1.Association Between Epicardial Atrioventricular Groove Fat Thickness and Prognosis of Patients With Dilated Cardiomyopathy

Objectives:To investigate the predictive value of epicardial fat volume(EFV)and atrioventricular groove fat thickness(AVGT)—morphological biomarkers of epicardial adipose tissue—for major adverse cardiovascular events(MACE)in patients with dilated cardiomyopathy(DCM).Methods:This study enrolled 216 DCM patients.EFV and AVGT were obtained from cardiac magnetic resonance imaging(CMR).Patients were divided into event-free group(n=142)and event group(n=74)based on MACE occurrence during follow-up.Receiver operating characteristic(ROC)curve analysis was used to determine optimal cutoff values.Survival differences were assessed using Kaplan-Meier analysis,Cox proportional hazards regression analysis was used to identify independent risk factors,and restricted cubic spline(RCS)models were used to evaluate dose-response relationships.Results:AVGT and EFV were significantly higher in the event group than in event-free group(both P<0.05).ROC analysis identified optimal MACE-predicting cutoffs as follows:AVGT≥7.74 mm(area under the curve[AUC]=0.57)and EFV≥78.6 ml(AUC=0.62).Kaplan-Meier analysis revealed significantly lower MACE-free survival rates in patients with AVGT≥7.74 mm and EFV≥78.6 ml(both P<0.05).Cox regression analysis confirmed that AVGT(HR=2.18,95%CI:1.34-3.54)and EFV(HR=1.81,95%CI:1.11-2.96)were independent MACE risk factors(both P<0.05)in this patient cohort.RCS models demonstrated the significant linear associations between EFV/AVGT and MACE risk(bothoverall P<0.05).Conclusions:EFV and AVGT,the non-invasive imaging biomarkers quantifying and characterizing fat distribution,are independently correlated with elevated MACE risk in DCM patients.These metrics serve as potential prognostic indicators,enriching risk stratification indicators for early identification of high-risk patients and guiding personalized medication strategies.
2.Preliminary study on the influence of the dimensional stability of 3D printed resin master model on the replication accuracy of implant replicas.

OBJECTIVES:
This study aimed to investigate the influence of the dimensional stability of 3D printed resin master model on the replication accuracy of implant replicas.
METHODS:
Ten digital impressions of patients undergoing continuous crowns or fixed bridge restoration supported by two implants were obtained, and resin models with implant replicas were 3D printed. Scanning rods were fixed on the replicas 3, 7, and 14 days after printing. The 3D, linear, and angular deviations of the scanning rods at different times were analyzed through Geomagic Wrap 2021 software.
RESULTS:
The position of the replicas shifted mesiolingually, in the same direction as the shrinkage of the model. From day 7 onward, the 3D, distance linear, and angular deviations of the replicas (scanning rod) significantly increased compared with those on the 3rd day (P<0.05). On the 14th day, the changes were even more pronounced, with the above deviations showing statistical significance (P<0.05) compared with those for the 3-day and 7-day groups. No statistical difference in height linear deviation was observed among the groups.
CONCLUSIONS
The insufficient dimensional stability of 3D printed resin models can lead to changes in the relative position and angle of the replicas, thereby affecting the accuracy of the replicas in recreating the implant's position. Complete manufacturing of prosthesis is recommended within 7 days after the model is printed.

3.Clinical Effect of Xiaozhi Hugan Capsules in Treatment of Patients with Non-alcoholic Steatohepatitis and Its Impact on Serum IL-6 and MCP-1

ObjectiveTo observe the clinical effect of Xiaozhi Hugan capsules in treating patients with non-alcoholic steatohepatitis (NASH) combined with phlegm-dampness and blood stasis syndrome and its effects on serum interleukin-6 (IL-6) and monocyte chemoattractant protein-1 (MCP-1). MethodsA total of 124 patients with NASH combined with phlegm-dampness and blood stasis syndrome who were admitted to the Department of Spleen, Stomach, and Hepatobiliary Diseases, the First Affiliated Hospital of Henan University of Chinese Medicine from July 2020 to December 2022 were selected. According to the random number table method, patients were randomly divided into an observation group (62 cases) and a control group (62 cases). The treatment duration was 6 months. The observation group received Xiaozhi Hugan capsules orally, while the control group received polyene phosphatidylcholine capsules. The efficacy indicators included alanine aminotransferase (ALT), aspartate aminotransferase (AST), controlled attenuation parameter (CAP), liver stiffness measurement (LSM), traditional Chinese medicine (TCM) syndrome scores (discomfort/dull pain/distending pain in liver region, fatigue, etc.), body mass index (BMI), waist-to-height ratio (WHtR), high-density lipoprotein cholesterol (HDL-C), total cholesterol (TC), triglycerides (TG), homeostatic model assessment for insulin resistance (HOMA-IR) [including fasting blood glucose (FBG) and fasting insulin level (INS)], free fatty acids (FFA), IL-6, and MCP-1. Adverse drug reactions were recorded. ResultsAfter treatment, the total effective rate in the observation group was 92.3% (48/52), while that in the control group was 75.5% (39/49). The total effective rate in the observation group was higher than that in the control group (χ2=5.339, P<0.05). After treatment, the TCM syndrome scores in both groups were significantly reduced (P<0.05), and the post-treatment scores in the observation group were better than those in the control group (P<0.05). After treatment, the levels of ALT, AST, TC, FFA, fasting insulin (FINS), HOMA-IR, MCP-1, IL-6, CAP, LSM, BMI, and WHtR were decreased (P<0.05) significantly in both groups, and the observation group showed superior improvement in the above indicators compared to the control group (P<0.05). The observation group exhibited significant reductions in TG and FBG (P<0.05) and an increase in HDL-C (P<0.05), while no significant changes were observed in the control group. The observation group was superior to the control group after treatment (P<0.05). No severe adverse reactions occurred in either group during the treatment. ConclusionXiaozhi Hugan capsules have significant clinical efficacy in treating patients with NASH combined with phlegm-dampness and blood stasis syndrome. It reduces hepatic steatosis, lowers liver stiffness, inhibits the expression of serum inflammatory factors, and alleviates liver inflammation. No obvious adverse reactions occur, suggesting it is suitable for clinical application.

5.Study on the prediction of Ki-67 expression level in nasopharyngeal carcinoma by multi-parameter MRI

Objective To explore the value of multi-parameter MRI in predicting the Ki-67 expression level in nasopharyngeal carcinoma(NPC).Methods The clinical and MRI data of 63 patients with pathologically confirmed NPC were prospectively collected.All patients underwent routine plain and enhanced nasopharyngeal MRI,diffusion weighted imaging(DWI),and arterial spin labeling(ASL)scans before treatment.Multivariate logistic regression analysis was used to identify the independent risk factors for the Ki-67 expression level.Results The degree of enhancement,the maximum blood flow(BFmax),and the minimum apparent diffusion coefficient(ADCmin)were independent risk factors for the Ki-67 expression level in NPC patients.The area under the curve(AUC)of the prediction model established based on these three factors was 0.920,with a sensitivity of 0.792 and a specificity of 0.897,respectively.Conclusion Multi-parameter MRI based on conventional enhancement,ASL,and DWI can effectively predict the Ki-67 expression level in NPC patients.

8.Mechanism of Youguiwan Regulating BMP-2/Smad Signaling Pathway to Promote Bone Formation in Postmenopausal Rats with Osteoporosis

ObjectiveTo observe the effects of Youguiwan on bone metabolism and bone morphogenetic protein-2 (BMP-2)/Smad signaling pathway in ovaries-removed rats with osteoporosis and study the mechanism of Youguiwan in the prevention and treatment of osteoporosis. MethodA postmenopausal rat model of osteoporosis was prepared by bilateral ovariectomy. The 40 female SD rats were randomly divided into five groups, including sham operation group, model group, alendronate sodium group (0.1 mg·kg-1), and high-dose and low-dose (5.36 and 2.68 g·kg-1) groups of Youguiwan. The drug was given seven days after modeling, once a day for 12 weeks. After the treatment, the changes in femur tissue structure were observed by micro-CT, including bone mineral density (BMD), bone volume/total volume (BV/TV), trabecular number (Tb.N), trabecular thickness (Tb.Th), bone surface/bone volume (BS/BV), and trabecular separation (Tb.Sp). Ossification was observed by saffrane-solid green staining, and serum levels of bone metabolism markers, including bone alkaline phosphatase (BALP), osteocalcin (BGP), type Ⅰ procollagen amino terminal propeptide (PINP), and tartrate-resistant acid phosphatase 5b (TRACP-5b), were determined by enzyme-linked immunosorbent assay (ELISA). The protein and mRNA expression levels of Runx2, BMP-2, and Smad1 in rat femur were detected by Western blot and Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). ResultCompared with the sham operation group, bone trabecula in the model group was sparse. BMD, BV/TV, Tb.N, and Tb.Th were decreased (P<0.05, P<0.01). BS/BV (P<0.05) and Tb.Sp were increased. The content of BGP, BALP, PINP, and TRACP-5b in serum was significantly increased (P<0.01). The mRNA and protein expressions of Runx2, BMP-2, and Smad1 in rat femur were significantly decreased (P<0.05, P<0.01). Compared with the model group, the number of bone trabeculae in the high-dose and low-dose groups of Youguiwan was increased, and the bone microstructure was improved. BMD, BV/TV, Tb.N, and Tb.Th were increased significantly (P<0.05, P<0.01), and BS/BV and Tb.Sp were increased. The content of bone metabolic markers decreased (P<0.05, P<0.01). ConclusionYouguiwan has certain preventive and therapeutic effects on postmenopausal osteoporosis, and its mechanism may be related to promoting bone formation by regulating the BMP-2/Smad signaling pathway.
9.Advances in molecular-targeted therapy for unresectable pancreatic cancer

Pancreatic cancer is one of the most prevalent malignant tumors of the digestive system, and its incidence and mortality rates are increasing year by year. Most patients with pancreatic cancer are unable to receive surgery due to the advanced stage. Although chemotherapy regimens based on gemcitabine and fluorouracil have prolonged the survival time of patients to some extent, some patients cannot tolerate chemotherapy and hence lose the opportunity for treatment. With the advent of the era of precision medicine, molecular-targeted therapy has exhibited an excellent therapeutic efficacy and has thus become one of the most important treatment techniques for tumors; however, due to the high heterogeneity of pancreatic cancer and its complicated tumor microenvironment, molecular-targeted therapy for pancreatic cancer has not achieved notable results. Therefore, it is imperative to seek new therapeutic targets and medications to overcome this issue. This article reviews the latest advances in the research on molecular-targeted therapy for unresectable pancreatic cancer based on common molecular targets and tumor immunity-related therapeutic targets, in order to provide new treatment strategies for patients with pancreatic cancer.
10.Meta-analysis of the correlation between ocular and neurological manifestations in 43 Chinese children with sialidosis type I

Objective:To investigate the correlation between macular cherry red spot (CS) and severity of neurological manifestations in Chinese children with sialidosis (SD) type I.Methods:A evidence-based medical study. "China", "Sialidosis" and "Sialidoses" were used as Chinese and English search terms. The literature was searched in CNKI, Wanfang and PubMed. The cases were all from China and matched the diagnostic criteria. According to the presence or absence of CS in the fundus, the SD children were divided into a group with CS (+) and a group without CS (-), and the correlation between the occurrence of ocular CS and neurological manifestations was compared with meta-analysis by RevMan 5.3 software.Results:Sixty-eight studies were initially retrieved according to the search strategy, and 17 studies were finally included, and 5 studies with CS+ and CS- were meta-analyzed. Among the 43 patients, 28 were male and 15 were female, with a median age of 12 years. Visual impairment was observed in 37 cases (90.2%, 37/41, 2 cases not recorded), and CS was present in 24 cases (55.8%, 24/43). The most common neurological manifestation was myoclonus (97.7%, 42/43), followed by cerebellar ataxia (95.1%, 39/41, 2 cases not recorded) and seizures (91.4%, 32/35, 8 cases not recorded). Pathogenic NEU1 gene mutations were detected in 42 cases and one case was undocumented. The incidence of seizure in group CS+ (100%, 20/20) was higher than that in group CS- (80%, 12/15). Meta-analysis showed that there was no statistically significant difference between the incidence of myoclonus or ataxia [relative risk ( RR)=1.13, 95% confidence interval ( CI) 0.79-1.63, P=0.49] and seizure ( RR=1.13, 95% CI 0.84-2.06, P=0.24) among the children in the CS+ and CS- groups. Conclusions:The incidence of ocular CS in Chinese children with type I SD was 55.8%. There was no correlation with neurological manifestations, however the incidence of seizure was significantly higher in patients with CS than in others without CS.
