1.Modified Ditan Tang Regulates Biorhythm-related Genes in Rat Model of Non-alcoholic Fatty Liver Disease

ObjectiveTo investigate the effects of modified Ditan tang on genes related to the transcription-translation feedback loop (TTFL) of biorhythm in the rat model of non-alcoholic fatty liver disease (NAFLD) and its mechanism for prevention and treatment of NAFLD. MethodsSixty-five healthy SPF male SD rats were randomly assigned into blank (n=20), model (n=15), and low-, medium-, and high-dose (2.68, 5.36, and 10.72 g·kg-1·d-1, respectively) modified Ditan tang (n=10) groups. Other groups except the blank group were fed a high-fat diet for 12 weeks. The modified Ditan tang groups were treated with the decoction at corresponding doses by gavage, and the blank and model groups were treated with an equal volume of normal saline from the 9th week for 4 weeks. The levels of triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), aspartate aminotransferase (AST), and alanine aminotransferase (ALT) in the serum were measured by an automatic biochemical analyzer. TG and non-esterified fatty acid (NEFA) assay kits were used to measure the levels of TG and NEFA in the liver. The pathological changes in the hypothalamus and liver were observed by hematoxylin-eosin staining, and the lipid deposition in the liver was observed by oil red O staining. The levels of brain-muscle ARNT-like protein 1 (BMAL1/ARNTL) in the hypothalamus and liver were determined by immunohistochemical staining. The mRNA and protein levels of BMAL1, circadian locomotor output cycles kaput (CLOCK), period circadian clock 2 (PER2), and cryptochrome1 (Cry1) in the hypothalamus and liver were determined by Real-time PCR and Western blot, respectively. ResultsCompared with the blank group, the model group showed elevated levels of TG, TC, LDL-C, AST, and ALT (P<0.01) and a lowered level of HDL-C (P<0.05) in the serum, elevated levels of TG and NEFA in the liver (P<0.01), pyknosis and deep staining of hypothalamic neuron cells, and a large number of vacuoles in the brain area. In addition, the model group showed lipid deposition in the liver, up-regulated mRNA and protein levels of CLOCK and BMAL1 (P<0.01), and down-regulated mRNA and protein levels of Cry1 and PER2 (P<0.01) in the hypothalamus and liver. Compared with the model group, all the three modified Ditan tang groups showed lowered levels of TG, TC, LDL-C, ALT, and AST (P<0.05, P<0.01) and an elevated level of HDL-C (P<0.05) in the serum, and lowered levels of TG and NEFA (P<0.05, P<0.01) in the liver. Furthermore, the three groups showed alleviated pyknosis and deep staining of hypothalamic neuron cells, reduced lipid deposition in the liver, down-regulated mRNA and protein levels of CLOCK and BMAL1 (P<0.05, P<0.01), and up-regulated mRNA and protein levels of Cry1 and PER2 (P<0.05, P<0.01) in the hypothalamus and liver. ConclusionModified Ditan tang can reduce lipid deposition in the liver and regulate the expression of CLOCK, BMAL1, Cry1, and PER2 in the TTFL of NAFLD rats.

With the rapid development of generative artificial intelligence(GAI)technologies,their widespread application in academic research and writing is continuously expanding the boundaries of sci-entific inquiry.However,this trend has also raised a series of ethical and regulatory challenges,inclu-ding issues related to authorship,content authenticity,citation accuracy,and accountability.In light of the growing involvement of AI in generating academic content,establishing an open,controllable,and trustworthy ethical governance framework has become a key task for safeguarding research integrity and maintaining trust within the academic community.This expert consensus outlines ethical requirements across key stages of AI-assisted academic writing-including topic selection,data management,citation practices,and authorship attribution.It aims to clarify the boundaries and ethical obligations surrounding AI use in academic writing,ensuring that technological tools enhance efficiency without compromising in-tegrity.The goal is to provide guidance and institutional support for building a responsible and sustainable research ecosystem.
3.Analysis on the current status of outpatient benefit policies for patients with hypertension and diabetes in urban and rural residents

Objective:To analyze the current policy design of outpatient benefit policies for patients with hypertension and diabetes(H&D)in urban and rural residents in China,and to provide references for optimizing the outpatient benefit mechanism for patients with H&D.Methods:A questionnaire survey was conducted to collect data on the basic features of General Outpatient Benefit(GOB),Outpatient Medication Guarantee Mechanisms for Hypertension and Diabetes(OMGM-H&D)and Outpatient Benefit for Patient with Chronic and Special Diseases(OB-C&S),focusing on deductibles,policy reimbursement ratios,and maximum payment limits.Descriptive statistical analysis was performed on the data.Results:A total of 334 regions were surveyed,of which 253 regions(75.34%)had implemented all three policies.Regional analysis revealed significant differences(P<0.001)in reimbursement ratios for GOB and the OMGM-H&D between the eastern,central,and western regions,with the western region having notably higher ratios than the eastern and central regions.However,no significant difference (P>0.05) was observed in the reimbursement ratios for OB-C&S across regions. Regarding deductibles,no significant regional difference (P>0.05) was found for GOB,but significant differences (P<0.05) existed for the OMGM-H&D and OB-C&S. Additionally,the annual maximum payment limits for all three policies showed significant regional variations (P<0.001). Conclusions:The outpatient benefits policy for patients with H&D have achieved full coverage nationwide among urban and rural residents in China,but regional equity in benefit levels requires improvement. It is recommended to strengthen data feedback mechanisms and promote provincial-level pooling of medical insurance.
4.Endurance exercise alleviates cerebral ischemia/reperfusion injury in rats by activating Mas/PKA/CREB/UCP2 signaling pathway

Aim To investigate the effects of endur-ance exercise(EE)on rats following cerebral ischemi-a/reperfusion injury(CI/RI)and to explore its rela-tionship with the Mas signaling pathway.Methods Seventy-two adult male SD rats were randomly divided into four groups(n=18 each):sham group,model group,EE group(group E),and A779 pretreatment group(group A).The CI/RI model was established u-sing middle cerebral artery occlusion method Rats in both group E and group A underwent regular running for four weeks before model preparation,while rats in group A were injected with A779 30 minutes before model preparation.The cognitive function of rats was evaluated using the Neurological Disability Score(NDS)and Morris water maze test.After intravenous injection of Evans blue(EB)for one hour,the rats were euthanized,and brain tissues were collected to measure the infarction volume,EB content,ROS con-tent,and the percentage of apoptotic neurons of the hippocampal CA1 region.The protein expression relat-ed to the Mas pathway was detected by Western blot.Results Compared with the model group,the learning and memory ability as well as the neurological function in group E exhibited a significant improvement(P<0.05).Both the cerebral infarction volume and the ap-optotic neuron percentage in the ischemic hippocampal CA1 region showed a significant reduction in group E(P<0.05).The ROS content along with the EB con-tent in the brain tissue significantly decreased in group E(P<0.05).Furthermore,the expressions of Mas/PKA/CREB/UCP2 pathway related proteins were sig-nificantly enhanced in group E(P<0.05).However,the Mas receptor antagonist A779 significantly inhibited these neurological effects(P<0.05).Conclusion EE may inhibit oxidative stress and alleviate CI/RI in rats by activating the Mas/PKA/CREB/UCP2 signaling pathway.
5.Application of the internet-based 4C continuity care model in home enteral nutrition for postoperative gastric cancer patients

Objective:This study aims to evaluate the effectiveness of the 4C continuous care model in home enteral nutrition management for patients after gastric cancer surgery.Methods:A control group consisting of 40 gastric cancer patients who were hospitalized in the Department of Gastrointestinal Surgery of Suzhou Ninth People's Hospital from January 2022 to June 2022 was chosen.This group received traditional nutritional management guidance,which included nutritional screening and assessment,nutritional monitoring,providing personalized nutritional guidance for each patient,and follow-ups via telephone and outpatient visits.An observation group consisting of 40 gastric cancer patients admitted between July 2022 and January 2023 received the same basic care as the control group,with the addition of the 4C continuous care plan based on an internet platform to improve their enteral nutrition status.The nutritional indicators of both groups were compared at 1 month,3 months,and 6 months post-intervention.Additionally,the results of the Nutritional Risk Screening(NRS 2002)and Patient-Generated Subjective Global Assessment(PG-SGA)at discharge and 6 months post-discharge were compared between the two groups.Results:At 1 month,3 months,and 6 months post-intervention,the observation group showed an increase in indicators such as albumin,prealbumin,hemoglobin,upper limb grip strength,and triceps skinfold thickness compared to the control group(P<0.05).At 6 months post-intervention,the PG-SGA and NRS 2002 scores in the observation group were improved compared to the control group(P<0.05).Conclusion:The implementation of the 4C continuous care model on an internet platform significantly improves home enteral nutrition status for patients after gastric cancer surgery.
6.Construction and Application of"On-Off-On"Fluorescence Sensor for Chlorpromazine Hydrochloride Based on Near Infrared Carbon Quantum Dots

In this work,near infrared carbon quantum dots(NIR-CDs)were synthesized by hydrothermal method using biomass material Clausena lansium leaves.The synthesized NIR-CDs emitted maximum fluorescence signal at 677 nm,which was independent of excitation wavelength.The characterization results showed that there were abundant groups on the surface of NIR-CDs.Pd2+could form non-fluorescent compounds with the surface groups of NIR-CDs,resulting in fluorescence quenching(Fluorescence signal was denoted as F0).Because chlorpromazine hydrochloride(CPZ)parent nucleus contained unoxidized S atom,CPZ could form stable colored complex with Pd2+under acidic conditions.In the presence of CPZ,Pd2+dissociated from the surface of NIR-CDs and bonded with CPZ,so that the fluorescence signal could be restored(Fluorescence signal was denoted as F).An"on-off-on"fluorescence sensor was thus constructed.The fluorescence signal recovery value of NIR-CDs(△F=F-F0)showed a good linear relationship with the concentration of CPZ in the range of 5.68-28.43 μg/mL,and the detection limit(3σ)was 0.078 μg/mL.The sensor was applied to determination of CPZ in pharmaceutical preparations,and the recoveries were 94%-106%.The developed fluorescence sensor was expected to be used in quality control of actual pharmaceutical preparations.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.
8.Development of a general practitioner training program for carotid plaque screening using portable intelligent point-of-care ultrasonography

Objective:To develop a training program for general practitioners (GPs) on carotid plaque screening using portable intelligent point-of-care ultrasonography (POCUS).Methods:A draft training program for GPs on carotid plaque screening using POCUS was initially formulated through focus group discussions involving experts in ultrasonography and general practice. Two rounds of Delphi consultation were conducted with 15 ultrasound experts from 6 secondary and tertiary public hospitals in Shanghai in February and April 2023. The analytic hierarchy process (AHP) was applied to assign weights to the indicators in the final program.Results:The final program consisted of 4 first-level indicators, 10 second-level indicators, and 47 third-level indicators. Among the first-level indicators,"training objects" had the lowest weight (7.69%), while the other three indicators were equally weighted (30.77% each). The top 3 third-level indicators by combined weight were "mastering key diagnostic points of carotid plaque"(10.26%),"mastering carotid plaque screening using portable POCUS"(6.84%), and "primary care hands-on training sessions"(6.39%).Conclusion:A structured training program for GPs has been successfully developed for carotid plaque screening using portable intelligent POCUS, comprising 4 first-level, 10 second-level, and 47 third-level indicators.
9.Effect and mechanism of CXCL8 on the paclitaxel sensitivity of cervical cancer cells

Objective The objective of this study was to explore the effect of knocking down CXCL8 on the efficacy of pacli-taxel chemotherapy in cervical squamous cell carcinoma and to investigate its potential mechanism of action.Methods The Hela cell model was used to specifically inhibit CXCL8 gene expression through lentivirus-mediated RNA interference(RNAi)technology.The optimal transfection conditions were HitransG P and a multiplicity of infection(MOI)of 100.The CCK-8 assay was used to screen the optimal intervention concentration of puromycin as 1.5μg/mL.The LV-CXCL8-RNAi(3 targets)and negative control lentivirus were transfected into cells under optimal transfection conditions and set up a blank control group.The qRT-PCR assay was used to select the sh-CXCL8-13 group lentivirus as the intervention sequence and virus for subsequent experiments.The experiments were divided into the blank control group,negative control group,sh-CXCL8 group,paclitaxel group,and sh-CXCL8+paclitaxel group.The prolifer-ative activity and invasive ability of cervical cancer cells were assessed by CCK-8 and cell invasion assays.The expression of CXCL8,Bcl2,Bax,and β-actin were detected by qRT-PCR and Western blot.Results Compared with the other four groups,the proliferative and invasive ability of Hela cells was significantly reduced in the sh-CXCL8+paclitaxel group,and the difference was statistically sig-nificant(P<0.01).The qRT-PCR results showed that the expression of CXCL8,Bcl2,PIK3CB,and Akt1 genes was significantly re-duced,and the expression of Bax gene was significantly increased in the sh-CXCL8+paclitaxel group.The difference between the groups was statistically significant(P<0.001).The results of Western blot showed that the expression of CXCL8,PIK3CB,and p-Akt1 proteins was reduced in the sh-CXCL8+paclitaxel group(P<0.05).Conclusion Knocking down CXCL8 can reduce the prolif-erative and invasive capacity of Hela cells,possibly by affecting the PI3K/Akt pathway to affect the drug sensitivity of Hela cells to paclitaxel.
10.Importation and analysis of data from a multi-center randomized controlled clinical research on total knee arthroplasty based on REDCap system

Objective To introduce how to import and analyze data using the Research Electronic Data Capture(REDCap)system,taking a multi-center randomized controlled clinical research of total knee arthroplasty as an example.Methods Various tools within the REDCap system,including data import tools,data export functions,reports and statistics,project dashboards,and coding manuals,were used to systematically process and analyze the multi-center randomized controlled clinical trial data for total knee arthroplasty.Initially,electronically collected clinical data were adjusted and standardized,then uploaded in bulk to the system using the REDCap data import tool.Subsequently,the data were organized through REDCap's data export feature,and basic descriptive statistical analysis was performed using its reporting and statistical functions to ensure data quality and completeness.Results An electronic data collection and management platform for clinical research on knee osteoarthritis wase successfully created by the REDCap system.The platform enabled real-time data collection from multiple centers,and ensured data accuracy and consistency through built-in data management and quality control mechanisms.With the statistical analysis features of REDCap,the research team could monitor the progress of data in real time,conduct effective quality assessments,and perform dynamic analysis for further in-depth statistical evaluations.Conclusion The REDCap system can be used not only to build a new clinical research project,but also to import and analyze data that has been previously digitized of ongoing clinical researches into the system,which improved the scientificity of data management and research efficiency.
