1.Training and use practices of specialty nurses in a tertiary hospital based on the ORTCC refined management theory

Objective:To apply the ORTCC (objective, rule, training, check, and culture) refined management model for the systematic management and overall construction of a specialty nurse team in a tertiary grade A general hospital, and to promote the cultivation and development of specialty nurses.Methods:The management framework of specialty nurses was investigated and a survey on their work was conducted. The ORTCC model was used to implement fine management of specialty nurses, including determining the integrated objective of "training, use, and management"(objective):, improving the rule system (rule), building a hierarchical training platform (training), streamlining the whole process check (check), and shaping the specialty nursing culture (culture). The effects were evaluated after one year of implementation.Results:After the implementation of ORTCC refined management, the number of specialty nurses increased to 367 in 2023, and a three-level progressive specialty nurse team was formed. The total and dimensional scores of perceived professional benefits ( P<0.001) and clinical competence ( P<0.001 or P<0.05) were higher than the scores in 2022. Additionally, 12 specialized nursing techniques were established, and six provincial or higher-level specialized quality improvement awards were obtained, further enhancing the capacity for specialized nursing services. Conclusions:The refined management practice of specialty nurses based on the ORTCC model provides a theoretical and practical basis for the hierarchical cultivation, effective use, and scientific management of specialty nurses in tertiary hospitals, thereby promoting specialty nurse team development and enhancing specialized service capacity.
2.A path analysis of the impact of death attitudes on negative emotion among nurses engaged in hospice care

Objective To investigate the current status of rumination and professional grief among hospice nurses,and to explore the mediating effect of death attitude between the two.Methods A total of 323 nurses in hospitals with hospice services and hospice wards in Hangzhou City were surveyed from April to July 2024 by convenience sampling method.They were investigated with general information questionnaire,Event Related Rumination Inventory,Death Attitude Profile-Revised,and Grief State Scale for Nurses by convenience sampling method.The structural equation model was used to analyze the mediating effect of death attitude between rumination and professional grief among hospice nurses.Results A total of 331 questionnaires were collected,among which 8 invalid questionnaires were deleted,and 323 valid questionnaires were retrieved,with the valid questionnaire recovery rate of 97.58%.The score of intrusive rumination dimension for hospice nurses was 13.34(7.00,20.00),and the score of deliberate rumination dimension was 16.92(11.00,24.00).The negative attitude towards death scored 38.35(29.00,48.00),while the positive attitude towards death scored 62.88(49.00,78.00).The total score of Grief State Scale for Nurses was 47.05(40.00,53.00).Hospice nurses'intrusive rumination was positively correlated with negative attitudes toward death,professional grief,and negatively correlated with positive attitudes toward death(all P<0.05);deliberate rumination was negatively correlated with negative attitudes toward death,professional grief,and positively correlated with positive attitudes toward death(all P<0.05).Negative death attitudes and positive death attitudes partially mediated between intrusive rumination and professional grief,with mediation effects of 0.053 and 0.037,accounting for 12.70%and 9.25%of the total effect.Negative death attitudes and positive death attitudes similarly partially mediated between deliberate rumination and professional grief,with mediation effects of-0.033 and-0.080,accounting for 10.03%and 21.33%of the total effect.Conclusion Death attitudes are a mediating variable between hospice nurses'rumination and professional grief.Nursing managers can take effective measures to improve hospice nurses'ruminative contemplation ability and positively guide positive attitudes toward death,thereby reducing their professional grief levels.

5.Construction of nursing quality evaluation indicators in perioperative period of heart transplantation

Objective To construct quality evaluation indicators for perioperative nursing in heart transplantation,and to provide standard and professional quantitative bases for monitoring and management of perioperative nursing quality.Methods This study was conducted based on the frame work of the three-dimensional"structure-process-outcome"quality model,using literature review,Delphi method and analytic hierarchy to determine the content of the indicators,and the weight of each index.Results A total of 22 experts from 14 qualified heart transplantation hospitals were included,and a total of 2 rounds of consultations were conducted.The effective recovery rates of 2 rounds of expert consultation questionnaires were 100%.The authority coefficients were 0.817.The variation coefficients of each item ranged from 0.025~0.169 and 0.039~0.157.The Kendall harmony coefficients were 0.126 and 0.225(P<0.001).The final evaluation indicators for perioperative nursing quality in heart transplantation included 3 first-level indicators,12 second-lever indicators and 59 third-level indicators.Conclusion The evaluation indicators of perioperative nursing quality in heart transplantation was scientific,comprehensive and specialized,which can provide references for the evaluation of perioperative nursing quality in heart transplantation.
6.Cardiovascular nursing discipline construction index system based on the integrated medical model

Objective:To establish an index system for the construction of cardiovascular nursing discipline based on the internal and external integration model of organ system, and to provide reference for the construction of cardiovascular nursing discipline with patients as the center and circulatory system as the core.Methods:Based on the health science cluster system theory, embodying the integrated nursing concept and the characteristics of cardiovascular discipline, the content of the cardiovascular nursing discipline construction index system and the weight of each index were determined through literature review, semi-structured interview and expert consultation. SPSS 23.0 was used to statistically analyze the obtained consultation data.Results:The positive coefficient of the experts was 92.308%, the authority coefficient was 0.812, and the coordination coefficients of the first-, second- and third-level indicators were 0.412, 0.421 and 0.347, respectively. The final indicator system consisted of 4 first-level indicators, 17 second-level indicators and 66 third-level indicators.Conclusion:The index system of cardiovascular nursing discipline construction is scientific and reliable, which has certain guiding significance to the construction of cardiovascular nursing discipline and the cultivation of talents under the integrated medical model.
7.The application of Six-Step standardized communication procedure in improving the communication ability of nurses

Objective To explore the effects of Six-Step standardized communication procedure in improving the communication ability of nurses.Methods C-I-CARE communication procedure was converted into Chinese.The Chinese version of Six-Step standardized communication procedure was Yi Kan,Er Yin,San Gaozhi,Si Wen,Wu Da,Liu Zaijian.The procedure was propagandized by making posters,setting up computer desktop,training nurses by lectures,video and role play.The effects were evaluated by pre and post self-control test among 100 nurses.Results The average score of communication behavior of 100 nurses after applying standardized communication procedure increased from 2.55±0.67 to 4.44±0.59 (t=-23.174,P<0.001).In 2015,the nursing department received 622 silk banners and commendatory letters for appreciating nursing work,increased by 18.9% compared to 2014.The discharged patient satisfaction increased from 94.2% to 96.93%.The satisfaction of doctors to nurses' collaboration was 99.86%.There was no nursing complaint or dispute caused by bad communication.Conclusion The application of procedure and standardized communication method can improve the communication ability of nurses,optimize the relationship between nurses and patients and increase patient satisfaction.
8.Components in Antineoplastic Actinomycete Strain(N2010-37)of Bottom Mud in Mangrove

Objective To study the antitumor components from an actinomycete strain(N2010-37)of bottom mud in Zhanjiang Mangrove,South China Sea.Methods The components were isolated and purified by chromatographic techniques and recrystallization,and the structures were identified by spectral methods together with physicochemical analyses.The antitumor effects of these components were tested in vitro by MTT method.Results Three compounds were identified including two anthrones and one novel lactone.They are(3S,4R,7R,8R,9S)-3,8-dihydroxy-4,7,9-trimethyl-2,6-cyclononanediolacetone(1),2-hydroxy-l-methoxy-3-methylanthraquinone(2),and 1,6,8-thihydroxy-3-methyl-anthraquinone(3).Conclusion Compound 1 is a new compound,and compounds 1 and 3 show the favorablecytotoxic activities against human chronic granulocytic leukemia cell line K562 strain by MTT method in vitro.
