Objective To investigate the in vitro antibacterial effect of Radix Sophorae Flavescentis on tigecycline-resistant carbapenem-resistant Klebsiella pneumonia(CRKP)and elucidate how Radix Sophorae Flavescentis restores the sensitivity of CRKP to tigecycline through the efflux pump pathway.Methods The CRKP standard strain BAA1705 was induced to be tigecycline-resistant using the microdilution broth method,and the expression of efflux pumps was verified by efflux pump inhibitors.The fractional inhibitory concentration index(FIC)was calculated using the microdilution checkerboard method to determine the minimum inhibitory concertration(MIC)of the two drugs in combination.Fluorescent real-time quantitative PCR(qRT-PCR)was used to analyze the expression of the AcrB gene before and after Radix Sophorae Flavescentis treatment,and the e?ux pump activity was validated by the Rhodamine 6G e?ux assay.Results The e?ux pump inhibition experiment on the artificially induced T-BAA1705 strain showed overexpression of e?ux pumps.The combined antibacterial test results indicated synergy between Radix Sophorae Flavescentis and tigecycline(FIC=0.375 2).The qRT-PCR results showed that the AcrB levels in the Radix Sophorae Flavescentis monotherapy group were lower than in the untreated group(0.018±0.002 vs 0.040±0.001),and the AcrB levels(0.009±0.001),RamA levels(0.184±0.003)and MarA(0.013±0.001)levels in the combination therapy group were all lower than in the untreated group(0.040±0.001,0.387±0.016,0.125±0.007),while the AcrR level was higher than in the untreated group(0.388±0.001 vs 0.288±0.001),and the differences were statistical significance(t=12.28～47.12,all P<0.05);the e?ux pump activity in the Radix Sophorae Flavescentis monotherapy group and the combination therapy group was lower than in the untreated T-BAA1705(60.667±0.448,60.267±0.376,65.133±0.296),and the differences were statistical significance(t=8.31,10.17,all P<0.05).Conclusion Radix Sophorae Flavescentis can restore the sensitivity of tigecycline-resistant CRKP to tigecycline and effectively inhibit bacterial growth when used in combination with tigecycline.The mechanism may involve Radix Sophorae Flavescentis inhibiting the expression of the AcrAB-TolC e?ux pump.

An effective herbal medicinal prescription of Shengjiang Xiexin decoction (SXD) was used in treating the inflammatory bowel disease in clinic. In this study, an ultrafast liquid chromatography-tandem mass spectrometry (UFLC-MS/MS) method was developed to separate and to simultaneously determine 14 major active ingredients in SXD. Chromatographic separation was successfully accomplished on an Acquity BEH C18 (100 mm×2.1 mm, 1.7 μm) column using gradient elution with 0.1% () formic acid water (A) and 0.1% () formic acid in methanol (B). Negative and positive electrospray ionization tandem mass spectrometry was used to detect the 14 analytes using its selective reaction monitoring (SRM) mode. A good linear regression relationship for each analyte was obtained over the range from 3.88 ng/mL to 4080 ng/mL. The precision was evaluated by intra- and inter-day assays with a relative standard deviation (RSD) of less than 6.25%. The recovery measured at three concentration levels varied from 98.72% to 103.47%. The overall limits of quantification (LOQ) ranged from 2.05 ng/mL to 4.72 ng/mL. The method was successfully implemented in the qualitative and quantitative analyses of the 14 chemical constituents in SXD. The results showed that the developed UFLC-MS/MS method was linear and accurate. The method could be used reliably as a quality control method for SXD.

AIM To observe the therapeutic effects of Shengjiang Xiexin Decoction (Zingiberis recens Rhizoma,Zingiberis Rhizoma,Coptidis Rhizoma,etc.) on irinotecan (CPT-11)-induced delayed diarrhea in colorectal carcinoma mice and to discuss its possible action mechanism.METHODS The AOM/DSS-induced female colorectal carcinoma mice were randomly divided into normal group,model group and Shengjiang Xiexin Decoction group.The Shengjiang Xiexin Decoction group was intragastrically administered with Shengjiang Xiexin Decoction,the normal group and the model group were intragastrically administered with normal saline.The diarrhea index and rectum pathologic morphology were measured,and the β-glucuronide activity,IL-15 content and UGT1A1 expression were detected.RESULTS The diarrhea index of Shengjiang Xiexin Decoction group was significantly lower than that of the model group,which might be related to the significant inhibition of β-glucuronide activity,and sig-nificant improvement of IL-15 content and UGT1A1 expression.CONCLUSION Shengjiang Xiexin Decoction shows therapeutic effects on irinotecan-induced delayed diarrhea in AOM/DSS-induced colorectal carcinoma mice.

Objective To observe the effects of ziprasidone and risperidone on schizophrenia patients and the change of serum leptin and adiponectin levels .Methods Totally 80 cases of schizophrenia patients were randomly divided into ziprasidone group and risperidone group ,which were treated for 8 weeks .Measure the positive and negative symptoms scale (PANSS) score and body weight of that number ,leptin and adiponectin at baseline ,treatment 4 weeks and 8 weeks respectively for patients ,at the end of the experiment ,the results for statistical analysis .Results Two groups of 4 ,8 weeks after treatment scores compared with baseline scores dropped significantly ,the difference was statistically significant(P<0 .05) .Risperidone group after treatment ,leptin levels significantly increased body mass index ,and adiponectin levels significantly decreased ,compared with the baseline before treatment was statistically significant difference(P<0 .05) .Conclusion Ziprasidone and risperidone in treatment of schizophrenia have similar efficacy .Ziprasidone has no significant effect on body weight ,leptin and adiponectin levels in treatment of schizophrenia patients . However ,risperidone has a significant effect ,long-term use should pay attention to the side effects .

Objective To observe the effects of ziprasidone and risperidone on patients with schizophrenia and their influence on bloodglucoseandlipids.Methods 96patientswithschizophrenicenrolledinthestudywererandomlydividedintotwogroups,zi‐prasidone and risperidone group ,and both were treated for 8 weeks .Their blood glucose ,blood lipid of base line and at the end of the 4th ,8th week were determined respectively .Results The positive and negative symptoms scores of the two groups by using Positive and Negative Syndrome Scale(PANSS) before and after treatment were not statistically different(P> 0 .05) .Compared with the baseline scores ,scores at the end of 4th and 8th week in both ziprasidone and risperidone groups significantly decreased(P<0 .05) ,but there was no statistically significant difference between the two groups(P>0 .05) .After 8 weeks′ treatment ,the ef‐fective rate was 91 .7% in ziprasidone groups and 89 .6% in risperidone group .There were no significant differences between the two groups(P>0 .05) .The blood lipids and glucose levels were less increased after ziprasidone treatment ,but was not statistically significant(P>0 .05) .The blood lipids and glucose levels significantly increased after risperidone treatment(P<0 .05) .Conclusion Ziprasidone and risperidone had the same effect on schizophrenia .Ziprasidone had no effect on blood glucose and lipids in schizo‐phrenic patients ,while risperidone could increase blood glucose and lipids level ,we should pay attention to the side effects of long‐term use .

Nine flavonoids were isolated and identified as luteolin (1), luteolin-4'-O-beta-D-glucoside (2), acacetin-7-O-beta-D-glucoside (3), rutin (4), acacetin (5), quercetin (6), quercetin-3-O-beta-D-glucoside (7), kaempferol-3-O-beta-D-glucoside (8), Isorhamnetin-3-O-beta-D-glucoside(9) from Lysimachia paridiformis var. stenophylla, and all these compounds were isolated from this plant for the first time.

BACKGROUND: The hot environmental condition of the isolated severe acute respiratory syndrome (SARS) ward makes the protein transformation of the medical personnel accelerated. The dietary protein should make up the special consumption and improve the immunity in addition to guaranteeing the normal metabolism for the purpose to promote the organic endurance and resist the communicable disease.OBJECTIVE: To study the scientific mode of quality and quantity of protein supply for the medical staff working in the SARS ward.DESIGN: A cross section analysis.SETTING: Trophological Department, General Hospital of Chinese People's Armed Police Forces.PARTICIPANTS: The 64 healthy doctors and nurses, 22 males and 42females, aged 29 averagely, worked in the SARS ward from May 1st to 20th 2003, were the volunteers to join the observation.METHODS: The weighing method was applied to measure the quantity of various food supplied by recipe and actually taken and the nutrition software was used to analyze the quantity of protein and amino acid contained in the food. The quantity of protein supply (taking male as standard) calculated according to the heavy consumption, 1.5 g/kg.d, in the hot environment was compared with the estimated consumption, male (108.00±12.63) g and female (85.21±9.55) g, and with the actual intake analyzed by computer. The corrected scoring of digestibility was employed to evaluate the reasonability of intake of various essential amino acids. The quantity of essential amino acids contained in every gram of protein of recipe/the amino acids contained in Reference Mode of Chinese Food × 93% being higher than 1.0 was said to be the quality protein meeting the need of human body.MAIN OUTCOME MEASURES:① The quantity and composition of protein supplied by the recipe. ② The quantity and composition of protein actually taken by the working staff.RESULTS: All of the 64 medical working staff entered the result analysis.① The protein supply of recipe (daily average) could totally meet the need of estimated consumption of male and female staff [(157.36±10.43) g,(108.00±12.63) g, (85.21±9.55) g, t=9.344, 18.278, P ＜ 0.01] and the need of actual intake [(157.70±58.14) g, (111.02±31.77) g, t= -0.015, 3.795, P＞ 0.05, P ＜ 0.01]. ② The corrected scores of 9 amino acids of digestibility, (1.9-3.1), were all higher than 1.0 and the percentage of amino acids was in accordance with the food mode of Chinese people.CONCLUSION: The protein quantity and quality of recipe can meet the special need of medical staff working in the hot environment with the heavy physical consumption in the SARS ward and can produce the best biological effect, being scientific.
