1.Efficacy and safety of cold forceps polypectomy utilizing the jumbo cup: a prospective study

BACKGROUND/AIMS: There are few prospective studies on cold forceps polypectomy (CFP) using jumbo cup forceps. Therefore, we examined patients with diminutive polyps (5 mm or smaller) treated with CFP using jumbo cup forceps to achieve an adenoma-free colon and also assessed the safety of the procedure and the recurrence rate of missed or residual polyp after CFP by performing follow-up colonoscopy 1 year later. METHODS: We included patients with up to 5 adenomas removed at initial colonoscopy and analyzed data from a total of 361 patients with 573 adenomas. One-year follow-up colonoscopy was performed in 165 patients, at which 251 lesions were confirmed. RESULTS: The one-bite resection rate with CFP was highest for lesions 3 mm or smaller and decreased significantly with increasing lesion size. Post-procedural hemorrhage was observed in 1 of 573 lesions (0.17%). No perforation was noted. The definite recurrence rate was 0.8% (2/251 lesions). The probable recurrence rate, which was defined as recurrence in the same colorectal segment, was 17%. Adenoma-free colon was achieved in 55% of patients at initial resection. Multivariate analysis revealed that achievement of an adenoma-free colon was significantly associated with number of adenomas and years of endoscopic experience. CONCLUSIONS: CFP using jumbo biopsy forceps was safe and showed a high one-bite resection rate for diminutive lesions of 3 mm or smaller. The low definite recurrence rate confirms the reliability of CFP using jumbo biopsy forceps. Number of adenomas and years of endoscopic experience were key factors in achieving an adenoma-free colon.

2.The impact of a school-based tooth-brushing program on dental caries: a cross-sectional study.

BACKGROUND:
Promotion of oral health in children is recognized as one of the components of health-promoting schools (HPSs). However, few studies have addressed supportive school environments for children's oral health. This study aimed to evaluate the status of dental caries in school children at HPSs, with the objective of examining the impact of a supportive school environment for oral health, considering the lifestyles of individual children and the socioeconomic characteristics of their communities.
METHODS:
Data of 2043 5th-grade students in 21 elementary schools in Ichikawa city between 2008 and 2013 were analyzed. Children's oral health status was evaluated using the decayed, missing, and filled permanent teeth (DMFT) index. A self-reported lifestyle questionnaire, a survey of the school environment promoting tooth-brushing, and community socioeconomic characteristics derived from the National Census data were included in the analyses. Bivariate analyses were conducted to evaluate the children's DMFT status, and zero-inflated negative binominal (ZINB) regression was used to assess the relationships between DMFT and other variables.
RESULTS:
Prevalence of dental caries in the permanent teeth of 5th-grade children (aged 10-11 years) was 33.3%, with a mean DMFT score (± SD) of 0.83 ± 1.50. According to multilevel ZINB regression analysis, children from schools with after-lunch tooth-brushing time showed a higher odds ratio (OR) for excess zero DMFT (OR = 1.47, 95% CI = 1.00-2.15, P = 0.049) as compared to those from schools without it. Neither bivariate analysis nor ZINB model analysis revealed any significant influence of children's gender or use of a toothpaste with fluoride.
CONCLUSIONS
The school-based environment supportive of oral health was significantly associated with a zero DMFT status in children. School-based efforts considering the socioeconomic characteristics of the area warrant attention even with declining prevalence of dental caries.
3.Improvement Effects of Drink Containing Ampelopsis glandulosa Extract on Knee Joint Discomfort

The prevalence of joint diseases in Japan is increasing yearly and it causes the need of nursing care and reduces quality of life. Therefore, there is an urgent need for the development of approaches to prevent and treat the diseases. In the present study, we investigated the effective, healthy food material focusing on the metabolism of joint cartilage. Ampelopsis glandulosa (A. g. ) extract improved exacerbation of hyaluronic acid metabolism and NFκB nuclear translocation caused by inflammatory cytokines, and it suppressed the onset of collagen-induced arthritis in mice. Moreover, intake of the drink containing A. g. extract for three months improved discomfort, pain, and bending angle of knee joint in activities of daily living. These results suggest that A. g. extract improves hyaluronic acid metabolism of joint cartilage, and it is expected to prevent and improve joint disease by long-term intake of the drink containing A. g. extract.
4.Magnified Endoscopic Findings of Multiple White Flat Lesions: A New Subtype of Gastric Hyperplastic Polyps in the Stomach.

BACKGROUND/AIMS:: While the occurrence of multiple whitish flat elevated lesions (MWFL) was first reported in 2007, no studies on MWFL have been published to date. The present retrospective observational study aimed to clarify the endoscopic findings and clinicopathological features of MWFL. METHODS:: Subjects were consecutive patients who underwent upper gastrointestinal endoscopy as part of routine screening between April 2014 and March 2015. The conventional white-light, non-magnifying and magnifying narrow-band images were reviewed. Clinical features were compared between patients with and without MWFL. RESULTS:: The conventional endoscopic findings of MWFL include multiple whitish, flat, and slightly elevated lesions of various sizes, mainly located in the gastric body and fundus. Narrow-band imaging enhanced the contrast of MWFL and background mucosa, and magnifying narrow-band imaging depicted a uniformly long, narrow, and elliptical marginal crypt epithelium with an unclear microvascular pattern. Histopathological findings revealed hyperplastic changes of the foveolar epithelium, and parietal cell protrusions and oxyntic gland dilatations were observed in the fundic glands, without any intestinal metaplasia. The rate of acid-reducing drug use was significantly higher in patients with MWFL than in those without (100% [13/13] vs. 53.7% [88/164], p < 0.001). CONCLUSIONS:: The present study indicated a relationship between the presence and endoscopic features of MWFL and history of acidreducing drug use.

5.Assessment of the First-year Grade Students in Junior High School Smoking Prevention Education and Student’s Attitudes Toward Smoking

To assess the changes in attitudes toward smoking after the smoking-prevention classes presented by Kumamoto Tobacco-Free Forum, this study administered a questionnaire survey to first-year junior high school students before and after a smoking-prevention class at a junior high school. The questionnaire that was used to pre and post comparison comprised thirteen items, ten from the Kano Test for Social Nicotine Dependence (KTSND-Youth) and three as follows: “I think I will be a smoker in the future,” “I think I will smoke about once,” and “What do you think of people around you who smoke tobacco?” The results of this pre- and post-class questionnaire indicate that attitudes toward seven items improved in a statistically significant way. These included four from KTSND-Youth: “Smokers cannot stop even if they want to,” “Smoking is adult-like and cool,” “Smoking is enjoys taste and a fragrance,” and “If it’s a place with an ashtray, it’s okay to smoke.” The smoking-prevention classes significantly decreased total KTSND-Youth scores. Furthermore, the analysis has clarified that the KTSND-Youth items: “Smoking is adult-like and cool” and “Doctor and school teachers say, ‘Tobacco is no good too much”, as well as the item “What do you think of people around you who smoke tobacco?” are significantly influencing factors for the item “I will be a smoker in the future.” These results indicate that the smoking-prevention classes presented by Kumamoto Tobacco-Free Forum were effective for decline in social nicotine dependence of the first-year students in junior high school.
6.Thyroid hormone alterations in trauma patients requiring massive transfusion: An observational study

BACKGROUND: Although non-thyroidal illness syndrome (NTIS) is considered a negative prognostic factor, the alterations in free triiodothyronine (fT3) levels in trauma patients requiring massive transfusion have not been reported. METHODS: A prospective observational study comparing 2 groups of trauma patients was conducted. Group M comprised trauma patients requiring massive transfusions (>10 units of packed red blood cells) within 24 hours of emergency admission. Group C comprised patients with an injury severity score >9 but not requiring massive transfusions. Levels of fT3, free thyroxine (fT4), and thyroid-stimulating hormone (TSH) were evaluated on admission and on days 1, 2, and 7 after admission. The clinical backgrounds and variables measured including total transfusion amounts were compared and the inter-group prognosis was evaluated. Results are presented as mean±standard deviation. RESULTS: Nineteen patients were enrolled in each group. In both groups, 32 were men, and the mean age was 50±24 years. In group C one patient died from respiratory failure. The initial fT3 levels in group M (1.95±0.37 pg/mL) were significantly lower than those in group C (2.49±0.72 pg/mL;P<0.01) and remained low until 1 week after admission. Initial inter-group fT4 and TSH levels were not significantly different. TSH levels at 1 week (1.99±1.64 μIU/mL) were higher than at admission (1.48±0.5 μIU/mL) in group C (P<0.05). CONCLUSION: Typical NTIS was observed in trauma patients requiring massive transfusions. When initial resuscitation achieved circulatory stabilization, prognosis was not strongly associated with NTIS.
7.Effect of increase in body temperature on cognitive function during prolonged exercise

This study examined the effect of prolonged exercise with and without hyperthermia on cognitive function. In this study, 9 male subjects cycled at 50% watt max for 60 min in a room maintained at 23°C. Water-perfused suits with water at 7°C (COOL) and 47°C (HOT) were randomly used during the 2 cycling periods. Rectal temperature, skin temperatures, heart rate, ratings of perceived exertion (RPE), and thermal sensation were evaluated during exercise. The Stroop color-word test (SCWT) and executive processing test (EPT) were conducted every 15min during exercise to evaluate cognitive function. In the COOL trial, the core temperature stabilized at 37.5 ± 0.1°C (mean ± SE), whereas in the HOT trial, the core temperature increased to 39.1 ± 0.1°C, with a concomitant increase in RPE and thermal sensation. Although the reaction time for the simple task of the SCWT improved when the body temperature increased slightly during the COOL trial, the conflict task of the SCWT was impaired during the latter stages of exercise during the HOT trial. The reaction time for the HOT trial was significantly longer than that for the COOL trial. Compared to the bigining of exercise the reaction time for the EPT was impaired at 60 min during the HOT trial, and this reaction time was longer than that for the COOL trial. The present findings indicate that exercise-induced hyperthermia during the latter stages of exercise impairs cognitive as well as physiological functions.
8.Nerve Growth Factor in Saliva Stimulated by Mastication

The aim of the present study was to measure the amount of nerve growth factor (NGF) present in saliva, and investigate the relationship with aging, gender, and number of retained teeth. The subjects were 93 healthy volunteers (35 males and 58 females) aged 18-87 years. Stimulated saliva was collected during gum chewing, and saliva secretion per minute and NGF concentration per unit volume were measured by enzyme-linked immunosorbent assay. We found that NGF in saliva was measurable in all subjects. The mean NGF concentration in saliva was 55.70 ± 38.27 pg/ml, and no relationship was found between NGF concentration and aging, gender, and the number of retained teeth. The mean NGF amount in saliva was 84.38 ± 71.06 pg/min. The NGF amount in saliva decreased with age, especially for the group aged 70 years and older. It was higher in male than female subjects, and was significantly higher in the group without tooth loss compared to the edentulous group. In addition, regarding the relationship between the saliva flow rate per minute and NGF amount in saliva, the latter increased significantly with a rise in the former. These results indicate that the NGF amount in saliva is influenced by aging, masticatory function, and saliva amount. It is speculated that human salivary glands play an important role in the synthesis and expression of NGF.
9.THE GENDER DIFFERENCES IN THERMOREGULATORY RESPONSES DURING EXERCISE RECOVERY

The purpose of this study was to investigate the gender differences in thermoregulatory responses during the recovery from prolonged exercise. Fourteen (7 males and 7 females) subjects cycled for 45 minutes at 55%VO2max and then rested for 105 minutes in the hot and humid environment (30℃, 80% relative humidity) or cool environment (20℃, 60% relative humidity). Both males and females showed same variations in rectal temperature (Tre) during exercise and recovery periods. Total sweat volume and water intake were significantly larger in males than in females during recovery in the both environments. In the heat condition, mean skin temperature (Tsk) decreased immediately after the cessation of exercise in females but not in males (the recovery period (R) 5–15 min : p<0.01, R15–20 min : p<0.05). These results suggested there are gender-related differences in the mechanisms and also the process maintaining the balance of heat production and heat loss response during recovery from prolonged exercise. Moreover the heat might modify gender differences of heat loss response during recovery as an important factor.
10.EPIDEMIOLOGICAL STUDY OF INFLUENZA VIRUS INFECTIONS IN YANGON, MYANMAR

Although influenza is a highly contagious acute respiratory illness of global importance, little is known about the disease in tropical countries. An influenza survey was conducted in three sentinel sites in Yangon, Myanmar from September 2003 to December 2004. Throat or nasal swabs were collected from 616 patients with influenza-like symptoms and tested using rapid diagnostic test kits and virus isolation. Influenza B virus was detected in 6 patients from September to October, 2003. Influenza A viruses were detected in 133 patients from June to September, 2004, and the 51 influenza A viruses isolated from 72 specimens were all A⁄H3N2. Influenza virus infections occurred mainly in the rainy season in Yangon, Myanmar, but continuous ongoing influenza surveillance is needed.
