1.Management of 99Tc m-MIBI-negative primary hyperparathyroidism complicated with papillary thyroid carcinoma

Primary hyperparathyroidism(PHPT) is a relatively common endocrine disorder in clinical practice. While the diagnosis and characterization of this condition are straightforward, cases with negative 99Tc m-methoxyisobutylisonitrile( 99Tc m-MIBI) imaging often complicates lesion localization, posing a clinical challenge in determining whether surgical intervention is warranted. This paper presents two cases of PHPT with negative 99Tc m-MIBI imaging, both diagnosed with papillary thyroid carcinoma(PTC). Following bilateral neck exploration, the surgeons successfully localized and excised the hyperparathyroid lesions and provided appropriate treatment for PTC. Postoperative follow-up indicated favorable recovery in both patients. This article aims to dive deeply into the necessity of surgical intervention and enhance clinical management of these cases.
2.Efficacy and safety of methylene blue,nano-carbon,and indocyanine green in sentinel lymph node identification for cervical cancer

Objective To investigate the guiding value and safety of three tracers in identifying sentinel lymph nodes(SLN)for lymph node dissection during radical surgery for early-stage cervical cancer.Methods A total of 114 patients with early-stage cervical cancer from January 2021 to No-vember 2022 were selected as the study subjects.They were randomly divided into methylene blue group,nano-carbon group,and indocyanine green(ICG)group according to the random number ta-ble method,with 38 cases in each group.All three groups underwent radical surgery.The methylene blue group used methylene blue for SLN identification during surgery,the nano-carbon group used nano-carbon,and the ICG group used ICG fluorescence imaging.The SLN identification status and surgery-related indicators were compared among the three groups.Trauma indicators[serum cortisol(Cor),epinephrine(E),adrenocorticotropic hormone(ACTH),and C-reactive protein(CRP)]were compared among the three groups before surgery and on 1,3,and 7 d after surgery.Tumor markers[carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),carbohydrate antigen 125(CA125),and carbohydrate antigen 199(CA199)]were compared among the three groups before surgery and 3 months after surgery.The complication rates and recurrence rates were also compared among the three groups.Results The tracing time in the ICG group was shorter than that in the nano-carbon and methylene blue groups,and the tracing time in the nano-carbon group was shorter than that in the methylene blue group(P<0.05).In terms of the success rate and number of identified SLN,the nano-carbon group was higher than the ICG and methylene blue groups,and the ICG group was higher than the methylene blue group(P<0.05).Regarding the SLN resection time,the nano-car-bon group had the shortest time,and the methylene blue group had the longest time,with a statisti-cally significant difference(P<0.05).The serum levels of CEA,AFP,CA125,and CA199 in the three groups decreased significantly 3 months after surgery compared with those before surgery(P<0.05).The within-group effects of one-way repeated-measures analysis of variance showed that the serum levels of Cor,E,ACTH,and CRP in the three groups increased first and then decreased on 1,3,and 7 d,with statistically significant differences(P<0.05).There was no statistically signif-icant difference in the complication rates among the three groups(P>0.05).During the 1-year fol-low-up,2 patients were lost during follow-up in the nano-carbon group,1 in the ICG group,and 2 in the methylene blue group.No recurrence was found in any of the three groups.Conclusion Among the three tracers,ICG has the shortest time for tracing SLN during radical surgery for early-stage cer-vical cancer.Nano-carbon exhibits relatively high application value compared with the other two tracers,and it guides the scope of lymph node dissection and demonstrates certain safety.
3.Teaching problems with postgraduate medical courses and countermeasures

Objective:To investigate the degree of satisfaction with courses of postgraduates in medicine in a military medical university, and to discuss the problems existing in the offering and teaching of postgraduate medical courses as well as targeted measures.Methods:A self-made questionnaire was administered through the Questionnaire Star platform to 533 postgraduate students in a military medical university to investigate the basic situation, the types of courses offered during their study period, their opinions about and degree of satisfaction with the curriculum system, the teaching effectiveness of various courses, and suggestions for improvement in education. SPSS 23.0 was used to perform the chi-square test to analyze the relationship between variables.Results:Among the 533 students, 391 (73.32%) were satisfied with course offering ( χ2=9.64, P=0.008), and 399 (74.95%) were satisfied with course teaching ( χ2=7.60, P=0.022). Twenty-four (4.47%), 3 (6.25%), 3 (10.81%), and 2 (6.45%) students in clinical medicine, public health and preventive medicine, nursing, and pharmacy, respectively, believed that course offerings did not quite fit with their majors. The numbers of students who believed that general compulsory courses, major-specific compulsory courses, and elective courses offered were appropriate in number were 309 (57.97%), 371 (69.61%), and 409 (76.74%), respectively. For the design of the curriculum system, 399 (74.86%), 367 (68.85%), 362 (67.92%), 361 (67.73%), 355 (66.61%), 352 (66.05%), and 357 (66.98%) were satisfied with "basic theory coverage", "major-specific knowledge coverage", "research interest cultivation", "innovative thinking cultivation", "academic reporting ability cultivation", "practical ability cultivation", and "academic writing ability cultivation", respectively. The numbers of master's and doctoral students who believed that there were appropriate numbers of courses on methodology and experimental skills were 285 (71.07%) and 90 (68.18%), respectively. Conclusions:In view of the current situation of postgraduate medical courses, general basic courses should be further optimized in terms of the number and proportion of different types of courses, and major-specific required courses should pay more attention to the cultivation of postgraduates' ability and quality in addition to the coverage of basic theories. Curriculum ideology and politics for medical postgraduates should be highlighted.

6.Exploration and practice of internal quality assurance system of graduate students in a military medical university based on training process management

This paper mainly discusses the practical significance of the internal quality assurance system of graduate students in a military medical university based on training process management. The integrated internal quality assurance system follows the direction of three core steps—course learning, scientific research practice training, and dissertation writing, and highlights nine key points—learning foundation in admission education and training, knowledge acquisition in course learning, application ability in teaching and scientific research practice, innovation ability in academic activities and innovation competitions, topic selection quality in thesis proposal, training quality in mid-term assessment, paper quality in dissertation writing, output quality in thesis defense, and comprehensive quality in diploma awarding. Our exploration and practical experience is shared in this article.
7.Applications of organ medicine and organ isolation protection therapy based upon normothermic machine perfusion in different research fields

Normothermic machine perfusion (NMP) is currently a research hotspot of organ preservation in the field of transplantation. Its safety and effectiveness of organ preservation have been clinically validated especially for preserving marginal donor organs. Various unique advantages of NMP are offered for in vitro organ function assessments, maintenance and organ injury repairing. Within the field of organ medicine, precise clinical interventions based upon in vivo or in vitro researches are implemented through an in vitro organ support system. With an aid of extracorporeal organ support system, an independent extracorporeal circulation system could be established for single or multiple organs more efficiently and safely. And physiological supports and other interventions for target organs are performed. Organ gene editing therapy, organ cell therapy, organ chemotherapy, organolytic virus therapy and organ physiological support therapy are all disciplines of organ quarantine therapy. This emerging theory provides a novel treatment for clinical organ diseases. This review summarized the use of NMP technology based upon the concepts and applications of organ medicine. Also the applications of organ isolation and protection therapy were discussed for organ transplantation and other research fields.
8.Path of constructing a "seven-in-one" medical postgraduate education quality assurance system in military medical universities

This paper analyzes the current situation of constructing postgraduate education quality assurance systems worldwide, and explores the specific path of constructing a "seven-in-one" medical postgraduate education quality assurance system. The system should be constructed according to the education practice in military medical universities and consider the following aspects: first-class students, first-class teaching, first-class tutors, first-class disciplines, first-class management, first-class culture, and first-class achievements. The system should be constructed by enrollment reform and innovation, teaching method reform, tutor team construction, discipline construction, cultural atmosphere creation, and emphasis on cultivating achievements.
9.Soybean GmGolS2-2 improves drought resistance of transgenic tobacco.

Galactinol synthase (GolS) genes play important roles in plant response to abiotic stress. In this research, the plant expression vector of soybean GmGolS2-2 gene was constructed and transformed into tobacco to study the drought tolerance of transgenic tobacco. A GmGolS2-2 gene with 975 bp coding sequence was cloned from soybean leaves by reverse transcription-polymerase chain reaction (RT-PCR). GmGolS2-2 was linked to the plant expression vector pRI101 by restriction enzyme sites Nde Ⅰ and EcoR Ⅰ, and transformed into tobacco by leaf disc method. Genomic DNA PCR and real-time PCR showed that three GmGolS2-2 transgenic tobacco plants were obtained. The growth status of GmGolS2-2 transgenic tobacco under drought stress was better than that of wild-type tobacco. After drought stress treatment, the electrolyte leakage and malondialdehyde content of transgenic tobacco were lower than those of wild-type tobacco, but the proline content and soluble sugar content were higher than those of wild-type tobacco. The results of real-time PCR showed that the heterologous expression of GmGolS2-2 increased the expression of stress-related genes NtERD10C and NtAQP1 in transgenic tobacco. The above results indicated that GmGolS2-2 improved drought resistance of transgenic tobacco.

Dizziness or vertigo is a common clinical symptom, and its underlying etiology is complex. Many clinicians are confused about its diagnosis and treatment. This article presents a case about chronic vestibular syndrome. And case appreciation and academic discussion are conducted by well-known domestic neurologists and otologists, so as to provide a good thinking model and basic ideas for the diagnosis and treatment of dizziness or vertigo, hoping to further improve the diagnosis and treatment level among clinicians.
