Objective:
To analyze the epidemilogical and seasonal characteristics of foodborne Salmonella-associated diarrhea among children aged 0-6 years in Guizhou Province from 2016 to 2023, so as to provide a basis for the prevention and control of foodborne diseases.
Methods:
Data were extracted from the Foodborne Disease Survellance System for cases reported between January 1, 2016, and December 31, 2023. The incidence, seasonal characteristics, and peak periods were analyzed by the method of concentration and circular distribution.
Results:
A total of 6 434 cases of diarrhea in children aged 0-6 years were collected, and 455 cases of Salmonella were detected, with a positive detection rate of 7.07%. Salmonella typhimurium was the main serotype causing diarrhea (59.34%). The peak of the disease was from May 3 to September 30, with certain seasonal characteristics. The highest detection rate was found in children aged 1-3 years (8.66%). Among food types, the positive detection rates of Salmonella were relatively high in other foods (17.39%), fruits and their products (10.22%), infant and toddler foods (10.09%), and aquatic animals and their products (9.80%). The processing and packaging methods of food were mainly home-made (9.38%) and bulk food (7.54%).
Conclusions
The detection rate of Salmonella in children aged 0-6 years is high in Guizhou Province, with strong seasonal characteristics. The detection rates of other foods, fruits and their products, infant and toddler foods, and aquatic animals and their products are high. Enhanced pathogen surveillance for susceptible populations and high-risk foods, coupled with public health education during summer/autumn, is recommended.

Zhishi Xiebai Guizhitang is a classical prescription for the treatment of chest impediment with the method of warming Yang. It is included in the Catalogue of Ancient Classical Prescriptions issued by the National Administration of Traditional Chinese Medicine (First Batch), with the effect of activating Yang, dissipating mass, moving Qi and resolving phlegm. Its main symptoms include chest fullness and pain, or even chest pain radiating to the back, wheezing, coughing, shortness of breath, and Qi reversal from the hypochondrium. In modern traditional Chinese medicine, Zhishi Xiebai Guizhitang is clinically used in the treatment of cardiovascular system, digestive system, respiratory system and other diseases, among which coronary heart disease, unstable angina pectoris, myocardial infarction, sinus bradycardia and other cardiovascular diseases have particularly significant effects. This paper reviewed the pharmacological studies of Zhishi Xiebai Guizhitang in the past 10 years. The results showed that each single medicine and the whole prescription alleviated the above cardiovascular diseases to a certain extent, with the pharmacological effects of improving intravascular environment, myocardial ischemia, myocardial ischemia-reperfusion injury, and myocardial hypoxia, anti-inflammation, plaque stabilisation, etc., and the pharmacological mechanism involved the regulation of relevant active substances in vivo as well as related signaling pathways and ion channels, mainly including thromboxane B2 (TXB2), prostacyclin I2(PGI2) and phosphatidylinositol 3-kinases/protein kinase B/endothelial nitric oxide synthase (PI3K/Akt/eNOS) signaling pathways, and ATP-sensitive potassium channels. In addition, the relationship between the pharmacological effects of some single medicines and transient receptor potential ankyrin 1 (TRPA1) has been reported that TRPA1 is a key to understanding the mechanism of Zhishi Xiebai Guizhitang in treating cardiovascular diseases, which is worth of further study.

Objective:To understand the genome sequence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and spike protein variations during different epidemic periods in Wuxi City. Methods:Nucleic acid was extracted from the nasopharyngeal swab samples of six local cases of coronavirus disease 2019 (COVID-19) (from January to February, 2020) and 13 imported cases of COVID-19 (from March to September, 2021) in Wuxi City, and the whole genome was amplified to construct the sequencing library. The second-generation sequencer was used for sequencing. The CLC Genomics Workbench (21 version) software was used to analyze the offline data with NC_045512.2 as the reference strain, and MEGA 7.0 software was used to construct the phylogenetic tree.Identification of type was conducted by Nextstrain typing method and phylogenetic assignment of named global outbreak lineages (Pangolin) typing method. Results:There were five subtypes in Nextstrain and seven subtypes in Pangolin of the nineteen patients with COVID-19. Compared with NC_045512.2, the median nucleotide mutation sites were 29 (range 0 to 42) and amino acid mutation sites were 20 (range 0 to 34). The six local and 13 imported cases had no common nucleotide mutation sites and were in different evolutionary branches. The sequences of the six local cases were highly homologous with the reference strain sequences (NC_045512.2) at the early stage of the pandemic, and the evolutionary distance was close to that of the reference strain. The 13 imported cases were obviously divided into three evolutionary branches (Alpha, Beta, Delta variant). The four Beta variants shared eight amino acid mutation sites in spike protein, and the two Alpha variants shared eight amino acid mutation sites in spike protein, and the seven Delta variants shared five amino acid mutation sites in spike protein. Conclusions:New mutations of SARS-CoV-2 are constantly emerging during the epidemic. The increase of the nucleotide sites number may result in the change of spike protein amino acid. Therefore, the whole-genome sequencing analysis plays an important role in the accurate tracing of epidemic origin and adjustment of prevention and control measures.

Objective:To evaluate the guidelines on discharged health education for solid organ transplant recipients at home and abroad, analyze the characteristics of each guideline, so as to provide reference for the development of health education for organ transplantation in China. Methods:This was a guideline quality evaluation study. The guidelines on health education for discharged recipients with solid organ transplantation were searched on the websites of clinical practice guidelines, the websites of transplantation professional associations, and Chinese and English databases at home and abroad through computers. The search time limit was from January 1, 2011 to January 1, 2022. The quality of guidelines that met the inclusion and exclusion criteria was evaluated using the Appraisal of Guidelines for Research and Evaluation Ⅱ (AGREE Ⅱ) .Results:A total of 8 guidelines were included. The overall quality evaluation of the guideline was Grade A ( n=2) and Grade B ( n=6) . In terms of standardized scores in six areas of quality evaluation, the average values of scope and purpose, stakeholder involvement, rigour of development, clarity and presentation, applicability as well as editorial independence were 87.62% (70.37%-100.00%) , 75.46% (44.44%-97.22%) , 66.54% (43.75%-93.06%) , 90.16% (77.78%-96.30%) , 48.18% (20.83%-68.75%) and 74.13% (8.30%-97.22%) , respectively. The recommendations of the guidelines covered 9 aspects: team composition and support services, drug management, infection prevention, complication management, diet and nutrition, daily life and social reintegration, contraception and reproduction, psychological guidance and follow-up review. Conclusions:The development of guidelines for different types of solid organ transplantation varies. The overall quality of the guidelines needs to be improved and the content needs to be refined. In the future, we can learn from foreign high-quality guidelines to carry out localization practice, so as to build a health education related guideline for discharge of solid organ transplantation that is suitable for China's national conditions as soon as possible.

ObjectiveTo investigate the mechanism of Renshentang, recorded in Synopsis of Golden Chamber, in the treatment of atherosclerosis (AS) based on the autophagic effect of transient receptor potential vanilloid subtype 1 (TRPV1) on arterial smooth muscle. MethodFourteen SPF-grade 8-week-old male C57BL/6J mice were assigned to the normal group and 70 8-week-old apolipoprotein E knockout (ApoE-/-) mice were assigned to the experimental group. The AS model was induced by a high-fat diet in the mice in the experimental group for eight weeks. The model mice were then randomly divided into model group, low-, medium-, and high-dose Renshentang groups (2.715, 5.43, and 10.68 g·kg-1·d-1), and simvastatin group (0.02 g·kg-1·d-1). Drug treatment lasted eight weeks. Serum was taken and serum total cholesterol (CHO), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) levels were measured by assay kits to observe the changes in lipid levels in mice. The aorta was stained with hematoxylin-eosin (HE) to observe the overall pathology of the aortic root and oil red O staining was used to detect the lipid deposition in the aortic plaque and calculate the percentage of the aortic root area to the lumen area. The protein expression of TRPV1, adenylate-activated protein kinase (AMPK), phosphorylated AMPK (p-AMPK), autophagy effector-1 (Beclin-1), and microtubule-associated protein 1 light chain 3 (LC3Ⅱ) in mouse aortic tissues was determined by Western blot. ResultCompared with the normal group, the model group showed increased serum CHO, TG, and LDL-C levels, decreased HDL-C, and increased aortic root plaque area (P<0.01). Compared with the model group, the Renshentang groups showed decreased levels of CHO, TG, and LDL-C in serum (P<0.05, P<0.01), especially in the low- and medium-dose Renshentang groups (P<0.01). Compared with the normal group, the simvastatin group and the Renshentang groups showed reduced aortic root plaque area (P<0.05), especially in the high-dose Renshentang group (P<0.01). Compared with the normal group, the model group showed decreased relative expression levels of TRPV1, p-AMPK/AMPK, Beclin-1, and LC3Ⅱ/LC3Ⅰ(P<0.05, P<0.01). Compared with the model group, the medium- and high-dose Renshentang groups showed increased relative expression levels of TRPV1, p-AMPK/AMPK, Beclin-1, and LC3Ⅱ/LC3Ⅰ(P<0.05,P<0.01). ConclusionThe anti-AS effect of Renshentang recorded in Synopsis of Golden Chamber may be achieved by up-regulating TRPV1 expression to restore the level of autophagy mediated by AMPK.
