1.Practical skills development for medical students in a medical college under the background of new medical science

Objective:To explore medical students'cognition,current status,and demands regarding practical skills training in a medical college under the background of new medical science.Methods:A cross-sectional survey was conducted among medical undergraduates from Year 1 to Year 5 in a medical college.A self-designed scale was used to investigate the cognition,current status,and demands related to new medical education concepts.Pearson correlation analysis was used to analyze the relationship between satisfaction with practical teaching facilities and variables including understanding of new medical education,curriculum design,and proportion of practical teaching.Multiple linear regression was conducted to investigate the factors influencing satisfaction.Results:Among 1 253 participants,70.71%acquired new medical science information through online media,while 52.99%learned about it via school courses.Over 80%endorsed"whole-cycle health management"(preventive care before disease onset 91.94%,disease treatment 81.72%,and post-illness rehabilitation 82.60%).Experimental courses(85.16%),clinical skills training(66.88%),and social practice(66.24%)were primary practical forms,but the participation rates of research practice(40.78%)and innovation/entrepreneurship practice(34.72%)was comparatively lower.The satisfaction with practical teaching was positively correlated with adequacy of faculty guidance(r=0.707)and curriculum rationality(r=0.522)(P<0.01).The multiple linear regression analysis showed that faculty guidance(β=0.436)and the proportion of practical teaching(β=0.319)were the key predictors of satisfaction.Conclusion:Medical students'cognition of practical skills training,curriculum optimization,and adequacy of faculty guidance significantly influence satisfaction with practical teaching,with faculty guidance and rationality of practical process playing key roles.

3.Effects of two irradiation methods on alkaloids in Sophorae Flavescentis Radix and its preparations

Objective:To study the effects of electron beam irradiation and 60Co irradiation on the composition changes of four alkaloids in Sophorae Flavescentis Radix, intermediate extracts of Sophorae Flavescentis Radix and Lixieling Tablets. Methods:Sophorae Flavescentis Radix, intermediate extracts of Sophorae Flavescentis Radix and Lixieling Tablets were irradiated at different doses of 0, 1.5, 3, 5, 7, 10, 20, 30, 40 kGy by electron beam irradiation and 60Co irradiation. The contents of oxymatrine, oxysophocarpine, matrine and sophocarpine were determined by HPLC, and the changes of the components before and after irradiation were compared. Results:Oxymatrine, oxysophocarpine, matrine and sophocarpine were among 0.046 9-0.937 4 μg, 0.020 5-0.410 4 μg, 0.098 9-1.977 9 μg, 0.048 7-0.973 1 μg, respectively. The linear relationship was good. The average recovery rates were 98.1%, 100.1%, 100.5%, 96.6%, respectively, and the RSDs were 1.69%, 2.03%, 3.14% and 1.10%, respectively. Electron beam irradiation and 60Co irradiation had no statistical significance on the changes of oxymatrine, oxysophocarpine, matrine and sophocarpine in Sophora flavescens, but had statistical significance in the contents of intermediate extracts of Sophorae Flavescentis Radix and Lixieling Tablets. Conclusion:The established method for the determination of matrine is accurate, reproducible, simple and practical, and can be used for the quality control of Lixieling Tablets. Irradiation has no significant effect on the content of Sophorae Flavescentis Radix, while high dose irradiation has significant effect on the intermediates and finished products of Sophorae Flavescentis Radix, which can provide a basis for quality control and sterilization irradiation of enterprises.
4.Study on the Quality Standard of Lixieling Tablets Based on Multicomponent Analysis

OBJECTIVE To establish the quality standard of Lixieling tablets. METHODS Thin layer chromatography was used to identify Bistortae Rhizoma, Andrographis Herba and Sophorae Flavescentis Radix in Lixieling tablets. The components of andrographolium and matrine were determined by HPLC. RESULTS The thin layer spots of Bistortae Rhizoma, Andrographis Herba and Sophorae Flavescentis Radix were clear, with good separation effect and repeatability. The linear relationship between andrographolide, neoandrographolide, 14-deoxygenated andrographolide, dehydrated andrographolide, sophorine, oxymatrine, sophorine and oxysophorine and their respective concentration ranges was good. The RSDs of instrument precision, repeatability, stability and recovery all met the requirements. CONCLUSION The method is simple and reproducible, and can be used for the quality control of Lixieling tablets.
5.Behavior preferences of pregnant women in Quzhou to obtain newborn health information

Objective:To explore the behavioral preferences of pregnant women in Quzhou to obtain newborn health information, so as to provide a reference for relevant departments to formulate policies.Methods:From January 2020 to April 2021, convenience sampling was used to select 226 pregnant women who gave birth in Quzhou Hospital of Traditional Chinese Medicine as the research object. The self-designed Behavior Preference of Pregnant Women to Obtain Newborn Health Information Questionnaire was used to investigate pregnant women through the form of online return visits. A total of 226 questionnaires were issued, and 215 valid questionnaires were returned with the valid response rate of 95.13%.Results:Among 215 pregnant women, they thought it was difficult to obtain information on the prevention and treatment of common diseases (such as diaper rash, impetigo) , distinguishing and nursing of physiological phenomena (such as jaundice, galactorrhea) , form and content of early education, mental behavior building and premature infant health monitoring. These 215 pregnant women preferred to obtain newborn health information from the WeChat public platform/ application (APP) , brochures/billboards, online teaching and expert consultation.Conclusions:Pregnant women have diverse needs for newborn health information, and they prefer many ways to obtain it. In clinical practice, we should broaden the ways of obtaining specialized information to provide pregnant women with scientific and accurate newborn health information.

7. Analysis of variants in complement genes in Han Chinese children with atypical hemolytic uremic syndrome

Objective:
To investigate the prevalence and characteristics of pathogenic variants in complement genes in Han Chinese children with atypical hemolytic uremic syndrome (aHUS).
Method:
Eleven Han Chinese children with aHUS, including 9 boys and 2 girls aged between 1 year and 4 months and 13 years, were investigated in Department of Pediatrics, Fuzhou General Hospital, from November 1998 to February 2014. Analysis of variants of all the exons of 10 complement genes (CFH, MCP, CFI, C3, CFB, CFHR1, CFHR2, CFHR3, CFHR4 and CFHR5), including 25 bases from 3′ end and 25 bases from 5' end, was performed in the 11 cases by targeted sequence capture and next generation sequencing. Significant variants detected by next generation sequencing were confirmed by Sanger sequencing. To understand pathogenicity of variants found in the captured genes, we investigated genetic conservation by multiple protein sequence alignment among different species, and analyzed whether the variants were located in protein domains or not, and investigated functional significance by functional computational prediction methods.
Result:
Twenty-seven percent of Han Chinese children with aHUS carried pathogenic variants in the 10 complement genes. Pathogenic variant CFB 221G>A (R74H) was detected in Patient 3 and Patient 9, which was not found in parents of Patient 3′ , and was found in healthy father of patient 9. Pathogenic variant CFHR5 242C>T (P81L) was found in Patient 2, and was found in healthy father of patient 2. However, no pathogenic variants in genes CFH, MCP, CFI, C3, CFHR1, CFHR2, CFHR3 and CFHR4 were identified.
Conclusion
Pathogenic variants in the 10 complement genes were identified in 3/11 of Han Chinese children with aHUS in our study and CFB was the most frequently mutated gene.
8.Effect of lipopolysaccharide on IL-6/STAT3 signal pathway in human intrahepatic biliary epithelial cells

Objective To investigate the expression of IL-6/STAT3 signal pathway induced by lipopolysaccharide (LPS) on human intrahepatic biliary epithelial cells (HiBECs) in vitro.Methods The HiBECs were cultured in vitro.The cells were stimulated by LPS at different concentrations (0,0.1,1,2,4,8 μg/ml) and different time (24,48,72 h).The expression of IL-6 was measured by ELISA to determine the best concentration and time of LPS.Then we chose the best concentration and time of LPS to intervene the cells in the further experiments.The levels of mRNA expression of c-Myc and Mcl-1 were detected by fluorescence quantitative reverse transcrption PCR (RT-qRCR).The proportion of p-STAT3/STAT3 protein and the expression of c-Myc and Mcl-1 protein in HiBECs were analyzed by Western blot.Results LPS upregulated the expression of IL-6, and reached its peak at 4 μg/ml and 24 h(F=16.492,17.763,P<0.001,P<0.01).LPS increased the mRNA expression of c-Myc and Mcl-1(P<0.01).Meanwhile, The ratio of p-STAT3/STAT3 (P<0.05) protein and the expression of c-Myc (P<0.001) and Mcl-1 (P<0.05) protein were upregulated after stimulating with LPS.Conclusion The IL-6/STAT3 signal pathway can be activated by LPS in HiBECs, and LPS may promote the expression of c-Myc and Mcl-1 through this signal pathway.
9.Survival Meta analysis for Entecavir versus Lamivudine therapy for patients with HBV-related liver failure

Objective To evaluate the short-term and long-term survival rate for Entecavir versus Lamivudine in the treatment of patients with HBV-related liver failure through Meta-analysis.Methods Relevant literature published from 2007 to 2015 in Chinese or English were searched,papers fully matching the inclusion criteria were brought into the study.RavMan 5.2 statistical software was utilized for Meta-analysis.The survival rate of 4-week,12-week,24-week,48-week were analyzed by forest plot.Results Total 1837 patients from 18 controlled clinical trials reported in Chinese and English were recruited in this study,which involving survival rate for Entecavir versus Lamivudine in the treatment of patients with HBV-related liver failure.Among them 12 papers were cohort and 6 were RCT(Randomized Controlled Trial) study.Meta analysis show:HBV DNA undetectable rate of 4-week、12-week、24-week and 48-week in Entecavir group were higher than that in Lamivudine group (4-week:RR =1.52,Z =4.11,P < 0.01;12-week:RR =1.26,Z =3.85,P <0.01;24-week:RR =1.26,Z =5.16,P <0.01;48-week:RR =1.22,Z =3.41,P < 0.01),survival rate of four-week,twelve-week,twenty four-week had no significant differences between Entecavir and Lamivudine group (4-week:RR =0.97,Z =0.55,P =0.58;12-week:RR =1.01,Z =0.15,P =0.88;24-week:RR =1.02,Z =0.56,P =0.57),fourty eight-week survival rate in Entecavir group was higher than that in Lamivudine group(48-week:RR =1.18,Z =2.84,P < 0.01).Conclusions The data indicated that Entecavir was more effective in the management of HBV levels and better improve the long-term prognosis of patients with HBV-associated liver failure than Lamivudine,which might be more suitable for long-term treatment to patients with HBV-related liver failure.
