1.Overview of Chinese Neonatal Network: current and future

The Chinese Neonatal Network(CHNN) was established in 2018 with the mission of establishing a national collaboration platform, conducting high-quality and collaborative research, and ultimately improving the quality of neonatal-perinatal care and health in China.At present, 112 hospitals across the country have joined CHNN.CHNN has established a national standardized cohort of very premature infants/very low birth weight infants with >10 000 enrollments each year, has been leading data-driven collaborative quality improvement initiatives, conducting multicenter clinical studies, and performing multi-level training programs.Guided by the principles of collaboration and sharing, data-driven, continuous improvement, and international integration, CHNN has become an important platform for clinical and research collaboration in neonatal medicine in China.
2.Histological Validation of Cardiovascular Magnetic Resonance T1 Mapping for Assessing the Evolution of Myocardial Injury in Myocardial Infarction:An Experimental Study

Objective:
To determine whether T1 mapping could monitor the dynamic changes of injury in myocardial infarction (MI) and be histologically validated.
Materials and Methods:
In 22 pigs, MI was induced by ligating the left anterior descending artery and they underwent serial cardiovascular magnetic resonance examinations with modified Look-Locker inversion T1 mapping and extracellular volume (ECV) computation in acute (within 24 hours, n = 22), subacute (7 days, n = 13), and chronic (3 months, n = 7) phases of MI. Masson’s trichrome staining was performed for histological ECV calculation. Myocardial native T1 and ECV were obtained by region of interest measurement in infarcted, peri-infarct, and remote myocardium.
Results:
Native T1 and ECV in peri-infarct myocardium differed from remote myocardium in acute (1181 ± 62 ms vs. 1113 ± 64 ms, p = 0.002; 24 ± 4% vs. 19 ± 4%, p = 0.031) and subacute phases (1264 ± 41 ms vs. 1171 ± 56 ms, p < 0.001; 27 ± 4% vs. 22 ± 2%, p = 0.009) but not in chronic phase (1157 ± 57 ms vs. 1120 ± 54 ms, p = 0.934; 23 ± 2% vs. 20 ± 1%, p = 0.109). From acute to chronic MI, infarcted native T1 peaked in subacute phase (1275 ± 63 ms vs. 1637 ± 123 ms vs. 1471 ± 98 ms, p < 0.001), while ECV progressively increased with time (35 ± 7% vs. 46 ± 6% vs. 52 ± 4%,p < 0.001). Native T1 correlated well with histological findings (R2 = 0.65 to 0.89, all p < 0.001) so did ECV (R2 = 0.73 to 0.94, all p < 0.001).
Conclusion
T1 mapping allows the quantitative assessment of injury in MI and the noninvasive monitoring of tissue injury evolution, which correlates well with histological findings.

4.Analysis on the postoperative effect of Ruin nutrient solution combined with dietary education on the postoperative patients with gastrointestinal neoplasms

Objective To investigate the effect of Ruin nutrient solution combined with dietary education on the postoperative patients with gastrointestinal neoplasms.Methods 88 patients with gastrointestinal neoplasms in Hangzhou tumor hospital from January 2016 to January 2017 were randomly divided into the control group(44 cases) and the observation group(44 cases).The control group were given the early stage total parenteral nutrition.The observation group were received the early stage enteral nutrition and dietary education.The recovery time, the occurrence of complications and the hospitalization time and hospitalization expenses were compared in the two groups.Results After the intervention, the recovery time of bowel sounds and recovery time of anal in the observation group were less than those in the control group, the difference was statistically significant(P<0.05);the incidence of complications in the observation group was lower than that in the control group(P<0.05).The hospitalization time and hospitalization cost in the observation group were better than those in the control group, the difference was statistically significant(P<0.05).Conclusion The combination of Ruin nutrient solution and dietary education is effective on the treatment of the postoperative patients with gastrointestinal neoplasms, which can effectively promote the recovery of intestinal function and reduce the incidence of complications, accelerate the rehabilitation of patients and deserve to be further promoted in clinic application.
5.Screening of the Base Formula of Compound Titanium Dioxide Cream

Objective:To screen the optimal base of compound titanium dioxide cream. Methods:Phenyl salicylate and titanium dioxide were respectively combined with 5 kinds of matrices to prepare compound titanium dioxide cream. The properties and stability of compound titanium dioxide cream were studied. Results:The 5 kinds of anionic emulsifiers all could be used to prepare compound titanium dioxide cream. The cream prepared with formula A was thick,and the cream prepared with formula B, C,D and E were delicate,luster and easy to smear. After the centrifugal test and low temperature test,the cream had no obvious changes. After the high temperature test,the cream prepared with formula A and B was dry and rough,the cream prepared with formula D had no significant changes when compared with that stored under room temperature,and the cream prepared with C and E was attenuated and changed into solution. Stored under the room temperature for 12 months,the cream prepared with formula D had no changes in the content and properties. Conclusion:Compound titanium dioxide cream prepared formula D is delicate and easy to smear with good stability,which shows good clinical application value.
6.Study of optimization for the cream base of benzhydramine hydroehloride

Objective To optimize the cream base of benzhydramine hydroehloride .Methods Five nonionic surfactants as an emulsifier were used to prepare benzhydramine hydroehloride creams .The five sorts of cream were evaluated from the ap-pearance ,preliminary stability ,accelerated test stability and room temperature stability .Results Benzhydramine hydroehloride cream could be prepared using five nonionic surfactants .On appearance of smoothness ,gloss and spread ,formula E was best but formula A was worst ,while formulas B and D were close and worse than formula E only .In the stability tests ,all of creams had no obvious change after the centrifugal experiment and the low temperature experiment .In the high temperature ex-periment ,the formula E was stable ,formulas A ,B and D showed a little appearance change ,and the formula C became layered bleeding .In room temperature ,the formula C became bleeding after five days ,which happened to the formula A after one month ,and formula B after two months .The formulas D and E had no obvious change until six months in room temperature . Conclusion The formulas D and E were stable ,which had clinical application value .
7.Study on disease-specific performance appraisal at public hospitals in Shanghai

This study summarized the experiences of disease-specific performance appraisal at tertiary hospitals in Shanghai,which was launched since year 2013 by Shanghai Hospital Development Center (SHDC). 38 tertiary hospitals in Shanghai were included in the study. A disease-specific performance appraisal system centering on quality and performance, by means of case-mix model, classified surgery management and typical disease screening, and leveraging disease and surgery difficulties analysis, and inter-hospital performance appraisal of typical diseases. This reform has established appraisal criteria of disease difficulty management,coding criteria and data norms,guiding such hospitals to consolidate their functional positioning of focusing on difficult,urgent and complicated cases in the medical service delivery system.All these efforts have paved the way for the reforms to build a hierarchical medical service system, pricing per disease, payment per disease, and consolidate performance appraisal of medical workers.
8.The relationship between characteristics of cervical cancer at contrast-enhanced ultrasound and prognostic factors

Objective To evaluate the relationship between characteristics of cervical cancer at contrast-enhanced ultrasound and prognostic factors.Methods Thirty-five patients with pathologically confirmed cervical cancer were enrolled in this retrospective analysis.All the patients accepted contrast-enhanced ultrasound examination before the surgery.Relationships between the characters of contrast-enhanced ultrasound and the expression of ki-67 antigen(cell proliferation marker),tumor diameter,lymph node metastasis and histologic grade were studied.Results (1)Compared to the surrounding normal tissue, the cervical cancer had statistically significant differences in time to peak and peak intensity(P <0.05),the former was shortened,and the latter was increased.(2 )The ratio of perfusion defect and heterogeneous enhancement were significantly higher in tissues,including the ki-67 positive expression tissue,poorly differentiated or larger tumors (≥ 2.0 cm in diameter)(P < 0.05 ).(3 )Compared to the ki-67 negative expression group,time to peak of the ki-67 positive expression group was shorter and the peak intensity was higher,the differences were statistically significant (P < 0.05 ).(4 )The peak intensity of poorly differentiated cervical cancers was significantly higher than that of the other groups (P <0.05).Conclusions Enhancement patterns and parameters of contrast-enhanced ultrasound were associated with prognosis, which may be useful in noninvasive prediction of the patients with cervical cancers.
9.Determination of Phenyl Salicylate in Compound Titanium Dioxide Cream by HPLC

Objective:To establish a method for the determination of phenyl salicylate in compound titanium dioxide cream. Meth-ods:The HPLC assay was carried out on an Agilent Zorbax SB C18 (250 mm × 4. 6 mm, 5 μm) column with methanol -water (78∶22) as the mobile phase. The sample was detected at 308 nm with a UV detector. The column temperature was set at 25 ℃ and the flow rate was 1. 0 ml·min-1 . Results:Phenyl salicylate could isolate from the other materials by HPLC. The linear range of phenyl salicylate was 59.460-198.200 μg·ml-1(r=0.999 5). The recovery was 101.84% with RSD of 0.64%(n=9). The content of phenyl salicylate in 3 batches of compound titanium dioxide cream was 107. 7%, 107. 5% and 109. 8%, respectively. Conclusion:The method is simple, rapid, exclusive, accurate and sensitive, which is suitable for the determination of phenyl salicylate in com-pound titanium dioxide cream.
10.Localization of the assessment of chronic illness care and item evaluation.

OBJECTIVE:
To translate the English version of the assessment of chronic illness care (ACIC) to Chinese, to conduct a trial in Hunan province and to develop a Chinese version ACIC.
METHODS:
According to the WHO rules, we translated the English version of the ACIC to Chinese and took the culture factor into account. Three hundred and ninety persons, who engaged in the work of chronic disease management in primary medical and health services in Hunan province, were enrolled for this study. One hundred and eighty three valid questionnaires were randomly selected. Six methods including discrete tendency method, good-poor analysis, correlation analysis, stepwise regression analysis, Cronbach's alpha coefficient and factor analysis were used for evaluation the questionnaire items.
RESULTS:
The items in Chinese version ACIC showed as follows: 1) good sensitivity. The standard deviation of each item was more than 2 and the P value from the t test between the good and poor group was less than 0.001; 2) good representation. The correlation coefficients range of each item with the total score and each dimension score was 0.588-0.916; 3) high internal consistency. Cronbach's alpha coefficient for the total scale was 0.975, and for each dimension range was 0.854-0.936; 4) independent. Factor loadings were greater than 0.40 entries, and factor loadings ranged from 0.487-0.798; 5) importance. Stepwise regression analysis α(in)=0.01, α(out)=0.05) showed 34 items were kept in the equation.
CONCLUSION
Items of Chinese version ACIC retains ACIC 7 dimensions of 34 items in the original English version, which are sensitivity and typical with the internal consistency, independence and importance.
