1.Umbrella review analysis of the safety and efficacy of vonoprazan in the treatment of peptic ulcers and post-ESD ulcers

OBJECTIVE To analyze the safety and efficacy of vonoprazan (VPZ) in the treatment of peptic ulcer (PU) and post-endoscopic submucosal dissection (ESD) ulcers, providing evidence-based pharmaceutical evidence for clinical practice and medical decision-making. METHODS Retrieved from CNKI, Wanfang, VIP, CBM, PubMed, Embase, Web of Science, and the Cochrane Library, meta-analyses/systematic reviews related to VPZ in the treatment of PU and post-ESD ulcers were collected. Two researchers independently performed literature screening, data extraction, quality assessment of included studies, and evaluation of literature overlap. By employing the umbrella review analysis, a fresh meta-analysis was conducted on all relevant raw research data when a high degree of overlap was identified among the included studies. RESULTS A total of 17 meta-analyses were included, with quality ranging from high to very low; all outcome measures involved showed a very high level of overlap in the included meta-analyses (corrected covered area: 22.22%-100%). In the treatment of post-ESD ulcers, compared to proton pump inhibitor (PPI), VPZ significantly improved the ulcer healing rate at 4 weeks post-ESD [RR=1.27, 95%CI (1.03, 1.56), Z=2.21, P= 0.027] and the ulcer contraction rate post-ESD [MD=0.08, 95%CI (0.00, 0.16), Z=2.09, P=0.037], while significantly reducing the ulcer recurrence rate in patients with a history of PU [RR=0.49, 95%CI (0.32, 0.73), Z=3.49, P=0.001]; the delayed bleeding rate in the VPZ group was significantly lower than that in the lansoprazole subgroup [RR=0.47, 95%CI (0.25, 0.90), Z=2.28, P=0.02]. In the treatment of PU, the incidence of adverse events with VPZ was significantly higher than that with PPI in the duodenal ulcer subgroup [RR=1.13, 95%CI (1.02, 1.26), Z=2.38, P=0.017]. CONCLUSIONS For post-ESD ulcers, VPZ demonstrates superior therapeutic efficacy compared to PPI and can reduce ulcer recurrence rates in patients with a history of PU. However, it does not offer advantages in terms of safety for duodenal ulcer treatment.
2.Spatiotemporal clustering analysis and ARIMA-SVM model prediction of hand-foot-mouth disease in Zibo City in 2018- 2023

Objective To analyze the spatial and temporal distribution characteristics of hand-foot-mouth disease (HFMD) in Zibo City, to explore the key incidence areas, and to find a suitable prediction model, so as to provide reference for the prevention and control of HFMD. Methods The spatiotemporal clustering characteristics of HFMD in Zibo City from 2018 to 2023 were analyzed by using SaTScan 10.0.2 software and ArcGIS 10.7 software. A combination model of ARIMA and SVM was established, and the prediction results were verified and compared. Results Spatial clustering analysis showed that there was spatial clustering of the incidence of HFMD in various townships of Zibo City from 2020 to 2022. The high-high clustering areas and Getis-Ord hot spot areas were mainly concentrated in some main urban areas of Zhangdian District, Zichuan District, and Huantai County. A total of 2-5 aggregation areas were detected by spatiotemporal scanning analysis. The first-type aggregation areas were mainly concentrated in the towns of Zhangdian District, Huantai County, Linzi District, Zhoucun District and Gaoxin District. The aggregation months were July, August, September and November. The model prediction results showed that the ARIMA-SVM combined model was more accurate than the traditional ARIMA model. Conclusion There is a spatiotemporal clustering of hand-foot-mouth disease in Zibo City. The ARIMA-SVM combined model can be used to predict the incidence of hand-foot-mouth disease in Zibo City, and to strengthen health education and disease monitoring in high-risk areas and populations during the epidemic months.
3.Surveillance of bacterial resistance in Liuzhou Workers'Hospital,Guangxi from 2020 to 2022

Objective To understand the changing profiles of antimicrobial susceptibility of the bacterial strains isolated from patients at Liuzhou Workers'Hospital in Guangxi from 2020 to 2022.Methods The bacteria were isolated,identified,and underwent antimicrobial susceptibility testing using VITEK 2 Compact,disk diffusion method,or E-test.The results were interpreted according to the breakpoints recommended by CLSI M100 32nd Edition in 2022.The data were analyzed using WHONET 5.6 software.Results A total of 26 254 nonduplicate strains were collected from 2020 to 2022,including Gram-positive bacteria(27.9%)and gram-negative bacteria(72.1%).The prevalence of methicillin-resistant strains was 20.0%in SS.aureus(MRSA),and 72.2%in coagulase-negative Staphylococcus(MRCNS).Methicillin-resistant staphylococcal strains were more resistant to most antimicrobial agents than methicillin-susceptible strains(MSSA and MSCNS).None of the staphylococcal strains was resistant to vancomycin,linezolid or tigecycline.Enterococcus faecium strains showed higher resistance rates to most antimicrobial agents than Enterococcus faecalis.None of enterococcal strains was resistant to vancomycin.A few enterococcal strains were resistant to linezolid.Overall,691 strains of the non-meningitis Streptococcus pneumoniae were isolated from children and 123 strains were isolated from adults.The prevalence of penicillin-resistant SS.pneumoniae(PRSP)was 0.4%in the strains from children and 1.6%in the strains from adults.None of S.pneumoniae strains was intermediate to penicillin.The prevalence of carbapenem-resistant Klebsiella pneumoniae(CRKpn)was 1.2%,1.2%,and 13.8%in 2020,2021,and 2022,respectively.The prevalence of carbapenem-resistant P.aeruginosa(CRPae)and carbapenem-resistant Acinetobacter baumannii(CRAba)was 10.7%and 68.4%in 2020,17.5%and 75.2%in 2021,14.3%and 77.3%in 2022,respectively.About 84.6%of the 1 269 strains of Haemophilus influenzae were isolated from children and 15.4%isolated from adults.The prevalence of beta-lactamase-producing strains was 39.4%in the isolates from children and 46.8%in the isolates from adults.The β-lactamase-producing H.influenzae was resistant to ampicillin.Furthermore,some β-lactamase-nonproducing ampicillin-resistant(BLNAR)H.influenzae strains(27.0%)were also identified.Conclusions Antimicrobial resistance is still serious in this hospital,especially high prevalence of carbapenem-resistant organisms(CRO).Hospital infection prevention and control measures,antibiotic stewardship,and proactive CRO screening should be strengthened.More clinical specimens should be collected for suspected infections.Antimicrobial treatment should be prescribed empirically in time and adjusted when the results of antimicrobial susceptibility testing are available.
4.Preliminary analysis of factors influencing the severity of tinnitus with normal hearing

Objective To investigate the influencing factors related to the severity of tinnitus with normal hearing and to provide a clinical basis for the treatment of such patients.Methods From November 2019 to May 2020,150 normal hearing patients with tinnitus as their first chief complain in the outpatient clinic of our center were selected.The severity of tinnitus was assessed by the tinnitus handicap inventory(THI),and the quality of sleep and psychological condition were assessed by the Pittsburgh sleep quality index inventory(PSQI)and the anxie-ty and depression scale(HADS).The relationship between tinnitus severity and patients'gender,age,duration of illness,tinnitus side,tinnitus dominant sound frequency,tinnitus dominant sound loudness,sleep quality,anxiety and depression were analyzed using Pearson's method and Logistic multi-factor regression.Results The Pearson correlation analysis suggested that sleep quality(r=0.667,P<0.001),anxiety status(r=0.603,P<0.001)and depression status(r=0.593,P<0.001)were correlated with the THI classification,and patients with poorer sleep quality and higher anxiety and depression scores had more severe tinnitus.Logistic multi-factor regression analysis showed that only sleep quality had a significant effect on THI classification(P<0.001).Conclusion Sleep quality may be related to the severity of tinnitus patients with normal hearing,and it is important to focus on their sleep sta-tus in the clinical management of such patients.
5.Multi-center study on the difficulty and discrimination of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire

Objective:To test the difficulty, discrimination, and reliability of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire.Methods:Two researchers independently translated the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire and cross-checked it to form a Chinese version of the questionnaire. The Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire consists of 24 items, with correct answers scoring one point and incorrect answers scoring zero points, with a total score of 24 points. Convenience sampling was used to select ICU nurses from 14 GradeⅢ Class A hospitals in five provinces/autonomous regions and two municipalities in China for the survey between April and July 2023. The difficulty index, discrimination index, and Cronbach's α coefficient of the questionnaire were analyzed.Results:A total of 1 121 questionnaires were distributed, with 1 020 valid responses, yielding a valid response rate of 90.99%. The mean score of the 1 020 ICU nurses on the Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire was (16.10±5.58) , with a minimum score of 4.00 and a maximum score of 24.00. The Cronbach's α coefficient of the Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire was 0.91. The questionnaire's overall difficulty and discrimination indexes were 0.67 and 0.59, respectively.Conclusions:The Chinese version of the ICU Nurse Pressure Injury Prevention and Care Knowledge Questionnaire has appropriate difficulty, moderate discrimination, and strong reliability, making it a valuable tool for assessing ICU nurses' knowledge of pressure injury-related topics.
6.Analysis of the Chemical Compositions of Olibanum of Different Origins Based on UPLC-Q/TOF-MS Combined with Multi-variate Statistics Methods

OBJECTIVE To analyze the differential chemical composition of Olibanum of different origins and to provide some references for the identification of Olibanum origin and the quality standards.METHODS The chemical compositions of Olibanum were analyzed by UPLC-Q/TOF-MS.Multivariate statistical methods were used to analyze Olibanum and processed products to screen the differential compounds in Olibanum of different origins.RESULTS Using positive and negative ion scanning modes,a total of 108 compounds were identified in Olibanum.The compounds were mainly terpenes,among which triterpenes were the highest.And through multivariate statistical analysis,it was found that there were significant differences in the chemical compositions of Olibanum of different regions,with a total of 34 differential compounds;furthermore,specific compounds unique to Olibanum from other sources were identified.CONCLUSION Different regions of Olibanum could be distinguished based on their differential chemical composi-tions.This study could serve as a basis for identifying different origins of Olibanum,and also provide experimental data for subsequent research on the pharmacological substance basis of Olibanum.
7.Establishment and validation of a column chart risk prediction model for aspiration in early enteral nutrition therapy of ICU patients

Objective To investigate the risk factors of aspiration during early enteral nutrition(EEN)support treatment in patients in intensive care unit(ICU)and establish and validate the corresponding nomogram risk prediction model.Methods A total of 348 ICU patients who received EEN between June 2022 and May 2024 were enrolled and divided into the aspiration group(n=74)and the non-aspiration group(n=274)based on the occurrence of aspiration.Clinical data were collected included age,sex,body mass index(BMI),history of diabetes,endotracheal intubation/mechanical ventilation status,plasma albumin(ALB)levels within 24 h after admission to ICU,disease type(severe pneumonia/stroke/septic shock),consciousness level(Glasgow Coma Scale,GCS),APACHE Ⅱscore,nasogastric tube insertion depth,infusion volume,nutritional risk(NRS2002 score≥3 indicating high risk),and nutrition mode(nasogastric/nasointestinal tube).Logistic regression was used to identify risk factors of aspiration,and a nomogram prediction model was constructed using R software.External validation was performed on 72 EEN-treated ICU patients admitted between June 2024 and January 2025.Results Logistic regression identified age(OR=2.701,95%CI:1.633-4.467),APACHE Ⅱ score(OR=2.125,95%CI:1.133-3.987),consciousness level(OR=2.826,95%CI:1.617-4.940),nasogastric tube insertion depth(OR=1.101,95%CI:1.006-1.136)and nutritional risk(OR=8.996,95%CI:5.017-16.132)were independent risk factors for aspiration(all P<0.05).A nomogram incorporating these factors was developed,converting cumulative scores into individualized aspiration risk probabilities.The model demonstrated strong predictive performance in internal validation(AUC=0.860,calibration curve slope=0.930)and external validation(AUC=0.831).Decision curve analysis(DCA)confirmed significant clinical net benefits across risk thresholds,supporting its practical utility.Conclusion The nomogram model exhibits good discrimination and accuracy,providing a valuable tool for individualized aspiration risk assessment in ICU patients receiving EEN.
8.Exploring Multi-target Effect of Erzhiwan on Improving Myocardial Injury in Ovariectomized Mice Based on Non-targeted Metabolomics

ObjectiveTo explore the target of Erzhiwan in reducing myocardial injury in ovariectomized mice through non-targeted myocardial metabolomics combined with experimental verification. MethodsOvariectomized mouse model was selected, 40 female C57BL/6 mice were randomly divided into sham operation group, model group, estrogen group(estradiol valerate, 1.3×10-4 g·kg-1), Erzhiwan low and high dose groups(3.12, 9.36 g·kg-1), with 8 mice in each group. Each administration group was given the corresponding dose of Erzhiwan by gavage, and the sham operation group and model group were given equal volume of distilled water by gavage for 12 weeks. Echocardiography was used to detect cardiac function, hematoxylin-eosin(HE) staining was used to observe myocardial morphological changes, and enzyme-linked immunosorbent assay(ELISA) was used to detect the levels of estrogen, N-terminal pro-brain natriuretic peptide(NT-proBNP), hypersensitive troponin T(hs-TnT), total cholesterol(TC), triglyceride(TG), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C), interleukin(IL)-1β, IL-18 and tumor necrosis factor-α(TNF-α). The non-targeted metabolomics of mouse myocardium were analyzed by ultra performance liquid chromatography-quadrupole-electrostatic field orbital trap high-resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS), and the differential metabolites and corresponding metabolic pathways were obtained. The mRNA expression levels of phosphatidylinositol 3-kinase(PI3K) and protein kinase B(Akt) in mouse myocardial tissues were detected by real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), and the protein expression levels of PI3K, Akt, phosphorylated(p)-Akt were detected by Western blot. ResultsCompared with the sham operation group, the model group showed abnormal cardiac function, increased myocardial fiber space, cardiomyocyte atrophy, sarcoplasmic aggregation, and occasional dissolution or rupture of muscle fiber, the level of estrogen in the serum was decreased, the levels of NT-proBNP, hs-TnT, IL-1β, IL-18, TNF-α, TG, TC and LDL-C were increased, and the level of HDL-C was decreased(P<0.01). Compared with the model group, Erzhiwan could increase the level of estrogen, improve the abnormal cardiac function, reduce the pathological injury of myocardial tissue, decrease the levels of myocardial injury markers(NT-proBNP, hs-TnT) and inflammatory factors(IL-1β, IL-18, TNF-α), decrease the levels of TG, TC, LDL-C, and increased the level of HDL-C(P<0.01). The results of non-targeted myocardial metabolomics showed that 31 of the 162 differential metabolites between the model group and sham operation group were significantly adjusted after administration of Erzhiwan, which were mainly glycerol phospholipid metabolites. Pathway enrichment results showed that Erzhiwan mainly affected glycerophospholipid metabolic pathway, PI3K-Akt pathway, cyclic guanosine monophosphate(cGMP)-protein kinase G(PKG) pathway and other metabolic pathways. Compared with the sham operation group, the levels of phosphatidylcholine(PC, 11 types) and phosphatidylethanolamine(PE, 5 types) in mouse myocardial tissue of the model group were increased(P<0.05, P<0.01), and the mRNA and protein expressions of PI3K and p-Akt were decreased(P<0.05, P<0.01). Compared with the model group, the levels of PC(11 types) and PE(5 types) were decreased(P<0.05, P<0.01) in myocardial tissue of Erzhiwan group, the mRNA and protein expressions of PI3K and p-Akt were elevated(P<0.01). ConclusionErzhiwan can alleviate the pathological injury of myocardium in ovariectomized mice, improve the abnormal cardiac function, improve lipid metabolism disorder, and reduce the levels of myocardial injury markers and inflammatory factors, which involves a number of signaling and metabolic pathways in the heart, among which glycerophospholipid metabolism pathway and PI3K/Akt pathway may have key roles.
9.Effects of superoxide dismutase inhibition of AFP expression on the malignant biological behavior of PLC/PRF/5 liver cancer cells

OBJECTIVE To explore the effect of superoxide dismutase (SOD) administration on the malignant behavior of PLC/PRF/5 liver cancer cells, and analyze the correlation between SOD and alpha-fetoprotein (AFP) expression, to provide new ideas for targeting AFP with SOD as a drug for hepatocellular carcinoma. METHODS Normal human liver cells L-02, AFP- negative human liver cancer cells HLE, and AFP-positive human liver cancer cells PLC/PRF/5 were used as experimental cells. Western blot assay and SOD activity detection kit were used to detect the expression of AFP, SOD and activity of SOD in cells before and after changing AFP expression; the effects of different concentrations of SOD [0 (control), 0.188, 0.375, 0.75, 1.5, 3 U/mL] administration on the migration and proliferation of PLC/PRF/5 cells were detected using cell scratch assay and CCK-8 assay. The effects of SOD overexpression on the expression of malignant biological behavior-related proteins AFP and sarcoma virus protein (Src) in PLC/PRF/5 cells were detected using Western blot. RESULTS Compared with L-02 group and HLE group, the expression levels of SOD1 and SOD2, and SOD activity in PLC/PRF/5 cells were significantly reduced (P<0.05). After down-regulating AFP expression in PLC/PRF/ 5 cells, compared with PLC/PRF/5 group, the expression levels of SOD1 and SOD2, as well as SOD activity, were significantly increased in the PLC/PRF/5-shAFP group (low-expression) (P<0.05). After 48 hours of SOD treatment, compared with control group, the scratch healing rates of PLC/PRF/5 cells in the 0.375, 0.75, 1.5 and 3 U/mL SOD groups were significantly reduced (P<0.05); after 72 hours of SOD treatment, compared with control group, the scratch healing rates of PLC/PRF/5 cells in the 0.375, 0.75, and 1.5 U/mL SOD groups were significantly reduced (P<0.05 or P<0.01). Compared with control group, proliferation rates of PLC/PRF/5 cells were significantly reduced in the 0.375, 0.75, 1.5 and 3 U/mL SOD groups (P<0.05 or P<0.01). Compared with the PLC/PRF/5 group before up-regulating SOD1 and SOD2 expression, the expression levels of AFP and Src in the PLC/PRF/5-oeSOD1 and PLC/PRF/5-oeSOD2 groups (over-expression) after up-regulating SOD1 and SOD2 expression were significantly reduced (P<0.05). CONCLUSIONS A certain concentration of SOD can inhibit malignant behavior such as migration and proliferation of PLC/PRF/5 cells, and the expression level and activity of SOD are negatively correlated with AFP.
