCompared with those in the control group, the rats in the model group exhibited a significantly prolonged escape latency and a markedly shorter duration in the target quadrant (P<0.01). The integrity of the colonic mucosal structure was compromised, with disordered gland arrangement and a reduced number of goblet cells. The Aβ1-42 deposition in the hippocampal region was significantly increased (P<0.01). The protein expression levels of TLR4 and NF-κB p65 in colonic tissues were significantly upregulated (P<0.01), while those of occludin and ZO-1 were downregulated (P<0.01). The contents of inflammatory factors such as IL-6, TNF-α, and SAA were significantly elevated (P<0.01), and the LPS level in the serum was markedly increased (P<0.01). In comparison to those in the model group, the rats in the Xixintang group showed a significantly shortened escape latency and a prolonged duration in the target quadrant (P<0.01). The colonic mucosal structure was ameliorated, with neat gland arrangement and an increased number of goblet cells. The Aβ1-42 deposition in the hippocampal region was reduced (P<0.01). The protein expressions of TLR4 and NF-κB p65 in the colon tissues were decreased (P<0.05,P<0.01), while the protein levels of occludin and ZO-1 were increased (P<0.01). The contents of IL-6, TNF-α, and serum amyloid A (SAA) were decreased (P<0.01), and the LPS level was reduced (P<0.01). ConclusionXixintang can significantly ameliorate cognitive dysfunction of AD model rats, by means of restoring the colonic mucosal barrier structure, reducing cerebral Aβ deposition, and suppressing peripheral and central inflammatory response. Its mechanism of action may be closely associated with the suppression of the TLR4/NF-κB signaling pathway activation, reduction of endotoxin levels, and regulation of the gut-brain axis.
7.Regulatory Effect of Danhe Granules on Oxidative Stress in Rats with Mixed Hyperlipidemia

ObjectiveTo investigate the therapeutic mechanism of Danhe granules in treating mixed hyperlipidemia based on network pharmacology, as well as animal and cell experiments. MethodsThe active compounds and targets of Danhe granules were screened using the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and the Encyclopedia of Traditional Chinese Medicine (ETCM). Related targets for mixed hyperlipidemia were obtained from the GeneCards database. The intersecting targets were subjected to Gene Ontology (GO) functional annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses. A high-fat model was established in human hepatocellular carcinoma cells (HepG2) induced by palmitic acid (PA), followed by intervention with Danhe granules to assess intracellular lipid accumulation and oxidative stress levels. A mixed hyperlipidemia rat model was also established and divided into low-, medium-, and high-dose Danhe granules groups (1.134, 2.268, and 4.536 g·kg-1, respectively), as well as a positive control group treated with pravastatin sodium (4.020 mg·kg-1). After eight weeks of intervention, serum lipid levels, inflammatory factors, oxidative stress indices, and the expression of key hepatic lipid metabolism-related proteins were determined. ResultsNetwork pharmacology identified 93 intersecting targets between Danhe granules and mixed hyperlipidemia, with peroxisome proliferator-activated receptor gamma (PPARG), peroxisome proliferator-activated receptor alpha (PPARA), tumor necrosis factor (TNF), interleukin-6 (IL-6), and IL-1B among the key nodes. The PPAR signaling pathway, AGE/RAGE signaling pathway, lipid metabolism, atherosclerosis and non-alcoholic fatty liver disease (NAFLD) were among the most significantly enriched pathways. Cellular experiments demonstrated that Danhe granules significantly reduced reactive oxygen species (ROS) and malondialdehyde (MDA) levels while increasing catalase (CAT) activity (P<0.05), thereby alleviating intracellular lipid accumulation and triglyceride (TG) content in HepG2. In animal experiments, Danhe granules markedly decreased serum total cholesterol (TC), TG, and low-density lipoprotein cholesterol (LDL-C) levels (P<0.05), reduced hepatic MDA levels, and elevated superoxide dismutase (SOD) and CAT levels. Histological analysis showed alleviation of hepatic steatosis, upregulation of hepatic PPARA and lipoprotein lipase (LPL) expressions, and downregulation of sterol regulatory element-binding protein 1 (SREBP1) expression (P<0.05, P<0.01). ConclusionDanhe granules improve lipid metabolism disorders in mixed hyperlipidemia by reducing MDA levels, enhancing SOD and CAT activities, scavenging excessive ROS, inhibiting oxidative stress, and mitigating liver injury. The underlying mechanism may involve the upregulation of PPARA and LPL and the suppression of SREBP1 expression.

After eight weeks of intervention, serum lipid levels, inflammatory factors, oxidative stress indices, and the expression of key hepatic lipid metabolism-related proteins were determined. ResultsNetwork pharmacology identified 93 intersecting targets between Danhe granules and mixed hyperlipidemia, with peroxisome proliferator-activated receptor gamma (PPARG), peroxisome proliferator-activated receptor alpha (PPARA), tumor necrosis factor (TNF), interleukin-6 (IL-6), and IL-1B among the key nodes. The PPAR signaling pathway, AGE/RAGE signaling pathway, lipid metabolism, atherosclerosis and non-alcoholic fatty liver disease (NAFLD) were among the most significantly enriched pathways. Cellular experiments demonstrated that Danhe granules significantly reduced reactive oxygen species (ROS) and malondialdehyde (MDA) levels while increasing catalase (CAT) activity (P<0.05), thereby alleviating intracellular lipid accumulation and triglyceride (TG) content in HepG2. In animal experiments, Danhe granules markedly decreased serum total cholesterol (TC), TG, and low-density lipoprotein cholesterol (LDL-C) levels (P<0.05), reduced hepatic MDA levels, and elevated superoxide dismutase (SOD) and CAT levels. Histological analysis showed alleviation of hepatic steatosis, upregulation of hepatic PPARA and lipoprotein lipase (LPL) expressions, and downregulation of sterol regulatory element-binding protein 1 (SREBP1) expression (P<0.05, P<0.01). ConclusionDanhe granules improve lipid metabolism disorders in mixed hyperlipidemia by reducing MDA levels, enhancing SOD and CAT activities, scavenging excessive ROS, inhibiting oxidative stress, and mitigating liver injury. The underlying mechanism may involve the upregulation of PPARA and LPL and the suppression of SREBP1 expression.
10.Visual analysis of treatment of adolescent idiopathic scoliosis

BACKGROUND:At present,the incidence of scoliosis is increasing year by year,especially in adolescent idiopathic scoliosis.Therefore,it is more and more important to strengthen the research on the treatment of adolescent scoliosis. OBJECTIVE:To summarize the current status,hotspots,emerging trends,and frontiers of global research on the treatment of adolescent idiopathic scoliosis to provide reference and guidance for future related research. METHODS:The literature related to the treatment of adolescent idiopathic scoliosis was retrieved on the Web of Science Core Collection(WOSCC)database from 2013 to 2023.CiteSpace 6.2.R1 software was used for visual analysis of countries,institutions,authors,and keywords. RESULTS AND CONCLUSION:(1)A total of 561 English articles were included in this study.Among countries,institutions,and authors,the United States has contributed the most.Nanjing University and Qiu,Yong(Affiliated Drum Tower Hospital,Nanjing University School of Medicine)are the most published institution and author.The academic journal with the largest number of articles is the European Spine Journal.(2)In the analysis of cited literature,the top 10 most cited articles mainly describe the effects of surgical treatment and conservative treatment on improving adolescent idiopathic scoliosis,especially improving the curvature of patients.(3)Through the summary of highly cited articles and the keyword clustering,keyword prominence in-depth mining,the research hotspots are currently the relationship between Cobb angle and treatment choice,the therapeutic effect of exercise therapy and the therapeutic effect of posterior vertebral fusion.(4)The prognosis of patients with different curvatures has not been studied in depth,and the etiology of adolescent idiopathic scoliosis has not been clarified,so the relationship between curvature and prognosis and the etiology of adolescent idiopathic scoliosis may be a new research trend in the future.
