1.Effect of flexible negative pressure suction sheath combined with flexible ureteroscopy for lower calyx renal stone treatment

Objective To investigate the clinical efficacy of the combination of a flexible negative pres-sure suction sheath with ureteroscopy in the treatment of lower calyx renal stones.Methods This study in-cluded 169 patients with lower calyx calculi admitted to Yongchuan Hospital Affiliated to Chongqing Medical University from January 2021 to December 2023.they were divided into a control group(n=81)and an exper-imental group(n=89).The control group underwent traditional flexible ureteroscopic lithotripsy,while the experimental group underwent ureteroscopic lithotripsy combined with a flexible negative pressure suction sheath.The surgical time,postoperative infection status,complications,stone clearance rate,and stone compo-sition analysis were compared between the two groups.Results The experimental group showed shorter oper-ation time than the control group(P<0.05).Postoperative inflammatory and infection indicators[procalcito-nin(PCT),C reactive protein(CRP),white blood cell count(WBC),systemic inflammatory response syn-drome(SIRS)incidence and positive blood culture rate]in the experimental group were significantly lower than those in the control group(P<0.05 or P<0.01).The experimental group also demonstrated lower inci-dence of postoperative fever,reduced back pain,lower urinary red blood cell counts and serum creatinine levels(P<0.05 or P<0.01)compared to the control group.The stone clearance rate was significantly higher in the experimental group(P<0.01)than in control group.There was no statistically significant difference in the composition of stones of the two groups(P>0.05).Conclusion The combination of a flexible negative pres-sure suction sheath with ureteroscopy for the treatment of lower calyx renal stones is an effective treatment method.
2.Clinical study on the treatment of children with tic disorder with heart and liver fire hyperactivity syndrome through acupuncture combined with low-frequency repetitive transcranial magnetic stimulation

Objective:To evaluate the clinical efficacy of acupuncture combined with low-frequency repetitive transcranial magnetic stimulation (rTMS) in the treatment of heart and liver fire hyperactivity syndrome in children with tic disorder; To explore its effects on the levels of serum neurotransmitters in children.Methods:A randomized controlled trial study was conducted. A total of 86 children with heart and liver fire hyperactivity syndrome of tic disorder in our hospital from March 2022 to March 2024 were selected as the observation objects and divided into 2 groups using the random number table method, with 43 cases in each group. The control group was treated with rTMS, while the observation group was treated with combined acupuncture on the basis of the control group. Both groups were treated continuously for 8 weeks. The TCM syndrome scores were conducted before and after the treatment respectively. The Yale Comprehensive Tic Severity Scale (YGTSS) was used to evaluate the severity of clinical symptoms, and the levels of serum γ -aminobutyric acid (γ-GABA), dopamine (DA), and 5-hydroxytryptamine (5-HT) were detected by ELISA. The adverse reactions during the treatment period were recorded and the clinical efficacy was evaluated.Results:The total effective rate was 90.70% (39/43) in the observation group and 72.09% (31/43) in the control group, with statistical significance ( χ2=4.91, P=0.027). After treatment, the scores of convulsion and peristalsis, flushed face and ears, irritability and anger, and red tongue with yellow coating in the observation group were lower than those in the control group ( t values were 3.80, 2.94, 3.26, and 4.22 respectively, P<0.01), and the scores of vocal tics, motor tics and the total score were lower than those in the control group (t values were 7.97, 5.23, and 6.44 respectively, P<0.01). After treatment, the level of γ-GABA in the observation group [(9.72±2.04) μg/L vs. (8.13±1.87) μg/L, t=3.77] was higher than that of the control group ( P<0.01), and DA [(8.98±1.94) μg/L vs. (12.10±2.27) μg/L, t=6.85], levels of 5-HT [(15.20±3.87) μg/L vs. (19.93±3.97) μg/L, t=5.59] were lower than those in the control group ( P<0.01). During the treatment period, the incidence of adverse reactions was 11.63% (5/43) in the observation group and 9.30% (4/43) in the control group, without statistical significance ( χ2=0.12, P=0.725). Conclusion:Acupuncture combined with rTMS can alleviate the tic symptoms of children of tic disorder with heart and liver fire hyperactivity syndrome, improve the neurotransmitter levels and main TCM syndromes of the children, enhance the clinical efficacy and have good treatment safety.

Surgical operation is the main treatment of oral and maxillofacial tumors.Dysphagia is a common postoperative complication.Swal-lowing disorder can not only lead to mis-aspiration,malnutrition,aspiration pneumonia and other serious consequences,but also may cause psychological problems and social communication barriers,affecting the quality of life of the patients.At present,there is no systematic evalua-tion and rehabilitation management plan for the problem of swallowing disorder after oral and maxillofacial tumor surgery in China.Combining the characteristics of postoperative swallowing disorder in patients with oral and maxillofacial tumors,summarizing the clinical experience of ex-perts in the field of tumor and rehabilitation,reviewing and summarizing relevant literature at home and abroad,and through joint discussion and modification,a group of national experts reached this consensus including the core contents of the screening of swallowing disorders,the phased assessment of prognosis and complications,and the implementation plan of comprehensive management such as nutrition management,respiratory management,swallowing function recovery,psychology and nursing during rehabilitation treatment,in order to improve the evalua-tion and rehabilitation of swallowing disorder after oral and maxillofacial tumor surgery in clinic.

Neck dissection(ND)is one of the main treatment methods for oral and maxillofacial malignancies.Although ND type is in con-stant improvement,but intraoperative peal-pull-push injury of the accessory nerve,muscle,muscle membrane,fascia and ligament induced shoulder syndrome(SS)is still a common postoperative complication,combined with the influence of radiochemotherapy,not only can cause pain,stiffness,numbness,limited dysfunction of shoulder neck and arm,but also may have serious impact on patient's life quality and phys-ical and mental health.At present,there is still a lack of a systematic evaluation and rehabilitation management program for postoperative SS of oral and maxillofacial malignant tumors.Based on the previous clinical practice and the current available evidence,refer to the relevant lit-erature at home and abroad,the experts in the field of maxillofacial tumor surgery and rehabilitation were invited to discuss,modify and reach a consenusus on the etiology,assessment diagnosis,differential diagnosis,rehabilitation strategy and prevention of SS,in order to provide clinical reference.

Objective To analyze the regulation of gene expression by adenosine deaminase RNA specific 1(ADAR1)in cervical cancer cell line Hela using RNA-seq technology and provide theoretical basis for understanding the role of ADAR1 in the occurrence and progression of cervical cancer.Methods RNA-seq sequencing of normal and ADAR1 knockdown Hela cell lines to identify differentially expressed genes.By conducting enrichment analysis using KEGG Pathway,GO cellular,and GSEA,the study analyzes the relevant signaling pathways and biological processes involving ADAR1 in Hela cell lines.Results Differentially expressed genes are mainly enriched in immune and inflammation-related signaling pathways(such as TNF-α/NF-κB,NIK/NF-κB,Jak/Stat-IL-6),Hippo signaling pathway,TGF-β signaling pathway,and are involved in interferon response,cellular amino acid metabo-lism regulation,protein ubiquitination/deubiquitination,viral transcription,and other biological processes.Further analysis of the NF-κB signaling pathway revealed a significant increase in the mRNA expression levels of NF-κB1 and TRAF5 after ADAR1 knockdown.Conclusion ADAR1 may regulate the expression of NF-κB signaling pathway-related factors and thereby regulate the occurrence and development of cervical cancer.

Objective:To investigate the pathological characteristics and the clinical significance of novel coronavirus (2019-nCoV)-infected pneumonia (termed by WHO as coronavirus disease 2019, COVID-19).Methods:Minimally invasive autopsies from lung, heart, kidney, spleen, bone marrow, liver, pancreas, stomach, intestine, thyroid and skin were performed on three patients died of novel coronavirus pneumonia in Chongqing, China. Hematoxylin and eosin staining (HE), transmission electron microcopy, and histochemical staining were performed to investigate the pathological changes of indicated organs or tissues. Immunohistochemical staining was conducted to evaluate the infiltration of immune cells as well as the expression of 2019-nCoV proteins. Real time PCR was carried out to detect the RNA of 2019-nCoV.Results:Various damages were observed in the alveolar structure, with minor serous exudation and fibrin exudation. Hyaline membrane formation was observed in some alveoli. The infiltrated immune cells in alveoli were majorly macrophages and monocytes. Moderate multinucleated giant cells, minimal lymphocytes, eosinophils and neutrophils were also observed. Most of infiltrated lymphocytes were CD4-positive T cells. Significant proliferation of type Ⅱ alveolar epithelia and focal desquamation of alveolar epithelia were also indicated. The blood vessels of alveolar septum were congested, edematous and widened, with modest infiltration of monocytes and lymphocytes. Hyaline thrombi were found in a minority of microvessels. Focal hemorrhage in lung tissue, organization of exudates in some alveolar cavities, and pulmonary interstitial fibrosis were observed. Part of the bronchial epithelia were exfoliated. Coronavirus particles in bronchial mucosal epithelia and type Ⅱ alveolar epithelia were observed under electron microscope. Immunohistochemical staining showed that part of the alveolar epithelia and macrophages were positive for 2019-nCoV antigen. Real time PCR analyses identified positive signals for 2019-nCoV nucleic acid. Decreased numbers of lymphocyte, cell degeneration and necrosis were observed in spleen. Furthermore, degeneration and necrosis of parenchymal cells, formation of hyaline thrombus in small vessels, and pathological changes of chronic diseases were observed in other organs and tissues, while no evidence of coronavirus infection was observed in these organs.Conclusions:The lungs from novel coronavirus pneumonia patients manifest significant pathological lesions, including the alveolar exudative inflammation and interstitial inflammation, alveolar epithelium proliferation and hyaline membrane formation. While the 2019-nCoV is mainly distributed in lung, the infection also involves in the damages of heart, vessels, liver, kidney and other organs. Further studies are warranted to investigate the mechanism underlying pathological changes of this disease.
7.Blocking the PD-1/PD-L1 axis enhanced cisplatin chemotherapy in osteosarcoma in vitro and in vivo.

BACKGROUND:
The blocking of the programmed cell death protein (PD-1)/programmed death-ligand 1 (PD-L1) axis has been found to have an anticancer activity against various types of cancer by enhancing T cell immunity, while there are no studies linking the PD-1/PD-L1 axis to chemotherapy drugs in osteosarcoma (OS). The present study aimed to investigate the effects of blocking PD-1/PD-L1 axis on the cisplatin chemotherapy in OS in vitro and in vivo.
METHODS:
Reverse transcription-quantitative polymerase chain reaction (RT-qPCR) was applied to detect PD-L1 mRNA in OS tissues. Cell proliferation and apoptosis were measured by Cell Counting Kit-8 (CCK-8) and flow cytometry assays, respectively. In vivo, the syngeneic mice were treated with cisplatin and anti-PD-1 antibody alone or jointly.
RESULTS:
In this study, it revealed that PD-L1 mRNA was highly expressed in OS tissues. Further inhibitory evaluation showed that the K7M2-LV cells (PD-L1 overexpression) co-cultured with PD-1 lymphocytes could promote K7M2 cell proliferation. Meanwhile, the combination of anti-PD-1 antibody and cisplatin significantly decreased the proliferation and increased the apoptosis of K7M2 cells in a co-culture system. In vivo, the combination of anti-PD-1 antibody and cisplatin significantly inhibited tumor growth, while the mechanisms did not involve regulatory T cells.
CONCLUSION
The present data suggested that the blocking of PD-1/PD-L1 axis had a positive prognostic value, which can enhance the chemotherapeutic effect of cisplatin in OS. These findings provide a rationale for utilizing PD1/PD-L1 blocking antibodies as a single agent to cure refractory OS in patients receiving cisplatin treatment.
8.The diagnostic value of six-slice coronal reformation in patients with acute midepigastric pain

Objective To evaluate the diagnostic value of six-slice coronal reformations in patients with acute midepigastric pain. Methods A total of 974 patients with acute midepigastric pain were included in this study and divided into group A(coronal reformation)and group B(non-coronal reformation).For group A,reconstructed coronal and oblique-coronal images were acquired.A comprehensive diagnosis was made based on coronal reformations and axial planes.Anatomical nomenclature was adopted,including kidney-ureter plane,abdominal aorta plane,superior mesenteric artery plane,ascending colon-appendix plane,stomach-cholecyst plane and colon-small intestine plane.For group B,the diagnosis was made based on axial planes.Finally,a comprehensive analysis was made,missed cases in these two groups were counted and compared,and statistical analysis was performed using the SPSS software(version SPSS V17).Results For group A,the missed diagnosis was made in 12 cases(1.23%)and it was 53 cases(6.58%)for group B.There was a statistically significant difference between two groups(P<0.05).Conclusion Combined with axial planes or oblique coronal reformations,six-sclice coronal reformation can reduce the the rate of missed diagnosis of acute midepigastric pain.
9.Analysis of risk factors of bile leakage in patients after laparoscopic common bile duct exploration and primary suture

Objective To explore the risk factors of bile leakage in patients with laparoscopic common bile duct exploration and primary suture for the purpose of providing clues for reducing occurrence of bile leakage.Methods The clinic data of 193 choledocholithiasis patients with laparoscopic common bile duct exploration and primary suture from October 2012 to March 2017 were retrospective analysed.All patients were divided into bile leakage group (23 patients) and non-bile leakage group (170 patients).Risk factors influencing the incidence of bile leakage were determined by analyzing 21 relevant factors with one-way analysis of variance and Logistic multivariate regression analysis.Count data and ordinal data was expressed as frequency or a percentage.Chi-square test was used to compare with groups of count data,rank-sum test was for comparison between groups of ordinal data,and Logistic regression was for multivariate analysis.Results Among all the patients,the incidence of bile leakage was 11.92% (23/193).The results of univariate analysis revealed that cholangitis,jaundice,bile characteristics,muddy stone,number of stones,incarcerated stone,open and close peristalsis of duodenal papilla were correlated with bile leakage (x2/Z =2.537,2.122,81.834,50.709,13.242,26.958,90.207,P <0.05).The result of multivariate analysis revealed that bile characteristics,muddy stone,incarcerated stone,open and close peristalsis of duodenal papilla was correlated with bile leakage (Wals =14.002,8.899,6.577,5.582,P <0.05).Conclusion Bile characteristics,muddy stone,incarcerated stone,open and close peristalsis of duodenal papilla were main risk factors of bile leakage in patients with laparoscopic common bile duct exploration and primary suture.
10.Effect of two-level community-based health education pattern on schistoso-miasis control

Objective To implement a two?level community?based health education pattern of schistosomiasis in residents of endemic areas in marshland and lake regions,so as to explore the suitable pattern of health education under hypo?endemic situa?tion. Methods Two schistosomiasis endemic villages in Jiangling County,Hubei Province were collected as study areas,and among which,one village was treated as an intervention group,where the two?level community?based health education pattern as well as regular control measures was implemented;the other village was a control group,where only regular control measures were implemented. The awareness rates on schistosomiasis control,the rates of correct behavior and the compliance rates of ex?amination,treatment and chemotherapy of the two groups before and after the intervention were compared. Results According to the results of the baseline survey in 2014,the awareness rates of schistosomiasis control of the intervention and control groups were 84.00%and 77.45%,respectively,the correct rates of behavior of the two groups were 72.00%and 63.73%,respectively, and the compliance rates of the treatment were 80.36%and 82.28%,respectively,there were no statistically significant differ? ences between all the above rates of the two groups(all P>0.05). After the intervention of the two?level community?based health education,the correct rates of behavior,and the compliance rates of examination and chemotherapy of the two groups were 92.31%and 80.37%,95.11%and 82.55%,84.13%and 63.64%,respectively,and the differences between all the rates above of the two groups were statistically significant(all P<0.05). When compared to those before intervention,the growing rates of the compliance rates of examination,treatment and chemotherapy of the intervention group were 20.97%,15.33%and 23.29%, respectively,while those of control group were 14.27%,4.17%,-3.77%,respectively,the growing rates of the intervention groups were higher than those of the control groups. Conclusions Through the two?level community?based pattern of health edu?cation,the compliance rates of examination and treatment of the residents have improved,and therefore,the pattern is suitable for popularization and application in marshland and lake regions.
