1.miR-21-5p Targetes STAT3 Reduce the OGD/R-induced Neuronal Injury

Objective To investigate the potential mechanism of miR-21-5p in alleviating cerebral ischemia-reperfusion injury by targeting STAT3.Methods The HT22 cells were induced by OGD/R to construct a cell model of cerebral ischemia reperfusion injury.The expression of miR-21-5p was detected by RT-qPCR.The CCK-8 assay,TUNEL staining and flow cytometry were respectively used to detect the cell viability and apoptosis.ELISA assay was used to determine the contents of inflammatory factors IL-6,IL-10 and TNF-α in the cell supernatant.Western blot was used to detect the expression levels of p-STAT3/STAT3,Cleaved-Caspase-3,Bax and Bcl-2 proteins.The TargetScan database was used to predict the binding sites of miR-21-5p and STAT3.The dual-luciferase reporter gene assay was used to verify the targeting relationship between miR-21-5p and STAT3.Results The relative expression level of miR-21-5p was down-regulated in HT22 cells which induced by OGD/R(P<0.001).The cell viability(P<0.0001)was decreased and the apoptosis rate(P<0.001)was increased in OGD/R induced-HT22 cells.The contents of pro-inflammatory factors IL-6(P<0.001)and TNF-α(P<0.001)was increased,while the content of anti-inflammatory factor IL-10(P<0.001)decreased.After transfection with miR-21-5p mimic,cell viability was enhanced,apoptosis rate was reduced and neuroinflammation was inhibited.MiR-21-5p could target and bind to STAT3.After miR-21-5p inhibitor transfection,cell viability decreased,apoptosis was promoted,and neuroinflammation occurred;STAT3 inhibitor Stattic could reverse the effect of miR-21-5p inhibitor.Conclusion MiR-21-5p could specifically bind to STAT3 and reduce the neuroinflammation and apoptosis of OGD/R induced-HT22 cells.
2.Cognitive trajectories modeling of rare reversion in mild cognitive impairment

Objective To construct a dynamic framework for bidirectional transitions of mild cognitive impairment(MCI),quantifying both rare reversion and high-risk progression trajectories in cognitive dynamics.Methods Patients diagnosed with MCI at baseline from 2005 to 2022 and completed at least two follow-up visits were selected from the Alzheimer's Disease Neuroimaging Initiative(ADNI),and a retrospective cohort was constructed.Demographic information,APOEε4 genotype,and neuropsychological scales data were collected.Longitudinal cognitive assessments were functionally reconstructed using multivariate functional principal component analysis(MFPCA),with functional principal components(FPCs)extracted based on cumulative variance contribution rate(PVE>90%).Functional multi-state Markov models were developed to estimate inter-state transition intensities,year to year transition probabilities,and covariate effects.Results Among 1,019 MCI patients(4,657 follow-up visits),93(9.1%)reverted to normal cognition,while 359(35.2%)progressed to Alzheimer's disease(AD).Longitudinal trajectory analysis revealed significant heterogeneity:progressive MCI>stable MCI>reverted MCI in the first functional principal component(MFPC1)scores.The transition intensity for MCI reversion(0.020)was approximately one-fourth of the AD progression risk(0.086),but the post-reversion cognitive re-impairment intensity was 0.138.Reduced MFPC1(HR=0.993,95%Cl:0.991,0.995)and elevated MFPC2(HR=1.004,95%Cl:1.001,1.007)were closely associated with MCI reversion.Conclusion MCI exhibits marked heterogeneity in longitudinal cognitive trajectories.Although reversion is rare,reversed patients remain at high risk of cognitive re-impairment.

4.Construction and practice of multilevel and inter-disciplinary management scheme for specialist nurses

Objectives To explore the establishment and effects of a multilevel and inter-disciplinary management scheme for specialist nurses.Methods The multilevel and inter-disciplinary management scheme included establishing the pyramid hierarchical training model,exploring superimposed training mechanism for specialist nurses with horizontal expansion,vertical extension,and deep development,and constructing stereoscopic clinical evaluation and quantitative assessment system.Four hundred-forty two specialized nurses have been recruited to join the multilevel and inter-disciplinary management scheme from July 2021 to August 2023.The core competencies of specialized nurses were compared before and after the implementation of management scheme.Results The quality development of specialized nursing have been improved,and the total scores and the scores of 3 dimensions(research innovation,management,and professional development)in core competencies of specialized nurses after the intervention of multilevel and inter-disciplinary management scheme were higher than those before(P<0.05).Conclusion The multilevel and inter-disciplinary management scheme can effectively improve the core competencies of specialized nurses,and promote the high-quality development of specialized nursing.
5.Effects of Danggui Wuji granules on 16S r DNA, metagenome, and metabolome in BDS mice

Blood deficiency syndrome (BDS) refers to a pathological state with blood dysfunction and organ dystrophy in traditional Chinese medicine. Danggui Wuji granules (DWG) was developed from a formula containing Angelicae Sinensis Radix and Musculus et Os Galli Domestici. Herein, we investigated the mechanism of DWG in treating BDS by modulating gut microbiota. We found that DWG protected mice from BDS by elevating the levels of red blood cell count, hemoglobin, and hematocrit in peripheral blood and increasing the erythrocyte membrane Na+-K+-ATPase activity. Danggui Wuji granules changed the composition and metabolites of colonic flora. Notably, Lactobacillus, Muribaculaceae, and Alistipes were the main genera showing changes after DWG treatment. Our findings revealed that DWG presented a positive therapeutic effect on BDS in mice by regulating the gut microbiota and metabolites. The protective mechanism of DWG was associated with pathways such as metabolic pathways, biosynthesis of secondary metabolites, ABC transporters, ribosome, thyroid hormone synthesis, lysine degradation, galactose metabolism, tyrosine metabolism, etc.
6.Mechanism of Huashi Baidu Prescription in Treating Influenza Viral Pneumonia Based on Transcriptome

ObjectiveTo clarify the therapeutic effect of Huashi Baidu prescription on pneumonia in mice caused by influenza A (H1N1) virus and explore its mechanism based on the transcriptome. MethodA mouse influenza viral pneumonia model was built by intranasal infection with influenza A virus, and mice were continuously administered the drug for five days, so as to investigate the general condition, lung index, viral load, pathological morphology of lung tissue, survival time, and prolongation rate of survival time of mice and clarify the therapeutic effect of Huashi Baidu prescription on influenza viral pneumonia. Transcriptome technology was used to detect the differentially expressed genes in the lung tissue of mice in the model group and the normal group, as well as the Huashi Baidu prescription group and the model group, and the potential core target of the Huashi Baidu prescription for the treatment of influenza viral pneumonia was screened. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to verify the effect of Huashi Baidu prescription on the mRNA expression level of core target genes. ResultCompared with the normal group, the lung index and viral load in the lung tissue of the model group were significantly increased (P<0.05, P<0.01). Compared with the model group, the high-dose group of Huashi Baidu prescription significantly prolonged the survival time of mice infected with influenza A virus (P<0.05) and significantly reduced the lung index value of mice (P<0.05) and the viral load of lung tissue. The high-dose, medium-dose, and low-dose groups of Huashi Baidu prescription could significantly reduce lung tissue inflammation, blood stasis, swelling, and other pathological changes in mice (P<0.05, P<0.01). Transcriptome analysis of lung tissue showed that core genes were mainly enriched in the nuclear transcription factor-κB (NF-κB) signaling pathway, interleukin-17 (IL-17) signaling pathway, cytokine-cytokine receptor interaction, and other pathways after the intervention of Huashi Baidu prescription. TRAF6, NFKBIA, CCL2, CCL7, and CXCL2 were the top five node genes with combined score values. Real-time PCR validation showed that Huashi Baidu prescription significantly downregulated the mRNA expression of key genes TRAF6 and NFKBIA in the NF-κB signaling pathway, as well as chemokines CCL2, CCL7, and CXCL2 (P<0.05, P<0.01). ConclusionHuashi Baidu prescription has a therapeutic effect on influenza viral pneumonia, possibly by inhibiting the expression of key nodes TRAF6 and NFKBIA in the NF-κB signaling pathway and that of chemokines CCL2, CCL7, and CXCL2, reducing the recruitment of inflammatory cells and viral load, and exerting anti-influenza viral pneumonia effects.
7.Multidisciplinary Treatment on a Case of ROSAH Syndrome

A 15-year-old female was referred to the hospital with intermittent fever, where multiple systemic abnormalities were found, such as splenomegaly, secondary hypersplenism, retinitis pigmentosa, and ectodermal dysplasia. Medical history revealed that she had suffered recurrent respiratory infections, blurred vision at night, and dysplasia of teeth and nail beds since childhood. Then she was suspected to be experiencing ROSAH syndrome, a rare disease newly recognized in recent years, which was finally confirmed by gene sequencing results. During a course of treatment with tumor necrosis factor inhibitors, recurrent fever with elevated inflammatory markers reappeared, and the child developed headaches. To guide the comprehensive treatment and improve the patient's quality of life, the multidisciplinary team in Peking Union Medical College Hospital discussed together and directed the following treatment.
8.Effects of palliative care based on multidisciplinary collaboration on cancer-related fatigue, quality of life and social support in cancer patients

Objective:To analyze the effect of palliative care based on multidisciplinary collaboration on cancer-related fatigue, quality of life and social support of cancer patients and to evaluate its application value.Methods:From April 2019 to May 2021, a total of 682 terminal cancer patients who were admitted to Affiliated Cancer Hospital of Zhengzhou University were selected as the research objects by the convenient sampling method. According to the random number table method, they were divided into the control group and the observation group, with 341 cases in each group. The control group received traditional cancer hospice medical treatment and nursing management, while the observation group received multidisciplinary hospice care on this basis. The cancer-related fatigue, quality of life, nursing satisfaction, and the degree of family and social support were compared between the two groups.Results:There was no statistically significant difference in the score of cancer-related fatigue between the two groups before intervention ( P>0.05) . After intervention, the scores of cancer-related fatigue in the two groups decreased and that of the observation group was lower than the control group, and the differences were statistically significant ( P<0.05) . There was no statistically significant difference in quality of life score between the two groups before intervention ( P>0.05) . After intervention, the quality of life scores of the two groups were improved and that of the observation group was higher than the control group, and the differences were statistically significant ( P<0.05) . There were no statistically significant differences in family support, friend support and other support between the two groups before intervention ( P>0.05) . After intervention, the scores of family support, friend support and other support in the two groups were improved, and the scores of family support and other support in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . 10.Application of home care platform in follow-up management of patients with gestational diabetes mellitus

Objective To construct the diabetic home care platform and to explore its effects in follow-up man agement of patients with gestational diabetes mellitus.Methods The diabetic home care platform contained clientsides for both nurses and patients,who could use the platform via identification and binding technology.Totally 200 patients with gestational diabetes mellitus in outpatient department from November 2015 to December 2016 were recruited using convenience sampling.Patients were divided into the experimental group and the control group with 100 cases in each group based on the voluntary principle.Both groups received routine follow-up management,and the experimental group also used diabetic home care platform management.Results There were no differences in gestational weeks,mode of delivery,postpartum hemorrhage,other complications,fetal intrauterine distress,neonatal hypoglycemia,and deformity between two groups(P>0.05).Rate of macrosomia in the experimental group was lower than that in the control group(P<0.05),and rate of breastfeeding was higher than the control group(P<0.05).The total score of Diabetes Self-Care Activeties-6 and scores of each dimension in the experiential group were higher than those in the control group 8 weeks after intervention(P<0.001);the total score of Problem Areas in Diabetes Scale and scores of each dimension in the experimental group were lower than those in the control group(P<0.05).Conclusion Application of diabetic home care platform is helpful for fetal weight control,and can improve rate of breastfeeding,promote women's self-management behavior and emotional disorders.
