1.Health risk assessment of zearalenone in commercially edible vegetable oils in Ningbo City in 2024

ObjectiveTo investigate the contamination levels of zearalenone (ZEN) in commercially available edible vegetable oils in Ningbo City and to assess its health risks to local residents. MethodsA total of 330 samples of commercially available edible vegetable oil samples (50 each of peanut oil, corn oil, and olive oil; 40 each of rapeseed oil and blended oil; 30 each of soybean oil, rice oil, and sunflower seed oil; and 10 of camellia oil) were collected in 2024. The samples were tested for ZEN using the first method specified in GB 5009.209‒2016 National Food Safety Standard―Determination of Zearalenone in Food, namely the liquid chromatography method, and the contamination status was analyzed. Additionally, combined with dietary consumption data of residents, the Monte Carlo simulation method was employed to evaluate the health risks of ZEN in edible vegetable oils. ResultsZEN was detected in 267 out of 330 samples, with a detection rate of 80.91%, and the median (P50) and the 25th, 75th percentiles (P25, P75) of ZEN concentrations were 2.02 (0.37, 17.90) μg·kg-1, with a maximum value of 342.00 μg·kg-1. The ZEN detection rates in corn oil, peanut oil, and blended oil were all 100.00%. The daily average exposure (P50) and daily high exposure (P95) to ZEN via edible vegetable oils among Ningbo residents were 0.001 μg·kg-1 (normalized to body weight, same below) and 0.060 μg·kg-1, respectively. However, 1.22% of Ningbo residents had a daily ZEN exposure exceeding the tolerable daily intake (TDI) of 0.25 μg·kg-1. The hazard quotients (HQ) for the daily average exposure (P50) and daily high exposure (P95) levels were 0.004 and 0.020, respectively, both substantially below 1. Nevertheless, the probability of health risk for Ningbo residents due to ZEN exposure from vegetable oil consumption remained at 1.02%. ConclusionEdible vegetable oils in Ningbo City were contaminated with ZEN, but the probability of ZEN exposure exceeding the TDI through edible vegetable oils was relatively low, and the associated health risk probability were also minimal, indicating an overall insignificant health risk.
2.Advances in the mechanisms of fibrosis at the electrode interface of invasive brain-computer interfaces and intervention strategies

The invasive brain-computer interface (BCI) is a method that involves implanting microelectrodes into brain tissue to collect neural electrical signals. The signals obtained through this method are often of high precision and relatively stable. However, the chronic fibrotic reaction resulting from long-term implantation can significantly impair the quality of the collected brain electrical signals. Therefore, ensuring the long-term stability of signal acquisition is a major challenge in the development of invasive BCI. This paper systematically reviews the formation mechanisms of fibrosis at the electrode interface, elaborating on the progression from acute inflammatory responses to the development of chronic glial scars and the formation of the extracellular matrix (ECM). It introduces the roles, advantages, and disadvantages of three anti-fibrosis strategies: material and surface optimization, drug and biological factor intervention, and integration of immune regulation and tissue engineering. This paper also evaluates their practical effects and limitations in animal and human clinical applications. Finally, it highlights the importance of establishment of standardized follow-up recording mechanisms in ensuring the long-term reliability and stability of invasive BCIs, providing references and insights for future in-depth interface optimization and clinical translation.
3.Increased risk of osteoporotic pathological fractures associated with sterol esters:evidence from IEU-GWAS and FinnGen databases

BACKGROUND:Although previous studies have reported associations between lipids and the risk of osteoporotic pathological fractures,the specific causal relationships between lipid level and osteoporotic pathological fractures remain unclear.OBJECTIVE:To elucidate the causal relationship between lipids and osteoporotic pathological fractures using a two-sample bidirectional Mendelian randomization analysis.METHODS:The data for 178 lipid metabolites were obtained from the IEU-GWAS database(developed by the MRC Integrative Epidemiology Unit at the University of Bristol,UK,which provides extensive summary data from genome-wide association studies),while osteoporotic pathological fracture data(from 173 619 European participants)were acquired from the FinnGen database(constructed by the Finnish national gene research program,focusing on investigating relationships between genomics and health/disease in the Finnish population).Osteoporotic pathological fracture data were used as the outcome variable,with lipids serving as exposures,for the bidirectional Mendelian randomization study to evaluate the causal effects of different lipids on osteoporotic pathological fractures.The UK Biobank database was employed as a validation set by switching the outcome variable to verify the findings horizontally.RESULTS AND CONCLUSION:(1)The inverse variance weighted analysis indicated that each unit increase in sterol ester(27∶1/20∶2)levels was associated with a 25.55%increase in the risk of osteoporotic pathological fractures(odds ratio=1.256,95%confidence interval:1.001-1.575,P=0.049),suggesting a significant positive correlation between elevated sterol ester levels and increased fracture risk.Reverse Mendelian randomization analysis revealed a significant negative association between osteoporotic pathological fractures and three types of phosphatidylcholine.Horizontal validation yielded consistent results,confirming sterol ester as a risk factor for osteoporotic pathological fractures.(2)The results indicate that sterol ester is a risk factor for osteoporotic pathological fractures,while phosphatidylcholine serves as a protective factor.These findings strengthen the evidence supporting the effect of lipids on the risk of osteoporotic pathological fractures.Although the GWAS data used in this study were derived from European populations,given the broad commonality of human genetics,the results provide valuable reference significance for improving osteoporosis in Chinese populations through lipid regulation.

5.Effect of medical-community linkage model on psychological status and motor function in community-dwelling patients with stroke

ObjectiveTo explore the effect of the medical-community linkage model on activities of daily living, psychological status and motor function of stroke patients in the community. MethodsA total of 60 stroke patients admitted to two community health service centers and their affiliated stations in Fengtai District, Beijing, from January, 2024 to August, 2025 were enrolled and randomly divided into control group (n = 30) and intervention group (n = 30). The control group received routine medicine, dietary care and rehabilitation management, while the intervention group underwent rehabilitation with the medical-community linkage model, for twelve weeks. They were assessed with modified Barthel Index (MBI), Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD) and Fugl-Meyer Assessment (FMA) before and after intervention. ResultsAfter intervention, the MBI, HAMA, HAMD and FMA scores of patients improved in both groups (|t| > 5.599, P < 0.001), and improved more in the intervention group than in the control group (P < 0.05), except MBI. The HAMA and HAMD scores of family members decreased in both groups (|t| > 10.333, P < 0.001), and decreased more in the intervention group than in the control group (t > 5.681, P < 0.001). ConclusionThe medical-community linkage model can further improve the motor function of stroke patients in community, as well as the psychological status of both patients and their family members.
6.Exploring the Application of "Cleaning Spleen and Restoring Defensive Qi" Method in Treatment of Pancreatic Cancer based on Neutrophil Extracellular Traps Abnormal Accumulation

The abnormal accumulation of neutrophil extracellular traps (NETs) can promote the initiation and progression of pancreatic cancer, which is considered a potential therapeutic target for this disease. The Miraculous Pivot·Inquiry About Statement (《灵枢·口问》) have recorded the concept of "defensive qi stagnation". Based on the recognition that the function of defensive qi is similar to the immune function of neutrophils, and combining traditional Chinese medicine theory with clinical practice, it is proposed that the abnormal accumulation of NETs may be a pathological product of "defensive qi stagnation", with the spleen being the critical site of pathology. Further exploring the application strategy of cleaning spleen and restoring defensive qi method in pancreatic cancer treatment, it is proposed to employ three approaches such as dredging method to eliminate spleen stagnation and inhibit pancreatic cancer proliferation, cleaning method to remove spleen dampness and suppress the inflammatory micro-environment, and tonifying method to strengthen Weiqi and to improve the immune microenvironment, which aims to provide new insights for the clinical treatment of pancreatic cancer with traditional Chinese medicine.

Therefore, plasma substitutes have become particularly important in fluid resuscitation, especially artificial colloidal solutions, which have a sustained volume expansion time and a good volume expansion effect, and can significantly improve the circulatory status of patients. This article aims to review the classification of artificial colloidal plasma substitutes and their research progress in clinical practice, in order provide a more rigorous, professional and standardized reference for medicine.

9.Research on the application of combined inflatable mediastinoscopy with laparoscopy guided by the concept of “reduced field and port” in esophageal squamous cell carcinoma

Objective:To evaluate the safety and efficacy of combined inflatable mediastinoscopy with laparoscopy guided by the concept of “reduced field and port” during esophagectomy for esophageal cancer.Methods:This is a retrospective cohort study. The clinical data of 497 patients with esophageal squamous cell carcinoma who underwent minimally invasive esophagectomy at the Center of Minimally Invasive Thoracic Surgery, the Second Affiliated Hospital of Naval Medical University, between January 2017 and December 2024 were retrospectively analyzed. There were 416 male and 81 female patients, with an age of (68.3±8.0) years (range: 44 to 89 years). Patients were divided into the traditional video-assisted thoracoscopic surgery group (Group A, n=354) and the combined inflatable mediastinoscopy with laparoscopic surgery group(Group B, n=143) based on the surgical approach. Furthermore, Group B was subdivided into the multiport laparoscopic group (Group B1, n=81) and the single-incision laparoscopic surgery plus one port group (Group B2, n=62). Perioperative indicators and postoperative survival differences were compared between the groups. Inter-group comparisons were performed using the independent sample t-test, χ2 test, or Fisher′s exact probability test. Survival curves were plotted using the Kaplan-Meier method, and the Log-rank test was used to analyze the survival differences between groups. Results:Compared with Group A, Group B demonstrated a significantly shorter operative time ((181.8±11.4) minutes vs. (196.7±8.1)minutes, t=16.09, P<0.01), a lower incidence of postoperative pulmonary complications (8.4% (12/143) vs. 17.8% (63/354), χ2=6.27, P=0.012), lower perioperative mortality (0 vs. 3.1%(11/354), P=0.039), and a shorter postoperative hospital stay ((16.2±2.2)days vs. (18.9±4.1)days, t=8.56, P<0.01). There was no significant difference in the anastomotic leak rate, number of lymph nodes dissected, or intraoperative blood loss between the two groups (all P>0.05). Overall survival time and recurrence-free survival time showed no significant difference between the two groups (all P>0.05). Subgroup analysis revealed no significant differences in perioperative indicators or postoperative complication rates between Group B1 and Group B2. Conclusions:Compared with traditional thoracoscopic combined with laparoscopic surgery, inflatable mediastinoscopy offered advantages in terms of lower postoperative pulmonary complication rates, shorter operative time, reduced postoperative hospital stay, and lower perioperative mortality. The “reduced field and port” concept could further minimize surgical trauma during the transmediastinal approach for esophagectomy while ensuring surgical safety and efficacy.
10.Analysis of the epidemic characteristics of notifiable infectious diseases in Xindu district,Chengdu from 2008 to 2022

Objective To analyze the epidemic trend and characteristics of notifiable infectious diseases in Xindu District of Chengdu,and to explore the changes of disease spectrum,so as to provide scientific basis for formulating prevention and control strategies.Methods The epidemic data of notifiable infectious diseases in Xindu District of Chengdu from 2008 to 2022 were collected and analyzed by descriptive epidemiological method.Results From 2008 to 2022,a total of 66 302 cases of 28 kinds of notifiable infectious diseases were reported in Xindu District of Chengdu,with an average annual reported incidence of 516.94/100 000.A total of 266 deaths were reported,with an average annual mortality rate of 2.07/100 000 and a case fatality rate of 0.40% .No Class A infectious diseases were reported,and Class B infectious diseases had no obvious seasonality.Class C infectious diseases had two incidence peaks from June to August and from October to December.The sex ratio of male to female was 1.41∶1,and the age of onset was mostly 0-4 years old.The reported cases were mainly scattered children,preschool children,students,farmers,housework and unemployed people.According to the main mode of transmission,the annual average reported incidence rate from high to low was intestinal infectious diseases,respiratory infectious diseases,blood and sexually transmitted infectious diseases,insect-borne and natural focal infectious diseases,and other infectious diseases.The mode of transmission gradually transitioned from intestinal infectious diseases to respiratory infectious diseases.From 2020 to 2022,except for COVID-19 and influenza cases,the reported incidence of infectious diseases in other transmission routes decreased.The reported diseases were mainly concentrated in tuberculosis,hepatitis B,syphilis,mumps,hand-foot-mouth disease,influenza and other infectious diarrhea,and the annual reported incidence order changed slightly.Conclusion From 2008 to 2022,the reported incidence of Class B infectious diseases in Xindu District of Chengdu showed a steady decline,while the reported incidence of Class C infectious diseases showed a fluctuating upward trend.The prevention and control measures of the new coronavirus epidemic are conducive to reducing the incidence of notifiable infectious diseases.It is necessary to focus on the prevention and control of high-risk infectious diseases such as hand-foot-mouth disease,influenza,other infectious diarrhea,tuberculosis,hepatitis B,and acquired immune deficiency syndrome,with highly reported mortality.Targeted health interventions should be carried out for key populations,key seasons,and key areas.At the same time,the monitoring of new and rare infectious diseases should be strengthened.
