OBJECTIVES:
To investigate the effect of Qianggu Kangshu Formula (QGKSF) for alleviating osteoclast differentiation in rheumatoid arthritis and the underlying mechanism.
METHODS:
RAW264.7 cells cultured under hypoxic conditions were treated with RANKL to induce osteoclast differentiation and incubated with normal rat serum or sera from rats medicated with methotrexate (MTX) or QGKSF at low and high doses. Cell viability, TRAP-positive multinucleated cells and F-actin ring formation in the treated cells were assessed with CCK-8 assay, TRAP staining and Phalloidin staining, respectively. Autophagy and autophagosomes in the cells were observed with MDC staining and transmission electron microscopy. ELISA was used to measure IL-6 and TNF-α levels in the culture supernatant, and the expressions of HIF-1α, BNIP3, Bcl-2, Beclin1, LC3-I, LC3-II, P62 and TRAP mRNAs and proteins were analyzed using RT-qPCR and Western blotting.
RESULTS:
In hypoxia- and RANKL-induced RAW264.7 cells treated with normal rat serum, significant increments of TRAP-positive cells and F-actin ring formation were observed with an enhanced autophagic fluorescence intensity and increased autophagosomes. Treatment of the induced cells with rat sera medicated with MTX and low- and high-dose QGKSF obviously reduced the TRAP-positive cells, F-actin rings and autophagosomes as well as the autophagic fluorescence intensity. RANKL treatment significantly increased IL-6 and TNF-α levels in RAW264.7 cells, which were obviously decreased by treatment with MTX- and QGKSF-medicated sera. RANKL also significantly increased the mRNA and protein expression levels of HIF-1α, BNIP3, Bcl-2, Beclin1, LC3 and TRAP and lowered P62 expressions, and these changes were effectively reversed by treatment with MTX- and QGKSF-medicated sera.
CONCLUSIONS
QGKSF attenuates RANKL-induced osteoclast differentiation in hypoxic RAW264.7 cells by inhibiting the HIF-1α/BNIP3 autophagy signaling pathway, suggesting its potential for treatment of bone destruction in rheumatoid arthritis.

Objective:To assess the efficacy and safety of pain control nursing for burn patients. Methods:Databases including CNKI, Wanfang Data Knowledge Service Platform, Weipu Database, China Biomedical Literature Database, Web of Science and Cochrane Library were searched to collect the literature on pain control nursing of burn patients from their inception to December 31, 2024. Literature screening, data extraction and quality assessment were performed independently by 2 researchers based on inclusion and exclusion criteria. Meta-analysis of the included studies was performed by applying RevMan 5.4 software. The efficacy of pain control care was assessed in terms of improving care satisfaction, alleviating negative emotions, reducing pain, and decreasing infection rate. Publication bias was analyzed on the related studies. Results:A total of 9 papers were included, all of which were randomized controlled trial (RCT) studies, involving 791 burn patients, with 392 in the pain control nursing group and 399 in the conventional care group. Meta-analysis showed that compared with the conventional care, pain control nursing significantly increased satisfaction of care ( OR=0.28, 95% CI 0.22, 0.34), improved negative emotions ( SMD=-3.06, 95% CI -4.65, -1.47), reduced pain ( SMD=-2.49, 95% CI -3.41, -1.58), and effectively reduced the incidence of infection ( OR=0.13, 95% CI 0.05, 0.34). The studies related to negative emotions were at the risk of publication bias. Conclusion:Compared with conventional care, pain control nursing can significantly increase care satisfaction, alleviate negative emotions, reduce pain, and lower infection risk in burn patients.

Objective:To understand the current status of scientific and technological personnel cognition regarding patent transformation and utilization in hospitals, explore the implementation effectiveness and existing problems of hospital patent operation management measures, and thereby identify key areas and provide targeted guidance for subsequent improvement efforts. Methods:A questionnaire survey was conducted among scientific and technological personnel at a tertiary public hospital in Shanghai. The survey assessed their patent knowledge, investigated their training needs related to patent transformation and utilization, and evaluated the perceived importance and satisfaction with the hospital′s patent operation management measures. Descriptive statistics, ANOVA, t-tests, and Importance-Performance Analysis (IPA) were used to analyze the personnel's improvement demands regarding patent operation. Results:A total of 261 scientific and technological personnel were included. Their overall patent knowledge assessment score was relatively low (46.86±15.52), and their training topic needs were dispersed. The mean scores for both importance (4.13±0.74) and satisfaction (3.90±0.80) regarding the 11 hospital patent operation management measures were above the midpoint. IPA results indicated that: four measures, including ″improving patent transformation/utilization regulations″ and ″implementing salary rewards for achievements transformation, ″ should be maintained (high importance, high satisfaction); two measures, including ″establishing special funds for achievements transformation″ and ″introducing professional service agencies″, require concentrated improvement (high importance, low satisfaction); three measures, including ″building medical-industry-academia-research collaboration platforms″ and ″implementing tiered and classified training″, could be opportunistically optimized (low importance, high satisfaction); two measures, including ″integrating achievements transformation into hospital priorities″ and ″linking patent transformation to performance evaluation and professional promotion″, showed no priority for improvement (low importance, low satisfaction). Conclusions:Effective hospital patent operation management necessitates establishing a robust organizational and institutional framework, cultivating scientific talent with a transformation-oriented mindset, optimizing resource inputs such as funding, technology, information, and services, and actively exploring new paradigms for medical-industry-academia-research collaboration.

Objective:To investigate the association between different sleep patterns and anxiety and depression in hemodialysis patients, thereby providing a reference for improving their psychological and sleep conditions. Methods:This study was a cross-sectional survey.A convenience sampling method was used to select patients undergoing regular dialysis at a hemodialysis centre in the Sixth People's Hospital of Huizhou from May 2023 to May 2024. Sleep quality, anxiety and depression symptoms were assessed using the Pittsburgh Sleep Quality Index (PSQI), Self-rating Anxiety Scale for (SAS) and Self-rating Depression Scale (SDS). Potential profiles were analyzed using Mplus 8.3 and mixed-effects Logistic regression was used to explore the association between sleep pattern category and anxiety-depression.Results:A total of 264 valid questionnaires were returned, of which 142 were males and 122 were females, aged (56.61 ± 12.69) years old. The sleep patterns of hemodialysis patients were divided into three potential categories:patients with overall better sleep quality31.4%(83/264), patients with poor sleep using hypnotic medication12.9%(34/264), and patients with poor sleep without hypnotic medication 55.7%(147/264). Significant differences were found in age, education, anxiety and depression across different sleep categories ( χ2 values were 9.75-25.72, all P<0.05). Compared with the group with overall better sleep quality, the risk of anxiety was higher in the group with sleep difficulties without hypnotic medication ( OR=5.409, P<0.05), and the risk of anxiety and depression ( OR=5.010, 6.488, both P<0.05) was higher in the group with sleep disorders using hypnotic medication. Compared with patients with poor sleep without hypnotic medication, patients with poor sleep using hypnotic medication had a higher risk of depression ( OR=6.501, P<0.05). Conclusions:There are three potential categories of sleep patterns in hemodialysis patients and significant correlations between them and anxiety and depression, and precise screening and individualized interventions need to be implemented in the clinic to improve patients' quality of life.

Journal of Traditional Chinese Medicine 2024;65(17):1753-1756
This article summarized the clinical experience of MEI Guoqiang in treating lung cancer of phlegm-heat obstructed in the lung syndrome with modified Xiaoxianxiong Decoction (小陷胸汤). It is believed that the key pathogenesis of lung cancer with phlegm-heat obstructed in the lung syndrome is the phlegm-heat toxin accumulation. According to the different pathogenic effects of qi stagnation, blood stasis, pathogenic toxin, phlegm-damp, qi deficiency, yin deficiency in the occurrence and development of the disease, it is advocated to clear heat and resolve phlegm, and additionally the methods of diffusing the lung and relieving cough, resolving toxins and dissipating masses, rectifying qi and activating blood, dispelling dampness, supplementing and boosting qi and yin are used if necessary. Multiple methods are used together and flexibly matched. In clinical practice, Xiaoxian-xiong Decoction with the function of clearing heat and relieving phlegm is recommended as the basic formula for further modification. For patients with mild lung symptoms, modified Xiaoxianxiong Decoction is commonly used, while for those with obvious symptoms, self-made Maxing Xianxiong Decoction(麻杏陷胸汤) in modifications is suggested. For patients with Shaoyang (少阳) diseases, modified Chaihu Xianxiong Decoction (柴胡陷胸汤) is often used.

Chinese Journal of Digestive Endoscopy 2022;39(9):714-718
Objective:To investigate the correlation of endoscopy findings with symptoms in patients undergoing gastroscopy. Methods:Patients who underwent gastroscopy for the first time in Renji Hospital of Shanghai Jiao Tong University School of Medicine from January to December 2017 were included in the questionnaire survey. The participants were asked to report their main symptoms of the digestive tract in details, and gastroscopy was completed within 2 weeks. Final gastroscopic diagnosis was made based on both gastroscopy and pathology, then patients were divided into the major-lesion (peptic ulcer and malignancy) group and the non-major-lesion (chronic gastritis, reflux esophagitis and others) group. The correlation of gastrointestinal symptoms with gastroscopic findings was analyzed. The risk for major gastroscopic lesions (peptic ulcer and malignant tumors) was assessed by multivariate Logistic regression analysis. Results:A total of 5 885 patients completed the questionnaire, 5 496 (93.4%) of whom completed gastroscopy. The detection rates of peptic ulcer were 12.3% (373/3 028), 12.6% (52/412), 17.9% (49/273), 9.5% (64/675) and 9.2% (102/1 108) in patients with dyspepsia, reflux, alarm, other symptoms and no symptoms, respectively, the detection rates of malignant tumors were 1.2% (36/3 028), 0.7% (3/412), 7.7% (21/273), 0.7% (5/675) and 0.4% (4/1 108), respectively in these patients. The most common symptoms was dyspepsia, accounted for 58.3% (373/640) and 52.2% (36/69) patients with peptic ulcer and malignant tumors, respectively. Alarm symptoms were found in 30.4% (21/69) patients with malignant tumors, and 15.9% (102/640) peptic ulcer patients and 5.8% (4/69) malignant tumor patients had no gastrointestinal symptoms. Compared to asymptomatic individuals, patients with dyspepsia ( P<0.001, OR=1.52, 95% CI: 1.21-1.92) and those with warning symptoms ( P<0.001, OR=2.87, 95% CI: 2.02-4.08) had significantly increased risk for major lesions (peptic ulcer and malignant tumors) detected by gastroscopy. Conclusion:Although dyspepsia and alarm symptoms are positively associated with upper gastrointestinal malignancy and peptic ulcer, they are of limited predictive value for upper gastrointestinal diseases.
