The Expert Consensus on Clinical Application of Qinbaohong Zhike Oral Liquid in Treatment of Acute Bronchitis and Acute Attack of Chronic Bronchitis (GS/CACM 337-2023) was released by the China Association of Chinese Medicine on December 13th, 2023. This expert consensus was developed by experts in methodology, pharmacy, and Chinese medicine in strict accordance with the development requirements of the China Association of Chinese Medicine (CACM) and based on the latest medical evidence and the clinical medication experience of well-known experts in the fields of respiratory medicine (pulmonary diseases) and pediatrics. This expert consensus defines the application of Qinbaohong Zhike oral liquid in the treatment of cough and excessive sputum caused by phlegm-heat obstructing lung, acute bronchitis, and acute attack of chronic bronchitis from the aspects of applicable populations, efficacy evaluation, usage, dosage, drug combination, and safety. It is expected to guide the rational drug use in medical and health institutions, give full play to the unique value of Qinbaohong Zhike oral liquid, and vigorously promote the inheritance and innovation of Chinese patent medicines.
2.Application of a new type of dermatologic surgical mold in standardized residency training

Objective:To explore the application effect of a new type of simulated dermatologic surgical mold based on 3D printing in standardized residency training of dermatology.Methods:In this study, 35 residents in Department of Dermatology, The Second Affiliated Hospital of Guangzhou Medical University from January 2018 to May 2019 were randomly divided into two groups: a new mold group based on 3D printing ( n=18) and a traditional mold group ( n=17). Two groups of residents were examined by dermatologic biopsy on the mold, and the effect of teaching was evaluated by questionnaire survey. SPSS 22.0 was used for data analysis, and the measurement data was expressed by (means ± standard deviation). The comparison between the new mold group and the traditional mold group was made by two independent samples t test. Results:The scores of dermatologic biopsy examination of residents in the new mold group were higher than those in the traditional mold group, and the differences were statistically significant ( P<0.01); and the scores of the questionnaires for the teaching effect of the new mold group were higher than those of the traditional mold group, and the differences were statistically significant ( P<0.01). Conclusion:The new type of simulated dermatologic surgical mold based on 3D printing, which has been successfully applied in the practical training of residents, lays a solid foundation for the actual clinical operation and significantly improves the teaching effect, providing a broad application prospect.
3.Preliminary study on metabolites of polyunsaturated fatty acids in seminal plasma of Guizhou Province population

Objective:To investigate the differences in polyunsaturated fatty acid metabolites in seminal plasma between normal group and low sperm motility group, and their relationship with sperm motility.Methods:Totally 581 human seminal plasmas collected from Guizhou Province were divided into normal group and low sperm motility group according to the motility rate. The metabolite products of 24 kinds of polyunsaturated fatty acids were detected by liquid phase tandem mass spectrometry/mass spectrometry (LC-MS/MS). Through the in vitro culture experiment of sperm, the culture supernatant was collected during the culture of the upstream screening sperm for 24 h, and the polyunsaturated fatty acid products with different differences detected in the different groups of seminal plasma were also verified by LC-MS/MS method. Results:Among the polyunsaturated fatty acid metabolites detected in seminal plasma, the concentrations of 9-hydroxyoctadecadienoic acid (9-HODE) and 13-hydroxyoctadecadienoic acid (13-HODE) in the seminal plasma of the low sperm motility group were significantly different from those in the normal seminal plasma group ( P=0.001, P=0.033). In vitro culture experiments showed that the concentration of 9-HODE and 13-HODE in the culture medium increased significantly with the prolongation of culture time and the decrease of sperm motility rate ( P=0.003, P=0.035). Statistical analysis showed that the concentrations of 9-HODE and 13-HODE were significantly negatively correlated with sperm motility ( r=-0.91, P=0.045; r=-0.95, P=0.026). Considering the sperm density factor in the analysis of polyunsaturated fatty acid metabolites and sperm motility in the seminal plasma of the population, the results showed that 9-HODE and 13-HODE were significantly higher in the low sperm motility group than in the normal group ( P<0.000 1). And there was a negative correlation ( r=-0.38) with the sperm motility rate ( P<0.000 1). Conclusion:9-HODE and 13-HODE are released into the culture medium and seminal plasma by sperm cell metabolism, which is a potential indicator of the degree of sperm membrane damage, which may be helpful in judging and detecting the asthenospermia.

5.Efficacy meta-analysis of laparoscope-assisted transanal total mesorectal excision and conventional laparoscopic excision for rectal cancer.

OBJECTIVETo compare the short-term efficacy of laparoscope-assisted transanal total mesorectal excision (LA-taTME) and conventional laparoscopic TME (LTME) for rectal cancer by meta-analysis.
METHODSClinical studies that compared clinical outcomes of LA-taTME and LTME were searched from form PubMed, Embase, Ovid, CNKI and Wanfang database before January 2016. Two reviewers independently screened the articles and assessed the quality of the included studies by using the MINORS standard which involves 12 items. The score is 0-2 for each item and the maximum score is 24, and the ideal global score should be above16. RevMan 5.3 software was used for meta-analysis and outcome measures included operation time, hospital stay, number of harvested lymph node, rate of conversion, positive rate of circumferential resection margin and the rate of incomplete mesorectum.
RESULTSSeven studies were included in the analysis, and the score of all the studies was more than 16 points. A total of 479 patients (208 in LA-taTME, 271 in LTME) were enrolled. There were no significant differences in terms of age, sex, tumor location and clinical stage between two groups (all P>0.05). Results of meta-analysis showed that LA-taTME had lower rate of incomplete mesorectum (OR=0.29, 95% CI:0.10 to 0.84, P=0.02), lower rate of complications (OR=0.59, 95% CI:0.35 to 0.97, P=0.04) and shorter hospital stay (MD=-1.66, 95% CI:-3.22 to -0.11, P=0.04) than those of LTME, with significant differences. In terms of operation time (MD=-14.49, 95% CI:-37.87 to 8.90, P=0.22), number of harvested lymph node (MD=-0.45, 95% CI:-1.98 to 1.08, P=0.56), the rate of conversion (OR=0.31, 95% CI:0.08 to 1.24, P=0.10) and positive rate of circumferential resection margin (OR=0.43, 95% CI:0.17 to 1.04, P=0.06), there were no significant differences between two groups.
CONCLUSIONCompared to LTME, LA-taTME has similar short-term efficacy for rectal cancer, but it can reduce the rate of complications and rate of incomplete mesorectum.

6.Meta-analysis of the efficacies of laparoscopic intersphincteric resection and laparotomy for ultra-low rectal cancer

Objective To compare the short-term efficacies of laparoscopic intersphincteric resection (ISR) and laparotomy for ultra-low rectal cancers by Meta-analysis.Methods We searched case-control trials that compared clinical outcomes of laparoscopic ISR and laparotomy from PubMed, EMBase, Ovid, CNKI and Wanfang database.Relevant published and unpublished data and conference papers were also retrieved.Two reviewers independently assessed the qualities of the included studies.Meta-analysis was performed by using of RevMan5.2 software.Results A total of 5 trials with 552 cases were included.The results of Meta-analysis showed that in terms of blood loss of the operation [mean difference (MD) =-65.42, 95% CI:-93.45--37.38, Z=4.57, P<0.000 01], flatus passage time (MD=-0.96, 95%CI:-1.45--0.47, Z=3.83, P=0.000 1) and hospital stays (MD=-1.69,95%CI:-2.19--1.19, Z=6.63, P<0.00001),laparoscopic ISR were significantly superior than those of laparotomy, with significant differences.In terms of operation time (MD =6.61,95 % CI:-21.29-34.51, Z =0.46, P =0.64), the positive rate of circumferential resection margin (OR =1.01, 95% CI: 0.37-2.80, Z =0.02, P =0.98) and postoperative morbidity (0R=0.73, 95% CI: 0.45-1.20, Z =1.23, P =0.22), there were no significant differences in the two groups.However, laparotomy may clean more numbers of lymph nodes than those of laparoscopic ISR (MD =-1.16, 95%CI:-2.14--0.18, Z =2.31, P =0.02), with significant difference.Conclusion The shortterm efficacy of laparoscopic ISR is superior than that of laparotomy in the treatment of ultra-low rectal cancer.

METHODSBSA-loaded genipin-crosslinked SF/CS microspheres were prepared by emulsion cross-linking technique. The micropheres were observed for surface morphology and size distribution under scanning electron microscope (SEM), and X-ray diffractometry (XRD) and fourier transform infrared spectroscopy (FTIR) were used to analyze their structural characteristics. BCA method was used for determining the drug entrapment, loading rate and cumulative drug release in 21 days.
RESULTThe microspheres were spherical and showed a smooth surface with an average diameter of 7.84∓0.97 µm. The drug entrapment efficiency of the microspheres was (50.16∓4.32)% with a drug loading ratio of (1.25∓0.11)% and a cumulative release of the total drug of (75.2∓2.53)% in 21 days.
CONCLUSIONGenipin-crosslinked SF/CS microspheres have a high drug entrapment efficiency and possess good capacity of sustained drug release.

10.Analysis of clinical and pathological features of familial gastric cancer

Objective:To explore the clinical and pathological features of familial gastric cancer, and to get the early discovery and early treatment of it.Methods:Two kindreds of familial gastric cancer were followed up and their clinical and pathological features were analyzed.Results:Six patients with gastric cancer were found in the 2 kindreds.Autosomal dominant inheritance was observed in these cases.clinical and pathological features of familial gastric cancer were showed according to the document analysis:early onset;poor prognosis;patients suffer simultaneous or metachronous carcinoma;CDH1 germline mutation carriers had higher penetrance;pathologically, tumors are mostly diffuse infiltrative type with lower differentiated degree and earlier metastasis to lymphnodes;in one kindred,the sites of the lesions were relatively consistent.Conclusion:Familial gastric cancer has particular clinical and pathological features:early onset;poor prognosis;patients suffer simultaneous or metachronous carcinoma;CDH1 germline mutation carriers have higher penetrance; pathologically,tumors are mostly diffuse infiltrative type with lower differentiated degree and earlier metastasis to lymphnodes.
