1.Role of CDH1 gene DNA methylation in autoimmune thyroiditis in population from different water-iodine regions

Objective:To study the role of cadherin 1 (CDH1) gene DNA methylation in autoimmune thyroiditis (AIT) in population from different water-iodine regions.Methods:From May to June 2019, the information of AIT cases and healthy individuals in Shandong Province were collected in three types of water-iodine regions: iodine-fortification (IF) region, iodine-adequate (IA) region and iodine-excess (IE) region. A case-control study design was applied to match 176 AIT cases (case group) with age, gender, body mass index, and place of residence in a 1 ∶ 1 ratio to 176 healthy individuals (control group). Fasting urine and whole blood samples were collected to test the contents of urinary iodine, thyroid function indicators [serum free triiodothyronine (FT 3), free thyroxine (FT 4), thyroid stimulating hormone (TSH)], and serum iodine. The DNA methylation levels of the target region of the CDH1 gene and its four CpG sites in whole blood were determined using methylation sequencing technology for target regions (MethylTarget TM). Results:The DNA methylation level of the target region of CDH1 gene in the case group was 0.832 ± 0.044, and that in the control group was 0.828 ± 0.049, there was no statistically significant difference between the two groups ( t = 0.76, P = 0.448). There was no statistically significant difference in DNA methylation levels of the four CpG sites in the target region of CDH1 gene between the case group and the control group ( P > 0.05). There was no statistically significant difference in the DNA methylation level of the CDH1 gene target region between the case group and the control group in IF, IA and IE regions ( P > 0.05). The detection results of DNA methylation levels at CpG sites in the target region of CDH1 gene in different water iodine regions showed that the DNA methylation level at site 83 in case group in IF region was higher than that in the control group ( t = 2.30, P = 0.023). However, there was no statistically significant difference in the DNA methylation levels of the four CpG sites between the case group and the control group in IA and IE regions ( P > 0.05). The DNA methylation level of CDH1 gene target region in AIT patients was not significantly correlated with urinary iodine, serum iodine, and serum FT 3, FT 4, and TSH contents ( P > 0.05), but was significantly negatively correlated with age ( r =-0.19, P = 0.014). Conclusions:The DNA methylation level at CpG site 83 of CDH1 gene in AIT patients in IF region is significantly higher than that in control population, indicating that DNA methylation at this locus may be involved in the occurrence and development of AIT after iodine fortification. The DNA methylation level of CDH1 gene is negatively correlated with age.
2.Established cell model and mechanism of visceral hypersensitivity and nerve hyperplasia in IBS using P815 and N2a co-culture

AIM:To establish a cell model of visceral hypersensitivity and nerve hyperplasia in irritable bowel syndrome(IBS)by conducting an in vitro co-culture of mouse P815 mast cells and N2a nerve cells and explore its possible mechanism.METHODS:Enzyme-linked immunosorbent assay(ELISA)with three replicates was used to confirm the C48/80-induced P815 degranulation.The length of neurites was observed under bright field microscopy to determine the number of differentiated neurons,thereby selecting the concentration of retinoic acid(RA)for stimulating the differentia-tion of N2a cells,with four replicates.A co-culture system of P815 and N2a cells was established using Transwell cham-bers with four replicates.The following groups were established:N2a cells cultured alone,N2a cells co-cultured with P815 cells,N2a cells co-cultured with P815 cells plus C48/80,and N2a cells plus RA group.After co-culturing,the num-ber of differentiated N2a cells was observed under bright field.The expression of nerve growth factor(NGF),tyrosine ki-nase receptor A(TRKA),growth-associated protein-43(GAP43),neuron-specific enolase(NSE),synapsin(SYN),and postsynaptic density protein-95(PSD-95)at both protein and gene levels in N2a cells was detected using Western blot and polymerase chain reaction(PCR),with four replicates.RESULTS:The best condition for N2a differentiation was stimulation with 10 μmol/L RA for 24 hours,whereas the best condition for degranulation was stimulation of P815 cells with 20 mg/L C48/80 for 24 hours.Compared with N2a cells cultured alone,the differentiation ratio of the N2a+P815+C48/80 and N2a+RA groups was significantly increased(P<0.01),and the protein and mRNA expressions of NGF,TRKA,GAP43,NSE,SYN,and PSD-95 were significantly increased(P<0.05).CONCLUSION:Our results revealed that mast cell degranulation enhances the level of nerve hyperplasia in enteric nerve cells and promotes changes in nerve structure and function.Synaptic remodeling regulated by abnormal expression of key proteins such as NGF,TRKA,and GAP43 is involved in the nerve hyperplasia induced by mast cell degranulation.
3.Therapeutic efficacy and influencing factors of ceftazidime/avibactam in lung transplant recipients with pulmonary infection caused by carbapenem-resistant Gram-negative bacilli

Objective To investigate the clinical application of ceftazidime/avibactam(CAZ/AVI)in lung trans-plant recipients with pulmonary infection caused by carbapenem-resistant Gram-negative bacilli(CRGNB),and ana-lyze the factors affecting the prognosis.Methods Lung transplant recipients who had CRGNB pulmonary infection and were treated with CAZ/AVI were included in the analysis.Based on 14-day clinical response,14-day microbial response,and 30-day survival status,the recipients were divided into a clinical response group and a clinical failure group,a microbial response group and a microbial failure group,as well as a survival group and a death group,re-spectively.Univariate analysis was conducted on various data from the two groups.Factors affecting therapeutic ef-ficacy and survival were included in a binary logistic regression model.Independent risk factors for CAZ/AVI anti-infective efficacy and all-cause mortality outcomes were analyzed.Results A total of 43 recipients were included.After 14-day anti-infective treatment,32 recipients(74.42%)achieved clinical response,and 30 recipients(69.77%)achieved microbial response.34 recipients(79.07%)survived 30 days after CAZ/AVI treatment.The Charlson comorbidity index(CCI),proportion of renal dysfunction,and incidence of shock in recipients in the clini-cal response group were all lower than those in the clinical failure group(P<0.05),while the serum albumin(ALB)level was higher(P<0.05).The incidence of shock in recipients in the microbial response group was lower than that in the microbial failure group(P<0.05).CCI,proportion of renal dysfunction,and incidence of shock in recipients in the survival group were all lower than those in the death group(all P<0.05),while ALB level was higher during treatment period(P<0.05).Multivariate analysis of 14-day clinical response and 30-day survival showed that higher CCI was an independent risk factor affecting 14-day clinical response of recipients(OR=2.22,95%CI:1.07-4.63),while lower ALB levels(OR=0.72,95%CI:0.54-0.98)and higher CCI(OR=5.27,95%CI:1.18-23.58)were independent risk factors for 30-day all-cause mortality in recipients with pulmonary in-fection after lung transplant.Conclusion CAZ/AVI may be an effective drug for treating pulmonary infection caused by CRGNB in lung transplant recipients.Higher CCI is an independent risk factor for 14-day clinical failure in recipients after CAZ/AVI treatment.Lower ALB level and higher CCI are independent risk factors for increased 30-day mortality in recipients.
4.Home-based volume management in patients with heart failure: a scoping review

Objective:To conduct a scoping review of current research on home-based volume management in patients with heart failure, in order to provide evidence for future research and nursing practice.Methods:A systematic search was performed in China National Knowledge Infrastructure, Wanfang Data, VIP, China Biology Medicine disc, PubMed, Web of Science, Cochrane Library, CINAHL, and Embase for studies related to home-based volume management in patients with heart failure. The search covered literature published from the inception of each database to January 5, 2024. The included studies were synthesized and analyzed.Results:A total of 26 studies were included. The results indicated that the ability of patients with heart failure to manage volume at home is relatively weak, and they often have negative experiences with volume management. Volume management is influenced by individual factors, social support, and environmental and resource-related factors. Relevant interventions were mainly carried out at home or through a combination of home and outpatient settings. Intervention formats included remote and in-person volume management. Core components of the interventions included information support and guidance, monitoring of volume-related signs and indicators, and peer support. Evaluation indicators covered six aspects: volume and cardiac function indicators, self-care ability and behavior, diet and nutrition, clinical outcomes, psychological status, and self-efficacy. A total of three different integrated assessment tools for home-based volume management were identified.Conclusions:The ability and experience of home-based volume management in patients with heart failure still require improvement. In the future, appropriate behavior change interventions should be adopted to enhance patients' adherence to and satisfaction with home-based volume management. Furthermore, enriching the content and formats of interventions and improving the evaluation system will help enhance the quality of volume management and stabilize the disease condition.

6.Influence of occupational hazard factors on incidence of hypertension in calcium carbide plant: Historical cohort study

Background The high work intensity and possible subsequently increased susceptibility to occupational hazards of calcium carbide plants may lead to hypertension in workers, but there are few studies on the relationship between occupational hazard exposure and hypertension in workers involving the production process of calcium carbide. Objective To explore the influence of occupational hazards on the incidence of hypertension in calcium carbide plants. Methods Using historical cohort design, the employees of a calcium carbide factory in the western part of Inner Mongolia Autonomous Region were selected as research subjects. According to the pre-determined inclusion and exclusion criteria, the study population comprised an exposure group of 377 employees (including furnace workers, inspection workers, and maintenance workers) exposed to dust, noise & carbon monoxide, and a control group of 388 employees (including central control workers, electricians, and administrative personnel) without above-mentioned exposure. The total sample size was 765 participants. The follow-up period was from April 2011 to October 2022, and the study endpoint was defined as the conclusion of the follow-up period or diagnosed hypertension in annual occupational health examination. Information on general demographic characteristics, living habits, and work status was collected from all study subjects. Cox proportional hazards regression model was used to analyze the association between occupational hazard exposure and the risk of hypertension among the calcium carbide plant employees. Results The average age, mean systolic and diastolic blood pressure, proportion of males, smoking rate, and alcohol consumption rate in the exposure group were higher than those in the control group (P<0.05). Compared to baseline, both systolic and diastolic blood pressure levels increased in the exposure group and the control group at the end of the follow-up (P<0.05). At the end of the follow-up, the average differences between systolic/ diastolic blood pressure and baseline values in the exposure group were higher than those in the control group (P<0.05). Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

8.Construction and validation of a nomogram model for the prediction of the prognosis of pulmonary large cell neuroendocrine carcinoma

Objective:To explore the prognostic influencing factors of patients with pulmonary large cell neuroendocrine carcinoma (LCNEC), to develop a nomogram-based predictive model for the overall survival (OS) of LCNEC patients and to make validation.Methods:The clinical data of 2 947 patients with LCNEC in the Surveillance, Epidemiology, and End Results (SEER) database (the modeling group) and 147 patients with LCNEC in Beijing Chest Hospital Affiliated to Capital Medical University from 2010 to 2023 (the validation group). The data of patients in the both groups were compared. Cox proportional hazards model was used to screen out the factors influencing the OS of patients with LCNEC. A nomogram model was constructed to predict the OS based on the multivariate analysis result. Internal validation of the predictive model's performance was conducted through 500 repeated samplings based on the Bootstrap method. The predictive performance of the nomogram model was evaluated by using the area under the curve (AUC) of the receiver operating characteristic (ROC) curve. The consistency index (CI) was used to analyze the discrimination of the nomogram model in predicting the survival of LCNEC patients; calibration curves were used to analyze the consistency between the survival predicted by the nomogram model and the actual survival outcomes; and the decision curve analysis (DCA) was used to assess the net benefit of the model for actual clinical decision-making.Results:The differences in the proportions of patients with different age, gender, race, tumor staging, N stage, M stage, hepatic metastasis or not, pulmonary metastasis or not, chemotherapy and radiotherapy or not between the modeling group and the validation group were statistically significant (all P < 0.05). The median OS time of LCNEC patients in the modeling group was 14.0 months, with the 1-year OS rate of 53.3% and the 5-year OS rate of 21.2%; the median OS time of LCNEC patients in the validation group was 17.5 months, with the 1-year OS rate of 58.7%; there was no statistically significant difference in OS between the 2 groups ( P = 0.280). In the modeling group, the median OS time of female and male LCNEC patients was 18.0 and 12.0 months, respectively, and the difference in OS between the 2 groups was statistically significant ( P < 0.05); for patients with stage Ⅰ-Ⅱ, Ⅲ, and Ⅳ LCNEC, the median OS time was 48.0, 16.0, and 6.0 months, respectively, and the difference in OS among the 3 groups was statistically significant ( P < 0.05); the median OS time of patients receiving surgery and not receiving surgery was 28.0 and 8.0 months, respectively, and the difference in OS between the 2 groups was statistically significant ( P < 0.05). The differences in OS among female and male, patients in stages Ⅰ-Ⅱ, Ⅲ and Ⅳ, patients who underwent surgery or not were statistically significant (all P < 0.05). The results of multivariate Cox regression analysis in the modeling group showed that patients aged >60 years old (>60 years old vs. ≤60 years old: HR = 1.234, 95% CI: 1.114-1.367, P < 0.01), M 1 stage (M 1 stage vs. M 0 stage, HR = 2.646,95% CI: 2.385-2.935, P < 0.001), T 2-4 stage (T 2-4 stage vs. T 1 stage: HR = 1.199, 95% CI: 1.147-1.252, P < 0.001), N 1-3 stage (N 1-3 stage vs. N 0 stage: HR = 1.281, 95% CI: 1.225-1.340, P < 0.001) were independent risk factors of the OS in patients with LCNEC; female (female vs. male: HR = 0.877, 95% CI: 0.805-0.956, P = 0.003), surgery (yes vs. no: HR = 0.612, 95% CI: 0.554-0.676, P < 0.001), chemotherapy (yes vs. no: HR = 0.520, 95% CI: 0.470-0.575, P < 0.001) were independent protective factors of the OS in patients with LCNEC. A nomogram model for predicting 1, 3, and 5-year OS rates of LCNEC patients was constructed based on age, gender, T stage, N stage, M stage, surgery and chemotherapy. The result of ROC curve analysis indicated that the AUC of the nomogram model for predicting 1, 3, and 5-year OS rates in the modeling group was 0.822, 0.821 and 0.821, respectively, while the AUC of 1-year OS rate predicted by the validation group was 0.660. The CI of the modeling group and the validation group was 0.756 and 0.660, respectively. The calibration curve showed that 1, 3, and 5-year OS rates predicted by the modeling group were highly consistent with the actual OS rates. The DCA showed that the nomogram model for predicting OS in the modeling group and the validation group both had good clinical net benefits. Conclusions:The constructed nomogram model for predicting the prognosis of LCNEC patients is proved to be reliable and has good clinical values.

10.Evaluation of the comprehensive intervention effect on lunch for primary and secondary school students in Minhang District of Shanghai

Objective:
To evaluate the comprehensive intervention effect of lunch for primary and secondary school students in Minhang District, so as to provide a theoretical and practical basis for lunch intervention in school.
Methods:
From October to December 2023, a convenience sampling method was used to select 1 937 students from one primary and secondary school in Minhang District.A comprehensive intervention measure focusing on "reducing oil and salt" for lunch recipe optimization and nutrition education was carried out, and a questionnaire survey was conducted to evaluate the intervention effect three months later. Chi square test and Wilcoxon rank test were used to compare the data before and after the intervention.
Results:
After intervention, the use of cooking oil and salt, the supply of protein and fat in primary and secondary school lunches were reduced, and had no obvious impact on energy and other major nutrients. After intervention, compared to before intervention, the proportion of primary school students who felt that lunch was greasy decreased (8.9%, 6.2%, χ 2=4.35), and the proportion of primary and secondary school students who felt that lunch were delicious decreased significantly (33.2%, 23.2%; 63.9%, 53.5%, χ 2=26.39, 17.52) ( P < 0.05 ). Secondary school students also felt reduced variety of food ingredients (46.9%, 38.3%, χ 2=16.05, P <0.05). In addition, after intervention, the total surplus rate of primary school students meals decreased (7.4%, 4.4%, χ 2=5.73), mainly reflected in the decrease of the surplus rate of staple foods (7.1%, 2.4%, χ 2=17.39), while the surplus rate of vegetable dishes increased ( 16.0 %, 21.2%, χ 2=6.01) ( P <0.05). Although there was no significant change in the total surplus rate of meals for secondary school students, the surplus rate of staple foods decreased (12.9%, 5.4%, χ 2=33.52), while the surplus rates of meat and vegetable dishes increased (11.2%, 26.9%; 17.5%, 33.2%, χ 2=74.26, 61.88) ( P <0.05). After intervention, there was no statistically significant difference in the overweight and obesity rates of primary school students ( χ 2=0.11,0.43) and secondary school students ( χ 2=0.01,0.00) compared to before intervention( P >0.05). After intervention, the lung capacity of primary school students [1 564 (1 269,1 890) mL] and sitting forward flexion [11.3 (7.6, 15.2) cm] increased compared to before intervention [1 522 (1 259, 1 819 ) mL, 10.5 (6.3, 13.5) cm] ( Z =2.20, 4.68, P <0.01), but there was no statistically significant difference in lung capacity and sitting forward flexion of secondary school students before and after intervention ( Z =-0.46, -0.08, P >0.05).
Conclusion
The comprehensive intervention of school lunch has promoted a significant decrease in the use of oil and salt in lunch and improved the quality of recipes, and has a positive impact on the situation of leftover lunch and the health of students to a certain extent.
