1.A Study on the application of evidence-based clinical guidelines combined with "rain classroom" smart teaching in standardized residency training in endocrinology department

Objective:To investigate the role of evidence-based clinical guidelines combined with "rain classroom" smart teaching in standardized residency training in endocrinology department.Methods:The 100 trainees who underwent standardized residency training in the Department of Endocrinology at Xiamen Hospital from September 2022 to September 2023 were selected as the research subjects. They were divided into groups based on the parity of their admission batches, with even-numbered batches assigned to the control group and odd-numbered batches to the experimental group, with 50 trainees in each group. The control group received traditional teaching, while the experimental group was taught using evidence-based clinical guidelines combined with "rain classroom" smart teaching. Statistical comparisons were made between the two groups in terms of exit examination scores, teaching effectiveness, teaching satisfaction, and evaluation of trainees by teachers. SPSS 22.0 was used for t test and Chi-square test. Results:The experimental group demonstrated significantly higher scores in theoretical knowledge [(25.62±3.12) vs. (22.71±3.27)], case analysis [(25.18±3.24) vs. (22.24±3.35)], clinical operation [(25.01±3.19) vs. (22.12±3.29)], and daily performance [(8.12±1.01) vs. (7.21±1.13)] compared to the control group in exit examination ( P<0.001). Additionally, the experimental group showed superior performance in self-learning [(8.57±1.12) vs. (7.64±1.24)], clinical thinking [(8.61±1.16) vs. (7.78±1.28)], evidence-based thinking [(8.52±1.13) vs. (7.69±1.26)], clinical skills [(8.49±1.17) vs. (7.61±1.23)], communication skills [(8.53±1.18) vs. (7.67±1.29)], and literature review [(8.59±1.15) vs. (7.71±1.27)], with statistically significant differences ( P<0.05). Teaching satisfaction in the experimental group (96.00%) was significantly higher than that in the control group (82.00%) ( P=0.025). The pass rates rated by teachers were higher in the experimental group in medical history collection (94.00% vs. 80.00%), physical examination (96.00% vs. 82.00%), clinical thinking (92.00% vs. 76.00%), communication skills (94.00% vs. 80.00%), literature review (96.00% vs. 84.00%), specialized skills (94.00% vs. 78.00%), and diagnosis and treatment plan formulation (92.00% vs. 74.00%), with statistically significant differences ( P<0.05). Conclusions:The implementation of evidence-based clinical guidelines combined with "rain classroom" smart teaching in the standardized residency training in endocrinology department can significantly improve the exit examination scores and teaching satisfaction of trainees, teaching effectiveness, and evaluation of trainees by teachers. This teaching method has a positive role in improving the clinical comprehensive ability of trainees such as evidence-based medical thinking and standardized clinical diagnosis and treatment.

3.Ultrasound vector flow imaging combined with singular value decomposition filtering for depicting deep microvasculature flow velocity of liver

Objective To observe the value of ultrasound vector flow imaging(VFI)combined with singular value decomposition(SVD)filtering for depicting deep microvasculature flow velocity of liver.Methods Grayscale ultrasound,CDFI and contrast-enhanced ultrasound(CEUS)were prospectively performed in a patient with suspected liver hemangioma.Images of CEUS were dealt with SVD filtering.Cross-correlation algorithm was used to obtain images of VFI based on grayscale ultrasound,original CEUS and SVD filtered CEUS,respectively,and the ability of the above images for depicting liver microvascular flow direction and velocity were compared.Results The signal-to-noise ratio(SNR)of liver grayscale ultrasound,original CEUS and SVD filtered CEUS images was 7.56,17.65 and 22.43 dB,respectively,while their contrast-to-issue ratio(CTR)was 1.12,7.56 and 16.34 dB,respectively.Compared with VFI based on grayscale ultrasound and original CEUS,VFI based on SVD filtered CEUS could display faster velocity and more uniform direction of blood flow.Before and after SVD filtering,liver microvascular flow velocity measured with VFI was 1.91(0.81,4.11)and 6.83(4.25,9.41)mm/s,respectively,which were significantly different(Z=-10.671,P<0.001).Conclusion Combined with SVD filtering could significantly improve the efficiency of VFI for depicting liver deep microvasculature flow velocity.
4.Evaluation of subjective scale in allergen nasal provocation test.

Objective:To compare the clinical value of visual analogue scale (VAS), Lebel scale and total nasal symptom scores (TNSS) in evaluating nasal allergen provocation test (NAPT). Methods:A total of 151 patients suspected of allergic rhinitis admitted to the Department of Otolaryngology-Head and Neck Surgery of our hospital from April 2020 to September 2020 were included, of which 76 were positive for house dust mites and 75 were negative for allergens. Nasal airway resistance(NAR) was measured by active anterior nasal manometry. Nasal symptoms were evaluated by VAS, Lebel and TNSS. House dust mite allergen was used for NAPT by spray method. An increase≥40% in NAR was used as the gold standard for objective evaluation of NAPT. ROC curves of VAS, Lebel and TNSS were drawn to compare the evaluation effectiveness of different subjective evaluation methods, and the optimal critical point of each ROC curve was obtained. Results:With NAR increased by ≥40% as the gold standard, the area under ROC curve of VAS was 0.884, and the sensitivity and specificity were 97.75% and 80.65%, respectively. The area under ROC curve of Lebel was 0.773, and the sensitivity and specificity were 68.54% and 75.81%, respectively. The area under ROC curve of TNSS was 0.792, and the sensitivity and specificity were 68.54% and 79.03%, respectively. There was no significant difference between Lebel and TNSS(P>0.05). The VAS differed significantly from Lebel and TNSS(P<0.05). The Kappa values of VAS, Lebel, TNSS and NAR were 0.803, 0.432 and 0.459, respectively. Conclusion:The VAS, Lebel, TNSS subjective scale and NAR are consistent in evaluating the efficacy of NAPT, with the VAS assessment showing highest consistency with NAR. As objective assessment instruments are not widely used in China, subjective assessment method could be adopted to evaluate the efficacy of NAPT in clinical practice, and VAS scale is recommended as a priority.

5.Susceptibility factors of occurrence of allergic rhinitis

Objective:To analyze the genetic and environmental factors that may contribute to the development of allergic rhinitis (AR) in patients, and provide a scientific basis for formulating intervention measures.Methods:This is a case-control study. A cross-sectional survey was conducted on AR patients who received treatment at The First Affiliated Hospital of Nanchang University from June 2017 to March 2019 using a self-designed questionnaire on AR susceptibility factors. Logistic regression analysis of AR susceptibility factors was performed.Results:There were 242 patients in the AR group and 258 patients in the non-AR group (control group). The average age of all patients was (13.16 ± 5.63) years. 56.8% of included patients were male. Multivariate logistic regression analysis showed that AR onset was associated with neonatal jaundice ( OR = 6.043), respiratory infections within 6 months after birth ( OR = 8.123), diarrhea ( OR = 3.868), eczema ( OR = 4.540), living in urban areas ( OR = 2.477), home decoration after birth ( OR = 3.042), moldy ceilings or walls ( OR = 38.255), home gardening ( OR = 3.752), and exposure to smoking environments ( OR = 2.574). Conclusion:The etiology of AR is complex, and its onset is influenced by various factors such as genetic factors, environmental factors, and related disease factors. It is necessary to strengthen the promotion and education of AR prevention knowledge, pay attention to environmental hygiene, and take personal protection measures.
6.Preliminary exploration on the formative assessment model in the integrated curriculum under the network environment

Curriculum integration has become the core content of the current medical education reform.With the rapid development of information technology,how to reflect the teaching and learning effects and to further improve them,is one of the difficulties in the reform of the integrated curriculum.In view of the problems of single form of assessment model,separate classroom teaching and extracurricular teaching evaluation,low utilization of network and electronic teaching resources,we make full use of network methods to carry out reform on the current formative teaching evaluation system.With the purpose of formative assessment and evaluation,the "Blackboard network teaching system" of the curriculum has been built,and the evaluation indexes of classroom teaching and extracurricular teaching have been established.And at the same time,the formative assessment index has been enriched and perfected in a diversified form,and the formative assessment model adapted to the integrated curriculum is preliminarily explored.
7.Influence of moist healing dressing on comfort degree and complications in patients with nasal ;packing for epistaxis patients

Objective To investigate the moist healing dressing nasal packing for epistaxis patients′physiological comfort and complications. Methods 70 patients of epistaxis patients with nasal packing by drawing lots were divided into A, B two groups of 35 cases. Group A used Vaseline gauze, and group B used moist healing dressings of composite fillings. Comparison of the two groups of pain, nasal odor, sleep disorders, hemostatic effect, nasal mucosa reaction, fever, nasal adhesions were conducted. Results Patients when nasal packing group A pain, sleep disorders and nasal odor scores were (9.80 ± 0.12), (7.10 ± 0.33) and (9.44 ± 0.25) points, group B were (3.82 ± 0.16), (3.22 ± 0.16) and (0.37 ± 0.09) points. Group A scored significantly higher than group B, the difference was statistically significant (t=-179.97,-7.22,-7.33, P <0.05). Group A fever and nasal adhesions were 33 cases and 6 cases respectively, while those figures in group B both were 0, the difference was statistically significant (χ2=62.43, 6.56, P<0.05). Conclusions Nasal packing varying degrees of discomfort in patients, the use of moist healing dressings for epistaxis targeted composite filling, can significantly increase the patients ’ physiological comfort and reduce the complications of nasal packing.
8.Clinical observation and nursing care of high dose hormone therapy in patients with uveitis

Objective To explore the nursing program of large dose hormone therapy for the treatment of uveitis patients , and to provide the reference for clinical nursing program .Methods A total of 196 patients with uveitis were randomly divided into observation group and control group with 98 cases 136 eyes each group by the random number table method .All patients were treated with high-dose hormone therapy .The control group received routine nursing care , while the observation group was given comprehensive nursing intervention measures.The clinical efficacy and recurrence rate of two groups were compared .Results After treatment, the total effective rate of observation group was 90.44%, and it was 89.71%in the control group (χ2 =0.041,P>0.05).Over 1 year of follow-up, the recurrence rate in the observation group was 5.15%, while it was 30.88%in the control group, and the recurrence rate of the observation group was significantly lower than that of the control group (χ2 =30.381,P<0.05).Conclusions During the process of high dose hormone therapy of uveitis patients , rigorous observation and comprehensive nursing interventions can help clinical treatment effect, reduce the recurrence rate of uveitis , and is worth of further application .
9.Clinical observation of the adverse effects of standardized dust mite allergen preparation in the treatment of allergic rhinitis.

OBJECTIVE:
To observe the adverse effects of specific immunotherapy (SIT) with standardized dust mite allergen preparation in the treatment of allergic rhinitis.
METHOD:
Three hundred and eighty-six patients with allergic rhinitis who received subcutaneous SIT with a standardized dust mite allergen preparation were enrolled in this study. The patients were treated for at least 15 weeks,adverse effects after each injection from dosing phase to maintenance phase were recorded and incidence of adverse effects were analyzed.
RESULT:
Of all the patients,adverse reactions occurred in 42 patients (10. 9%),10 local reactions (2. 6%) and 36 systemic side effects (9. 3%) which included 34 mild ,1 moderate and 1 severe side effects (no fatal) were reported respectively. None had anaphylactic shock. Among three treatment options, incidence of routine program was the highest (21.1%),followed by adult cluster program (11. 9%), adverse effects of children cluster program was the least (1. 5%). The adverse effects often happened in the middle and late phase of does addition period and early phase of maintenance period.
CONCLUSION
SIT with standardized dust mite allergen preparation in the treatment of allergic rhinitis is a safe and effective treatment by complying with the guidelines and taking specific interventions.

10.Analysis of aeroallergen spectrum in patients with allergic rhinitis in Nanchang area

Objective To determine the distribution of allergens in patients with allergic rhinitis in Nanchang area and provide the clinical reference for management strategies with regional character.Methods Patients with symptoms of allergic rhinitis were given skin prick test (SPT) from March 2012 to May 2013.Among the patients,the distribution of all allergens was analyzed.The positive rate was compared with gender,age,occupation,season,course of disease and so on.SPSS 17.0 software was used to analyze the data.Results Among 5 654 patients,the total positive rate of SPT was 56.07%,with dermatophagoides pteronyssinus (46.94%),dermatophagoides farina (35.99%),blomia tropicalis (31.34%) as the major allergens.With age increasing,the positive rate of allergen decreased.There was statistically significant difference (x2 =261.372,P < 0.01).There was significant difference of the positive rate in different occupations (x2 =170.859,P <0.01),the highest rate was found in student(66.12%) and the lowest was found in farmer (41.56%).There was significant difference of variation with months (x2 =227.333,P < 0.01),the most in October,68.88%.There was significant difference of the positive rate of dust mites in different seasons (x2 =204.343,P < 0.01),the highest in autumn,the lowest in winter.There was no correlation between the positive rate and course of disease (P > 0.05).Conclusions Dust mites were identified as the most common allergens among patients with allergic rhinitis in Nanchang area.Occupation is one of the influeneing factors for the positive rate.
