Strenuous exercise induces upper respiratory tract infection (URTI), whereas moderate exercise prevents URTI. This study aimed to assess the incidence of URTI and the association between URTI episodes and exercise duration in Japanese collegiate athletes. A cross-sectional survey was conducted with 1,740 participants; 1,235 responses were eligible for analysis. Participants were classified into three groups: control group (weekly total exercise duration <60 min, n=405), exercise group (weekly total exercise duration >60 min, n=193), and athlete group (joined a sports club and weekly exercise frequency >5 days, n=637). We requested the following information from participants: basic characteristics, incidence of URTI symptoms (fever, runny or plugged nose, sore throat, and cough) or influenza for each month over the past year, day of the week exercise is typically performed, and duration of exercise per week. The incidence of URTI episodes per year was significantly lower in the athlete group (2.0 ± 2.4 episodes) compared to the control group (2.6 ± 2.4 episodes, p<0.001). The incidence of URTI episodes did not significantly differ between the control group and exercise group (2.9 ± 2.9 episodes, p=0.607). Although the frequency of URTI episodes and exercise duration were not significantly associated among male athletes (p=0.209), they were positively associated in female athletes (p=0.027). These results indicate that Japanese collegiate athletes experience fewer URTI episodes than non-athletes, but athletes who exercise for long durations may frequently experience URTI, particularly female athletes.

Horseshoe kidney is an unusual abnormality occurring in 0.25% of the population. In surgery for AAA with horseshoe kidney, reconstruction of aberrant renal and preservation of renal isthmus is important. We report 3 cases of AAA with horseshoe kidney treated successfully without division of the isthmus.

In post-genome era, the greatest challenge of post-genome research is how we can apply genomic outcome to practical field like clinical medicine through discovering effective findings from its complex and meta-molecular network. From the viewpoint of reducing health care cost, preventive medicine that can avoid diseases should be essential target. Balneology that contains preventive medicine in part through unspecified bio-modulation effect should be a principal field of genome science based application. Balneology has expectations to be applied to practical clinical field or health promotion through translational research to modern medicine or health science. This translational research needs establishment of bridging knowledge and its bi-directional migration as the essence of translation. Integration of in silico knowledge among balneology, modern medicine, and genomic science is the fundamental basis of this translation. Single knowledge architecture that has anatomically hierarchical structure, logical conceptual unit and its supportive evidences makes integration logically seamless and establishes smooth translation. This paper reports knowledge architecture in balneologic translational research and its prototype.

The basal metabolic rate (BMR) of 70 postmenopausal women (age: 60.6±4.2 yrs., height: 154.9±5.2cm, body weight (BW) : 52.7±6.2kg; mean ±SD) was evaluated in relation to body composition (body fat mass (FM) : 17.3±3.9kg, lean body mass (LBM) : 35.3±3.6kg) . BMR was 1, 148±126kcal/day, 21.9±2.2kcal/kgBW/day, 32.7±3.2kcal kgLBM/day in all subjects. BMR (kcal/day) significantly correlated with BW (r=.635, p<0.001) and LBM (r=.598, p<0.001) . When divided into two groups, (lower %fat group (LF; <35%fat) and higher %fat group (HF; ≥35%fat) ), BW and FM were significantly higher in HF than in LF (BW: 56.6±6.4 vs 51.0±5.3 kg, FM: 21.7±2.8 vs 15.5±2.7 kg, p<0.001, respectively) . No difference was observed in LBM between the two groups (34.9±3.9 vs 35.5±3.5 kg) . BMR (kcal/kgBW/day) was lower in HF than in LF (21.0±1.8 vs 22.3±2.3, p<0.05), but HF group had higher BMR in terms of kcal/kgLBM/ day than LF (34.0±3.1 vs 32.1±3.1, p<0.05) . Multiple regression analysis was performed to predict BMR. A single predictor LBM and a pair of predictors LBM and FM explained 35.7% and 42.7% of the variance of BMR. This study suggested that a decrease in LBM is a major factor in affecting the reduction of BMR in postmenopausal women, whereas FM gained after menopause could be considered to have metabolic activity related to BMR.

The present study was performed to investigate heart rate responses and occurrence of arrhythmias caused by various diving maneuvers and swimming in infants. Forty four healthy infants (mean age 2.5±1.1, mean ±SD) served on this study. Condition of telemetry electrocardiograph recording was 1) rest on the land and in the water, 2) diving, 3) swimming and 4) “pass in water (PW) ”. Moreover, diving styles was classified for five patterns by presence of body movement or expiration (Apneic not move diving, Expiratory not move diving, Apneic kick diving. Expiratory kick diving and Underwater swimming) . “Diving bradycardia” was observed during diving and PW, and the magnitude of heart rate reduction was grater in diving without body movement than diving combined with body movement. Presence of expiration did not influence on the level of bradycardia during diving. Any kinds of arrythmias was observed 13 out of 44 subjects. In premature contractions, supraventricular premature contraction (SVPC, 7 subjects) was observed more often than ventricu-lar premature contraction (PVC, 3 subjects) . Another arrhythmias observed was atrioventricular junctional rhythm (AVJR, 1 subject), atrioventricular junctional escaped beat (AVJEB, 3 subjects) and wandering pacemaker (1 subject) . The danger arrhythmias which is necessary to restrict on exercise or swimming was not observed in this study.

A 62-year-old man was diagnosed as having atrial septal defect (ASD) and atrial fibrillation (Af) upon admission to our hospital with acute myocardial infarction. He was placed on medication for 3 years but surgery was considered necessary because of the further complication of angina pectoris. Coronary arterial bypass grafting, ASD closure and right atrial separation procedure which was a modification of the right-sided maze operation, were performed simultaneously. Although chronic Af disappeared immediately after surgery, it reappeared on the 8th postoperative day, and medicinal and electrical defibrillation had no effect. We considered that the main cause of Af accompanied by ASD had existed in the right atrium before surgery. Also, as the right atrial separation procedure was less invasive than the Cox/maze procedure for such complications in patients with ischemic heart disease, we chose this method. In our patient, postoperative left ventricular loading was considered to have been the cause of Af recurrence. Therefore the indications for this procedure should be decided carefully in patients with preoperative left ventricular hypofunction or left atrial dilatation.
