1.Arthroscopic versus open Brostr?m-Gould repair of the anterior talofibular ligament for chronic ankle instability in young men undergoing high intensity exercise

Objective:To compare arthroscopic versus open Brostr?m-Gould repair of the anterior talofibular ligament (ATFL) in the treatment of chronic ankle instability (CAI) in young men undergoing high intensity exercise.Methods:A retrospective study was conducted to analyze the 61 young male patients with CAI undergoing high-intensity exercise who had been treated at Department of Trauma and Orthopedics, 947th Army Hospital of Chinese People’s Liberation Army from January 2016 to July 2020. Their age was (25.9±2.7) years and their disease duration (13.9±2.8) months. According to their different treatment methods, they were divided into an arthroscopic group ( n=26) in which their ATFL was repaired by arthroscopic Brostr?m-Gould surgery and a Brostr?m-Gould group ( n=35) in which their ATFL was repaired by open Brostr?m-Gould surgery. The 2 groups were compared in terms of operation time and intraoperative bleeding, as well as the ankle-hindfoot scores of American Orthopaedic Foot & Ankle Society (AOFAS), Karlsson ankle functional (KAF) scores and visual analogue scale (VAS) pain scores at postoperative 3, 6, 12, and 24 months. Results:There were no significant differences in the preoperative general data between the 2 groups, indicating comparability ( P > 0.05). The operation time [(35.8±3.9) min] and intraoperative bleeding [(6.6±2.6) mL] in the arthroscopic group were significantly less than those in the Brostr?m-Gould group [(52.1±4.6) min and (16.1±4.0) mL] ( P < 0.05). The AOFAS ankle-hindfoot scores and KAF scores in the arthroscopic group were significantly higher than those in the Brostr?m-Gould group at postoperative 3 and 6 months, but the AOFAS ankle-hindfoot score and KAF score at postoperative 24 months were significantly lower than those in the Brostr?m-Gould group ( P < 0.05). There was no statistically significant difference in AOFAS ankle-hindfoot score or KAF score between the 2 groups at postoperative 12 months, as well as in VAS pain scores at postoperative 3, 6, 12, and 24 months between the 2 groups ( P > 0.05). Conclusions:In young men undergoing high intensity exercise, compared with open Brostr?m-Gould surgery, arthroscopic Brostr?m-Gould surgery may lead to better clinical outcomes in a short-term (3 months after surgery). However, open Brostr?m-Gould surgery may result in better long-term efficacy than arthroscopic Brostr?m-Gould surgery (24 months after surgery).
2.Comparison of the quality of bowel preparation for colonoscopy between 2.0 L and 1.5 L polyethylene glycol under optimized dietary restrictions: a multicenter randomized controlled study

Objective:To compare bowel preparation quality between 2.0 L and 1.5 L polyethylene glycol (PEG) regimens with optimized dietary restrictions.Methods:This study was a randomized controlled trial conducted in three hospitals: the First Affiliated Hospital of Naval Medical University ( n=57), Huadong Hospital Affiliated to Fudan University ( n=30), and General Hospital of Northern Theater Command ( n=30) from May 5th to 30th, 2024. Participants consumed food for special medical purpose one day before examination or therapeutic colonoscopy and were randomized to receive either 2.0 L PEG (group A) or 1.5 L PEG (group B). Outcomes included the completion rate of bowel preparation, the adequate/excellent bowel preparation rate, Boston bowel preparation scale scores, the subject/endoscopist satisfaction, the willingness to repeat the preparation regimen, and incidence of adverse events. Results:A total of 60 subjects in group A and 57 in group B were included. There was no significant difference in baseline characteristics between the two groups ( P>0.05). The adequate bowel preparation rate [81.7% (49/60) VS 64.9% (37/57), χ2=4.21, P=0.040] and endoscopist satisfaction [88.3% (53/60) VS 70.2% (40/57), χ2=5.91, P=0.015] in group A were significantly higher than those in group B. There were no significant differences in bowel preparation completion rates, the excellent bowel preparation rate, the bowel preparation score, subject satisfaction, willingness to repeat the preparation regimen, or incidence of adverse events ( P>0.05). Conclusion:When combined with optimized dietary restrictions, 2.0 L PEG provides superior bowel preparation quality compared with 1.5 L PEG.

Objective:To compare the short-term efficacy of robotic-assisted sacrocolpopexy(RASC)and lap-aroscopic sacrocolpopexy(LSC)in overweight/obese patients with pelvic organ prolapse(POP),and to evaluate the advantages of RASC.Methods:We retrospectively collected the clinical data of overweight/obese(body mass index≥24 kg/m2)patients over 50 years old with POP who underwent sacrocolpopexy in the Department of Gynecology of the First Affiliated Hospital of Zhengzhou University from November 2014 to June 2023.According to the surgical method,they were divided into robotic group(68 cases)and laparoscopic group(110 cases).The intraoperative,postoperative and short-term follow-up outcomes of the two groups were compared.Pelvic Floor Distress Inventory-short form 20(PFDI-20),Pelvic Floor Impact Questionnaire-short form 7(PFIQ-7)and the Patient Global Impression of Improvement(PGI-Ⅰ)were used to evaluate surgical outcomes.Results:①No intra-operative conversion to laparotomy was found in both groups.Compared with the laparoscopic group,the robotic group had shorter operation time,less intraoperative blood loss,shorter postoperative anal exhaust time,and shor-ter postoperative hospital stay.The difference was statistically significant(P<0.05).②There were no intraopera-tive complications in the robot group.While in the laparoscopic group,there was 1 case of bladder injury.③Within the two groups,the PFDI-20 and PFIQ-7 scores at 3 and 12 months after surgery were lower than those before surgery,and the difference was statistically significant(P<0.05).PGI-Ⅰ scores were both"significantly im-proved".Subjective and objective cure rate were 100%.④The POP-Q indicator points(Aa,Ba,C,Ap,Bp,gh,pb,TVL)of both groups at 3 and 12 months after surgery were better than those before surgery(P<0.05),achie-ving satisfactory anatomical reduction.Conclusions:RASC is safe and feasible in the treatment of overweight and obese patients with POP.Compared to LSC,RASC has significantly shorter operation time,less blood loss,faster postoperative intestinal function recovery,shorter postoperative hospital stay,and stable short-term effect.Therefore,RASC can provide a new choice for overweight/obese patients with POP.
