1.Impact of rigid container material type and loading volume on the sterilization of thoracoscopic instruments

ObjectiveTo explore the impacts of material type and loading volume of rigid containers on the hydrogen peroxide low temperature plasma sterilization of thoracoscopic instruments, to identify the best rigid containers and loading volume of thoracoscopic instruments. MethodsThoracoscopic instruments sterilized by STERRAD® 100NX hydrogen peroxide low temperature plasma in Shanghai Pulmonary Hospital affiliated to Tongji University from August to September 2024 were selected as the research items. According to the material of rigid containers, the instruments were divided into polyethylene case group (A), stainless steel case group (B) and silicone resin case group (C). In terms of the loading volume, the rigid containers were divided into (loading capacity <80%) groups of 8, 10 and 12 instruments. The results of physical monitoring, the first type of chemical indicator card monitoring, and the five types of card luminal chemical process challenge device (PCD) monitoring of the 9 groups of A8, A10, A12, B8, B10, B12, C8, C10 and C12 were compared and evaluated. ResultsCompared to A8, A10 A12, C8, C10 or C12 groups, the thoracoscope instruments in the stainless steel containers in B8, B10 or B12 group had higher hydrogen peroxide concentrations and shorter elapsed time in the pressure check phases 1 and phases 2, with the differences statistically significant (P<0.05), followed by the silicone resin case group and the polyethylene case group. The nine groups of physical parameter monitoring, the first type of chemical indicator monitoring, and the five types of chemical PCD monitoring for lumen sterilization achieved 100% qualification rates, and there were no significant differences in the qualified rates of sterilization among the 9 groups (P>0.05). ConclusionWhen using hydrogen peroxide low temperature plasma to sterilize thoracoscopic instruments, it is recommended to use stainless steel or silicone resin rigid containers with a controlled loading capacity (≤12) to ensure optimal sterilization quality.

The patient was diagnosed with Angiostrongyliasis depended on the clinical presentation, epidemiological history, and etiological results. The patient recovered after receiving albendazole anthelmintic and dexamethasone anti-inflammatory therapy. This article incorporates literature review to sort out the diagnosis and treatment of this patient, in order to provide feasible reference for clinicians.

4.A study on the association between insulin resistance and genome-wide DNA methylation based on Shanghai monozygotic twins

Objective:To explore the association between insulin resistance (IR) and genome-wide DNA methylation based on Shanghai twin study.Methods:Monozygotic twins (MZ) from Shanghai were recruited during 2012-2013, 2017-2018, and 2022-2023. Data were collected by questionnaire survey, physical examination and laboratory tests. Genome-wide DNA methylation was quantified. Generalized linear mixed effect model was applied to analyze the association between methylation level at each site and homeostatic model assessment 2-insulin resistance (HOMA2-IR). Non-paired and paired designs were used to assess the association between DNA methylation and phenotype of IR. Cluster analysis was conducted to identify the clusters of top significant sites. Generalized linear regression was performed to examine the differential methylation patterns from clusters.Results:A total of 100 MZ pairs were included in this study. Hypermethylated cg10535199-2q23.1 ( β=0.74%, P=1.51×10 -7, OR=1.06, 95% CI: 1.03-1.09) and ch.17.49619327- SPOP ( β=0.23%, P=7.54×10 -7, OR=1.17, 95% CI: 1.08-1.28) were identified with suggestive significance. After correcting for multiple testing, no sites reached genome-wide significance. There was no statistical significance in the paired analysis. Two clusters with hypomethylated ( β=-0.39%, P<0.001) and hypermethylated ( β=0.47%, P<0.001) patterns were observed for HOMA2-IR. Conclusions:IR was significantly associated with DNA methylation, and genetic factors might contribute to the association.
5.Analysis of clinical characteristics of antiglutamic acid decarboxylase 65 antibody-positive autoimmune-associated epilepsy

Objective:To explore the clinical manifestations, auxiliary examinations, treatment, and outcomes of autoimmune-associated epilepsy (AAE) with anti-glutamic acid decarboxylase 65 (GAD65) antibodies.Methods:Nine patients diagnosed with AAE with anti-GAD65 antibodies between July 2019 and December 2023 at the Department of Neurology of Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology, were enrolled. Their clinical data and prognosis were retrospectively analyzed.Results:The onset age of these 9 cases was 13.0-59.0 (29.0±13.4) years, with 8 females and 1 male. The main clinical phenotypes at onset included limbic encephalitis/extralimbic encephalitis (6 cases) and chronic epilepsy (3 cases, of whom 1 case was accompanied with stiff person syndrome). The former often demonstrated T 2/fluid attenuated inversion recovery (FLAIR) hyperintensities and enlargement of hippocampus and amygdala (5 cases), and may present with concomitant meningoencephalitis (2 cases) or hypertrophic pachymeningitis (1 case). The latter showed normal (2 cases) or increased T 2/FLAIR signals in the hippocampal tails (1 case) in magnetic resonance imaging. Electroencephalogram findings were usually characterized by slow activities (6 cases) and temporal epileptiform discharges (6 cases). Five cases concurrently presented with one or more other autoimmune diseases, including the autoimmune polyendocrine syndrome type Ⅱ in 1 case. All cases received first-line immunotherapy and 7 cases received long-term immunosuppressive agents. Eight cases were followed up for 5-45 months and 2 cases experienced relapse of autoimmune encephalitis (presented with new-onset seizures). At the last follow-up, all cases achieved favourable outcomes according to the modified Rankin Scale scores (all≤2) and 4 patients had reduced seizures, of whom 2 were seizure-free. Conclusions:Anti-GAD65 antibody-positive AAE is mainly composed of postencephalitic epilepsy and chronic temporal lobe epilepsy. The co-occurrence of systemic autoimmune diseases is common, and meningeal lesions sometimes can be involved in MRI findings. Patients usually have poor response to immunotherapy and anti-seizure medication,and some postencephalitic epilepsy patients who receive early immunotherapy and long-term immunosuppressants can achieve sustained seizure freedom.
6.Risk prediction of low birth weight infants in Shanghai

ObjectiveTo investigate the risk factors of fertility behaviors with preterm birth and low birth weight, and to develop a nomogram model to predict the occurrence of low birth weight. MethodsBirth registration information in Shanghai from 2010 to 2020 was collected, and ANOVA and Chi-square tests were used to compare the differences in reproductive behavior factors and newborn health status across time. The odds ratio (OR) value and 95%CI were calculated by a multi-classification logistic regression model to determine the association between reproductive behavior factors and preterm birth or low birth weight infants. A nomogram model was established based on logistic model and the area under the ROC curve was used to assess the effect of the model. ResultsThis analysis included 2 089 384 live newborns. The incidence of full-term low birth weight, preterm normal weight and preterm low birth weight in Shanghai was 0.94%, 2.48% and 2.01%, respectively. From 2010 to 2020, 40.00% women had a history of abortion, the proportion of women who gave birth at age ≥40 years old increased from 1.05% to 2.24%, the proportion of fathers aged ≥40 years increased from 4.79% to 7.48%, and the proportion of women with postgraduate or above increased from 4.81% to 11.74%. The incidence of preterm low birth weight in Shanghai showed an increasing trend over time. Logistic regression analysis showed that the risk of preterm low birth weight was lower in female than in male infants (OR=0.97, 95%CI: 0.95‒0.98), and the risk of full-term low birth weight was higher than in male infants (OR=1.85, 95%CI: 1.80‒1.90). The risk of preterm birth and low birth weight was lower for couples of childbearing age with higher education. The risk of preterm low birth weight in newborns tended to increase with maternal age at childbirth >30 years, paternal age ≥40 years, and the number of abortions >2 times. Mother <25 or >35 years, father aged 30‒34 years, and the number of abortions >3 times were the risk factors of full-term low birth weight infants. ConclusionCouples of childbearing age who choose to have children at too high or too low age may increase the risk of preterm birth or low birth weight, so it is necessary to strengthen population awareness and promote age-appropriate childbirth. Multiple abortions are also associated with preterm birth and low birth weight, and it is advisable to popularize the scientific knowledge of contraception and birth control to reduce unnecessary abortions. The nomogram in the study can visualize the risk of full-term and low birth weight infant at different levels of factors, which can assist couples preparing for pregnancy in making decisions about the timing of childbirth and understanding the level of risk.

8.Role of melatonin receptor 1B gene polymorphism and its effect on the regulation of glucose transport in gestational diabetes mellitus.

Melatonin receptor 1B (MT2, encoded by the MTNR1B gene), a high-affinity receptor for melatonin, is associated with glucose homeostasis including glucose uptake and transport. The rs10830963 variant in the MTNR1B gene is linked to glucose metabolism disorders including gestational diabetes mellitus (GDM); however, the relationship between MT2-mediated melatonin signaling and a high birth weight of GDM infants from maternal glucose abnormality remains poorly understood. This article aims to investigate the relationship between rs10830963 variants and GDM development, as well as the effects of MT2 receptor on glucose uptake and transport in trophoblasts. TaqMan-MGB (minor groove binder) probe quantitative real-time polymerase chain reaction (qPCR) assays were used for rs10930963 genotyping. MT2 expression in the placenta of GDM and normal pregnant women was detected by immunofluorescence, western blot, and qPCR. The relationship between MT2 and glucose transporters (GLUTs) or peroxisome proliferator-activated receptor γ (PPARγ) was established by western blot, and glucose consumption of trophoblasts was measured by a glucose assay kit. The results showed that the genotype and allele frequencies of rs10830963 were significantly different between GDM and normal pregnant women (P<0.05). The fasting, 1-h and 2-h plasma glucose levels of G-allele carriers were significantly higher than those of C-allele carriers (P<0.05). Besides, the protein and messenger RNA (mRNA) expression of MT2 in the placenta of GDM was significantly higher than that of normal pregnant women (P<0.05). Melatonin could stimulate glucose uptake and GLUT4 and PPARγ protein expression in trophoblasts, which could be attenuated by MT2 receptor knockdown. In conclusion, the rs10830963 variant was associated with an increased risk of GDM. The MT2 receptor is essential for melatonin to raise glucose uptake and transport, which may be mediated by PPARγ.

9.Evaluation of the performance of a 34-layer ResNet model-based artificial intelligence application in the diagnosis of skin diseases

Objective:To evaluate the performance of Autoderm, an artificial intelligence application, in the diagnosis of skin diseases in Chinese patients.Methods:Totally, 920 patients with confirmed skin diseases were prospectively recruited in the Department of Dermatology, the First Affiliated Hospital of Zhengzhou University. Every patient provided 1 clinical image, which was uploaded onto the Autoderm application for the diagnosis of skin diseases. The diagnostic sensitivity, specificity and accuracy of the Autoderm application were estimated, and the kappa values for the diagnostic agreement between the Autoderm application and dermatologists were calculated.Results:Among the 920 patients, 871 (94.7%) could be diagnosed with an Autoderm′s in-distribution skin disease, whereas 49 (5.3%) had out-of-distribution skin diseases. According to the top 1 and 3 diagnoses given by the Autoderm application for the 920 patients separately, its mean diagnostic sensitivities were 41.8% and 65.8%, mean specificities 96.8% and 91.5%, and mean accuracies 92.9% and 89.9%, respectively, and there was moderate overall agreement between the Autoderm application and dermatologists (κ = 0.420, 0.464, respectively). However, for an out-of-distribution skin disease, the Autoderm application could output 5 definitely false diagnoses.Conclusion:Autoderm may be used as as clinical decision support tool for the diagnosis of common skin diseases in most Chinese patients, with moderate diagnostic sensitivity, high specificity, and high accuracy, but misdiagnosis may occur.
10.Analysis of dietary intake in the residents aged 15 years and above in Shanghai

ObjectiveTo provide basic data of daily dietary intake from various food categories as well as in different regions, seasons, genders, and age groups in Shanghai residents aged 15 and over. MethodsMultistage stratified proportional probability sampling (PPS) was used to extract the samples, and food frequency questionnaire (FFQ) was used to investigate the dietary intake of the subjects in four seasons from 2012 to 2013. The weighted statistical analysis of the samples comprehensively considered the sampling design weights, the stratified adjustment weights, and the non-response adjustment weights. ResultsThe total daily dietary intake (excluding drinking water) of residents aged 15 years and above was 1 174.71 g, and the highest three daily dietary intake categories were cereals (252.31 g), vegetables (205.36 g) and fruits (141.00 g). The total daily dietary intake of the residents in the urban area, the suburban area and the rural area was 1 209.15 g,1 172.27 g and 948.50 g, respectively, and the total daily dietary intake in the outer suburb area was significantly lower than that in other areas (F=74.12,P<0.001). The total daily dietary intake in different seasons was 1 232.47 g in spring, 1 166.80 g in summer, 1 241.15 g in autumn and 1 088.83 g in winter, respectively. The total daily dietary intake in winter was lower than that in other seasons (F=15.96,P<0.001). Fruits and beverages intake showed apparent seasonality. The total daily dietary intake in male and female residents was 1 234.03 g and 1 112.32 g, respectively, and the total daily dietary intake of male was higher than that of female (F=78.59,P<0.001). The total daily dietary intake of residents in different age groups was 1 218.64 g for 15‒44 years old, 1 141.27 g for 45‒59 years old, and 1 064.54 g for 60 years old and above (F=20.28,P<0.001). ConclusionThe daily intake of cereals, livestock and poultry meat, aquatic products, eggs and edible oil is relatively balanced, but the daily intake of vegetables, fruits and milk is relatively insufficient for the residents aged 15 years and above in Shanghai. The daily intake of different food types shows distinguishable characteristics in urban and rural areas, seasons, age groups and genders.
