Objective To understand the current status of the job competency of full-time healthcare-associated in-fection(HAI)management professionals in all levels and types of medical institutions across China, and provide in-formation and basis for professional training, competency improvement, and career planning. Methods The strati-fied sampling method was adopted to select HAI management professionals from medical institutions in 31 provin-cial-level administrative regions across the country as the research subjects. The designed content of questionnaire involved four parts, including the surveyed personnel's basic information, daily job competency assessment,satis-faction level towards the job,as well as opinions and suggestions on the management of full-time HAI management professionals. The assessment on daily job competency was divided into 13 dimensions, ranging from very incompe-tent to very competent in 5 levels. The scores of HAI management professionals with different professional back-grounds were compared and analyzed. Results A total of 8 709 valid questionnaires were collected, with 3 475 and 3 697 surveyed personnel from tertiary and secondary medical institutions, respectively, and 1 537 from primary or unclassified medical institutions. The overall average score for the competency assessment of full-time HAI manage-ment professionals was(4.17±0.80)points.The scores of professionals with different professional backgrounds, from high to low, were as follows:nursing([4.12±0.81]points),clinical medicine([4.07±0.86]points),pre-ventive medicine([3.93±0.92]points),laboratory medicine([3.88±0.93]points),pharmacy([3.86±0.94]points),and health management([3.85±0.95]points).For the core competency of HAI management professio-nals,such as monitoring and analyzing HAI cases, identifying and investigating HAI outbreaks, the assessment scores for professionals with medical backgrounds were the highest(both P<0.05).For the basic work of HAI prevention and control, such as checking and guiding the implementation of rules and regulations, guiding occupa-tional protection, management and communication, and implementing HAI management training, professionals with a nursing background had the highest assessment scores(all P<0.05).Full-time HAI management professionals were relatively satisfied with their training, while those with a background in preventive medicine had lower satisfac-tion with their training, career development, and job benefits(all P<0.05).Conclusion There are significant differences in the competency of HAI management professionals with different professional backgrounds. It is nece-ssary to optimize division of labor and leverage the strengths, providing ideas and models for promoting the construction of a specialized and professional HAI management team.

Objective:The minimal ablative margin (MAM) after radiofrequency ablation (RFA) was evaluated based on magnetic resonance imaging (MRI) image registration to analyze its effect on the prognosis of patients with hepatocellular carcinoma (HCC).Methods:Clinical data of 120 patients with HCC undergoing RFA in the General Hospital of Ningxia Medical University from January 2017 to April 2022 were retrospectively analyzed, including 88 males and 32 females, aged (58.4±8.5) years. The enhanced MRI images of patients before and after treatment were imported into a 3D Slicer software to show the ablative margin, and patients were divided into two groups according to whether MAM exceeded the peritumor safety boundary of 5 mm: MAM<5 mm group ( n=75) and MAM≥5 mm group ( n=45). Clinical data were recorded such as gender, age, tumor length and location. Patients were followed up by outpatient review to record whether local tumour progression occurred. Kaplan-Meier method was used for survival analysis, and log-rank test was used for survival comparison. Cox regression analysis was performed to analyze the risk factors of local tumour progression after RFA in patients with HCC. Results:There were significant differen-ces in tumor volume, whether the tumor is located around the vessels, and the mode of RFA guidance between the two groups (all P<0.05). The cumulative local tumour progression-free survival rates at 6, 12 and 24 months after RFA were 100%, 100% and 98% in MAM ≥5 mm group, superior to those in MAM<5 mm group (92%, 84% and 69%, respectively, χ2=47.22, P<0.001). Multivariate Cox regression analysis showed that MAM<5 mm ( OR=9.992, 95% CI: 4.358-22.913), tumor diameter ≥2 cm ( OR=1.758, 95% CI: 1.025-3.015) and perivascular tumor ( OR=2.344, 95% CI: 1.379-3.985) were risk factors for local tumour progression after RFA in patients with HCC (all P<0.05). Conclusion:The MAM evaluated based on MRI image registration is an influential factor on prognosis of patients with HCC. Patients with MAM<5 mm suffer an increased risk of postoperative local tumour progression.

Since the Beijing health and family planning commission promulgated the new medical technology management system,we have accepted and reviewed 14 key medical technologies in Beijing, such as resection of the skull base tumor (intracranial tumor outside communication),resection of intracranial important functional areas and large vascular malformation, renal vascular reconstruction technique, the technique of artificial joint replacement, coronary interventional diagnosis and treatment technology, etc.Through the existing laws and regulations requirements from medical new technology, the ethical review way, submit materials requirements and ethical review focus is analyzed, and combining with the experiences of our hospital in the medical technical review, aims to explore suitable for medical technology ethical review specification.
7.Clinical analysis of the lie delivery in promoting natural delivery functionsmothly
Clinical Medicine of China 2016;32(2):120-122
Objective To investigate the effect of lie delivery in promoting natural delivery functionsmoothly. Methods One hundred and sixty pregnant women hospitalized and deliveried in the Maternal and Child Care Service Centre of Shijiazhuang from June 2013 to June 2014 were randomly divided into observation group of lie delivery and control group of bladder lithotomy postion delivery,80 cases in each group,The second stage of labor time, postpartum hemorrhage (2 h > 500 ml),postpartum comfort and neonatal asphyxia (Apgar score ≤ 7 points) of two groups were observed and compared. Results The second stage of labor time of observation group was significantly shorter than that of the control group taking lithotomy position childbirth, and thepostpartum hemorrhage (2.5% vs 7.5%;x2 =3.247,P=0.034),cervical laceration(3.75% vs 21.25%;x2 =4.657,P=0.047) and the incidence of postpartumdiscomfort incidence(16.25% vs 50.0%;x2 =18.357,P =0.008) were less than that in the control group, the differences between two groups were statistically significant (P<0.05).Fetal distress (2.50% vs 8.75%;x2 =4.672,P =0.030),neonatal asphyxia rate(1.25% vs 7.50%;x2 =4.142,P =0.035) of two groups have statistical significance (P < 0.05).Conclusion The parturient labor process in lying position to take, can effectively shorten the birth process, reduce the incidence of fetal distress and neonatal asphyxia.
