BACKGROUND:Research has demonstrated a close association between ferroptosis and vascular dementia.Tongmai Kaiqiao Pill has a certain effect on improving the cognitive function of vascular dementia patients, but its mechanism is unclear. OBJECTIVE:To explore the interventional effects and molecular mechanisms of Tongmai Kaiqiao Pill for vascular dementia based on the regulation of ferroptosis by the nuclear factor erythroid-2-related factor 2(Nrf2)/heme oxygenase-1(HO-1)/glutathione peroxidase 4(GPX4)signaling pathway. METHODS:Among eighty-four SD male rats,12 rats were used as the sham-operated group, and the rest of them were prepared as a model of vascular dementia by the modified 2-VO method, and then randomly divided into the model group, the Tongmai Kaiqiao Pills high-,moderate-,and low-dosage(27.6,13.8,and 6.9 g/kg)groups,the combined group(Tongmai Kaiqiao Pill high-dosage+ML385,20 mg/kg),and the donepezil hydrochloride group(0.45 mg/kg).The drug was given once a day by intragastric administration. The combined group was also intraperitoneally injected Nrf2 inhibitor ML385,once a day,for 4 weeks.Morris water maze was used to detect the learning memory ability of rats.Hematoxylin-eosin staining was used to observe the histopathological changes in the hippocampus of rats in each group.Colorimetric assay was used to detect the content of reduced glutathione, ferrous ion(Fe2+),and malondialdehyde in the serum of rats.Prussian blue staining was used to detect the iron deposition in the hippocampal tissue of rats.Transmission electron microscopy was used to observe the ultrastructural changes of mitochondria in rat hippocampal tissues.Western blot assay was used to detect the protein expression levels of Nrf2,HO-1,GPX4,XCT,and ferritin heavy chain 1(FTH1)in rat hippocampal tissues. RESULTS AND CONCLUSION:(1)In comparison to the sham operation, rats in the model group exhibited a significantly prolonged latency period(P<0.05)and a reduced number of platform crossings(P<0.05).Additionally,the hippocampal tissues of these rats displayed loosely organized structure, deeply stained cell nuclei, and solidified or lysed chromatin.Ferri ions aggregated in CA1 region. There were atrophied mitochondria with dissolved cristae and thickened mitochondrial membranes.Fe2+,malondialdehyde,and reduced glutathione levels in rat serum were found to be elevated(P<0.05).A significant reduction in the expression of GPX4,HO-1,XCT,Nrf2,and FTH1 proteins was detected in the hippocampus(P<0.05).(2)Compared to the model group, the average escape latency of the rats was significantly reduced following intervention with Tongmai Kaiqiao Pills and donepezil hydrochloride(P<0.05),with an increased number of platform crossings(P<0.05).Hippocampal neurons showed significant recovery.Notably, iron aggregation in the CA1 region was significantly reduced, and mitochondrial structure and function were improved. There were significant reductions in Fe2+and malondialdehyde levels, while the levels of GPX4,HO-1,XCT,Nrf2,and FTH1 in rat hippocampal tissues, and reduced glutathione in serum were significantly increased(P<0.05).(3)The high-dose Tongmai Kaiqiao Pills exhibited a treatment effect comparable to that of donepezil hydrochloride(P>0.05),with a significant prolongation of water maze escape latency(P<0.05),a reduced number of platform crossings(P<0.05),and insignificant neuronal pathological changes in the CA1 area. However, the combined group showed increased iron deposition, elevated malondialdehyde and Fe2+levels in blood serum(P<0.05),reduced glutathione content(P<0.05),hippocampal tissue mitochondrial atrophy, and reduced expression of Nrf2,XCT,HO-1,GPX4,and FTH1 proteins(P<0.05).Within a certain range, higher doses of Tongmai Kaiqiao Pills demonstrated a more pronounced effect, comparable to the efficacy of high-dose donepezil hydrochloride.(4)It is concluded that Tongmai Kaiqiao Pills have been shown to mitigate histopathological changes in the rat hippocampus and enhance cognitive function in rats with vascular dementia. The mechanism of action is likely associated with the suppression of ferroptosis through the activation of the Nrf2/HO-1/GPX4 signaling pathway.

Objective:To investigate the longitudinal trajectories of symptom clusters in elderly patients with laparoscopic surgery for colon cancer, and explore the predictive factors for each trajectories subgroup. Methods:Using a longitudinal survey, elderly patients with laparoscopic surgery for colon cancer patients from Shanxi Bethune Hospital from January 2021 to January 2024 were collected by convenient sampling method. The Chinese Version of the MD Anderson Symptom Inventory Gastrointestinal Cancer Module was used to conduct follow-up surveys of the selected patients at 2 weeks, 1 month, 3 months and 6 months after surgery. The symptom cluster was extracted by exploratory factor analysis, and the latent category growth model was conducted to identify the trajectory subgroups of each symptom cluster, the predictive factors of each trajectory subgroup was analyzed by multiple Logistic regression analysis. Results:A total of 118 cases were concluded in the present study, there were 69 males and 49 females with an age of (74.85 ± 3.29) years old. There were 4 symptom clusters after surgery, which were named as energy deficit symptom cluster, digestive tract symptom cluster, sleep mental symptom cluster, and psychological fatigue symptom cluster, the variance contribution rates were respectively 62.486%, 71.209%, 73.937%, 63.476%. The results from latent class growth model showed that there were 3 trajectory subgroups in the symptom cluster: high level-slow decline group accounted for 33.0% (39/118), moderate level-stable decline group accounted for 39.0% (46/118), low level-rapid decline group accounted for 28.0% (33/118). College education or above tends to developed into low level-rapid decline group ( OR=0.365, 95% CI 0.083-0.603, P<0.05), while patients undergoing traditional laparoscopic surgery ( OR=3.679, 95% CI 1.297-4.432, P<0.05) and primary tumor stages of Ⅲ-Ⅳ were more likely to developed into high level-slow decline group ( OR=0.333, 95% CI 0.120-0.920, P<0.05). Conclusions:There are 4 symptom clusters in elderly patients with laparoscopic surgery for colon cancer, and the characteristics of postoperative symptom cluster changes demonstrate significant heterogeneity, medical staff should pay attention to the management of symptom clusters trajectory categories, dynamically adjust intervention plans to improve nursing quality.

Objective The purpose of the study is to breed homozygous interferon-γ knockout(IFN-γ-/-)mice and optimize the breeding strategies to achieve continuous and stable reproduction of IFN-γ-/-mice,which could be used as an ideal animal model for fundamental research. Methods Initially, heterozygous IFN-γ knockout(IFN-γ+/-)C57BL/6J mice were used as the parental generation for breeding.Subsequently,3 breeding strategies were employed using the offspring:(1)female heterozygotes mated with male heterozygotes;(2)male homozygotes mated with female heterozygotes;(3)female homozygotes mated with male homozygotes. The number and survival rate of IFN-γ-/-mice were compared across the three breeding strategies to determine the optimal breeding strategy.Under the optimal strategy, the effects of female mating age and diet type on the reproductive performance of IFN-γ-/-mice were further evaluated.Data from the first three litters of 60 IFN-γ-/-female mice, including litter size, number of weaning survivors, and weaning survival rate, were recorded and analyzed. In addition, the effects of dietary supplementation of pregnant mice and environmental optimization measures, such as the provision of shelters, were evaluated. Results Under conditions where the nutritional needs of pregnant mice were adequately met by supplementation with egg yolk and sunflower seeds, mating of female and male IFN-γ-/-mice result ed in a litter size of five to eight IFN-γ-/-mice,demonstrating higher efficiency compared to other breeding strategies. In addition, diet type and mating age significantly influenced female reproductive performance. When 7~9 weeks old female IFN-γ-/-mice were mated to male IFN-γ-/-mice and fed a high-protein breeding diet, litter size(6.9±1.7),weaning survival number(6.5%±2.0%)and weaning survival rate(93.2%±17.8%)were higher than those under other conditions. In addition, providing shelters to prevent fighting between breeding pairs further improved reproductive outcomes. Conclusions By adopting an optimized breeding strategy, combined with a high-protein diet, nutritional supplementation, and standardized mating age management, the breeding efficiency and stability of IFN-γ-/-mice can be significantly improved. This provides a reliable animal model for related research.
