3.Clinical evaluation of elderly patients with sarcopenia receiving Shenling baizhu powder combined with nutrition and exercise intervention

OBJECTIVE To explore the safety, effectiveness, and cost-effectiveness of elderly patients with sarcopenia receiving Shenling baizhu powder combined with nutrition and exercise intervention, providing a reference for rational clinical drug use. METHODS A total of 237 elderly sarcopenia patients were randomly assigned to an observation group (118 cases) and a control group (119 cases). Both groups of patients received nutrition and exercise intervention; the observation group added Shenling baizhu powder (6 g each time, three times daily) on this basis. The safety, effectiveness, and cost-effectiveness of the two plans were compared after 3 months. RESULTS Both groups of patients completed the follow-up. Before intervention, no significant difference was observed in skeletal muscle index (SMI), grip strength, and 6-minute walk test (6MWT) speed between the two groups (P>0.05). After intervention, the grip strength of the patients in the observation group was significantly greater than that of the control group (25.05 kg vs. 23.18 kg, P<0.01); the treatment response rate of sarcopenia, SMI, and 6MWT speed were higher than those of the control group, butthe differences were not statistically significant (P>0.05). The adverse reaction/event rate of the patients in the observation group was lower than that of the control group (14.41% vs. 16.81%, P=0.611), but the difference was not statistically significant. Compared with the control group’s plan, the cost of the observation group’s plan was higher (981.25 yuan vs. 913.94 yuan), and the effect was better (effectiveness rate: 0.618 6 vs. 0.563 0), with an incremental cost-effectiveness ratio of 1 210.61 yuan. The results of the sensitivity analysis were consistent with the cost-effectiveness analysis results. CONCLUSION Elderly patients with sarcopenia who receive Shenling baizhu powder combined with nutrition and exercise intervention can significantly strengthen grip strength without increasing the incidence of adverse reactions/events. Compared with the control group plan, the observation group needs to spend an additional 1 210.61 yuan for each additional effective patient with sarcopenia.
4.Preparation Process, Quality Evaluation and Antipyretic Pharmacodynamics of Bupleurum Nanoemulsion

OBJECTIVE
To screen the prescription and preparation method of Bupleurum nanoemulsion, and evaluate its quality, study the antipyretic effect.
METHODS
The emulsifier and co-emulsifier of the nanoemulsion were preliminarily screened, and then the prescription was screened by pseudo-ternary phase diagram. The quality evaluation of the appearance, particle size distribution, structure type, stability and content of the prepared Bupleurum nanoemulsion was performed. Wistar rats were further randomly divided into blank control group, model control group, positive control group(aspirin group), Bupleurum nanoemulsion high-dose, medium-dose and low-dose groups(18.00, 9.00, 3.00 g·kg−1). Except for the blank control group, the pathological model of fever rats was prepared in the other groups. According to the scheduled experimental requirements, rats in each group were given the corresponding drugs. And the temperature changes of rats in each group were recorded at 0.5, 1, 1.5, 2, 3 h to observe the antipyretic effect of Bupleurum nanoemulsion.
RESULTS
The best prescription of Bupleurum nanoemulsion: Tween-80 6 g and n-butanol 3 g, Bupleurum extract dissolved in pure water as water phase 20 mL, Bupleurum oil as oil phase 2 g. At room temperature, the Bupleurum nanoemulsion was a yellow-brown clear and transparent liquid, O/W nanoemulsion, with an average particle size of (77.21±3.66)nm, polydispersity index of 0.28±0.04, Zeta potential of (–18.81±1.42)mV, and saikosaponin content of 3.071 mg·mL−1, with good stability. In animal experiments, compared with the model control group, the rectal temperature of aspirin group and Bupleurum nanoemulsion high-dose group was significantly lower after the first administration(P<0.01), the rectal temperature of Bupleurum nanoemulsion middle-dose group was significantly lower after the first administration 2, 3 h(P<0.01).
CONCLUSION
The Bupleurum nanoemulsion is transparent and stable, and it has good antipyretic effect on fever rat model.
5.Evidence summary on preventive management of high output and dehydration in ileostomy patients

Objective To retrieve, evaluate, and summarize evidence on preventive management of high output and dehydration in ileostomy patients from both domestic and international sources. Methods Based on the "6S" evidence model, a top-down approach was employed to retrieve evidence on prevention and management of high output and dehydration in ileostomy patients. Databases searched included BMJ best practice, UpToDate, National Guideline Clearinghouse (NGC), National Institute for Health and Care Excellence (NICE), Scottish Intercollegiate Guidelines Network (SIGN), Registered Nurses' Association of Ontario (RNAO), Wound Ostomy and Continence Nurses Society (WOCNS), World Council of Enterostomal Therapists (WCET), Medlive Clinical Guidelines website, Joanna Briggs Institute (JBI), Cochrane Library, Embase, PubMed, Web of Science, CNKI, Wanfang Data, China Biology Medicine (CBM), and VIP Database. The search period was from January 2019 to April 2024. Results A total of 13 articles were included after retrieval and screening, comprising 2 guidelines, 2 expert consensuses, 4 systematic reviews, 4 evidence summaries, and 1 clinical decision aid. After translation, summarization, and organization of the included articles, 19 pieces of evidence across 11 categories were formulated, focusing on four items: definition and risk factors of high output in ileostomy, prevention and management strategies related to high output in ileostomy, prevention and management strategies related to dehydration in high-output ileostomy, and follow-up strategies for prevention of high output and dehydration after ileostomy surgery. Conclusion The evidence summarized based on the "6S" evidence model for prevention and management of high output and dehydration in ileostomy patients can provide a reference for clinical practice among healthcare professionals and medical decision-makers, thereby enhancing nursing quality and reducing patient rehospitalization rates.
6.Current status and influential factors of self-management ability in patients with systemic lupus erythematosus

Objective:To investigate the current status of self-management ability in patients with sys-temic lupus erythematosus(SLE),and to analyze the related factors affecting the self-management ability of SLE patients.Methods:A total of 180 SLE patients who were selected from the outpatient department and ward of the Department of Rheumatology and Immunology of a Tertiary Hospital in Beijing from January 2024 to March 2024.General information questionnaire,SLE self-management ability assessment scale,general self-efficacy scale(GSES)and family concern index questionnaire(APGAR)were used for questionnaire investigation,so as to investigate the current status and related influencing factors of self-management ability in patients with SLE.Results:A total of 170 questionnaires were effectively collected,and the total score of self-management ability was(90.94±14.26)points,of which 103 pa-tients were 89-110 points,accounting for 60.6%;60 patients were 67-88 points,accounting for 35.3%;7 patients were 0-66 points,accounting for 4.1%;The results of univariate analysis showed that personal monthly income,follow-up frequency,family caring index,self-efficacy and SLE self-management ability score had statistical significance(P<0.05).Spearman rank correlation analysis showed that family caring index,self-efficacy scores were positively correlated with the scores of various dimensions and the total score of SLE self-management scores(P<0.001).Multivariate Logistic regres-sion analysis showed that family caring index(OR=1.503,95%CI=1.186-1.906),self-efficacy(OR=1.103,95%CI=1.03 8-1.172),personal monthly income of 5 000-8 000 yuan/month(OR=0.120,95%CI=0.022-0.645)and 1-2 weeks return frequency(OR=0.044,95%CI=0.003-0.575)were significant influencing factors for SLE patients'self-management ability.Conclusion:The re-sults of this study indicate that patients with SLE have a good level of self-management ability.In the process of chronic disease management,medical staff should formulate detailed and layered intervention measures to further improve self-management ability with SLE patients,and at the same time,help SLE patients establish good family caring index and patient self-efficacy,which is conducive to improving self-management ability of SLE patients,so as to effectively promote disease management and improve the quality of life.
7.The predictive value of CT pulmonary angiography combined with echocardiography for the degree of pulmonary hypertension

【Objective】 To compare the value of CT pulmonary angiography (CTPA) and echocardiography in predicting the degree pulmonary hypertension (PH). 【Methods】 Fifty-four patients in our hospital who underwent right heart catheterization, CTPA and echocardiography for suspected or confirmed PH from November 2013 to April 2021 were retrospectively recruited. Pulmonary artery systolic pressure (PASP) and mean pulmonary artery pressure (PAMP) were measured by right heart catheterization. According to PAMP, the patients were divided into non-PH group and mild, moderate, and severe PH groups. The three-dimensional model of the pulmonary artery was obtained by computer-aided segmentation, and the best fitting circle diameter (Dfit), inscribed circle diameter (Dmin), circumscribed circle diameter (Dmax), hydraulic diameter (Dh), cross-sectional area (Area), circumference (Scf), and the largest area and largest short diameter of the right ventricle were measured. Echocardiography was used to estimate PASP. The differences in the above parameters between different PH groups were compared, the correlations of the above parameters with PASP and PAMP were analyzed, and statistically significant indicators were included to establish three predictive models of PAMP (Model 1: CTPA pulmonary artery; Model 2: CTPA pulmonary artery+CTPA right ventricle; Model 3: CTPA pulmonary artery+CTPA right ventricle+echocardiography), and receiver operating characteristic (ROC) curves were used to compare the diagnostic performance of the three models. 【Results】 Some parameters of main pulmonary artery (Dfit, Dmin, Dmax, Dh, Area, Scf), right pulmonary artery (Dmax, Dh, Area, Scf), left pulmonary artery (Dfit), and right ventricular short diameter increased with the increase of PAMP (P<0.05). Dfit, Dmin, Dh, and area of main pulmonary artery, right pulmonary artery and left pulmonary artery were positively correlated with PASP and PAMP (P<0.05). Right ventricular short diameter and right ventricular maximum area were positively correlated with PASP (P<0.05), and right ventricular short diameter was positively correlated with PAMP (P<0.05). The estimated value of pulmonary artery systolic blood pressure in echocardiography was positively correlated with PASP and PAMP (P<0.05). Model 1, Model 2 and Model 3 could all be used to identify mild PH and moderate PH, among which Model 3 had the best performance in identifying non-PH and mild PH, moderate PH and severe PH. 【Conclusion】 CTPA and echocardiography are helpful in assessing the degree of PH, and the combination of the two has better accuracy in distinguishing non-PH from mild PH, moderate and severe PH.
8.Drug hypersensitivity syndrome induced by propylthiouracil: One case report and literature review

This article reports a case of a 38-year-old female who developed drug hypersensitivity syndrome one month after receiving propylthiouracil. The patient showed improvement with adalimumab, corticosteroids, intravenous immunoglobulin, and plasma exchange. Propylthiouracil is a rare medication associated with drug hypersensitivity syndrome. Additionally, during the follow-up after discharge, rapid changes in thyroid autoantibodies and thyroid function were observed in this patient. By analyzing the progression of this case and reviewing literature, it aims to enhance clinical understanding and management.
9.Construction and in vitro evaluation of an LNP system for mRNA delivery

Objective To construct lipid nanoparticles DLin-LNP for mRNA delivery. Methods DLin-LNP was prepared by thin film hydration method, and DLin-LNP/mRNA was further constructed by using EGFP-mRNA as model drug. The particle size, zeta potential, and appearance morphology were measured. Furthermore, the intracellular distribution and transfection of DLin-LNP/mRNA in RM-1 cells was investigated by laser scanning confocal microscope. Results DLin-LNP was successfully prepared. The average particle size was about (151.1±2.1) nm, the no-load potential was (23.7±0.5) mV. The cytotoxicity of DLin-LNP was far lower than that of the commercially available liposomal Lipo8000. The results of transfection experiment indicated that DLin-LNP has high transfection efficiency for mRNA delivery with low cytotoxicity and good stability. Conclusion DLin-LNP could become a potential mRNA vector for gene therapy.
10.Application status and obstacle factors of nursing evidence movement of oral exercise intervention in premature infants

Objective:To comprehensively evaluate the clinical application status of oral movement intervention in preterm infants, analyze the obstacles in the process of clinical application of the best evidence and formulate countermeasures, so as to provide reference for clinical evidence transformation.Methods:Based on the theoretical guidance of the "Clinical Application Model of Evidence" of the Joanna Briggs Institute (JBI) Evidence-Based Health Care Center in Australia, 12 pieces of the best evidence for oral movement intervention in premature infants were included, and 13 review indicators were formulated. From May to July 2021, a status review was conducted in Department of Neonatology in Suzhou Hospital Affiliated to Nanjing Medical University. The Ottawa Model of Research Use (OMRU) was used to analyze the barriers and facilitators in the process of evidence application and formulate effective intervention strategies and action plans.Results:Among the 13 reviewed indicators, only 2 items had a compliance rate of 100%, 2 items had a compliance rate of more than 60%, 6 items had a compliance rate of less than 60%, and 3 items had a compliance rate of 0. The main obstacle factors were lack of standardized process and assessment tools, lack of oral motor intervention related knowledge and training for preterm infants, increased clinical workload due to evidence transformation and the low level of knowledge and action among medical staff. The main promoting factors were support from managers for the development and reform of evidence-based learning, good learning atmosphere for doctors and nurses, departments with material and hardware conditions for evidence transformation, effective evidence, and parents' willingness to accept the reform. Through analysis, countermeasures were drawn up to formulate feasible and suitable standardized procedures and introduce assessment tools. Managers formulated and implemented incentive policies, adopted various training methods, strengthened quality supervision in the process of evidence transformation, timely gave feedback of progress results and improved human resource allocation.Conclusions:There is a certain gap between the best evidence of oral movement intervention in premature infants and the current clinical nursing practice. It is necessary to formulate corresponding countermeasures according to the obstacle factors and promoting factors, promote the evidence transformation and constantly promote the evidence-based practice of oral movement intervention in premature infants.
