1.Construction of Nomogram Prediction Model for Malnutrition in Patients with Lung Cancer undergoing Chemotherapy

Objective To analyze the influencing factors of malnutrition in patients with lung cancer undergoing chemotherapy,and construct a predictive nomogram model according to the results.Methods A retrospective analysis was made on 100 patients with lung cancer who received postoperative chemotherapy in our hospital from March 2022 to April 2024.After all the patients had completed five cycles of chemotherapy,the incidence of malnutrition was counted.According to the nutritional evaluation results,the patients were divid-ed into two groups:well-nourished group and malnourished group.The clinical data of patients were compared and the related influen-cing factors were screened by Logistic regression analysis.The nomogram model for risk prediction of malnutrition risk of lung cancer pa-tients undergoing chemotherapy was constructed,and the predictive value and fitting degree of the model were evaluated by drawing the re-ceiver operating characteristic(ROC)curve and calibration curve.Results There were 29 cases of malnutrition in 100subjects,and the other 71 patients had normal nutritional status.Among the general data with significant differences between the two groups,TNM is stageⅣ(OR=2.275,95%CI:1.242-4.169),Accompanied by digestive system adverse reactions(OR=2.142,95%CI:1.133-4.050),Low albumin(OR=2.601,95%CI:1.370-4.937),Low hemoglobin(OR=2.697,95%CI:1.483-4.903),Low prealbumin(OR=2.686,95%CI:1.460-4.941)is a risk factor for malnutrition in patients with lung cancer undergoing chemotherapy(P<0.05).Based on the above related risk factors,a nomogram model was established to predict the occurrence of malnutrition in patients with lung cancer undergoing chemotherapy.ROC curve analysis showed that the AUC value and 95%CI of the model for predicting malnutrition in patients with lung cancer undergoing chemotherapy were 0.972 and 0.918-0.995.The calibration curve results show that the nomogram model has a good fitting degree.Conclusion TNM is stage Ⅳ,accompanied by digestive system adverse reactions,low KPS score before chem-otherapy and low levels of albumin,hemoglobin and prealbumin are all related risk factors for malnutrition in patients with lung cancer un-dergoing chemotherapy.The nomograph model based on this is also of high predictive value.

8.Distribution of COVID-19 transmission-related receptors ACE2 and TMPRSS2 in human conjunctival tissue and its significance

Objective:To investigate the expression of coronavirus disease 2019 (COVID-19) transmission-related receptors angiotensin-converting enzyme 2 (ACE2) and transmembrane serine protease 2 (TMPRSS2) in human conjunctival tissue and its clinical significance.Methods:Fifty human conjunctival tissue specimens from 50 patients including 10 normal conjunctival tissues, 15 conjunctival papilloma tissues, 15 conjunctival nevus tissues and 10 conjunctival cyst tissues were collected from June 2019 to June 2020 at Xi'an People's Hospital.Ten corneal tissue samples from 10 patients with eyes removed due to trauma were collected as control.The distribution of ACE2 and TMPRSS2 in different corneal tissues was detected by the immunohistochemistry.The expression of ACE2 and TMPRSS2 was scored and compared.Reuse of the human samples and the research protocol was approved by an Ethics Committee of Xi'an People's Hospital (No.20190022). Written informed consent was obtained from each patient.Results:ACE2 and TMPRSS2 were both expressed in normal conjunctival epithelium, epithelial cells in conjunctiva papilloma and conjunctival nevus, and cells in conjunctiva cyst wall.ACE2 was mainly distributed in the superficial and intermediate cells of conjunctival epithelium, but not in the basal cells and goblet cells.TMPRSS2 was found in different layers of cells.The positive expression rates of ACE2 and TMPRSS2 in conjunctiva were both 100%.There was no significant difference in the expression intensity of ACE2 and TMPRSS2 among normal conjunctival tissue, conjunctival papilloma, conjunctival nevus and conjunctival cyst (all at P>0.05). Weakly expressed in corneal tissues, ACE2 and TMPRSS2 were more moderately and strongly expressed in conjunctival tissues.There were significant differences in the number of differently graded ACE2 and TMPRSS2 expression between normal conjunctival tissues, conjunctival papilloma, conjunctival nevus, conjunctival cyst and corneal tissues (ACE2: Z=-3.473, -4.183, -3.970, -3.873, all at P<0.01; TMPRSS2: Z=-4.119, -4.472, -4.443, -4.147, all at P<0.001). Conclusions:COVID-19 transmission-related receptors ACE2 and TMPRSS2 are expressed in human conjunctival tissue, which provides organological evidence for ocular surface transmission of COVID-19.
9.The Effect of Trunk Motion Control on Biomechanical Parameters of Knee During Landing

Objective To determine the effect of trunk motion control on knee biomechanics during single-leg landing. Methods Twenty male healthy basketball players were recruited. The kinematics, kinetics parameters and surface electromyogram (EMG) of knee joints under trunk motion control during single-leg landing were studied by using Vicon motion Analysis system, Kistler force platform and Noraxon surface EMG system. Results Compared with natural landing, there were significant differences in flexion angle at initial contact moment, peak flexion angle and peak flexion moment during deeply inspiratory landing and landing with a stick, and a significantly larger flexion angle at initial contact moment, a larger peak flexion angle and smaller peak flexion moment were also found. Compared with natural landing, there was no statistical difference in peak valgus angle and peak valgus moment, as well as EMG activity of hamstrings, quadriceps muscles of the knee during deeply inspiratory landing and landing with a stick. Conclusions Deeply inspiratory landing or landing with a stick reduce the risk of anterior cruciate ligament (ACL) injury, and the results provide the theoretical basis for prevention of ACL injuries in basketball players during landing.
10.Clinical analysis of nine cases of neuroimmune disease with antibodies against glutamic acid decarboxylase 65

Objective:To explore the clinical features, auxiliary examinations, therapies and prognoses of patients with antibodies targeting glutamic acid decarboxylase 65 (GAD65).Methods:The nine patients with anti-GAD65 neuroimmune disease, admitted to Xuanwu Hospital, Capital Medical University from October 2018 to October 2020, were analyzed, retrospectively.Results:The onset age of nine cases was 17-68 (43.6±20.5) years old, and six cases were female. Two cases had preceding infection. The data of initial symptoms were collected and analyzed, including epileptic onset in four cases, memory impairment in two cases, dizziness in two cases and limb stiffness in one case. As the disease continued to advance, one case developed cerebellar ataxia, one case presented with isolated epilepsy, five cases suffered from limbic encephalitis, one case had stiffman syndrome, one case had brainstem encephalitis. Five cases had antibodies against thyroid peroxidase. Brain magnetic resonance imaging scan showed abnormal signals of T 2/fluid attenuated inversion recovery sequences in four cases, mainly involved bilateral temporal lobes or hippocampus. Epileptiform discharges of frontal or temporal regions of electroencephalography were observed in six cases. All cases received immunotherapy and long-term follow-up was performed in seven cases. Four cases benefited from the immunotherapy. Among the four patients, one fully recovered and returned to work, the other three cases developed neurologic sequelae, including seizures (two cases), and short-term memory loss (one case). The remaining three patients were unresponsive to treatment. Conclusions:GAD65 antibody-mediated neuroimmune disease is a rare neurological disorder, presenting with various syndromes including limbic encephalitis or stiffman syndrome, which is more susceptible to young female. The clinical manifestations included epileptic onset, limb stiffness, cognitive impairment and cerebellar ataxia, etc. Detection of GAD65 antibody in serum or cerebrospinal fluid was gold standard. Early immunotherapy contributed to improving the prognosis of patients, especially for those patients with epileptic onset as the main symptom.
