1.Application of doctors-nurses-patients theory of goal attainment combined with StMarys cluster intervention mode in patients with tracheotomy after stroke

Objective:To exploring the application effect of doctors-nurses-patients theory of goal attainment combined with StMarys cluster intervention mode in patients with tracheotomy after stroke.Methods:Using convenient sampling method, a total of 130 stroke patients with tracheostomy who were admitted to Shangqiu First People's Hospital from August 2020 to February 2022 were selected as the research objects. The patients were divided into the control group and the observation group using the random number table method, with 65 cases in each group. The control group received routine nursing care, while the observation group applied doctors-nurses-patients theory of goal attainment combined with StMarys cluster nursing intervention mode on the basis of the control group. After intervention, scores of Self Rating Anxiety Scale (SAS) and Self Rating Depression Scale (SDS) and patient nursing satisfaction were compared between the two groups.Results:After intervention, the SAS and SDS scores of the observation group were lower than those of the control group, and the nursing satisfaction was higher than that of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The doctors-nurses-patients theory of goal attainment combined with StMarys cluster intervention mode can help improve the psychological state of patients and increase nursing satisfaction.
2.Competency and professional activities for Chinese pediatric rehabilitation therapists based on World Health Or-ganization rehabilitation competency framework

Objective To construct the competency and professional activities of pediatric rehabilitation therapists in China based on World Health Organization rehabilitation competency framework(RCF). Methods Competencies,activities,and tasks in the field of pediatric rehabilitation were collected through literature,offi-cial websites and interviews with key informants using RCF.Competency requirements suitable for pediatric re-habilitation therapists were selected,competency dimensions and activities were adjusted based on the context of pediatric rehabilitation therapy.Similar content was integrated through thematic framework analysis and content analysis.Competencies were matched with typical professional activities.A competency framework and profes-sional activities for Chinese pediatric rehabilitation therapists were developed,and the Delphi method was used to survey 22 experts,ultimately confirming competency dimensions and activity content. Results The enthusiasm coefficient of experts was 88.00%,the authority coefficient was 0.84,and the expert opinions were more focused on importance.The CV values of all competency dimensions and activities were≤0.25,and the W test was statistically significant(P<0.05),indicating a high degree of coordination among experts.A com-petency framework for pediatric rehabilitation therapists in China,included 17 competency dimensions and 17 professional activities. Conclusion To explore the competency of typical professions through RCF may enhance the application of research re-sults in the field of rehabilitation,which provides a reference for clarifying the job content and cultivating the competency of Chinese pediatric rehabilitation therapists.
3.Competency and professional activities of pediatric rehabilitation therapists in three provinces/cities of East China

Objective To investigate the current situation of competency and professional activities of pediatric rehabilitation thera-pists in Jiangsu,Shanghai and Zhejiang. Methods A questionnaire survey was conducted on 168 pediatric rehabilitation therapists in Jiangsu,Shanghai and Zheji-ang,from December,2022 to February,2023,and the general information and the competency were obtained.A total of 35 pediatric rehabilitation therapists were interviewed one-on-one,from October,2022 to January,2023,using thematic framework. Results A total of 162 effective questionnaires were collected.The level of competency of pediatric rehabilitation thera-pists was(3.44±0.87);and it was higher in professional literacy(3.90±0.77)and practice(3.83±0.77),and lower in management and leadership(2.92±1.44),and research(2.14±1.64).Latent class analysis showed that the thera-pists could be divided into four categories,of which the"medium competency"accounted for the most proportion(34.57%),and"low learning and research"was the least(8.64%).The interviews identified problems of compe-tency such as maintaining communication with children's families and managing self-professional development. Conclusion Self-reported rehabilitation competency levels of pediatric rehabilitation therapists in three provinces and cit-ies are above three points in all four dimensions,except for the management and leadership dimension,where the score is below three points.
4.Design and implementation of post level management and secondary performance assessment and distribution in Central Supply Service Department

Objective:To explore the effects of post level management and secondary performance distribution in the Central Supply Service Department.Methods:From January 2019, the job categories, job labor intensity, technical risk level, division of professional staff, specialist training, job description revision, job capability and other aspects of all staff ( n=67) of Central Supply Service Department of the First People's Hospital of Shangqiu, Henan were set up, and the competitive employment mechanism was applied. Based on the total amount of performance issued by the Nursing Department according to the total number of people in the disinfection supply center, the scientific design and implementation of post performance secondary assessment distribution and post promotion were carried out according to post workload, work quality, technical research, service satisfaction and so on. Results:The difference in the results of comprehensive skills assessment of nurses before and after the performance of secondary assessment and distribution management was statistically significant ( P<0.05) . The difference of job satisfaction of disinfection and supply center among different departments was statistically significant ( P<0.05) . There was statistical significance in the comparison of quality control defects in quality control examination of department ( P<0.05) . In the democratic evaluation of staff, 43 were very satisfied with the performance distribution, 3 were dissatisfied. The content of dissatisfaction was the coefficient in lactation has no difference from other posts. Conclusions:The implementation of the post level management and secondary performance assessment in the Central Supply Service Department has stimulated the potential of nurses at all levels, aroused their enthusiasm for work, and set up goals for everybody. All staff participating in the management improved the work efficiency and quality, guaranteed the nursing safety and the work operation, achieving the satisfactory effect.
5.Current status of mental resilience of patients undergoing hysteroscopy and its influencing factors

Objective:To understand the current status of mental resilience of patients undergoing hysteroscopy and its influencing factors so as to provide guidance for clinical health education.Methods:By the convenient sampling method, patients who underwent hysteroscopy in the gynecological clinic in a ClassⅢGradeA hospital in Jiaozuo from January to June 2019 were selected as research objects. Connor-Davidson Resilience Scale (CD-RISC) , Perceived Social Support Scale (PSSS) and General Data Questionnaires were used to conduct survey to explore the current status of mental resilience of patients undergoing hysteroscopy and its influencing factors. This study retrieved 157 valid questionnaires.Results:The score of CD-RISC in 157 patients undergoing hysteroscopy was (58.20±27.45) , with a minimum of 16 points and a maximum of 107 points. There were statistically significant differences in the total scores of CD-RISC among patients of different ages, education background and residential area who underwent hysteroscopy ( P<0.05) . The correlation analysis showed that the level of mental resilience of patients undergoing hysteroscopy was positively correlated with the score of social support ( P<0.05) . Multiple linear regression analysis showed that age, education background, living area, family support were the main factors influencing the mental resilience of patients undergoing hysteroscopy ( P<0.05) . Conclusions:The mental resilience of patients undergoing hysteroscopy is relatively poor. Age of patients, educational background of patients, living area, family support are the important factors affecting mental resilience. Targeted disease education and psychological counseling should be carried out for different groups. Multiple channels and diversified ways should be adopted to strengthen social support so as to improve mental resilience of patients, enable them to cope with the disease in a positive way and promote recovery.
6.Genetic analysis of three children with disorders of sex development caused by structural rearrangements of Y chromosome.

OBJECTIVE:
To explore the genetic basis of three children with disorders of sex development (DSD) in association with rare Y chromosome rearrangements.
METHODS:
The three children, who all featured short stature and DSD, were subjected to G banding chromosomal karyotyping, multiplex PCR for Y chromosomal microdeletion, sequencing of the whole SRY gene, SNP-array analysis for genomic copy number variations, and fluorescence in situ hybridization (FISH).
RESULTS:
The combined analysis revealed chromosomal abnormalities in all of the three children, including 46,X,t(X;Y)(p22.3;q11.2) in case 1, mos 45,X,der(7)pus dic(Y:7)(p11.3p22)del(7)(p21.2p21.3) del(7)(p12.3p14.3) [56]/45,X [44] in case 2, and mos 45,X [50]/46,X,idic(Y)(q11.22) [42]/47,X,idem×2 [4]/47,XYY [2] in case 3.
CONCLUSION
Combined use of genetic techniques can delineate complex rearrangements involving Y chromosome in patients featuring short stature and DSD. Above findings have enabled molecular diagnosis and genetic counseling for the patients.

7.Analysis of monitoring results of coal-burning-borne arsenic poisoning in Xiangyang City, Hubei Province from 2014 to 2018

Objective:To analyze the monitoring results of coal-burning-borne arsenic poisoning in Xiangyang City, Hubei Province from 2014 to 2018.Methods:In 2014-2018, according to "the Monitoring Plan for Coal-burning-borne Endemic Arsenic Poisoning in Hubei Province", five villages in Xiangyang City were selected as monitoring sites every year. Coal samples from 8 households were collected by four-point method after multi-point sampling and mixing in each site every year, coal arsenic content was detected. Urine samples of 10 adults (half male and half female) were collected to detect the arsenic content in urine. The disease condition of residents with coal-burning-borne arsenic poisoning was surveyed.Results:From 2014 to 2018, there was a significant difference in coal arsenic content ( F=21.572, P < 0.05), and the coal arsenic content in 2018 was significantly lower than those in other years ( P < 0.05); there was no significant difference in arsenic content in adult urine (χ 2=1.647, P > 0.05). During the past five years, 266 suspected cases, 736 mild cases, 633 moderate cases and 18 severe cases were detected. There was no significant difference in the detection rates of arsenic poisoning among different years (χ 2=1.094, P > 0.05). Conclusions:The detection rate of coal-burning-borne arsenic poisoning in Xiangyang City from 2014 to 2018 is relatively stable, and the management of high arsenic coal has achieved initial results. It is necessary to further strengthen disease monitoring, energy diversification transformation and health education of residents, so as to further improve the prevention and management mechanism of coal-burning-borne arsenic poisoning.
8.Influence of centralized management mode on disinfection and sterilization of flexible endoscopes in Central Sterile Supply Department

ObjectiveTo explore the influence of centralized management mode on disinfection and sterilization of flexible endoscopes in Central Sterile Supply Department(CSSD). MethodsBy cluster sampling,the endoscopes processed in the First People's Hospital of Shangqiu in He'nan province from 2015 to 2016 were selected for investigation. The endoscopes processed from 2015 to 2018 were put into the control group (n=47 250)and those processed from 2017 to 2018 were in the experimental group(n=55 319). The cleaning and sterilization room was set up to manage the flexible endoscopes in the control group while the CSSD centralized management mode was adopted in the experimental group. The quality of sterilization and disinfection in the two groups were compared. ResultsThere were statistically significant differences between the two groups of endoscopes in the sterilization and disinfection quality monitoring results(P<0.05). ConclusionsThe cleaning, disinfection and sterilization of flexible endoscopes under the CSSD centralized management mode ensures the quality of disinfection and sterilization of flexible endoscopes and the safety of doctors, nurses and patients by reasonably equipping facilities and personnel and strict quality control.
9.The Influence of Transitional Care on the Quality of Life and Psychology of Patients with Acute Myocardial Infarction after PCI

Objective:To discuss the effect of transitional care on the quality of life ( QOL) and psychology in acute myocardial infarction patients after PCI. Methods:The acute myocardial infarction patients accepted PCI in our university affiliated hospitals were selected and divided into experimental group and control group randomly. The control group received routine care during the period of hospitalization. The experimental group received transi-tional care including home visits, massed learning, telephone follow-up, and the establishment of WeChat and QQ group. The QOL, anxiety, and depression were investigated pre-intervention and six months post-interven-tion to evaluate the effect of transitional care. Results:After transitional care six months later, the QOL scores in the experimental group were significantly higher than those in the control group, SAS and SDS scores were lower in experimental group than those in the control group ( P<0 . 05 ) . Conclusions:Transitional care for acute myocar-dial infarction patients after PCI could improve their QOL, and relieve their anxiety and depression level in different degrees.
10.Polymorphisms of catechol-O-methyltransferase and monoamine oxidase B genes among Chinese patients with Parkinson's disease.

OBJECTIVETo study polymorphisms of catechol-O-methyltransferase (COMT) and monoamine oxidase B (MAO-B) genes among Chinese patients with Parkinson's disease.
METHODSGenotypes of the COMT and MAO-B genes of 1408 patients with Parkinson's disease was sequenced using Sanger method. And these patients were recruited by Chinese Parkinson Study Group from 29 research centers throughout the country.
RESULTSThe genotypic frequencies of COMT rs4680 AA, AG, GG were 8.9%, 42.0% and 49.1%. Those of rs4818 CC, CG, GG were 42.5%, 45.6% and 11.9%, respectively. The genotype frequencies of MAO-B rs1799836 A/AA, AG, G/GG were 74.4%, 14.1% and 11.5%, respectively. The haplotype formed by COMT rs4680 (GG) and MAO-B rs1799836 (A/AA) genotype has a frequency of 36.86%.
CONCLUSIONPolymorphisms of COMT and MAO-B genes has a unique characteristics among Chinese patients with Parkinson's disease. They may be related with differences in drug response in such patients.
