1.Analysis and Prediction of Disease Burden of Depression in Old Age in China from 1990 to 2021

Objective To analyze the trends in disease burden and risk factors of depression among the elderly population in China from 1990 to 2021,and to provide a theoretical basis for the prevention,treatment,and policy-making of geriatric depression in China.Methods Data on the disease burden of geriatric depres-sion in China from 1990 to 2021,including the number of incident cases,disability-adjusted life years(DALYs),incidence rate,and DALY rate,were extracted from the 2021 Global Burden of Disease(GBD)database.The Joinpoint regression model was used to analyze the trends by calculating the annual percentage change(APC)and average annual percentage change(AAPC).The autoregressive integrated moving average(ARIMA)model was employed to predict the disease burden of geriatric depression over the next five years.Population attributable fractions(PAFs)were used to describe the risk factors for geriatric depression in China in 1990 and 2021.Results From 1990 to 2021,the number of incident cases and the incidence rate of geri-atric depression in China showed an overall upward trend.The most significant increase in incidence was ob-served in the 60-64 age group,while the prevalence rate increased notably in the ≥ 95 age group.The DALY rate showed the most pronounced upward trend in the 65-69 age group.The incidence,prevalence,and DALY rates of geriatric depression were higher in women than in men.Major risk factors included child-hood sexual abuse and intimate partner violence,with the impact of intimate partner violence being particular-ly significant among women.The ARIMA model predicted that the incidence,prevalence,and DALY rates of geriatric depression in China would decline over the next five years,with a greater decline observed in women than in men.Conclusions From 1990 to 2021,the incidence,prevalence,and DALY rates of geriatric de-pression in China showed an overall upward trend,with higher rates observed in women than in men.Greater attention should be paid to the elderly female population,with a focus on early prevention to reduce the dis-ease burden of geriatric depression.

Ulcerative colitis(UC)is a chronic inflammatory bowel disease affecting the colon(particularly the descending colon and sigmoid)and rectum.UC primarily presents with persistent or recurrent diarrhea,abdominal pain,bloody stools,and other symptoms.The primary pathological mechanism of UC involves intestinal barrier injury.When the intestinal barrier function is compromised,characterized by loss of epithelial layer integrity,thinning of the mucus layer,and microbiota dysregulation,pathogenic microorganisms can infiltrate the lamina propria from the intestinal lumen through the damaged barrier,triggering and exacerbating the intestinal inflammatory response.Current treatments for UC are limited by high costs,numerous adverse reactions,and a high likelihood of relapse.Consequently,there is an urgent need for the development of new drugs that can effectively and safely treat UC.Natural products have become significant research targets in treating various diseases due to their broad biological activity,multiple action targets,low toxicity,and easy availability.They play a crucial role in the targeted repair of the intestinal barrier,with potential mechanisms including enhancing intes-tinal epithelial cells and their secreted proteins,regulating gut microbiota and its metabolism,and balancing immune cell subsets.Additionally,it is essential to consider the synergistic effects,bioavailability,and safety of natural products.This paper summarizes the natural products reported in the past five years for their anti-UC properties by repairing the intestinal barrier,providing a theoretical basis for the development and application of natural products in anti-UC drugs.

5.Intracranial seizure-onset electroencephalogram patterns in mesial temporal lobe epilepsy

Objective:To explore the seizure-onset intracranial electroencephalogram patterns in mesial temporal lobe epilepsy (MTLE) by stereo-electroencephalography (SEEG).Methods:The clinical data of 16 patients with focal epilepsy admitted to Epilepsy Center, Shanghai Deji Hospital from March 2017 to December 2019 were retrospectively analyzed. Tonic discharge was taken as sign of the initial period; transition period from the interictal period to the initial period was named as the preictal period, characterized by clonic discharge that was different from the interictal period. The initial and preictal discharge patterns were described, and their corresponding relationship were studied; at the same time, the spatial distribution and synchrony of the discharges at initial and preictal periods were observed.Results:One hundred and thirty-two seizures were recorded in these 16 patients. Two patterns at initial period were noted: low-voltage fast activity (LVFA)+slow depolarizing shifts (SPSs, n=29, 22%), and rhythmic activity (RA) of low-medium amplitude without SPSs ( n=103, 78%); spatial spread of LVFA+SPSs was much wilder than that of RA. In addition, 3 patterns at preictal period were summarized: hypersynchronous (HYP, n=93, 70.5%, most commonly found in the anterior hippocampus [87/219]), sentinel spike ( n=22, 16.7%), and pattern of smooth transition from the interictal period to the initial period ( n=17, 12.9%). No special correspondence in discharge patterns was noted between the initial period and preictal period. Conclusion:Seizure is a process of gradual evolution from interictal period, preictal period to initial period. HYP potential is the most common pattern of seizure in MTLE at preictal stage, mainly appeared in the anterior hippocampus; RA potential is the most common pattern of seizure in MTLE at initial stage, with lower frequency and more limited spatial spread.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.
7.Gland segmentation in colorectal pathological image using dual-branch network based on weakly supervised learning

To address the issue that the existing weakly supervised segmentation methods have difficulties in obtaining fine-grained glandular features from colorectal pathological images,leading to the inability to generate high-quality pseudo-labels and compromising the gland segmentation performance,a dual-branch network based on weakly supervised learning is proposed for gland segmentation in colorectal pathological image.The patch-level colorectal pathological images are input into the first branch network,where the interaction and fusion of local and global features of patch-level images are achieved through the feature interaction module and affinity attention fusion module,and fine-grained glandular features are obtained.Subsequently,image-level colorectal pathological images are input into the second branch network,where the gland locations are located using the partial class activation attention module,and coarse-grained class activation maps are obtained.Finally,high-quality pseudo-labels are derived from the fine-grained glandular features and coarse-grained class activation maps,and gland segmentation is realized in the segmentation network through the cross-scale connected spatial perception module.The tests on two colorectal pathological image datasets(GlaS and CRAG)reveal that the proposed method is superior to other segmentation methods in segmentation performance,confirming its effectiveness.
8.Melanoma image segmentation method based on edge key points and edge attention

High-precision segmentation of melanoma images is crucial for early diagnosis and improving patient survival.However,the blurring of the edge region of melanoma,which presents irregular shapes,makes it difficult for existing segmentation methods to obtain edge feature information,affecting the accuracy of melanoma image segmentation.To solve this problem,a melanoma image segmentation method based on edge key points and edge attention is proposed.An edge key point selection module for point rendering and a combined convolution transformer block are designed in the encoder to guide the acquisition of local and global features of the edge by selecting edge key points.Then,the edge refinement module is designed in the encoder to refine the edge features of the deep network,and finally,the multi-scale edge attention module is designed in the skip connection,which enables the capture of the edge shape features at multiple scales.The tests on two datasets(ISIC 2018 and PH2)demonstrate that the proposed method has better segmentation performance than the existing segmentation methods.
9.Application of HEOS hysteroscopic surgery in patients with uterine fibroids and its effect on ovarian function and trauma response indicators

Objective:To explore the therapeutic effect of HEOS hysteroscopic surgery on patients with uterine fibroids and its effect on ovarian function and trauma response indicators.Methods:The clinical data of 104 patients with uterine fibroids admitted to Shangqiu Fifth People's Hospital from Jan 2020 to Mar 2023 were retrospectively analyzed and were divided into 2 groups according to different surgical protocols.A total of patients who underwent HEOS hysteroscopic surgery were enrolled in HEOS group and 52 patients who underwent conventional hysteroscopic electroresection(TCRPR)were enrolled in TCRPR group.The surgical and postoperative recovery indicators,surgical effects,ovarian function before and after surgery[estradiol(E2),luteinizing hormone(LH),follicle-stimulating hormone(FSH)],trauma response indicators[interleukin-6(IL-6),interleukin-17(IL-17),angiotensin Ⅱ(Ang Ⅱ)],and incidence of complications were compared between the two groups.Results:The operation time,postoperative exhaust time,time of the white blood cell count returning to normal,and hospitalization time in HEOS group were shorter than those in TCRPR group,and the intraoperative blood loss was less than that in TCRPR group(P<0.01).At 1 d after operation,serum levels of IL-6,IL-17 and Ang Ⅱ in 2 groups were higher than those before operation,and serum levels of IL-6,IL-17 and Ang Ⅱ in HEOS group were lower than those in TCRPR group(P<0.01).The excellent and good rate of operation in HEOS group was 98.08%(51/52),which was higher than that in TCRPR group[84.62%(44/52)](P<0.05).At 3 months after operation,the serum E2 level was lower and the serum LH and FSH levels of 2 groups were higher than those before operation,and the serum E2 level of HEOS group was lower than that of TCRPR group,and the serum LH and FSH levels were higher than those of TCRPR group(P<0.01).The incidence of complications in HEOS group was 1.92%(1/52),which was lower than that of TCRPR group[17.31%(9/52)](P<0.05).Conclusion:Compared with conventional TCRPR in the treatment of uterine fibroids,hysteroscopic surgery with HEOS is more helpful to improve the surgical effect,optimize the surgical and postoperative recovery indicators,improve ovarian function,reduce complications,and had less traumatic reactions.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in healthcare facilities in major regions of China in 2023.Methods Clinical isolates collected from 73 hospitals across China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2023 Clinical & Laboratory Standards Institute (CLSI) breakpoints.Results A total of 445199 clinical isolates were collected in 2023,of which 29.0% were gram-positive and 71.0% were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species (excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi) (MRSA,MRSE and MRCNS) was 29.6%,81.9% and 78.5%,respectively.Methicillin-resistant strains showed significantly higher resistance rates to most antimicrobial agents than methicillin-susceptible strains (MSSA,MSSE and MSCNS).Overall,92.9% of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 91.4% of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis had significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 93.1% in the isolates from children and and 95.9% in the isolates from adults.The resistance rate to carbapenems was lower than 15.0% for most Enterobacterales species except for Klebsiella,22.5% and 23.6% of which were resistant to imipenem and meropenem,respectively .Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.6% to 10.0%.The resistance rate to imipenem and meropenem was 21.9% and 17.4% for Pseudomonas aeruginosa,respectively,and 67.5% and 68.1% for Acinetobacter baumannii,respectively.Conclusions Increasing resistance to the commonly used antimicrobial agents is still observed in clinical bacterial isolates.However,the prevalence of important crabapenem-resistant organisms such as crabapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a slightly decreasing trend.This finding suggests that strengthening bacterial resistance surveillance and multidisciplinary linkage are important for preventing the occurrence and development of bacterial resistance.
