1.Differential Diagnosis of Insomnia in Traditional Medicine

Background:
Problems related to sleep affect approximately 23-56% of the world’s population. In Mongolia,
the prevalence of non-organic sleep disorders is estimated at 27.9%, among which 42.2% of
the population have poor sleep quality. Long-term insomnia can have a negative impact on
an individual’s normal work and life, increasing the risk of various health problems. Severe
insomnia can reduce work efficiency and concentration, thereby causing serious harm.
Traditional medicine has a long history of treating insomnia. However, there are currently no
systematic studies on insomnia based on ancient traditional medical books.
Goal:
Based on the ancient medical literature of traditional medicine, identify the underlying
diseases that cause insomnia and compare their symptoms.
Materials and Methods:
We adopted the checklist method to list the relevant information of insomnia from the literature,
providing a prerequisite for further analysis and the issue of syndrome differentiation of
insomnia was analyzed by using comparison methods.
The research protocol was reviewed and approved by the Medical Ethics Committee of the
Mongolian National University of Medical Sciences at its meeting on June 24, 2025 (No. 04),
and ethical clearance was granted for the conduct of this study.
Results:
The research materials we selected did not describe insomnia as a dedicated chapter, but
mentioned the content related to insomnia in the form of disease symptoms, such as terms
like ”sleeplessness”, ”reduced sleep” and ”light sleep”. We explore the underlying diseases
of insomnia based on the above symptoms related to insomnia and analyze the syndrome
differentiation of insomnia. Exploring the underlying disease of insomnia:
1. Insomnia caused
by diseases resulting from Wind include Wind delirium, palpitations, bone marrow Wind,
heart Wind, lung Wind, Wind-induced diabetes, Wind tsbs.
2. Insomnia caused by diseases
resulting from Bile include heat Bile, heart Bile, liver sdembu, deficiency heat, Invisible heat
and epidemic heat.
3. Other diseases include mugpo spreads to the heart, heart chuser,
trichomoniasis and childhood evil spirit disease.
We also compared the symptoms of the
underlying diseases that cause insomnia and listed their similarities and differences.
Conclusion
The underlying etiologies of insomnia were categorized into Wind, Bile, Mugpo, yellow fluid,
parasitic and evil spirit disorders. Accordingly, effective management of insomnia requires
differentiation based on the primary disease and an individualized treatment approach that
integrates both etiological and symptomatic therapies.
2.5-Fluorouracil upregulates the activity of Wnt signaling pathway in CD133-positive colon cancer stem-like cells.

BACKGROUND AND OBJECTIVECD133-positive colon cancer stem like cells (CSLCs) are resistant to the conventional cytotoxic drug 5-fluorouracil (5-FU). Wnt signaling pathway plays important roles in colon cancer carcinogenesis and metastasis, and regulates the self-renewal capacity of CSLCs. In the present study, we explored the impact of 5-FU on Wnt signaling pathway of CD133-positive colon CSLCs, and the relation between Wnt signaling pathway and drug resistance of CD133-positive colon CSLCs.
METHODSMagnetic activation cell separation was used to collect CD133-positive cells from colon cancer cell line DLD1, which was transfected with luciferase reporter for Wnt signaling activity. The activity of Wnt signaling pathway was compared between CD133-positive and CD133-negative cells. After the treatment with 1 μg/mL of 5-FU, the cell proliferation rates of DLD1 cells, CD133-positive cells, and CD133-negative cells were compared. After the treatment with 1 μg/mL and 10 μg/mL of 5-FU for 48 h, Wnt activity was compared between CD133-positive and CD133-negative cells. The expression of CD133 and cell apoptosis of CD133-positive cells was detected after exposure to 50 ng/mL of dickkopf (DKK)-1, a Wnt pathway inhibitor.
RESULTSAfter the treatment with 5-FU, the cell proliferation rate of CD133-positive cells was higher than that of CD133-negative cells and the sensitivity of CD133-positive cells to 5-FU decreased. Wnt activity was higher in CD133-positive cells than in CD133-negative cells [(46.3 ± 0.3)% vs. (33.9 ± 2.7)%, P = 0.009]. After the treatment with 1 μg/mL and 10 μg/mL of 5-FU, Wnt activity of CD133-positive cells was (90.1 ± 10.0)% (P = 0.012) and (52.9 ± 2.5)% (P = 0.047), respectively, whereas that of CD133-negative cells was (35.5 ± 3.3)% (P = 0.434) and (26.5 ± 0.4)% (P = 0.046), respectively. CD133 expression in CD133-positive cells decreased from (87.2 ± 5.3)% to (60.6 ± 3.1)% (P = 0.022) after treatment with DKK-1, whereas the cell apoptosis rate increased from (11.8 ± 0.2)% to (28.3 ± 0.6)% (P = 0.013).
CONCLUSIONSWnt activity is higher in CD133-positive DLD1 cells than in CD133-negative DLD1 cells. 5-FU can upregulate Wnt activity of CD133-positive colon CSLCs. Blocking Wnt activity may reverse drug sensitivity of CD133-positive cells to 5-FU.

3.Parentage testing anomalies in Hong Kong SAR of China.

OBJECTIVETo determine if there are any differences in the number of exclusions from paternity of men using an anonymous parentage testing service compared with that of men using an in-person parentage testing service provided by the same company in Hong Kong SAR of China.
METHODSComparable numbers of consecutive anonymous and in-person parentage tests conducted by the same company were analyzed.
RESULTSMen using an anonymous parentage testing service were excluded from paternity at a significantly greater rate (P < 0.001), compared with men using an in-person parentage test service.
CONCLUSIONSThe results obtained from anonymous parentage testing indicate that the number of families containing children of doubtful parentage is much greater than expected previously. As illegitimate children are known to suffer greater degrees of abuse and neglect, this finding poses serious social questions regarding the welfare of families, which the relevant authorities should acknowledge and address.
