In February 2024, a patient with left lumbar pain for over 1 month was admitted to Heze Municipal Hospital and was diagnosed as retroperitoneal mass by enhanced CT and PET/CT examination.Laparoscopic retroperitoneal mass resection was performed.The postoperative pathological diagnosis was Erdheim-Chester disease.The patient was treated with interferon (IFN) alpha after surgery and was followed up for 5 months with no recurrence or metastasis. Erdheim-Chester disease(ECD) is rare and prone to be misdiagnosed clinically. Biopsy or resection of the tumor should be performed to make a definite diagnosis.

ObjectiveTo investigate the expression levels of miR-128-3p， SIRT1， and AMPK in the peripheral blood of patients with type 2 diabetes mellitus （T2DM） comorbid with nonalcoholic fatty liver disease （NAFLD）， as well as the role of miR-128-3p in predicting NAFLD in T2DM patients. MethodsA total of 80 patients with T2DM who were hospitalized in The First Affiliated Hospital of Anhui University of Chinese Medicine from September 2022 to August 2023 were enrolled and divided into T2DM group with 40 patients and NAFLD group with 40 patients， and according to the NAFLD fibrosis score （NFS）， the patients were further divided into progressive liver fibrosis group with 16 patients and non-progressive liver fibrosis group with 64 patients. General data and biochemical parameters were collected； quantitative real-time PCR was used to measure the mRNA expression levels of miR-128-3p， SIRT1， and AMPK in peripheral blood， and Western blot was used to measure the protein expression levels of SIRT1 and AMPK. The independent-samples t test was used for comparison of normally distributed data between two groups， and the Mann-Whitney U test was used for comparison of data with skewed distribution between two groups； the chi-square test was used for comparison of categorical data between two groups. The logistic regression analysis was used to identify the influencing factors for the presence of NAFLD and progressive liver fibrosis， and the receiver operating characteristic （ROC） curve analysis was used to determine the optimal cut-off value of miR-128-3p for predicting NAFLD. ResultsThere were significant differences between the NAFLD group and the non-NAFLD group in body mass index， fasting plasma glucose， glycated hemoglobin， fasting insulin， fasting C-peptide， alanine aminotransferase （ALT）， aspartate aminotransferase， gamma-glutamyl transpeptidase， alkaline phosphatase， fibronectin， triglycerides， high-density lipoprotein cholesterol， total triiodothyronine （TT3）， Homeostasis Model Assessment of Insulin Resistance （HOMA-IR）， and NFS （all P<0.05）. Compared with the non-NAFLD group， the NAFLD group had a significantly higher mRNA expression level of miR-128-3p in peripheral blood （t=-8.765， P<0.001） and significant reductions in the mRNA and proteins expression levels of SIRT1 and AMPK （P<0.001）. There were significant differences between the progressive liver fibrosis group and the non-progressive liver fibrosis group in age， ALT， free triiodothyronine， TT3， superoxide dismutase， and miR-128-3p （all P<0.05）. The logistic regression analysis showed that miR-128-3p was an independent risk factor for the development of NAFLD （odds ratio ［OR］=8.221， 95% confidence interval ［CI］： 2.735 — 24.714， P<0.001） and progressive liver fibrosis （OR=1.493， 95%CI： 1.117‍ ‍—‍ ‍1.997， P=0.007）. The ROC curve analysis showed that miR-128-3p had an area under the ROC curve of 0.890 （95%CI： 0.829 — 0.950）， with an optimal cut-off value of 13.165， a sensitivity of 89.3%， and a specificity of 72.7%. ConclusionThere is an increase in the expression of miR-128-3p in peripheral blood of T2DM patients with NAFLD， while there are reductions in the expression levels of SIRT1 and AMPK， suggesting that miR-128-3p has a certain diagnostic value in identifying NAFLD and liver fibrosis in such population.

Insufficient remyelination due to impaired oligodendrocyte precursor cell (OPC) differentiation and maturation is strongly associated with irreversible white matter injury (WMI) and neurological deficits. We analyzed whole transcriptome expression to elucidate the potential role and underlying mechanism of action of lipocalin-2 (LCN2) in OPC differentiation and WMI and identified the receptor SCL22A17 and downstream transcription factor early growth response protein 1 (EGR1) as the key signals contributing to LCN2-mediated insufficient OPC remyelination. In LCN-knockdown and OPC EGR1 conditional-knockout mice, we discovered enhanced OPC differentiation in developing and injured white matter (WM); consistent with this, the specific inactivation of LCN2/SCl22A17/EGR1 signaling promoted remyelination and neurological recovery in both atypical, acute WMI due to subarachnoid hemorrhage and typical, chronic WMI due to multiple sclerosis. This potentially represents a novel strategy to enhance differentiation and remyelination in patients with white matter injury.

To compare the clinical efficacy of calcium sodium edetate （EDTA） and the low-dose sodium dimercaptopropanesulfonate（DMPS） in treating cerebral Wilson’s disease. Methods Eighty patients with neurological Wilson’s disease in the neurology department of the first affiliated hospital of Anhui traditional Chinese medicine from February 2019 to September 2021 were collected and divided into the EDTA treatment group（n=39） and the low-dose DMPS control group（n=41）based on treatments. Propensity score matching （PSM） was performed on the two groups of patients and the results were compared after matching. The copper displacement effects of the two groups were compared，and the deterioration of copper displacement and adverse reactions were observed. Results After PSM，35 patients in EDTA group and 35 patients in the low-dose DMPS group were included. UWDRS Ⅰ in EDTA group showed significant statistical difference after treatment［（57.03±21.06） versus （54.40±20.02），t=7.256，P<0.001］，while UWDRS Ⅰ in the low-dose DMPS group showed no significant statistical difference［（53.49±26.82） versus （52.37±27.17），t=1.544，P=0.129］. Compared with the low-dose DMPS group，there was no significant difference in 24-hour urinary copper detection in the EDTA group［551.18 （417.77，843.47） versus 489.20 （389.64，839.47），Z=0.029，P>0.05］. The neurological deterioration （P=0.025） and adverse reactions were significantly reduced. Conclusion EDTA and low-dose DMPS have the same effect in expelling copper in patients with neurological WD，with better improvement，less neurological deterioration probability，and fewer adverse reactions. EDTA can be used as a copper removing treatment for patients with neurological WD.

Objective: To evaluate the severity of median nerve damage in patients with carpal tunnel syndrome (CTS) , and to analyze its relationship with body mass index (BMI) and wrist joint index. Methods: From May 2016 to January 2017, 23 patients with mild CTS (mild group) and 35 patients with moderate to severe CTS (moderate to severe group) were enrolled in this study. And 22 healthy volunteers matched for sex and age were selected as control group. The neuroelectrophysiological monitor was used to measure the median nerve movement and sensory nerve conduction in the subjects. The BMI and wrist joint index were calculated. The relationship of neuroelectrophysiological parameters with BMI and wrist joint index was analyzed in the CTS patients. Results: Compared with the control group, the mild group and the moderate to severe group had significantly higher wrist joint index, significantly longer distal motor latency (DML) of the median nerve, and significantly lower sensory nerve conduction velocity (SNCV) and sensory nerve action potential (SNAP) amplitude of the finger 1-wrist and finger 3-wrist (P<0.01) ; the moderate to severe group had significantly higher BMI and significantly lower composite muscle action potential (CMAP) amplitude (P<0.01) . The wrist joint index and BMI were positively correlated with DML of the median nerve and negatively correlated with SCV and SNAP amplitude of the finger 1-wrist and finger 3-wrist (all P<0.05) . The patients with a wrist joint index of >0.73 had a significantly higher risk of CTS than those with a wrist joint index of <0.73 (odd ratio=30.67, 95% confidence interval: 3.79-248.36) . Conclusion A wrist joint index of >0.73 is an independent risk factor for CTS in manual laborers. CTS should be prevented in the manual laborers with high wrist joint index and BMI.

Objective To evaluate the relationship of the plasma levels of osteopontin(OPN),lipoprotein (a) [Lp (a)] and apolipoproteinA/apolipoproteinB (ApoB/ApoA1) ratio with atherosclerosis.Methods 120 elderly patients with atherosclerosis (observation group) and another 120 healthy elderly subjects (control group) from Health Check Department in Tianjin Third Center Hospital were enrolled from Mar.2014 to Nov.2015.The levels of plasma OPN,Lp(a) and ApoB / ApoA1 ratio were measured and their relationship with atherosclerosis was analyzed.Results In observation group versus control group,the levels of plasma OPN[(33.2±10.1) μg/L vs.(30.6± 9.1) μg/L],ApoB/ApoA1 ratio[(0.8±0.7) vs.(0.6±0.1)]and Lp(a)[(357.6±66.5) mg/L vs.(120.5±35.7) mg/L] were increased with statistically significant differences (t =8.91,8.43,6.01;P=0.021,0.035,0.041).The concentrations of plasma OPN,ApoB/ApoA1 ratio and Lp (a) were increased along with aggravation of artery lesion,with statistically significant differences (F =8.50,7.38,6.41;P=0.031,0.039,0.037).Conclusions The plasma levels of OPN,Lp(a) and ApoB/ ApoA1 ratio are increased in patients with atherosclerosis and increased along with the lesion aggravation.

Objective To explore the effects of diethylenetriamine/nitric oxide (DETA/NO)on capillary spasm and early brain injury (EBI)after subarachnoid hemorrhage (SAH)in rats.Methods Sixty-nine male Sprague-dawley rats were randomized into three groups:sham group,SAH group and DETA/NO group.SAH model was established by wearing out the willis ring with thread and then Garcia neurological score was observed in the general animals.The expressions of alpha smooth muscle actin (αSMA)and PDGFRβwere detected by dual immunofluorescence staining;nitric oxide kit was used for detecting brain tissue NO concentration.Changes in the hemoglobin-stimulated capillaries were observed in rat slices.Results Three days after surgery,neurological deficit score was remarkably improved in DETA/NO group compared with that in SAH group (P<0 .0 5 ). Immunofluorescence results showed that the expressions of peri-capillaryαSMA and PDGFRβwere significantly increased after SAH (P<0.05 ),and that DETA/NO could down-regulate the expressions (P<0.05 ).NO concentration was greatly reduced about 3 hours after SAH and then gradually increased;DETA/NO could maintain the concentration of NO at an early stage (P<0 .0 5 ).The capillary contraction was observed in slices perfused with hemoglobin;DETA/NO could alleviate capillary spasm.Conclusion DETA/NO can alleviate the severity of capillary spasm and EBI after SAH in rats.
