1.Characteristics of Traditional Chinese Medicine Syndromes and Their Correlation with Ocular Manifestations in Chronic Hepatitis B Complicated by Metabolic Dysfunction-associated Fatty Liver Disease

ObjectiveThis paper aims to investigate the traditional Chinese medicine syndrome types in patients with chronic hepatitis B (CHB) complicated by metabolic dysfunction-associated fatty liver disease (MAFLD) and explore the correlations between these syndrome types and clinical indicators, as well as ocular manifestation characteristics, thereby providing a reference for syndrome differentiation and treatment strategies in traditional Chinese medicine. MethodsGeneral data, information from the four diagnostic methods of traditional Chinese medicine, clinical indicators, and ocular manifestation data were collected from 506 patients with CHB complicated by MAFLD enrolled at the Public Health Clinical Center of Chengdu between June 2024 and December 2024. Cluster analysis, principal component analysis, and complex network models were employed to identify the distribution patterns of traditional Chinese medicine syndromes. Correlations between different syndrome types and clinical indicators, as well as ocular manifestation characteristics, were further analyzed. ResultsThe predominant syndromes identified in patients with CHB complicated by MAFLD were dampness and heat accumulation (51.58%), liver depression with spleen deficiency (31.62%), blood stasis obstructing collaterals (8.89%), and Qi-Yin deficiency (7.91%). No statistically significant differences were found among the four syndrome types in routine blood tests and liver function indicators. However, patients with the dampness and heat accumulation type exhibited significantly higher levels of total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), liver stiffness measurement (LSM), controlled attenuation parameter (CAP), and alpha-fetoprotein (AFP), along with lower levels of high-density lipoprotein cholesterol (HDL-C), compared with those with other syndrome types. Regarding ocular manifestations, the incidence of moon halo signs was significantly higher in patients with the blood stasis obstructing collaterals type than in those with other syndrome types. Additionally, the incidence in scleral zone 3 (corresponding to the large intestine) was higher in patients with the damp and heat accumulation type. ConclusionDampness and heat accumulation is the core syndrome type in patients with CHB complicated by MAFLD, commonly accompanied by spleen deficiency, liver depression, blood stasis, and Yin deficiency. A complex syndrome pattern characterized by a predominance of dampness and heat, along with a mixture of deficiency and excess, is formed. Different traditional Chinese medicine syndrome types are associated with distinct clinical indicators and ocular manifestation characteristics. Among them, patients with the dampness and heat accumulation type exhibit more pronounced metabolic disturbances and liver injury, whereas those with the blood stasis type show a higher incidence of moon halo signs. Abnormalities in scleral zone 3 are also more prevalent in patients with dampness and heat type.

Paraplegia caused by spinal cord injury is a serious neurological complication, for which surgery is currently the main treatment method. Due to different surgical approaches, patients are usually expected to maintain a passive prone position for a long time or switch between the supine and prone positions. Affected by multiple factors such as neurogenic sensory disorders, pathological changes in muscle tone and operative duration, the risk of intraoperative acquired pressure injury (IAPI) is significantly increased. Current clinical prevention strategies for IAPI in these patients predominantly focus on localized pressure relief during positioning, lacking systematic, standardized comprehensive prevention protocols or evidence-based guidelines. To address it, Department of Nursing, Orthopedics Branch, China International Exchange and Promotive Association for Medical and Health Care, Spinal Trauma Professional Committee, Orthopedics Branch, Chinese Medical Doctor Association, Nursing Group of Spine and Spinal Cord Professional Committee of Chinese Association of Rehabilitation Medicine organized experts in relevant fields to formulate Guideline for the prevention of intraoperative acquired pressure injury in paraplegic patients with spinal cord injury ( version 2025), based on evidence-based medical evidence and latest research results and clinical practice at home and abroad. Eleven recommendations were put forward from the aspects of preoperative risk assessment, intraoperative prevention strategies, postoperative handover and monitoring, and supportive mechanisms for IAPI prevention, aiming to standardize the prevention measures and management strategies of IAPI in paraplegic patients with spinal cord injury and accelerate the recovery of patients and improve the therapeutic effect.

We performed descriptive analyses on hospitalised patients diagnosed with IBD between 2019 and 2023, who received TCM treatment.Results:The study included 11 095 case records from 4 760 patients, with TCM diagnoses primarily indicating diarrhoea and abdominal pain. Ulcerative colitis (UC) accounted for 4 782 hospitalizations (3 103 patients), while Crohn's disease (CD) accounted for 6 313 hospitalizations (1 657 patients). Patient demographics showed a trend towards younger age and a higher proportion of males. Treatment utilisation was highest in southern Jiangsu compared with the central and northern regions. In terms of disease burden, all treatment costs showed a downward trend. In terms of external TCM therapies, UC patients tend to prefer plasters and enemas, while CD patients are more inclined to use acupuncture. Regarding herbal medicine, licorice, white atractylodes, and white peony root are commonly used single herbs for IBD patients.Conclusions:The number of IBD patients treated with TCM in Jiangsu Province has steadily increased from 2019 to 2023. It is important to identify effective TCM treatment methods to reduce the burden of patients.

7.Non-contrast CT findings of acute ischemic stroke for predicting early prognosis after mechanical thrombectomy

Objective To explore the value of non-contrast CT findings of acute ischemic stroke(AIS)for predicting early prognosis after mechanical thrombectomy.Methods Data of 161 AIS patients from clinical center 1 who underwent mechanical thrombectomy were retrospectively analyzed.The patients were divided into training set(n=113)and internal test set(n=48)at the ratio of 7∶3,while 79 AIS patients who underwent mechanical thrombectomy from clinical center 2 were retrospectively enrolled as external test set.According to the National Institutes of Health stroke scale(NIHSS)scores 7 days after thrombectomy,patients'prognosis were classified as good(<15 points)or poor(≥15 points).Pre-treatment non-contrast CT images of patients were reviewed,and CT findings were comparatively analyzed.Independent predictors of patients'early prognosis after mechanical thrombectomy were obtained with sequential univariate and multivariate logistic regressions,and a predicting model was established and visualized as a nomogram.The receiver operating characteristic curve was drawn,and the distinction was assessed with the area under the curve(AUC),then calibration was assessed with Hosmer-Lemeshow goodness of fit test,and the net benefit was evaluated with decision curve analysis(DCA).Results Alberta stroke program early CT score(ASPECTS),hyperdense middle cerebral artery sign(HMCAS)and basal ganglia calcification were all independent predictors of early prognosis of AIS after mechanical thrombectomy(all P<0.05).The predictive model was established combining the above 3 variables and then visualized as a nomogram to predict prognosis of AIS after mechanical thrombectomy,with AUC of 0.776 in internal test set(χ2=6.052,P=0.417)and 0.800 in external test set(χ2=2.269,P=0.811).DCA showed that the nomogram might provide clinical net benefit within certain threshold probability ranges.Conclusion ASPECTS,HMCAS and basal ganglia calcification were all independent predictors of early prognosis of AIS after mechanical thrombectomy.The nomogram originated from predicting model combining the three could be used to somewhat accurately predict poor early prognosis after mechanical thrombectomy.
8.Analysis on the Inheritance and Innovation Policy of Traditional Chinese Medicine in China Based on Policy Tools

OBJECTIVE:To know about the use of policy tools for trad itional Chinese medicine (TCM)inheritance and innovation policies in China ,and to provide reference for subsequent improvement of TCM inheritance and innovation policies. METHODS:The content analysis method was used to classify and code the policy texts related to TCM inheritance and innovation policies at the national level before Sept. 2020. Statistical analysis was carried out from the two dimensions of basic policy tools and the TCM inheritance and innovation system ,and related suggestions were put forward. RESULTS :Nine related policy texts were collected ,which were published during 2007-2020. In terms of basic policy tool dimensions ,environmental policy tools were the most (46.47%),followed by supplying policy tools (41.36%)and demanding policy tools (12.17%). The supply-based policy tools included scientific research and informatization support (25.67%),talent training (7.06%),infrastructure construction (5.6%),public health services (2.07%),and capital investment (used the least ,0.97%). The demand-based policy tools included international exchanges and cooperation (5.11%),organization coordination (3.77%),industry shaping (1.82%),pilot/ demonstration projects (1.46%),the latter two contents were used less. Among the environmental policy tools ,laws and regulations were used the most (24.94%),followed by target planning (9.73%),intellectual property (6.57%),financial support (2.43%),policy publicity (1.95%),quality evaluation (0.61%),and medical insurance support (0.24%). The dimensions of TCM inheritance and innovation system included scientific research and innovation (34.67%),management mechanism (19.59%), Chinese medicine industry (18.37%),service system (18.00%)and team building (9.37%),among which team building was related less. CONCLUSIONS :In order to better implement TCM inheritance and innovation policies in China ,it is recommended to optimize supply-based policy tools and highlight policy promotion functions ;increase demand-based policy tools to form an effective“pull force ”;adjust the structure of environmentalpolicy tools to improve the external environment for inheritance and innovation ;promote the full use of policy tools in the inheritance and innovation system to achieve planning goals.
9.Development of the "Third-Generation" Hybrid Rice in China.

Rice is a major cereal crop for China. The development of the "three-line" hybrid rice system based on cytoplasmic male sterility in the 1970s (first-generation) and the "two-line" hybrid rice system based on photoperiod- and thermo-sensitive genic male-sterile lines (second-generation) in the 1980s has contributed significantly to rice yield increase and food security in China. Here we describe the development and implementation of the "third-generation" hybrid rice breeding system that is based on a transgenic approach to propagate and utilize stable recessive nuclear male sterile lines, and as such, the male sterile line and hybrid rice produced using such a system is non-transgenic. Such a system should overcome the intrinsic problems of the "first-generation" and "second-generation" hybrid rice systems and hold great promise to further boost production of hybrid rice and other crops.

10.Experimental study on the effect of sleeve type guide growth rod on spinal growth of piglet

Objective To evaluate the efficacy of the sleeve type guide growth rod sliding automatically with the growth of piglet spine.Methods Fourteen 4-month-old piglets were randomly divided into growth rod group (n=8) and control group (n=6).Animals in growth rod group were implanted with the sleeve type guide growth rod system through the small incision of the tho racodorsal.Whereas ones in control group were just expose the bilateral lamina.On the day of surgery,6 and 12 weeks of postoperation,all piglets accepted the spinal X-ray examination.Measured the mean height of T8,T9 vertebral bodies,the average height of T7-8,T8-9,Tg-10 discs and the mean length of the rods through X-ray film.Spinal MRI was measured 3 months after the operation.The morphological changes of the plate were observed after HE staining.The morphological changes of the nucleus pulposus cells were observed and the thickness of the growth plate and the length of the growth bar were measured.Results Two piglets of growth rod group did not complete the experiment,1 piglet died of aspiration due to anesthesia resuscitation,and 1 piglet had infection after operation.The average height of T8,T9 vertebral bodies of the piglets in the growth rod group on the day after surgery and 3 months after operation was 18.26±3.26 mm and 27.15±4.18 mm,respectively,and the difference was statistically significant (F=16.868,P=0.002).Similarly,the mean height of T7-8,T8-9,Tg-10 discs of the piglets in growth rod group on the day after surgery and 3 months of postoperation was 1.79±0.24 mm and 2.14±0.23 mm,respectively,and the difference was statistically significant (F=6.455,P=0.029).The average height of T8,T9 vertebral bodies in the control group on the day after surgery and 3 months after operation was 17.86±3.50 mm and 28.63±4.45 mm,respectively,and the difference was statistically significant (F=21.654,P=0.001).Similarly,the mean height of T7-8,Ts-9,Tg-10 discs in control group at the day after surgery and 3 months of postoperation was 1.78±0.27 mm and 2.17±0.30 mm,respectively,the difference was statistically significant (F=5.928,P=0.035).There was no statistically significant difference in average height between T8 and T9 vertebral bodies in growth rod group and the control group at 3 months after operation (F=0.352,P=0.566).Similarly,There was no statistically significant difference in average height of T7-8、Ts-9、Tg-10 discs in growth rod group and the control group at 3 months after operation (F=0.053,P=0.823).The average sliding length of the growth rod in growth rod was 65.74 mm.Furthermore,growth plate chondrocytes and nucleus pulposus cells in EG and SG,all showed that the cells were arranged in order,the cell volume was large,the shape was round and the cytoplasm was rich.Simultaneously,there was no significant difference between the two groups in growth plate chondrocytes (P > 0.05).Conclusion The sleeve type guiding growing rod can automatically slip to extend due to the driving force of young pig's spine growth,without obvious effects on the normal growth of piglet's spine.
