Objective To evaluate the safety and immunogenicity of a booster dose of bivalent types Ⅰ and Ⅲ oral poliovirus vaccine(bOPV) of sugar pill and liquid dosage forms in children who were sequentially vaccinated with oral poliovirus vaccine(OPV) and Sabin strain-based inactivated poliovirus vaccine(sIPV) at the age of 48 months.Methods From January2018 to May 2020, a total of 485 healthy children aged 48 months who completed basic immunization and had paired serum antibody detection results before and after immunization were selected in the phase 3 clinical study of sequential immunization combined with OPV and sIPV in Liujiang District, Liucheng County and Rongan County of Liuzhou City to conduct enhanced immunization study. All the subjects were inoculated with 1 dose of bOPV for enhanced immunization, the occurrence of adverse events within 28 days after enhanced immunization was observed, and serum samples were collected before and 28 days after enhanced immunization to detect neutralizing antibodies against poliovirus typesⅠ,Ⅱ and Ⅲ. The incidence of adverse events, poliovirus neutralizing antibody positive rate, seroconversion rate, and geometric mean titer(GMT) were analyzed.Results A total of 485 subjects were inoculated with bOPV for enhanced immunization, among which, 485 cases were included in safety evaluation, and 342 cases in immunogenicity evaluation. The overall incidence of adverse events at 28 days after enhanced immunization was 31. 75%(154/485), and the incidence of vaccination-related adverse events was 24. 74%(120/485). Among 120 cases of vaccination-related adverse reactions, the severity was mainly grade 1 and grade 2, and no adverse reactions of grade 4 occurred. Before booster immunization, the positive rate of poliovirus type Ⅰ neutralizing antibody was 97. 56% in 2 conventional inactivated poliovirus vaccine(cIPV) + trivalent oral poliovirus vaccine(tOPV)(liquid dosage) group, and 100. 00% in other groups. The positive rate of type Ⅱ poliovirus neutralizing antibody was 75. 00% in sIPV + 2 bOPV(liquid dosage) group, and was more than 90. 00% in other groups. The positive rate of type Ⅲ poliovirus neutralizing antibody was 100% in all groups. At 28 days after booster immunization, the positive rate of typesⅠ, Ⅱ and Ⅲ poliovirus neutralizing antibody was 100. 00% in all groups. The seroconversion rate of typeⅠpoliovirus neutralizing antibody was more than 50. 00% and the seroconversion rate of type Ⅲ poliovirus neutralizing antibody was more than 40. 00%, while the seroconversion rate of type Ⅱ poliovirus neutralizing antibody was relatively low.The GMT of typeⅠpoliovirus neutralizing antibody increased by about 5-20 times, the GMT of typeⅡpoliovirus neutralizing antibody increased by about 1. 5-5 times, and the GMT of type Ⅲ poliovirus neutralizing antibody increased by about5-15 times, compared with that before immunization. The difference of GMT of neutralizing antibodies against poliovirus typesⅠ, Ⅱ and Ⅲ before and after booster immunization was statistically significant(t =-20. 769,-6. 128 and-19. 609,respectively, each P < 0. 01).Conclusion The infants who completed the sequential immunization of OPV and IPV at 2 months of age showed good safety and immunogenicity in enhanced immunization with bOPV at 48 months.

Background/Aims: Anti-reflux mucosal ablation (ARMA) is a promising endoscopic intervention for proton pump inhibitor (PPI)-dependent gastroesophageal reflux disease (GERD). However, the effect of ARMA on esophageal motility remains unclear. Methods: Twenty patients with PPI-dependent GERD receiving ARMA were prospectively enrolled. Comprehensive self-report symptom questionnaires, endoscopy, 24-hour impedance-pH monitoring, and high-resolution impedance manometry were performed and analyzed before and 3 months after ARMA. Results: All ARMA procedures were performed successfully. Symptom scores, including GerdQ (11.16 ± 2.67 to 9.11 ± 2.64, P = 0.026) and reflux symptom index (11.63 ± 5.62 to 6.11 ± 3.86, P = 0.001), improved significantly, while 13 patients (65%) reported discontinuation of PPI. Total acid exposure time (5.84 ± 4.63% to 2.83 ± 3.41%, P = 0.024) and number of reflux episodes (73.05 ± 19.34 to 37.55 ± 22.71, P < 0.001) decreased significantly after ARMA. Improved esophagogastric junction (EGJ) barrier function, including increased lower esophageal sphincter resting pressure (13.89 ± 10.78 mmHg to 21.68 ± 11.5 mmHg, P = 0.034), 4-second integrated relaxation pressure (5.75 ± 6.42 mmHg to 9.99 ± 5.89 mmHg, P = 0.020), and EGJ-contractile integral(16.42 ± 16.93 mmHg · cm to 31.95 ± 21.25 mmHg · cm, P = 0.016), were observed. Esophageal body contractility also increased significantly (distal contractile integral, 966.85 ± 845.84 mmHg · s · cm to 1198.8 ± 811.74 mmHg · s · cm, P = 0.023). Patients with symptom improvement had better pre-AMRA esophageal body contractility. Conclusions ARMA effectively improves symptoms and reflux burden, EGJ barrier function, and esophageal body contractility in patients with PPIdependent GERD during short-term evaluation. Longer follow-up to clarify the sustainability of ARMA is needed.

Methods: We retrospectively analyzed the clinical data of patients with superficial ESCN who underwent ESD or ERFA between January 2015 and December 2021. Objective: To create a prediction model for stroke risk among middle-aged and elderly populations, so as to provide a basis for early identification of high-risk population for stroke. Methods: From October to December 2023, residents aged ≥45 years in Chongchuan District, Nantong City, Jiangsu Province were selected using a multi-stage stratified random sampling method. The demographic information, life behavior, and chronic disease data were collected through a questionnaire survey. The standardized prevalence of stroke was calculated using data from the seventh National Population Census. The subjects were randomly divided into the training set and the internal validation set according to the ratio of 8∶2. The basic demographic information, life behavior, and chronic diseases of residents aged ≥45 years in Rugao City were collected from July to August 2023 as the external validation set. Predictive factors were selected using multivariable logistic regression model, and a nomogram for stroke among residents aged ≥45 years was established. The prediction effect was evaluated using the area under the curve (AUC) of the receiver operating characteristic (ROC), calibration curve, and Hosmer-Lemeshow goodness of fit test. Results: A total of 6 290 residents aged ≥45 years were included, including 2 975 males (47.30%) and 3 315 females (52.70%). The average age was (61.90±10.20) years. The prevalence of stroke was 3.80%, and the standardized prevalence was 3.36%. The multivariable logistic regression showed that age, smoking, hypertension, and hyperlipidemia were predictors of stroke risk among residents aged ≥45 years, and the prediction model was ln[p/(1-p)]=-4.619+0.046×age+0.383×smoking+0.887×hypertension+0.678×hyperlipidemia. The AUC values of the training set, internal validation set, and external validation set were 0.748, 0.755, and 0.738, respectively. The consistency indexes were 0.748, 0.755, and 0.738, respectively. The Hosmer-Lemeshow goodness of fit test showed a good fitting effect (P>0.05). Conclusion The prediction model based on age, smoking, hypertension, and hyperlipidemia has good discrimination and calibration, and can be used to predict the risk of stroke among middle-aged and elderly populations aged ≥45 years.

Objective: To investigate trends in incidence and mortality of colorectal cancer in Nantong City, Jiangsu Province from 2013 to 2022. Methods: Data on incidence and mortality of colorectal cancer from 2013 to 2022 in Nantong City were collected through the Nantong City cancer registry. The crude incidence, crude mortality, average age at onset, and average age at death of colorectal cancer were calculated. Chinese population-standardized incidence, Chinese population-standardized mortality, Chinese population-standardized average age at onset and Chinese population-standardized average age at death were calculated using the age structure of the standard population from the Fifth National Population Census in 2000. Trends in incidence and mortality of lung cancer from 2013 to 2022 were evaluated using average annual percent change (AAPC). Trends in the Chinese population-standardized average age at onset and Chinese population-standardized average age at death of lung cancer from 2013 to 2022 were evaluated using the linear regression model. Results: From 2013 to 2022, the crude incidence and Chinese population-standardized incidence of colorectal cancer in Nantong City increased from 33.63/105 and 16.05/105 to 53.82/105 and 19.62/105, respectively, showing upward trends (AAPC=5.665% and 2.467%, both P<0.05). The crude mortality increased from 15.99/105 in 2013 to 25.65/105 in 2022, also showing an upward trend (AAPC=5.514%, P<0.05), while no statistically significant trend was found in the Chinese population-standardized mortality (P>0.05). The Chinese population-standardized incidence of colorectal cancer showed upward trends in both males and females (AAPC=2.666% and 1.790%, both P<0.05). The Chinese population-standardized mortality showed an upward trend in males (AAPC=1.966%, P<0.05), but no statistically significant trend was found in females (P>0.05). The crude incidence of colorectal cancer in the groups aged 40-<50 years, 50-<60 years, 60-<70 years, 70-<80 years, and ≥80 years showed upward trends (AAPC=4.045%, 2.833%, 2.300%, 1.948%, and 1.775%, all P<0.05), and the crude mortality in the group aged ≥80 years showed an upward trend (AAPC=3.240%, P<0.05). The average age at onset of colorectal cancer increased at an annual average of 0.156 years (P<0.05), while the trend in the Chinese population-standardized average age at onset was not statistically significant (P>0.05). The average age at death and the Chinese population-standardized average age at death increased at an annual average of 0.325 and 0.153 years, respectively (both P<0.05). Conclusions From 2013 to 2022, both the crude incidence and mortality of colorectal cancer in Nantong City showed upward trends. Males and individuals aged ≥40 years faced a higher risk of both incidence and mortality. It is recommended to implement comprehensive prevention and control measures targeting these high-risk populations to reduce the burden of colorectal cancer.
