1.Depressive symptoms and associated factors among middle school and college students from 2021 to 2023 in Hunan Province

Objective:
To investigate the current status and trends of depressive symptoms among middle school and college students in Hunan Province, and to explore the primary related factors of depressive symptoms, so as to provide a scientific basis for strengthening mental health among students.
Methods:
A total of 279 382 students in Hunan Province were selected through a stratified cluster random sampling method from 2021 to 2023. National Survey Questionnaire on Common Diseases and Health Influencing Factors among Students was adopted for the survey, and the Center for Epidemiological Studies Depression Scale was used to assess their depressive symptoms. The χ 2 test and trend χ 2 test were used to analyze depressive symptoms prevalence and trends, and multivariable Logistic regression was used to analyze the related factors of depressive symptoms.
Results:
The prevalence of depressive symptoms among students in Hunan Province from 2021 to 2023 were 19.66%, 20.17% and 21.47%, respectively, showing an upward trend ( χ 2 trend =9.07, P <0.01). In addition, the results of the multivariable Logistic regression analysis showed that students with healthy diet ( OR=0.43, 95%CI =0.40-0.45), adequate sleep ( OR=0.88, 95%CI =0.86-0.90), and acceptable screen time ( OR=0.61, 95%CI =0.60-0.62) had lower risks in depressive symptoms detection, while students with smoking ( OR= 1.95, 95%CI =1.88-2.02), secondhand smoke exposure ( OR=1.33, 95%CI =1.30-1.36) and Internet addiction ( OR= 4.19 , 95%CI =4.05-4.34) had higher risks in depressive symptoms detection, with differences in the degree of association among different genders, educational stages and urban rural groups ( OR=0.40-6.04, Z =-12.69-11.98) ( P <0.05).
Conclusions
There is an increasing trend of depressive symptoms among middle school and college students in Hunan Province from 2021 to 2023.Targeted depression prevention measures should be taken for students with different demographic characteristics to promote their mental health.
2.4 Weeks of HIIT Modulates Metabolic Homeostasis of Hippocampal Pyruvate-lactate Axis in CUMS Rats Improving Their Depression-like Behavior

ObjectiveTo investigate the role of 4-week high-intensity interval training (HIIT) in modulating the metabolic homeostasis of the pyruvate-lactate axis in the hippocampus of rats with chronic unpredictable mild stress (CUMS) to improve their depressive-like behavior. MethodsForty-eight SPF-grade 8-week-old male SD rats were randomly divided into 4 groups: the normal quiet group (C), the CUMS quiet group (M), the normal exercise group (HC), and the CUMS exercise group (HM). The M and HM groups received 8 weeks of CUMS modeling, while the HC and HM groups were exposed to 4 weeks of HIIT starting from the 5th week (3 min (85%-90%) Smax+1 min (50%-55%) Smax, 3-5 cycles, Smax is the maximum movement speed). A lactate analyzer was used to detect the blood lactate concentration in the quiet state of rats in the HC and HM groups at week 4 and in the 0, 2, 4, 8, 12, and 24 h after exercise, as well as in the quiet state of rats in each group at week 8. Behavioral indexes such as sucrose preference rate, number of times of uprightness and number of traversing frames in the absenteeism experiment, and other behavioral indexes were used to assess the depressive-like behavior of the rats at week 4 and week 8. The rats were anesthetized on the next day after the behavioral test in week 8, and hippocampal tissues were taken for assay. LC-MS non-targeted metabolomics, target quantification, ELISA and Western blot were used to detect the changes in metabolite content, lactate and pyruvate concentration, the content of key metabolic enzymes in the pyruvate-lactate axis, and the protein expression levels of monocarboxylate transporters (MCTs). Results4-week HIIT intervention significantly increased the sucrose preference rate, the number of uprights and the number of traversed frames in the absent field experiment in CUMS rats; non-targeted metabolomics assay found that 21 metabolites were significantly changed in group M compared to group C, and 14 and 11 differential metabolites were significantly dialed back in the HC and HM groups, respectively, after the 4-week HIIT intervention; the quantitative results of the targeting showed that, compared to group C, lactate concentration in the hippocampal tissues of M group, compared with group C, lactate concentration in hippocampal tissue was significantly reduced and pyruvate concentration was significantly increased, and 4-week HIIT intervention significantly increased the concentration of lactate and pyruvate in hippocampal tissue of HM group; the trend of changes in blood lactate concentration was consistent with the change in lactate concentration in hippocampal tissue; compared with group C, the LDHB content of group M was significantly increased, the content of PKM2 and PDH, as well as the protein expression level of MCT2 and MCT4 were significantly reduced. The 4-week HIIT intervention upregulated the PKM2 and PDH content as well as the protein expression levels of MCT2 and MCT4 in the HM group. ConclusionThe 4-week HIIT intervention upregulated blood lactate concentration and PKM2 and PDH metabolizing enzymes in hippocampal tissues of CUMS rats, and upregulated the expression of MCT2 and MCT4 transport carrier proteins to promote central lactate uptake and utilization, which regulated metabolic homeostasis of the pyruvate-lactate axis and improved depressive-like behaviors.
3.Four Weeks of HIIT Modulates Lactate-mediated Synaptic Plasticity to Improve Depressive-like Behavior in CUMS Rats

ObjectiveThis study aimed to investigate the effects of 4-week high-intensity interval training (HIIT) on synaptic plasticity in the prefrontal cortex (PFC) of rats exposed to chronic unpredictable mild stress (CUMS), and to explore its potential mechanisms. MethodsA total of 48 male Sprague-Dawley rats were randomly divided into 4 groups: control (C), model (M), control plus HIIT (HC), and model plus HIIT (HM). Rats in groups M and HM underwent 8 weeks of CUMS to establish depression-like behaviors, while groups HC and HM received HIIT intervention beginning from the 5th week for 4 consecutive weeks. The HIIT protocol consisted of repeated intervals of 3 min at high speed (85%-90% maximal training speed, Smax) alternated with one minute at low speed (50%-55% Smax), with 3 to 5 sets per session, conducted 5 d per week. Behavioral assessments and tail-vein blood lactate levels were measured at the end of the 4th and 8th weeks. After the intervention, rat PFC tissues were collected for Golgi staining to analyze synaptic morphology. Enzyme-linked immunosorbent assays (ELISA) were employed to detect brain-derived neurotrophic factor (BDNF), monocarboxylate transporter 1 (MCT1), lactate, and glutamate levels in the PFC, as well as serotonin (5-HT) levels in serum. Additionally, Western blot analysis was conducted to quantify the expression of synaptic plasticity-related proteins, including c-Fos, activity-regulated cytoskeleton-associated protein (Arc), and N-methyl-D-aspartate receptor 1 (NMDAR1). ResultsCompared to the control group (C), the CUMS-exposed rats (group M) exhibited significant reductions in sucrose preference rates, number of grid crossings, frequency of upright postures, and entries into and duration spent in open arms of the elevated plus maze, indicating marked depressive-like behaviors. Additionally, the group M showed significantly reduced dendritic spine density in the PFC, along with elevated levels of c-Fos, Arc, NMDAR1 protein expression, and increased concentrations of lactate and glutamate. Conversely, BDNF and MCT1 contents in the PFC and 5-HT levels in serum were significantly decreased. Following HIIT intervention, rats in the group HM displayed considerable improvement in behavioral indicators compared with the group M, accompanied by significant elevations in PFC MCT1 and lactate concentrations. Furthermore, HIIT notably normalized the expression levels of c-Fos, Arc, NMDAR1, as well as glutamate and BDNF contents in the PFC. Synaptic spine density also exhibited significant recovery. ConclusionFour weeks of HIIT intervention may alleviate depressive-like behaviors in CUMS rats by increasing lactate levels and reducing glutamate concentration in the PFC, thereby downregulating the overexpression of NMDAR, attenuating excitotoxicity, and enhancing synaptic plasticity.
4.Rapid health technology assessment of toripalimab combined with chemotherapy in the treatment of locally advanced or metastatic non-small cell lung cancer

OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of toripalimab (Tor) combined with chemotherapy (CT) in the treatment of locally advanced or metastatic non-small cell lung cancer (NSCLC). METHODS PubMed, the Cochrane Library, Embase, Web of Science, CBM, CNKI, Wanfang Data, and Health Technology Assessment (HTA) related websites were searched to collect the HTA reports, systematic reviews/meta-analyses and pharmacoeconomic studies of Tor+CT in the treatment of locally advanced or metastatic NSCLC from database/website inception to March 31, 2025. After data extraction and quality evaluation, the results of the included studies were analyzed descriptively. RESULTS A total of eleven studies were included, involving five systematic reviews/meta-analyses, and six pharmacoeconomic studies. Among the five systematic reviews/ meta-analyses, two were of high quality, while there was one each of moderate, low, and very low quality. All six pharmacoeconomic studies were of good quality. In terms of efficacy, compared with CT, Tor+CT significantly improved patients’ progression-free survival (PFS) and overall survival (P<0.05). In addition, compared with ipilimumab+CT, durvalumab, durvalumab+tremelimumab and sugemalimab+CT, Tor+CT could also improve the PFS (P<0.05). In terms of safety, there was no significant difference in the incidence of grade≥3 adverse events between patients receiving Tor+CT and CT (P>0.05); while Tor+CT had a lower incidence of grade≥3 adverse E-mail: events, compared with camrelizumab+CT, pembrolizumab+ 3233255290@qq.com ipilimumab, nivolumab+CT and atezolizumab+CT (P<0.05).In terms of cost-effectiveness, Tor+CT treatment had certain cost-effectiveness advantages, compared with CT. CONCLUSIONS Compared with CT, other programmed death-1/programmed death-ligand 1 inhibitors alone, or their combination with CT, Tor+CT for the treatment of locally advanced or metastatic NSCLC has good efficacy, safety and cost-effectiveness.
5.Interpretation of the radiologist training system in Canada and enlightenment

This paper aims to discuss the ideas and experience about the radiology residency training system of Canada with a presentation of its base accreditation standards for five aspects, competency goals for seven roles, four stages of training arrangement, and two types of final assessment questions. Although the Canada's radiology residency program differs from China's standardized resident and specialist training programs for radiology, there are still several points that are worth referencing, including emphasizing the training priority of competency goals, providing a specific basis for the stratification of training, offering clear guidance for the implementation of training content, and improving assessment methods to focus on competency goals. These points are of great value for improving the standardized radiology resident and specialist training programs in China, so as to provide a reference for the training of excellent radiologists in China.
6.Clinical Value of Systemic Immune Inflammation Index to Predict Left Ventricular Reverse Remodeling in Dilated Cardiomyopathy Patients With Heart Failure With Reduced Ejection Fraction

Objectives:To investigate the predictors of left ventricular reverse remodeling(LVRR)and prognosis in dilated cardiomyopathy(DCM)patients with heart failure with reduced ejection fraction(HFrEF). Methods:A total of 480 patients with HFrEF were continuously recruited.The patients were divided into LVRR group(n=235)and without LVRR group(n=245).The clinical data of patients with and without LVRR were compared.At the same time,the incidence of LVRR and complex cardiovascular adverse events in patients with different SII tripartite groups was compared.The study population was stratified according to SII tertiles:the baseline tertiles of the SII(group 1:<332.8[n=160],group 2:332.8-563.1[n=160],and group 3:>563.1[n=160]).Composite cardiac events include heart failure re-hospitalization,fatal arrhythmias,and cardiac death.Logistic regression analysis was used to analyze the predictive factors of LVRR.The dose-response relationship between systemic imnune inflammation index(SII)and LVRR were evaluated by restricted cubic spline(RCS).Receiver operating characteristic curves,net reclassification index(NRI),integrated discrimination improvement(IDI)and Akaike information criterion(AIC)were drawn to assess the performances of predictors to predict LVRR and prognosis.The predictive efficacy of these predictors were compared with traditional biomarkers(N-terminal pro-brain natriuretic peptide[NTpro-BNP]and soluble growth stimulation expressed gene 2 protein[sST2]).The survival analysis was performed using the Kaplan-Meier method. Results:A total of 235(49.0%)patients experienced LVRR.The results of logistic regression analysis showed that baseline SII was an independent predictor of non-LVRR(OR=1.005,95%CI:1.004~1.007,P<0.01).RCS showed a positive linear relationship between SII and non-LVRR(Pnonlinear=0.455).Compared with traditional biomarkers NT-proBNP and sST2,ROC,NRI,IDI and AIC results proved that SII had the best predictability regarding non-LVRR.All 480 patients completed the follow-up,there were 108(22.5%)composite cardiac events and 37(7.7%)all-cause deaths.Patients in the SII>563.1 group had higher rate of composite cardiovascular events than those in the SII<332.8 and SII 332.8 to 563.1 groups(43.1%vs.9.4%vs.15.0%,log-rank P<0.01). Conclusions:This study indicates that increased SII at admission could predict non-LVRR in DCM related HFrEF patients and its predictive efficacy is better than traditional biomarkers.
7.Research progress on self-care of patients with left ventricular assist devices

With the increase in the number of patients with heart failure and the improvement of medical care,the prevalence of patients with end-stage heart failure is increasing.Due to the limited donor supply for heart transplantation,the implantation of left ventricular assist devices has become a major alternative treatment option for patients with end-stage heart failure.Patients with left ventricular assist device have to face potential heart failure and long-term self-care related to left ventricular assist devices.It is of great significance to comprehensively evaluate the level of self-care behavior of patients with left ventricular assist devices and to formulate targeted self-care guidance programs to improve their prognosis.This article reviewed the concept and connotation,assessment tools,current status,influencing factors and intervention of self-care of patients with left ventricular assist devices,in order to provide theoretical basis and practical guidance for improving the self-care level of patients with left ventricular assist devices,so as to improve the quality of life and improve the prognosis of patients.
8.Risk of ICU aspiration and construction of a prediction model in postoperative patients with brain glioma

Objective To explore the risk factors for aspiration in intensive care unit(ICU)patients with brain glioma after surgery and construct a risk prediction model for aspiration.Methods Clinical data of 379 glioma patients undergoing surgical treatment in our hospital between March 2021 and March 2024 were collected and retrospectively analyzed.In a ratio of 2∶1,they were assigned into a modeling group(252 cases)and a verification group(127 cases).According to presence or absence of postoperative ICU aspiration,the patients in the modeling group were di-vided into aspiration subgroup(49 cases)and non-aspiration subgroup(203 cases).Multivariate logistic regression analysis was applied to identify the risk factors for ICU aspiration.Based on the regression coefficients(β)of these risk factors,a risk prediction model was constructed.ROC curve was employed to evaluate the discrimination efficiency of the risk prediction model.Results There was no significant difference in incidence of ICU aspiration between the modeling group and the verification group(x2=0.594,P=0.441).Multivariate logistic regression analysis showed that COPD,mechanical ventilation,enteral nutrition and postoperative coma time ≥24 h were risk fac-tors for ICU aspiration in brain glioma after surgery(P<0.01).The incidence of ICU aspiration was 7.69%,39.13%and 64.29%,respectively in the low-,medium-and high-risk patients of the modeling group,with an AUC value of 0.771(95%CI:0.700-0.842,P<0.01).And the incidence was 8.05%,51.61%and 66.67%,respectively in the low-,medium-and high-risk patients of the validation group,with an AUC value of 0.789(95%CI:0.698-0.881,P<0.01).Conclusion COPD,mechanical ventilation,enteral nutrition and continuous postoperative coma can increase the risk of ICU aspiration in postoperative patients with brain glioma.Our constructed prediction model based on above risk factors has good predictive value.
9.The regulatory role of the hypothalamus in the thermogenesis of brown and beige adipose tissue

There are three types of adipose tissue: white adipose tissue that stores energy, brown adipose tissue that expends energy, and beige adipose tissue which results from the browning of white adipose tissue. Research has established that adipose tissue is vital for energy metabolism. However, the complex regulatory mechanisms limit the development of effective therapeutic strategies for obesity and related metabolic diseases. This review explores the characteristics and functions of adipose tissue types, emphasizing the role of central hypothalamic nuclei in regulating peripheral adipose tissue function Additionally, it provides a summary and comparison of current research on leveraging brown and beige adipose tissue thermogenesis as a treatment for metabolic diseases, offering insights for the advancement of targeted therapies for obesity and related disorders.
10.Efficacy of Fried Licorice Decoction Plus-minus combined with PD-1 for advanced non-small cell lung cancer with Qi-Yin deficiency

Objective:To evaluate the clinical efficacy of Fried Licorice Decoction Plus-minus in combination with PD-1 therapy for patients with advanced non-small cell lung cancer(NSCLC)with Qi-Yin deficiency.Methods:From December 2020 to December 2022,72 patients diagnosed with advanced NSCLC with Qi-Yin deficiency were enrolled in this study.The patients were recruited from Jilin Provincial Cancer Hospital and the Affiliated Hospital of Changchun University of Chinese Medicine.The patients were randomly assigned into two groups,with 36 patients in each group.The control group received PD-1 treatment alone,and the treatment group received PD-1 combined with Fried Licorice Decoction Plus-minus.The measures of primary outcomes included immune function(CD3+,CD3+/CD4+,and CD3+/CD8+)and quality of life score(Kamofsky,KPS).The measures of secondary outcomes included the objective response rate(ORR);traditional Chinese medicine(TCM)symptom score;serum tumor marker carcinoembryonic antigen(CEA);neuron-specific enolase(NSE);cytokeratin fragment(CYFRA211);and incidence of immune-related adverse events such as gastrointestinal toxicity,pulmonary toxicity,hepatotoxicity,and thyroid toxicity.Results:Regarding immune function regulation,a significant improvement was observed in CD3+,CD3+/CD4+,and CD3+/CD8+.Furthermore,at T3 and T4 time points,an improvement was observed in patient quality of life indicators such as the tumor markers CEA and CY211.In addition,symptoms related to TCM syndrome,including coughing,fatigue,palpitations,and diarrhea were significantly reduced compared to the baseline levels.Moreover,the treatment group exhibited superior outcomes compared to those of the control group in terms of reducing immune-associated pneumonia and diarrhea(P<0.05).The ORR for both groups was similar;36%for those receiving com-bination therapy vs.25%for those receiving PD-1 monotherapy.Therefore,no significant differences between these two groups were ob-served(P>0.05).Conclusions:The combination of Fried Licorice Decoction Plus-minus and PD-1 in the treatment of patients with advanced NSCLC showed a significant enhancement in immune function,improvement in clinical efficacy,reduction in immune-related adverse reac-tions,and had an excellent safety profile.
