1.Genomic variant surveillance of SARS-CoV-2 positive specimens using a direct PCR product sequencing surveillance (DPPSS) method.

BACKGROUND AND OBJECTIVE
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) as the causative agent of COVID-19 has significantly challenged the public health landscape in late 2019. After almost 3 years of the first ever SARS-CoV-2 case, the World Health Organization (WHO) declared the end of this global health emergency in May 2023. Although, despite the subsequent drop of COVID-19 cases, the SARS-CoV-2 infection still exhibited multiple waves of infection, primarily attributed to the appearance of new variants. Five of these variants have been classified as Variants of Concern (VOC): Alpha, Beta, Gamma, Delta, and the most recent, Omicron. Therefore, the development of methods for the timely and accurate detection of viral variants remains fundamental, ensuring an ongoing and effective response to the disease. This study aims to evaluate the feasibility of the application of an in-house approach in genomic surveillance for the detection of SARS-CoV-2 variants using in silico designed primers.
METHODSThe primers used for the study were particularly designed based on conserved regions of certain genes in the virus, targeting distinct mutations found in known variants of SARS-CoV-2. Viral RNA extracts from nasopharyngeal samples (n=14) were subjected to quantitative and qualitative tests (Nanodrop and AGE). Selected samples were then analyzed by RT-PCR and amplicons were submitted for sequencing. Sequence alignment analysis was carried out to identify the prevailing COVID-19 variant present in the sample population.
RESULTSThe study findings demonstrated that the in-house method was able to successfully amplify conserved sequences (spike, envelope, membrane, ORF1ab) and enabled identification of the circulating SARS-CoV-2 variant among the samples. Majority of the samples were identified as Omicron variant. Three out of four designed primers effectively bound into the conserved sequence of target genes present in the sample, revealing the specific SARSCoV-2 variant. The detected mutations characterized for Omicron found in the identified lineages included K417N, S477N, and P681H which were also identified as mutations of interest. Furthermore, identification of the B.1.448 lineage which was not classified in any known variant also provided the potential of the developed in-house method in detecting unknown variants of COVID-19.
CONCLUSIONAmong the five VOCs, Omicron is the most prevalent and dominant variant. The in-house direct PCR product sequencing surveillance (DPPSS) method provided an alternative platform for SAR-CoV-2 variant analysis which is accessible and affordable than the conventional diagnostic surveillance methods and the whole genome sequencing. Further evaluation and improvements on the oligonucleotide primers may offer significant contribution to the development of a specific and direct PCRbased detection of new emerging COVID-19 variants.

2.Colon adenocarcinoma presenting as splenic abscess in a young filipino female, A case report.

The spleen is a very hostile environment for tumor cells due to its anatomic location, blood supply, and rich immunological property – which makes it one of the most unique organ to be involved in metastatic diseases.15 Splenic metastases from non-hematologic malignancies are rare ranging from 0.6 to 7.1% base on autopsy reports of cancer patients, and 1.1 to 3.4% base on review of splenectomy cases.14 Moreover, isolated splenic metastases are more infrequent with only 31 cases reported from 1969 to October 2015.16 A splenic abscess is an unusual formation and is usually caused by hematogenous spread from an infection. Such expected frequency varies in different autopsy studies between 0.14% and 0.7%.1 Albeit rare, abscess can also result from migration of gut flora brought about by direct invasion of tumor cells from a neighboring neoplasm.17 This is a case of a 36-year-old female who came in with a history of abdominal pain, chills and fever for seven months. CT scan of the whole abdomen revealed splenic abscess with suspicion of a splenic rupture. The patient underwent exploratory laparotomy with abscess evacuation, splenectomy and double barrel colostomy and given with intravenous antibiotics. Histopathology results showed metastatic adenocarcinoma in the spleen. Thorough deliberation of her case was done and she was eventually managed as a case of Colon Cancer Stage IV and underwent chemotherapy. Splenic abscess developing from splenic metastasis from a colonic adenocarcinoma is rare and with concomitant high mortality rate. More often than not, splenic metastasis is discovered in advanced stage together with metastatic tumor in other organs while isolated splenic metastasis is even more uncommon. A splenic abscess as an initial demonstration of a colon cancer is not a common daily encounter of physicians hence a high index of suspicion coupled with sensitive and specific imaging is necessary in order to provide prompt medical and surgical intervention.

3.Quality of care among patients with acute heart failure at the emergency room and adherence of physicians at the University of the Philippines – Philippine General Hospital to the division of cardiovascular medicine – heart failure pathway:A retrospective cohort study.

OBJECTIVES
Clinical pathways (CPs) ensure adherence to heart failure (HF) management guidelines. To optimize quality care in a low resource setting, an evidence-based care pathway for the management of acute HF was implemented at the emergency department (ED) of the Philippine General Hospital (PGH), the designated national tertiary hospital and referral center. This study aimed to describe the characteristics of adults with acute HF admitted at the ED and evaluate the quality of care they received, measured using physician adherence to the hospital’s acute heart failure CP.
METHODSThis was a retrospective, descriptive cohort study. We reviewed the inpatient charts of all adult patients with acute HF admitted to the ED of the PGH and referred to the Division of Cardiovascular Medicine between December 1, 2022 and May 31, 2023. Quality of care was assessed based on adherence to quality indicators adapted from routine and conditional order sets detailed in the pathway. Descriptive statistics was utilized to describe patient characteristics, quality of care, and outcomes.
RESULTSTwo hundred thirty-six (236) patients were included, with a mean age of 51.8 years. Majority were male (53.4%); hypertension (61.4%) and ischemic heart disease (53.8%) were the most common comorbidities, and infection the most common precipitant of decompensation (60.6%). There were optimal adherence rates to routine orders, which included referrals to Internal Medicine and Cardiology, baseline vital signs monitoring, fluid intake and output monitoring, chest radiograph, complete blood count, blood urea nitrogen, sodium, potassium, prothrombin time, partial thromboplastin time, arterial blood gas, urinalysis, and N-terminal pro b-type natriuretic peptide. Conditional orders, such as oxygen support, focused echocardiography, thyroid - stimulating hormone, and the use of vasopressors, diuretics, and venous thromboembolism prophylactic agents, were optimally performed when warranted. However, we noted suboptimal adherence to certain resource-intensive conditional orders, such as hourly monitoring of urine output (61.4%), hooking to cardiac monitor (53.8%), and performance of 12-lead ECG within 10 minutes (56.8%). Further, only 43.9% of patients were referred to the intensive care unit. Troponin I, calcium, magnesium, and albumin were ordered in excess.
CONCLUSIONOverall adherence rate of physicians to the hospital’s Acute Heart Failure Pathway was satisfactory. Work is needed to improve adherence to hourly urine output monitoring, consistent hooking to cardiac monitor, and timely performance of 12-lead ECG – an effort that begins with expanding in-hospital diagnostic equipment and human resource supply. We recommend continuous pathway implementation with periodic evaluation and stakeholder feedback to further improve quality of care.

4.A bibliometric analysis of research productivity on Kawasaki disease in Southeast Asia: Trend and socioeconomic drivers.

OBJECTIVES
The increasing prevalence of Kawasaki disease in Southeast Asia (SEA) and its potential relation with Coronavirus Disease 2019 (COVID-19) infection resulted in heightened interest in KD in the region, thus, this paper aimed to determine the trend and the socioeconomic facilitators of scientific productivity of KD research within the region. Specifically, this article determined the number of publication and citations related to KD per country, institution, and journal. We also explored the networks of countries within the region to the rest of the world and the keywords mostly associated with KD research in the region. Lastly, correlation of these bibliometric indices with socioeconomic factors in the region was analyzed.
METHODSA literature search of KD papers in SEA was performed using Scopus database. We obtained bibliographic data from the available literature and visualized network of existing collaborations and keywords using VOSviewer software.
RESULTSA total of 196 papers were included in the study. Bibliometric analysis showed a rising trend in publication within the region, most of which were from institutions in Singapore and Thailand. The most common topics on KD studies included clinical features, complications, treatment, and comorbidities.
Country characteristics such as gross domestic product (GDP) per capita, research and development (R&D) expenditure (% GDP), and number of physician and R&D researchers were positively correlated with bibliometric indices of KD research in SEA. Moreover, number of international linkages was significantly associated with KD research productivity in the region.
CONCLUSIONIn summary, we showed an increasing trend of KD research in SEA. Funding allocation and capacity building are necessary to strengthen research productivity within the region.

5.A rare case of acute purulent pericarditis secondary to invasive streptococcal infection (S. pyogenes) with cardiac tamponade in an immunocompetent 37-year-old Female.

Purulent pericarditis is a rare occurrence in the era of modern antibiotics. It is most often caused by organisms such as Staphylococcus aureus, Streptococcus pneumoniae, Viridans streptococci, Haemophilus influenzae and anaerobic bacteria with Streptococcus pyogenes (S.pyogenes) being a possible, though very uncommon etiology. This case represents an occurrence of S. pyogenes pericarditis in an apparently healthy female with no known immunocompromising condition. A 37-year-old female, married, real estate agent with no comorbidities came in for chest pain radiating to the upper back, relieved with leaning forward. Cardiac biomarker was normal, ECG demonstrated diffuse ST-segment elevation and PR segment depression, while imaging showed lobar pneumonia. Blood tests showed leukocytosis with neutrophilic predominance and workup for immunocompromised state was negative. The 2D echo showed large pericardial effusion with tamponade physiology. An urgent pericardiocentesis was done. Cultures grew Streptococcus pyogenes confirming the diagnosis of acute purulent pericarditis. Daily drainage of pericardial effusion, colchicine, ibuprofen was initiated together with antibiotics and the patient had resolution of pericardial effusion. Acute pyogenic pericarditis with cardiac tamponade is a rare but serious condition that requires prompt recognition and intervention. Early diagnosis, through a combination of clinical suspicion, ECG and echocardiography is crucial for initiating timely treatment.

6.Current Situation and Global Perspective of Nipah Virus

Abstract
The Nipah virus (NiV) is a highly pathogenic zoonotic virus associated with recurrent outbreaks in South and Southeast Asia. Since its first identification in 1998 among the pig farmers in Sungai Nipah Village in Malaysia, NiV has demonstrated high case fatality rates (40–75%) and significant epidemic potential. At that time, the virus spread to Singapore, and human cases were also identified in India, Bangladesh, and Philippine afterwards. No human cases have been reported in Indonesia; the country remains on high alert due to geographic concerns and the intensity of mobilization of people. To review the current global epidemiological situation of Nipah virus (as of 2026), evaluate public health preparedness, and analyze ongoing research efforts, including vaccine and therapeutic development.Nipah virus outbreaks remain geographically concentrated in Bangladesh and India, with seasonal spillover events linked primarily to bat-to-human transmission through contaminated food products. Human-to-human transmission occurs but remains limited. Improved surveillance, rapid case isolation, and contact tracing have reduced outbreak sizes in recent years. Vaccine candidates are currently in Phase II clinical trials.While the global pandemic risk remains low at present, the Nipah virus continues to represent a high-consequence emerging pathogen. Sustained surveillance, vaccine development, ecological research, and strengthened health systems are critical to mitigating future risks.

7.Overwhelming Opportunistic Infections as the Initial Presentation of Severe Cushing Syndrome

Introduction:
Severe hypercortisolism is associated with profound
impairment of both innate and adaptive immune responses,
predisposing affected individuals to opportunistic infections. Excess glucocorticoids alter leukocyte trafficking,
suppress pro-inflammatory cytokine production, and
impair cellular immunity, increasing susceptibility to
bacterial, viral, and fungal pathogens. In some cases, severe
infections may precede the diagnosis of Cushing syndrome
and represent the initial clinical manifestation. Early
recognition is important as untreated hypercortisolism can
lead to substantial morbidity and mortality.
Case:
A young adolescent male presented with progressive
facial fullness and facial hyperpigmentation for 4 months,
followed by 1 month of intermittent fever, cough, lower
limb weakness, and hallucinations. On examination, he
was tachypneic with cushingoid features including moon
facies, pigmented acne over the face and chest, and nail bed
hyperpigmentation. He was hypertensive and had severe
hypokalemia with lymphopenia. Radiological imaging demonstrated multiple cavitary lung
lesions and intracranial tuberculomas. Bronchoalveolar
lavage identified multiple opportunistic pathogens,
including Pneumocystis jirovecii, Aspergillus fumigatus, and
Haemophilus influenzae, while cerebrospinal fluid testing
was positive for cytomegalovirus.
Given the unusual combination of infections, an underlying
immunocompromised state was suspected. Endocrine
evaluation revealed markedly elevated serum cortisol, with
loss of diurnal rhythm and elevated adrenocorticotropic
hormone (ACTH). Twenty-four-hour urinary cortisol
was significantly increased, confirming severe ACTHdependent Cushing syndrome. Magnetic resonance
imaging of the pituitary gland and computed tomography
imaging of the thorax, abdomen, and pelvis did not identify
the source of ACTH secretion.
Conclusion
This case highlights that overwhelming opportunistic
infections may be the first manifestation of severe
Cushing syndrome in children. Excess cortisol disrupts
host defenses by impairing neutrophil chemotaxis and
macrophage phagocytosis, suppressing T-cell-mediated
immunity, and reducing cytokine signaling necessary
for pathogen clearance. These mechanisms contribute to
susceptibility to simultaneous bacterial, fungal, and viral
infections. Clinicians should therefore consider underlying
hypercortisolism in patients presenting with multiple or
unusual opportunistic infections to enable earlier diagnosis
and appropriate multidisciplinary management.

8.Utility of performing routine screening tests of infections in the clinical management of preterm labor in a tertiary hospital in the Philippines

OBJECTIVE
Preterm birth is a major cause of complications leading to death of children under 5 years old. Infections are important to be identified because antimicrobial treatment may prevent or delay progression to preterm delivery. This study determined if routine screening tests of infections are useful in the clinical management of preterm labor.
METHODSA cross-sectional (descriptive) study was done involving 417 pregnant patients who had preterm labor and was subsequently admitted from 2015 to 2019 at a tertiary hospital in the Philippines, using review of past medical records, inpatient charts, and admission charts.
RESULTSMajority of the patients delivered at less than 34 weeks, most of the culture results turned out negative, and urine tests were more commonly employed as screening tests for preterm labor. The endocervical and rectovaginal swab studies had no significant growths. Asymptomatic bacteriuria was diagnosed in 1 out of 10 patients and they were subsequently started on antibiotic treatment. Majority of the patients who were given antibiotics delivered within 48 hours from admission.
CONCLUSIONThe routine use of cultures in the assessment of preterm labor is costly and is unlikely to be helpful in the clinical management of patients in preterm labor.

9.Association between vaccination status and the incidence of breakthrough COVID-19 infections among University of Santo Tomas – Faculty of Medicine and Surgery students: A case-control study.

INTRODUCTION
With the introduction of COVID-19 vaccines, schools around the world have slowly started to reimplement on-site classes with guidelines to prevent outbreaks. The University of Santo Tomas has devised their own set of guidelines, including safety protocols, vaccinations and daily health declarations. These were monitored using the
Thomasian Online Medical Services and Support (ThOMedSS).
Through a case-control study design, the study aims to determine an association between the vaccination status of UST Faculty of Medicine and Surgery (UST-FMS) students and breakthrough COVID-19 infections in the first semester of the academic year 2022-2023, with population data acquired via records of students participating in face-to-face classes from the UST Health Service Office, and categorized based on breakthrough infections and vaccination status.
STATISTICAL ANALYSISResults were analyzed using Pearson’s chi-square test and Fisher’s exact test (pCONCLUSION
The study suggests that vaccination status did not have a statistically significant association (p = 0.3451) with breakthrough infections. The majority (99.94%) of students have received the complete primary COVID-19 vaccination series and only 6.61% of this population developed breakthrough COVID-19 infections, all occurring in those completely vaccinated. Breakthrough infections were 1.76 times more likely for those with booster shots than those without. The possible reason for this is the emergence of the Omicron variant. To improve the study, external exposures and individual behaviors must be considered as potential factors influencing infection rates.

10.Role of antibiotic delivery system targeting bacterial biofilm based on ε-poly- L-lysine and cyclodextrin in treatment of bone and joint infections.

OBJECTIVE:
To explore the mechanism of antibiotic delivery system targeting bacterial biofilm with linezolid (LZD) based on ε-poly- L-lysine (ε-PLL) and cyclodextrin (CD) (ε-PLL-CD-LZD), aiming to enhance antibiotic bioavailability, effectively penetrate and disrupt biofilm structures, and thereby improve the treatment of bone and joint infections.
METHODS:
ε-PLL-CD-LZD was synthesized via chemical methods. The grafting rate of CD was characterized using nuclear magnetic resonance. In vitro biocompatibility was evaluated through live/dead cell staining after co-culturing with mouse embryonic osteoblast precursor cells (MC3T3-E1), human umbilical vein endothelial cells, and mouse embryonic fibroblast cells (3T3-L1). The biofilm-enrichment capacity of ε-PLL-CD-LZD was assessed using Staphylococcus aureus biofilms through enrichment studies. Its biofilm eradication efficacy was investigated via minimum inhibitory concentration (MIC) determination, scanning electron microscopy, and live/dead bacterial staining. A bone and joint infection model in male Sprague-Dawley rats was established to validate the antibacterial effects of ε-PLL-CD-LZD.
RESULTS:
In ε-PLL-CD-LZD, the average grafting rate of CD reached 9.88%. The cell viability exceeded 90% after co-culturing with three types cells. The strong biofilm enrichment capability was observed with a MIC of 2 mg/L. Scanning electron microscopy observations revealed the effective disruption of biofilm structure, indicating potent biofilm eradication capacity. In vivo rat experiments demonstrated that ε-PLL-CD-LZD significantly reduced bacterial load and infection positivity rate at the lesion site ( P<0.05).
CONCLUSION
The ε-PLL-CD antibiotic delivery system provides a treatment strategy for bone and joint infections with high clinical translational significance. By effectively enhancing antibiotic bioavailability, penetrating, and disrupting biofilms, it demonstrated significant anti-infection effects in animal models.
