1.The relationship between fibrinogen to albumin ratio,PNI,SII index and prognosis of patients with lung cancer undergoing thoracoscopic surgery

Objective To explore the relationship between fibrinogen to albumin ratio(FAR),prognostic nutritional index(PNI),systemic immune inflammation index(SII)and prognosis of patients with lung cancer undergoing thoracoscopic surgery.Methods The clinical data of 287 patients with lung cancer who underwent thoracoscopic lobectomy in the hospital from February 2018 to February 2020 were retrospectively collected.The FAR,PNI,and SII values were measured within one week before surgery.The 3-year survival after operation was counted.The relationship between clinical data,preoperative FAR,PNI,SII and 3-year survival prognosis was observed.The receiver operating characteristic curve(ROC)was used to determine the optimal cut-off values of preoperative FAR,PNI,and SII values for predicting the 3-year survival prognosis after surgery,and the prognostic value of single index and combination of each index was observed.Multivariate Logistic regression analysis was used to evaluate the influencing factors of prognosis.Pearson correlation analysis was used to explore the correlation between FAR,PNI and SII values.Results Finally,242 patients with lung cancer were included in the study,and 199 patients survived 3 years after operation,with an overall survival rate of 82.23%.43 cases(17.77%)died in 3 years.Age,serum carcinoembryonic antigen(CEA)level,lymph node metastasis,TNM stage,preoperative FAR,PNI and SII value were related to the 3-year survival prognosis of lung cancer patients after thoracoscopic lobectomy(P<0.05).The ROC curve showed that the preoperative FAR,PNI and SII values had certain predictive value for the 3-year survival prognosis of patients after operation.The combined prediction of the three showed better predictive efficacy,and the area under the curve(AUC)was 0.772.Multivariate Logistic regression analysis showed that lymph node metastasis,TNM stage,FAR value and SII value were independent risk factors for postoperative survival and prognosis.PNI was an independent protective factor(P<0.05).Pearson correlation analysis showed that there was a negative correlation between FAR and PNI,PNI and SII,and a positive correlation between FAR and SII(P<0.05).Conclusion The values of FAR,PNI and SII are independently related to the prognosis of patients with lung cancer undergoing thoracoscopic surgery.The combined application has a higher prognostic value.

Objective To explore the neuroelectrophysiological mechanism of the decline of overall cognitive function and prospective memory,the syndrome manifestation of liver failing to govern conveyance and dispersion of the middle-aged and elderly people in the community with long-term emotional disturbance were evaluated.Methods Eysenck personality questionnaire(EPQ)and general condition questionnaire were used to screen the middle-aged and elderly people from 46 to 65 years old in the community who met the high and low neuroticism criteria,which were included in the long-term emotional disturbance group and the control group.According to the principle of matching sex,age(±2 years)and years of education(±2 years),60 subjects in each group were included.The scores of livers meridian syndrome,anxiety factor,depression factor and the total scores of three factors in the two groups were detected with the emotion rating scale for Ganzangxiang of traditional Chinese medicine.The overall cognitive function,the overall level of prospective memory and the dual-task paradigm of event-related memory were evaluated and event-related potential components were detected.Results Compared to the control group,the total scores of emotion rating scale for Ganzangxiang of traditional Chinese medicine and the scores of liver meridian syndrome factor,anxiety factor and depression factor all increased in the long-term emotional disturbance group significantly(P<0.05).The total scores of MoCA scale(after correction)and the level scores of visual space and executive function,attention and delayed recall decreased significantly(P<0.05),and the total scores of prospective memory questionnaire decreased significantly(P<0.05).The correct rates of ongoing tasks and prospective memory tasks in the event-based prospective memory dual task paradigm were decreased,and the reaction time was prolonged in the long-term emotional disorder group significantly(P<0.05).The amplitude of P200 of ongoing task and prospective memory task were increased in the long-term emotional disorder group(0.05

Rapid brain aging has become a social and medical problem to be solved urgently,among which amnesia mild cognitive impairment(aMCI)has attracted much attention because it is more likely to develop into Alzheimer's disease(AD)dementia.Many studies at home and abroad have reported that executive function decline is the initiating factor,key damage and sensitive index of cognitive aging,and long-term negative emotional accumulation and liver failing to govern conveyance and dispersion is an important risk factor for aMCI.Soothing liver-qi stagnation therapy can improve the overall cognitive function of aMCI patients,but the above mechanisms need to be further studied and explained.This paper focuses on the cognitive flexibility level of early sensitive components of cognitive decline and one of the core components of executive function,and summarizes the pathogenesis,risk factors,sensitive indicators,event-related potentials and eye tracking techniques of aMCI,and focuses on the effects and research progress of long-term negative emotional accumulation and liver failing to govern conveyance and dispersion on aMCI and cognitive flexibility.The purpose of this study is to provide reference and reference for the intervention and treatment of emotional pathogenesis in patients with aMCI,and to provide new perspectives,methods and ideas for the prevention and treatment of diseases with excessive cognitive decline in traditional Chinese medicine.

6.Analysis of synovial fluid Asporin levels in patients with temporomandibular joint disorders

Objective To investigate the changes and clinical significance of synovial fluid Asporin level in patients with tem-poromandibular joint disorders(TMD).Methods A total of 48 TMD patients who were treated in our hospital from January 2021 to December 2023 due to irritant pain and mouth opening limitation were randomly selected as the observation group,and 48 healthy vol-unteers were selected as the control group.The synovial fluid Asporin levels of the two groups were detected by enzyme-linked immu-nosorbent assay(ELISA).The difference of synovial fluid Asporin levels between the two groups was compared.The correlation be-tween the synovial fluid Asporin levels and the clinical symptoms of TMD was analyzed.Results The synovial fluid Asporin level in the experimental group was significantly higher than that in the control group(P<0.05).The synovial fluid Asporin level was positively correlated with the pain degree of TMD patients(Rs=0.825,P<0.001),negatively correlated with the degree of mouth opening(Rs=-0.945,P<0.001).Conclusion The level of Asporin in synovial fluid of TMD patients was significantly increased.The level of As-porin in synovial fluid of TMD patients is correlated with the clinical symptoms of TMD,which provides a basis for the diagnosis and evaluation of TMD.
7.The Acceleration of Spatial Navigation Aging Process by Liver Failing to Convey and Disperse and Its Neuroelectrophysiological Mechanisms

Objective To investigate the influence of the liver failing to convey and disperse on space navigation aging.Methods High and low neuroticism subjects screened by Eysenck personality questionnaire were included in liver failing to convey and disperse group and liver controlling conveyance and dispersion group,respectively.The two groups were then divided into youth and elderly groups based on age.Finally,spatial navigation task was conducted to record and analyze behavioral(reaction time and accuracy)and EEG data(amplitude and latency of P2 and N2 components)of all the four groups(30 subjects each group).Results Compared with liver controlling conveyance and dispersion group,the accuracy in subjects with liver failing to convey and disperse decreased significantly(P<0.001)and reaction time was prolonged significantly(P<0.05).The interaction effect between age and liver regulation status showed a marginal significant difference(P=0.078).The accuracy of elderly people was lower than that of youth people(P=0.002)for liver failing to convey and disperse subjects,while there was no significant difference between the two groups for subjects with liver controlling conveyance and dispersion.The P2 amplitude in the elderly group was significantly smaller than that in the youth group(P=0.027).The amplitude of P2 in group of liver failing to convey and disperse was significantly smaller than that in group of liver controlling conveyance and dispersion(P=0.042).The interaction effect of P2 amplitude between age and liver regulation status showed a marginal significance(P=0.073).For youth subjects,the P2 amplitude in group of liver controlling conveyance and dispersion was significantly larger than that in group of liver failing to convey and disperse(P=0.007),while there was no significant difference in P2 amplitude between the two groups for the elderly subjects.The N2 amplitude for tasks of allocentric frames of reference was significantly greater than that of egocentric frames of reference(P=0.024).Conclusion Liver failing to convey and disperse caused by long-term emotional disturbance accelerates spatial navigation aging,and selective attention feature inhibition disorder may be the underlying ERPs neuroelectrophysiological mechanism.

Objective To investigate the influence of the and mechanism of liver failing to convey and disperse on age-related changes in attentional search based on ERPs.Methods oddball attention search task was administrated to record and analyze behavioral and EEG data(N2pc、SPCN、N2pc-Ptc components)of 120 subjects.Results Compared with liver controlling conveyance and dispersion group,the accuracy in subjects with liver failing to convey and disperse decreased significantly(P<0.05).The elderly group had a lower accuracy(P<0.001)and a longer reaction time(P<0.001)compared to the young group.The N2pc amplitude in subjects with liver failing to convey and disperse was significantly greater than that in subjects with liver controlling conveyance and dispersion(P<0.05).The interaction effect of SPCN amplitude between age and liver failing to convey and disperse status was significant(P=0.024).And in the elderly group,SPCN amplitude in subjects with liver dysregulation was significantly smaller than that of liver controlling conveyance and dispersion(P=0.042).The N2pc-Ptc peak to peak amplitude interaction effect between age and liver regulation status was marginal significant(P=0.087),and in liver failing to convey and disperse group,N2pc-Ptc peak to peak amplitude of the elderly was significantly smaller than that of the young(P=0.008).Conclusion Attention search ability is impaired in the elderly with liver failing to convey and disperse,and the electrophysiological abnormalities,such as directed attention allocation,spatiotemporal dynamic cohesion and short-term memory maintenance,may be part of the mechanism.

The concept of "spirit" in traditional Chinese medicine (TCM) aligns closely with "the liver governs the designing of strategy". By exploring the relationship between the liver and executive function decline, it is proposed that prolonged liver constraint leads to indecisiveness in strategy designing, which is the initiating factor for executive function decline; liver blood deficiency causes difficulties in executing strategy, which forms an essential foundation for the progression of executive function decline; obstruction in the "liver-du mai-brain" pathway leads to unclear strategy designing, which accelerates executive function decline. This relationship is examined from the perspectives of TCM, modern medicine, and cognitive psychology, aiming to provide insights into addressing executive function decline through treatments focused on the liver.
10.Patient-reported health status vs . N-terminal pro-B-type natriuretic peptide levels in patients with acute heart failure.

BACKGROUND:
Changes in N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels may not fully translate into patient-reported health status in patients with heart failure (HF). We aimed to evaluate the correlation between NT-proBNP levels and patient-reported health status changes at one month after discharge of patients, and their associations with risk of death and rehospitalization in patients with acute HF.
METHODS:
We used data from the China Patient-centered Evaluative Assessment of Cardiac Events Prospective Heart Failure Study (PEACE 5p-HF Study). Patient-reported health status was measured by the 12-item Kansas City Cardiomyopathy Questionnaire (KCCQ-12). Patients who were hospitalized for HF and completed the KCCQ-12 and NT-proBNP tests before and one month after discharge were eligible in our study. We stratified patients into different groups based on NT-proBNP levels (i.e., improved, stable, and deteriorated) and KCCQ-12 scores (i.e., not deteriorated and deteriorated). We also examined the associations of the joint NT-proBNP and KCCQ-12 change with the risk of one-year and four-year clinical outcomes.
RESULTS:
A total of 2461 patients were included in the analysis. The mean age was 64.06 ± 13.51 years, and 36.37% (895/2461) of the study population were female. Among patients with improved NT-proBNP levels, 115 (10.95%) patients had deteriorated KCCQ-12 scores. The correlation between the change in the KCCQ-12 score and NT-proBNP level was weak ( r2 = 0.002, P = 0.013). Stratification by changes in the KCCQ-12 score revealed subgroups with distinctive risks, such that patients with deteriorated KCCQ-12 scores in any of the NT-proBNP change groups exhibited an increased risk of one-year all-cause death than participants with not deteriorated KCCQ-12 scores in any of the NT-proBNP change groups. Patients with improved NT-proBNP levels and deteriorated KCCQ-12 scores presented greater risks of one-year all-cause death (hazard ratio [HR]: 2.45, 95% confidence interval [CI]: 1.34-4.48) than patients with stable NT-proBNP levels and not deteriorated KCCQ-12 scores (HR [95% CI], 1.77 [1.25-2.53]).
CONCLUSIONS:
A discrepancy between changes in NT-proBNP levels and KCCQ-12 scores was common. The change in NT-proBNP levels was not sufficient to characterize critical aspects related to HF during one month after discharge of patients. Changes in the KCCQ-12 score exhibit complementary information to NT-proBNP levels for the prediction of clinical outcomes in patients with acute HF.
