1.Preparation of Mycobacterium tuberculosis ESAT6-Fc fusion protein and evaluation of its efficacy and safety in allergic rhinitis mice

ObjectiveTo prepare Mycobacterium tuberculosis ESAT6-Fc fusion protein and evaluate its efficacy and safety on ovalbumin(OVA)-induced allergic rhinitis(AR) in mice, so as to provide experimental basis for the development of novel AR immunothera-peutic agents.MethodsThe recombinant plasmid pcDNA3.1(+)-Rv3875-Fc was transfected into CHO-K1 cells to obtain ESAT6-Fc fusion protein, which was purified by Protein G affinity chromatography. The female C57 BL/6J mice of AR model sensitized by OVA were randomly divided into five groups: normal control, OVA, CpG, ESAT6-Fc + CpG and dexamethasone(DEX) group, six mice each group, which were challenged with atomized OVA after the corresponding preparation was given to the nasal cavity for intervention. HE staining method was used to evaluate the inflammatory infiltration of mouse nasal mucosa, and ELISA method was used to detect the content of histidine(HIS), leukotriene B4(LTB4), IgE, IL-4,IL-13 and IL-17A in the nasal lavage fluid. The structural changes of heart, liver, spleen and kidney tissues were observed by HE staining, and the levels of glutamic oxaloacetic transaminase(AST) and lactate dehydrogenase(LDH) in mouse heart,liver, spleen and kidney were measured by ELISA.ResultsESAT6-Fc fusion protein with high purity(> 95%) was successfully obtained. Compared with the normal control group, the inflammatory cell infiltration in nasal mucosa of the OVA group was significantly aggravated(t = 11. 180, P < 0. 001), and the levels of HIS, LTB4, IgE, IL-4, IL-13 and IL-17A in the nasal lavage fluid significantly increased(t = 3. 843, 4. 237, 5. 996, 3. 561, 4. 544, 5. 773, respectively, each P < 0. 05). Compared with the OVA group, the infiltration of inflammatory cells in nasal mucosa of mice in the ESAT6-Fc + CpG and DEX groups was significantly reduced(t = 4. 472 and 5. 582, respectively, each P < 0. 001), and the levels of HIS, IgE, LTB4, IL-4, IL-13 and IL-17A in the nasal lavage fluid significantly decreased(t = 0. 000-1. 061, each P < 0. 05). There was no significant difference in the levels of HIS, IgE, LTB4, IL-4, IL-13 and IL-17A between the ESAT6-Fc + CpG and DEX groups(t = 0. 048, 0. 000,0. 968, 1. 061, 0. 375 and 0. 638, respectively, each P > 0. 05). Intranasal immunization with ESAT6-Fc did not cause pathological damage to the heart, liver, spleen, and kidney of mice, and the levels of AST and LDH in the above organs showed no significant difference from those in the normal control group(t = 0. 001-1. 036, each P > 0. 05).ConclusionThe prepared ESAT6-Fc fusion protein has high purity, and intranasal immunization with the fusion protein can safely and effectively alleviate OVA-induced AR symptoms in mice, and inhibit the expression of related inflammatory factors, without obvious organ toxicity observed, indicating that ESAT6-Fc fusion protein is expected to become a new type of immunotherapeutic agent for AR.
2.PPARα activation alleviates lithocholic acid-induced liver injury by inhibiting pyroptosis

Background and aims
The mechanism of cholestatic liver injury (CLI) is unclear, and effective therapies are lacking. While peroxisome proliferator-activated receptor alpha (PPARα) agonists show potential hepatoprotective effect and pyroptosis is implicated in hepatocellular damage, how PPARα activation mitigates lithocholic acid (LCA)-induced pyroptosis remains unknown.
Methods
The hepatoprotective effect of PPARα agonists was evaluated in a mouse model of intrahepatic cholestasis induced by LCA. Liver injury was assessed via serum biochemistry, hematoxylin and eosin and TUNEL staining, and electron microscopy. Pyroptosis pathways were analyzed using real-time quantitative polymerase chain reaction, Western blot, and co-immunoprecipitation.
Results
Combined morphological, histopathological, and biochemical analyses confirmed that PPARα activation protects against CLI. Compared with LCA treatment alone, PPARα activation significantly attenuated the elevation of serum lactate dehydrogenase (LDH), the increased TUNEL-positive cells, and the formation of hepatocyte membrane pores. Mechanistically, PPARα activation suppressed both NOD-like receptor protein 3 (NLRP3) inflammasome-mediated pyroptosis and apoptosis protease-activating factor-1 (APAF-1)/CASPASE-3/GSDME-mediated pyroptosis. Furthermore, PPARα agonist pretreatment inhibited activation of the nuclear factor-kappa B (NF-κB) and forkhead box O1 (FOXO1) signaling pathways.
Conclusions
PPARα protects against LCA-induced CLI by inhibiting both NLRP3 inflammasome-mediated pyroptosis associated with NF-κB and APAF-1/CASPASE-3/GSDME-mediated pyroptosis associated with the FOXO1 signaling pathway.
3.Mechanism of circ-0058063 on ferroptosis in esophageal cancer cells

Objective To investigate the mechanism of action of circ-0058063 regulating ERK/MAPK signaling pathway on ferroptosis in esophageal cancer cells.Methods EC9706 cells were randomly divided into the Control group,the si-NC group,the si-circ-0058063 group,and the si-circ-0058063+ISO group.qRT-PCR was used to detect the expression of circ-0058063 in esophageal cancer tissues and cells;CCK-8 and clone formation assay were used to detect the cell proliferation ability.The levels of Fe2+,malondialdehyde(MDA),glutathione(GSH),and reactive oxygen species(ROS)in cells were detected.Western blot was used to detect the ERK/MAPK signaling pathway and the expression of ferroptosis-related protein in cells.Results The expression level of circ-0058063 in esophageal cancer tissue was significantly higher than that in adjacent tissues(P<0.05);the expression level of circ-0058063 in human esophageal cancer cell EC9706 was significantly higher than that in human normal esophageal epithelial cell HEEC(P<0.05).Compared with the Control group,the expression level of circ-0058063 in cells,cell survival rate,number of cloned cells,GSH level,ratios of p-ERK1/2/ERK1/2 and p-p38MAPK/p38MAPK,as well as the expression levels of SLC7A11 and GPX4 protein in the si-circ-0058063 group were significantly reduced(P<0.05),while the levels of Fe2+,MDA,and ROS in cells were significantly increased(P<0.05).Compared with the si-circ-0058063 group,the expression level of circ-0058063 in cells,cell survival rate,number of cloned cells,GSH level,ratios of p-ERK1/2/ERK1/2 and p-p38MAPK/p38MAPK,as well as the expression levels of SLC7A11 and GPX4 protein in the si-circ-0058063+ISO group were significantly increased(P<0.05),while the levels of Fe2+,MDA,and ROS in cells were significantly decreased(P<0.05).Conclusion Knockdown of circ-0058063 can inhibit the proliferation of esophageal cancer cells and induce ferroptosis,and its mechanism of action may be related to the inhibition of ERK/MAPK signaling pathway activation.

5.Protective effect and mechanism of Ginkgo biloba extracts against cerebral ischemia-reperfusion injury

Objective To investigate the mechanism of Ginkgo biloba extracts(GB)in the treatment of cerebral ischemia-reperfusion(CIR)injury based on network pharmacology and animal experiments.Methods The intersection targets of CIR and GB were obtained from TCMSP,GeneCards and other databases.Cytoscape software and Metascape database were used to analyze and map the related targets.The model of transient middle cerebral arterial occlusion(t-MCAO)was constructed in mice by suture method,and the effects of GB on the neurological function of mice after t-MCAO were observed by the neurological deficit score and Morris water maze test.HE staining was used to observe the pathological structural changes of neuron in the hippocampus of mice,and Western blot was used to verify the signal pathways screened by network pharmacology.Results Network pharmacology predicted that GB contained 33 active ingredients,and 116 potential targets of GB in treatment of CIR included Caspase3,Bax,etc.In addition,GB may play a protective role through signaling pathways such as PI3K-Akt and AMPK.Animal experiments showed that GB treatment could significantly improve the neural function and learning spatial memory ability of mice,alleviate the brain histopathological injury,and activate p-Akt/Akt signaling pathway.Conclusion GB has the characteristics of multi-target and multi-pathways therapy for CIR,which may reduce neuronal apoptosis by activating the p-Akt/Akt signaling pathway.
6.Efficacy and safety analysis of combined telitacicept in 25 patients with systemic lupus erythematosus based on standard therapy

Objective:To evaluate the efficacy and safety of telitacicept in the treatment of systemic lupus erythematosus (SLE) .Methods:The clinical data of 25 SLE patients who received standard therapy combined with telitacicept at the Department of Dermatology, Xiangya Second Hospital, Central South University, from 2021 to 2024 were retrospectively collected. Baseline demographic and clinical characteristics were analyzed. Changes in skin lesions, joint pain symptoms, complete blood count, and biochemical parameters at 4, 12, and 24 weeks of treatment were compared with baseline (week 0). The Wilcoxon signed-rank test was used to compare complement C3 and C4 levels before and after treatment, and univariate logistic regression analysis was performed to explore factors influencing the efficacy of telitacicept.Results:Among the 25 SLE patients, 3 were male (12.0%) and 22 were female (88.0%). Based on the SLE Disease Activity Index (SLEDAI) -2000 scores, 8 patients were mild, 13 were moderate, and 4 were severe. Of the 11 SLE patients with rashes before treatment, 6 achieved complete remission at 12 weeks. Among the 7 patients with joint pain before treatment, 4 experienced symptom resolution at 24 weeks. The proportion of patients with leukopenia at baseline and at 4, 12, and 24 weeks was 10/25 (40.0%), 0/24 (0), 1/22 (4.5%), and 2/19 (10.5%), respectively. The proportion of patients with thrombocytopenia was 6/25 (24.0%), 3/24 (12.5%), 1/22 (4.5%), and 1/19 (5.3%), respectively, and the proportion of patients with anemia was 7/25 (28.0%), 3/24 (12.5%), 1/22 (4.5%), and 1/19 (5.3%), respectively. At baseline, 11 out of 25 patients (44.0%) had proteinuria. At 12 weeks, the urinary protein quantification level (0.4 [0, 0.6] g/L) was significantly lower than at baseline (0.9 [0.8, 1.2] g/L). The SLE responder index-4 (SRI4) response rates at 4, 12, and 24 weeks were 14/18, 15/17, and 12/14, respectively. Complement C3 and C4 levels were significantly higher at 4, 12, and 24 weeks compared to baseline (all P < 0.001). Univariate logistic regression analysis showed that age, disease duration, glucocorticoid dosage, baseline complement C4 levels, antinuclear antibody titer, and SLEDAI-2K score did not significantly affect the efficacy of telitacicept (SRI4 response rate at 12 weeks) (all P > 0.05). No serious adverse reactions related to telitacicept were observed in patients. Conclusions:Telitacicept improved skin lesions, complement C3 and C4 levels, and anti-double-stranded DNA antibody levels in SLE patients. No association was found between the efficacy of telitacicept and baseline SLEDAI-2K scores, antinuclear antibody titers, or complement C4 levels, suggesting that telitacicept is an effective and safe treatment for SLE patients.
7.Effect and mechanism of LINC01088 on proliferation,migration and in-vasion of breast cancer cells

Objective:To investigate the expression of LINC01088 in breast cancer and its effects on cell prolifera-tion,migration,and invasion.Methods:GEPIA and bc-GenExMiner were used to analyze the correlation between LINC01088 expression levels and clinical characteristics as well as prognosis.The expression of LINC01088 in MCF10A and MDA-MB-231,BT-549,MCF7 were detected by Real-time PCR.The effect of LINC01088 on the biological func-tion of breast cancer cells was examined by overexpressing LINC01088 in breast cancer cells.Cell proliferation was as-sessed using the Incucyte assay,while cell migration and invasion were evaluated using Transwell assays.Western blot-ting was employed to detect the expression of proteins associated with cell proliferation and metastasis.Results:LINC01088 expression was significantly lower in breast cancer tissues compared to normal breast tissues(P<0.05).Data from the bc-GenExMiner database revealed higher LINC01088 expression in HER2 positive patients(P<0.0001),corre-lating with longer overall survival(P=0.0006)and disease-free survival(P=0.0002).The mRNA expression level of LINC01088 in normal breast epithelial cell line was higher than that in breast cancer cell lines(P<0.05).Overexpression of LINC01088 significantly reduced proliferation,migration,and invasion in three breast cancer cell lines(P<0.01).Addi-tionally,LINC01088 upregulated p21 and p27(P<0.01),while downregulating Snail,Slug,PI3K,and phosphorylated Akt(P<0.05).Conclusion:LINC01088 expression was significantly reduced in human breast cancer.In vitro,LINC01088 in-hibited the proliferation,migration,and invasion of breast cancer cells.This effect may be attributed to its role in sup-pressing the PI3K-AKT pathway and epithelial-mesenchymal transition.
8.Rehabilitation effect of aerobic exercise combined with discharge rehabilitation guidance in patients with coronary heart disease

Objective:To investigate the rehabilitation effect of aerobic exercise combined with discharge rehabilita-tion guidance in patients with coronary heart disease(CHD).Methods:This randomized controlled study enrolled 225 CHD patients admitted in Huai'an First People's Hospital between June 2020 and June 2023.Patients were ran-domly divided into control group(n=112)and intervention group(n=113).Patients in the control group received discharge rehabilitation guidance intervention,while those in the intervention group received additional aerobic ex-ercise training.After 6-month intervention,cardiac function,pulmonary function,vascular endothelial function indexes,quality of life,exercise treadmill scores,walking distance and patient satisfaction with rehabilitation effect were compared between the two groups.Results:Compared to those in the control group after intervention,patients in the intervention group had significantly higher left ventricular ejection fraction(LVEF)[(69.82±7.92)%vs.(63.34±6.88)%],stroke volume(SV)[(74.74±8.09)ml vs.(68.03±7.70)ml],cardiac output(CO)[(4.85±0.82)L/min vs.(4.18±0.73)L/min],cardiac index(CI)[(3.05±0.64)L·min-1·m-2 vs.(2.42±0.59)L·min-1·m-2],forced vital capacity(FVC)[(2.88±0.59)L vs.(2.37±0.56)L],maximum oxygen uptake(VO2max)[(23.23±4.08)ml·kg-1·min-1 vs.(19.79±3.86)ml·kg-1·min-1],anaerobic threshold(AT)[(21.10±4.18)ml·kg-1·min-1 vs.(17.21±3.90)ml·kg-1·min-1],nitric oxide(NO)[(33.10±5.18)mg/L vs.(27.21±5.90)mg/L],scores of General Quality of Life Inventory 74(GQOLI-74)[(85.43±8.19)points vs.(79.67±7.86)points],Duke treadmill score(DTS)[(9.08±2.03)points vs.(7.14±1.78)points],6-min walking distance(6MWD)[(342.14±36.24)m vs.(317.21±34.90)m](P<0.001 all),and significantly lower levels of von Willebrand factor(vWF)[(23.43±4.39)ng/L vs.(29.37±4.56)ng/L]and endothelin-1(ET-1)[(47.12±5.28)pg/ml vs.(52.79±5.96)pg/ml](P<0.001 all).We detected significantly higher total satisfac-tion with rehabilitation effect(92.73%vs.78.18%,P=0.002)in intervention group.Conclusion:Aerobic exercise combined with discharge rehabilitation guidance had good rehabilitation effect in CHD patients.It could improve their cardiopulmonary function,vascular endothelial function,and prognosis quality of life,and increase patient sat-isfaction.
9.Analysis on regional disparities and dynamic evolution of primary healthcare resource allocation in China under high-quality development:Based on Dagum Gini Coefficient Decomposition and Kernel Density Estimation

Objective:To explore the regional equity status and dynamic evolution patterns of primary healthcare resource allocation in China from 2009 to 2023.Methods:Based on statistical data of China's primary healthcare resources,the Dagum Gini coefficient decomposition method was used to analyze regional equity differences in primary healthcare resource allocation in China from 2009 to 2023,and Kernel density estimation was employed to investigate the spatial distribution and dynamic evolution process of resource allocation.Results:From 2009 to 2023,the total volume of primary healthcare resources in China continued to grow,with institutions,beds,and personnel increasing by 15.20%,65.53%,and 101.83%respectively,but significant regional disparities persist.Different types of resource allocation showed divergent equity trends:the Gini coefficient for personnel allocation decreased from 0.14 to 0.07,while the Gini coefficient for bed allocation increased from 0.15 to 0.22,and institutional allocation remained relatively stable.Kernel density estimation revealed that personnel allocation transformed from a bimodal distribution to a concentrated unimodal distribution,while bed allocation became more dispersed.Conclusion:China's primary healthcare resource allocation exhibits a divergent phenomenon with concurrent improvement in personnel allocation equity and deterioration in bed allocation equity.It is necessary to establish a differentiated allocation system with targeted policies,implement regional coordinated development strategies,improve monitoring and evaluation mechanisms,and strengthen technological support.
10.Research on practice guideline of health conversational artificial intelligence

The development of artificial intelligence(AI)technology has provided new avenues for the public to access health information.While both domestic and international research and practical applications of health conversational AI have emerged,there remains a lack of consensus on the key issues surrounding their development.Based on a comprehensive review of domestic and international literature and multiple rounds of expert consultations,the study focuses on six issues:the definition,characteristics,application scenarios,evaluation elements,evaluation methods,as well as prospects and challenges of AI-driven health consultation systems,and develops the practice guideline for health conversational artificial intelligence.The findings aim to provide scientific reference for promoting the innovative application of AI technology in the healthcare field.
