1.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
2.From tradition to modernity: the integrated development of medical humanities and narrative medicine

Medical humanities consistently run through the entire process of medical development and educational reform. However, with the increasingly prominent dominance of evidence-based medicine in clinical practice, the medical humanities have gradually been weakened in both medical education and clinical practice. Narrative medicine, through telling and listening to patients’ stories, enhances healthcare professionals’ empathy, fosters doctor–patient communication, and facilitates a return to the humanistic essence of medical education and clinical practice. By sorting out and reviewing related literature and developmental trends both at home and abroad, this paper pointed out the existing structural problem of an imbalance between technological priority and humanistic care in medical education, focusing on how to achieve an effective integration of medical humanities and narrative medicine in medical education. This paper also systematically analyzed the significance of both medical humanities and narrative medicine in the medical education system and proposed promoting the deep embedding of narrative medicine in medical education from three entry points, namely, curriculum integration, interdisciplinary collaboration, and the construction of teaching evaluation systems. The aim was to provide theoretical support and practical experience for medical education reform, foster the coordinated development of professional competence and humanistic spirit among medical talents, and truly achieve the goal of cultivating well-rounded medical talents.

Renal fibrosis (RF) is a prevalent clinical symptom of numerous chronic kidney illnesses and a significant pathological alteration in end-stage renal disease resulting from various mechanisms, such as abnormally activated signaling pathways, microRNAs, aging, autophagy disorders, and fibrotic ecological niches, all of which contribute to RF development. Inhibiting, blocking, or delaying the aforementioned mechanisms may yield novel approaches for treating RF. This article explores advancements in the comprehension of the mechanisms and therapeutic approaches for RF.
4.An in vitro experimental study on the efficiency of dissolving pediatric ammonium urate stones in solutions with different pH values

Objective To evaluate the dissolution effects of pediatric ammonium urate stones in solutions with different pH values, so as to provide reference for the clinical treatment of this type of stones. Methods Eight pure ammonium urate stones(ammonium urate content ≥90%)of similar size and mass(median mass 4.6 g)were obtained from 8 children(<14 years)undergoing percutaneous nephrolithotomy at the People's Hospital of Xinjiang Uygur Autonomous Region. The stones were placed in sodium chloride solutions with pH values of 5.0, 6.0, 7.0, and 8.5 and artificial urine with pH values of 4. 6, 5.0, 7.0, and 8.5 at a constant temperature of 37 ℃. The mass of the stones was recorded before placement and at 24, 48, and 72 h after placement. The dissolution rates of the stones were calculated. Results The pediatric ammonium urate stones could be dissolved in sodium chloride solution, and the total dissolution rate was the highest(94.6%)after 72 hours when the pH value was 6.0. The total dissolution rate of ammonium urate stones in artificial urine after 72 hours were all over 50%, and the total dissolution rate was the highest(79.7%)when the pH value was 4.6. Conclusion Pediatric ammonium urate stones are soluble in sodium chloride solution and artificial urine, and the total dissolution rate is higher in acidic environment.
5.Clinical application of transperineal four-hole prostate puncture

Objective To compare the efficacy and safety of the transperineal four-hole prostate puncture with conventional transperineal prostate puncture. Methods A total of 91 patients undergoing transperineal prostate puncture at the Second Hospital of Hebei Medical University during Jan. 2024 and Jan. 2025 were enrolled. Based on the puncture methods, the patients were divided into the four-hole group(n=46)and conventional group(n=45). The four-hole group underwent four-hole puncture, while the conventional group received standardized parallel needle insertion. The operation time, intraoperative blood loss, pain score, positive biopsy rate, and complications were compared between the two groups. Results The intraoperative blood loss [(7.05±2.48)mL vs.(9.15±3.49)mL] and pain score [(2.33±1.55)vs.(3.02± 1.54)] of the four-hole group were significantly lower than those of the conventional group(P<0.05). No significant differences were observed in the positive biopsy rate [47.8%(22/46)vs. 46.7%(21/45)] or operation time [(18.83±2.49)min vs.(18.07±1.66)min](P>0.05). The total complication rate in the four-hole group was significantly lower than that in the conventional group [21.7%(10/46)vs. 42.2%(19/45), P<0.05]. Conclusion Compared with the conventional parallel needle insertion method, the four-hole puncture technique reduces the intraoperative blood loss, alleviates trauma severity and pain response, and reduces the complication rate.
6.Preparation and evaluation of in-house Factor Ⅷ inhibitor-positive quality-control samples

Objective To prepare in-house coagulation factor Ⅷ(F Ⅷ)inhibitor-positive control material and evaluate its perform-ance.Methods Frozen plasma samples from hemophilia A patients with positive factor Ⅷ inhibitors were pooled,and diluted with Owren's Veronal Buffer(OVB)to 1 BU/mL of the inhibitor concentration in the mixture,then aliquoted and freeze-stored.The homo-geneity and stability of the in-house quality control material were verified,and its suitability was further assessed through intra-laborato-ry reproducibility among different technologists and inter-laboratory comparisons.Results Twenty-one aliquots were randomly tested for homogeneity assessment,yielding an average of 1.05 BU/mL(range 0.9-1.15 BU/mL),with a standard deviation(SD)of 0.083 and coefficient of variation(CV)of 7.90%.The freshly prepared inhibitor-positive control samples contained a concentration of 1.03 BU/mL.After storage at-80℃ for 24 hours,1 week,1 month,2 months,3 months,4 months,5 months,6 months,7 months,8 months,and 9 months,thawed the samples showed relative deviations of 9%,0%,10%,9%,14%,15%,6%,0%,-10%,-5%,and 2%,respectively.The intra-laboratory CV value from different technologists at this center was 7.28%,and the inter-labora-tory CV across different centers was 18.75%.Conclusion The prepared in-house positive control material of Factor Ⅷ inhibitor ex-hibited adequate uniformity and stability.

To formulate an expert consensus on the principles of preoperative hair removal and provide scientific guidance for standardized removal of hair before surgical procedures so as to reduce the incidence of surgical site infections.METHODS Led by the Hospital Management Institute of National Health Commission of the People's Republic of China,this consensus was reached with the joint efforts from the expects of relevant fields such as surgeries,interventional therapies,nursing,and infection prevention and control.The consensus facilitates the classification and evaluation of literatures by following the evidence grade formulated by Oxford Evidence-based Medicine Center and focuses on the association of preoperative hair removal with surgical site infection,it reaches the evidence grade of expert consensus and recommendation intensity by integrating with discussions on meetings and clinical experience of the expects from relevant fields.RESULTS A total of 6 items of consensus were reached by summarizing the latest evidence on the aspects including the indications for preoperative hair removal,tools,range,timing and places.CONCLUSION The consensus,to some extent,make supplements to and complete the exiting regulations and standards.It provides guidance for the medical institutions to carry out the preoperative hair removal.
8.Application of VWF Antigen and Activity Testing Based on ABO Blood Group in Risk Assessment of Deep Vein Thrombosis

Objective:To explore the clinical value of plasma von Willebrand factor antigen(VWF:Ag)and VWF activity(VWF:GPIbM)based on ABO blood group in the risk assessment of deep vein thrombosis(DVT).Methods:A total of 163 patients with DVT who sought medical treatment from March 2021 to December 2022 were selected as the case group,and 135 healthy volunteers during the same period were selected as the control group.The differences of ABO blood groups,plasma VWF:Ag and VWF:GPIbM levels between the two groups were compared.Receiver operating characteristic(ROC)curves were used to evaluate the clinical value of VWF testing in predicting DVT events.Logistic regression analysis was applied to identify risk factors for DVT.Results:The levels of plasma VWF:Ag and VWF:GPIbM in the DVT group were significantly higher than those in the control group both overall and across ABO blood type subgroups(P<0.01).Within the DVT group,the levels of plasma VWF:Ag and VWF:GPIbM in patients with non-O blood type were significantly higher than those with blood type O[VWF:Ag:219.74%±63.64%vs 162.21%±56.03%,P<0.01;VWF:GPIbM:228.10%(185.15%,249.10%)vs 148.25%(116.48%,225.48%),P<0.01].The area under the ROC curve(AUC)of VWF:Ag for predicting DVT events was 0.855,with a cut-off value of 142.4%,sensitivity of 82.2%and specificity of 72.6%;the AUC of VWF:GPIbM was 0.861,with a cut-off value of 141.2%,sensitivity of 84.7%,and specificity of 71.1%.Univariate analysis showed that both VWF:Ag and VWF:GPIbM were influencing factors for DVT events(P<0.05).Multivariate logistic regression analysis indicated that VWF:Ag>142.4%(OR=13.961,95%CI:7.654-25.464,P<0.01)and VWF:GPIbM>141.2%(OR=17.615,95%CI:9.155-33.892,P<0.01)were independent risk factors for DVT events.Conclusion:Levels of VWF:Ag and VWF:GPIbM are significantly elevated in non-O blood type DVT patients.VWF:Ag>142.4%and VWF:GPIbM>141.2%are independent risk factors for DVT events.VWF testing based on ABO blood group aids in the precision prevention and control of DVT.
9.Analysis of Gene Types and Clinical Characteristics of Thalasse-mia in Children in Nanchong Area

Objective:To investigate the positive rate,mutation type and distribution characteristics of thalassemia gene detection in children in Nanchong area.Methods:The common α and β-thalassemia gene mutation sites were detected in 1 254 children suspected of thalassemia by hematological screening in our hospital from January 2017 to December 2023,and the genotypes,detection rates and clinical characteristics of thalassemia in local children were statistically analyzed.Results:Among 1 254 children with suspected thalassemia,490 carriers were screened out,with a positive detection rate of 39.07%.Among them,220 cases(17.54%)were α-thalassemia,251 cases(20.02%)wereβ-thalassemia,and 19 cases(1.52%)were αβ compound thalassemia.Among 220 cases of α-thalassemia,the main genotypes were--SEA/αα,-α3.7/αα,-α3.7/--SEA and-α4.2/αα,accounting for 63.64%,18.64%,5.91%,and 5.00%,respectively.Among 251 cases of β-thalassemia,CD17,CD41-42,and IVS-Ⅱ-654 genotypes were the most common,accounting for 40.24%,29.88%,and 17.93%,respectively.In 19 cases of αβ compound thalassemia,the most common genotypes were-α3.7/αα compound CD41-42,--SEA/αα compound CD41-42,--SEA/αα compound CD17,accounting for 26.32%,15.79%,and 15.79%,respectively.In addition,compared with healthy individuals,red blood cell(RBC)in the thalassemia gene carriers was significantly increased,while hemoglobin(Hb),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean corpuscular hemoglobin concentration(MCHC)and red blood cell distribution width-standard deviation(RDW-SD)were significantly decreased(all P<0.01).The ROC curve analysis showed that the area under the curve(AUC)of RDW-SD,MCHC,MCH,MCV,Hb and RBC were 0.827,0.707,0.823,0.863,0.603 and 0.882,respectively.The thalassemia gene carrying rates from 2017 to 2023 in Nanchong were 35.6%(54/154),28.43%(56/197),34.74%(74/213),40.56%(58/143),42.69%(73/171),45.86%(83/181),and 47.18%(92/195),respectively,showing an upward trend year by year.Conclusion:The positive detection rate of children's thalassemia gene in Nanchong is relatively high,and the genetic types are complex,with β-thalassemia as the main type.The genetic pattern shows obvious regional distribution characteristics.The genotypes of thalassemia in children are mainly--SEA/αα,-α3.7/αα,CD17,CD41-42 and IVS-Ⅱ-654,which are consistent with the genotypes of adults in this area,but different from high-risk areas such as Dongguan and Guangxi.
10.Structural analysis of influencing factors for continuity of care and care coordination:A DEMATEL-AISM approach

Objective:To identify key influencing factors and their structural relationships for continuity of care and care coordination in integrated healthcare systems,providing evidence for systemic improvement strategies.Methods:Taking the Yuhuan Health Consortium in Zhejiang Province as an example,this study integrated the Decision-Making Trial and Evaluation Laboratory(DEMATEL)and Adversarial Interpretive Structure Modeling(AISM).An expert questionnaire and literature review were used to construct a factor system,quantifying the influence degree,centrality,and hierarchical structure of 17 continuity and 14 coordination factors.Results:In the system of continuity of care,division-of-labor and linkage mechanisms(influence degree:2.516)and payment methods(causal degree:1.043)were identified as core drivers,forming a four-level interaction network.For care coordination,health planning(centrality:4.452)and health insurance policies(causal degree:1.131)emerged as root causes,establishing a three-tier hierarchical structure.Topological analysis revealed that continuity relies on the"institutional design-process articulation-patient perception"pathway,while coordination depends on the"policy traction-management synergy-technical support"linkage mechanism.Both systems shared disease characteristics(causal degree:1.650/1.384)as underlying drivers,yet service processes(centrality:4.680)and managerial awareness(centrality:4.754)served as unique hub nodes.Conclusion:Differentiated interventions are required:continuity improvement should prioritize payment reform and division-of-labor mechanisms,while coordination enhancement necessitates strengthened health planning and policy synergy.
