1.A Single-Centre Malaysian Cohort Study on Predictors of Glycemic Improvement 12 Months After Bariatric Surgery

Introduction:
Bariatric surgery is an established intervention for obesity
and its metabolic complications, including diabetes
mellitus. However, most studies are from Western
populations with limited data in Malaysia. This study
evaluated glycemic status improvements and identified
predictors of hemoglobin A1c (HbA1c) reduction at 12
months following bariatric surgery to contribute regional
data to Southeast Asia.
Methodology:
This retrospective analysis included 110 patients who
underwent bariatric surgery between 2015 and 2025.
Pre- and post-operative parameters, including HbA1c,
body mass index (BMI), and lipid profiles, were collected
at 1, 3, 6, 9, and 12 months and evaluated as potential
predictors of glycemic status improvement at 12 months.
Univariate analysis followed by stepwise multivariate
linear regression was performed to identify predictors of
HbA1c improvement at 12 months. Regression coefficients
(B), 95% confidence intervals, and p-values were reported.
Results:
The mean age was 40.5 years with a mean baseline BMI of
43.49 kg/m²; 58% were female. At baseline, 60.9% (n = 67)
had diabetes, 30.9% (n = 34) prediabetes, and 8.2% (n = 9)
normal HbA1c. In univariate analysis, baseline HbA1c (B
= 0.579, p <0.001), age (B = −0.051, p = 0.034), and sex (B =
1.123, p = 0.044) were significantly associated with HbA1c
change. In multivariate analysis, the final model identified
baseline HbA1c, BMI difference at 12 months, and TG
difference at 12 months as independent predictors of HbA1c change. Baseline HbA1c was the strongest predictor (B =
0.857, p <0.001) indicating that higher baseline HbA1c was
associated with greater change over time. BMI difference
at 12 months was also significantly associated with HbA1c
change (B = 0.046, p = 0.016), indicating that greater weight
loss contributed to improved glycemic control. Similarly,
TG difference at 12 months was a significant predictor (B =
0.337, p = 0.023). The model explained 86.8% of the variation
in HbA1c change (adjusted R² = 0.868).
Conclusion
Bariatric surgery resulted in significant weight loss and
improvement in glycemic status at 12 months, with baseline HbA1c, BMI differences, and TG reductions emerging
as key predictors of glycemic improvement in a Malaysian
cohort.

Keropok lekor is a fish-based snack and traditional food originating from the East Coast of
Peninsular Malaysia, especially in Terengganu. This study was conducted to identify the
ingredients and packaging used in the manufacturing of keropok lekor by entrepreneurs in
Selangor. This study is a qualitative study that requires in-depth interviews. The Nutritionist
Pro application was used to determine the nutrients in keropok lekor. The sampling method for
this study is purposive sampling and involves five keropok lekor operators around Selangor.
The ingredients used in making keropok lekor are salt, fish filling (fringescale sardine), baking
powder, monosodium glutamate (MSG), margarine, and sweet potato flour. Through this study,
it was found that the main source of sodium in keropok lekor is salt. For keropok lekor
packaging, all keropok lekor operators use the same packaging method, which is vacuum
packaging. In addition, the majority of keropok lekor operators (80%) have been introduced
to Modified Atmosphere Packaging (MAP). The results of the study show that the average
sodium content in keropok lekor is 372.11mg/100g and is categorized as medium-level sodium.
Although the majority of keropok lekor operators have been introduced to MAP packaging,
further research needs to be conducted to find out whether keropok lekor operators have
renewed their packaging methods or vice versa. In conclusion, based on the findings, the
content of sodium in the manufacturing of keropok lekor in Selangor is still under control. The
results of this study can be used to help the government monitor the sodium content found in
keropok lekor around Selangor.
3.Correlation of Vitamin D With Bone Mineral Density by Dual Energy X-ray Absorptiometry (DXA) Scan Among Healthy Malay Adult

Introduction: Scarcity of data found in regard to association of vitamin D level with bone mineral density (BMD). Our study aimed to determine the correlation of vitamin D with BMD and intact parathyroid hormone (iPTH) among healthy Malay adult. Methods: This cross-sectional study recruited 126 healthy Malay volunteers (aged 21–45 years old) from Kota Bharu, Malaysia. Serum total calcium, albumin, phosphorus, 25-hydroxyvitamin D (25(OH)D), and iPTH were measured. BMD was assessed with dual energy X-ray absorptiometry (DXA) scan over left hip (right hip in case of problem with left hip) and lumbar spine (L1 – L4 vertebrae). Results: The mean serum 25(OH)D was 38.91 ± 14.07 nmol/L. Out of 126 study subjects, 104 subjects (82.5%) had insufficient level of vitamin D (< 50 nmol/L). Mean hip and lumbar BMD were 0.952 ± 0.145 g/cm2 and 1.006 ± 0.133 g/cm2 respectively. According to T-score, 93 subjects (73.8%) had normal T-score of ≥ -1, 33 subjects (26.2%) had osteopenia (T-score -2.5 to -1) and none had osteoporosis (T-score ≤ -2.5). Significant positive correlation between serum 25(OH)D and hip BMD (r = 0.234, p = 0.009) was observed, but no correlation was found between serum 25(OH)D and lumbar BMD. Meanwhile, significant inverse correlation between serum 25(OH)D and iPTH was observed (r = -0.324, p < 0.001). Conclusion: High prevalence of vitamin D insufficiency was observed among healthy Malay population, but majority had normal bone density. Nonetheless, serum 25(OH)D was positively correlated with BMD and inversely correlated with iPTH. Our findings support the role of vitamin D for maintaining bone health.
4.Development Of Clinical Pathway For Mild Cognitive Impairment And Dementia To Quantify Cost Of Age-Related Cognitive Disorders In Malaysia

As the Malaysian population ages, the burden of age-related cognitive disorders such as dementia and Alzheimer’s disease will increase concomitantly. This is one of the sub-study under a research project titled by quantify the cost of age-related cognitive impairment in Malaysia, which was undertaken to develop a clinical pathway for Mild Cognitive Impairment (MCI) and Dementia. The clinical pathway (CP) will be used to support the costing studies of MCI and Dementia. An expert group discussion (EGD) was conducted among selected experts from six (6) government hospitals from different states of Malaysia, Ministry of Health, and United Nations University, International Institute for Global Health, UKM and UPM. The expert group includes psychiatrist specialists and public health medicine specialists. A total of 15 participants took part in the EGD. The group was presented with the different approach in managing MCI and Dementia. Finally, the group came to the consensus agreement on the most appropriate and efficient ways of managing the two conditions. In the EGD, an operational definition for MCI and Dementia was agreed upon and a pathway was developed for the usual practice in the Malaysian health system. A typical case used, as a reference is a 60-year-old patient referred to a memory clinic with complaint of “forgetfulness”. After three outpatient visits in the clinic, the diagnosis of MCI and Dementia could be clinically established. The clinical pathways covered all active clinical and non-clinical management of the patient over a period of one year. The experts identified the additional resources required to manage these patients for the whole spectrum of lifetime based on the expected life expectancy. The Clinical pathway (CP) for MCI and Dementia was successfully developed in EGD with strong support from practitioners in the health system. The findings will help the researchers to identify all-important clinical activities and interventions that will be included in the costing study.
5.Ventricular dyssynchrony is common among heart failure patients with narrow QRS complex

Current selection guideline for CRT uses broad QRS duration (>120 ms) as a marker for ventricular
dyssynchrony. However, more recent data supports mechanical marker specifically measured
by Tissue Doppler Imaging (TDI) as a better criterion to predict response to CRT. Sixty seven
patients with significant left ventricular dysfunction (EF less than 40%) and narrow QRS complex
were prospectively enrolled. They underwent Tissue Doppler Imaging (TDI) study to evaluate
intraventricular mechanical dyssynchrony. Dyssynchrony index which is defined as standard
deviation of time to peak systolic velocity in twelve ventricular segments was measured. A value
greater than 32.6 is taken to reflect significant ventricular dyssynchrony. Overall 38 patients
(56.7%) demonstrated significant dyssynchrony. There was no significant correlation between QRS
duration and the Ts-SD-12 (r = 0.14, p = 0.11). Ventricular mechanical dyssynchrony is common
in patients with normal QRS duration. Therefore, QRS duration alone will miss a substantial
proportion of suitable patients for CRT and therefore deny them this adjunct therapy. We propose
echocardiographic parameters, specifically TDI, to be included in patient selection criteria for CRT.
