2.Construction of laboratory biosafety evaluation index system for emergency public health events in medical institutions from the perspective of integrating routine and emergency measures

Objective:To construct a biosafety evaluation index system for major emergency public health events in medical institutions.Methods:Based on previous laboratory biosafety evaluation work, relevant regulations and standards on biosafety in China were collected through literature research and expert consultations. Candidate indicators for constructing the biosafety evaluation system for major emergency public health events in medical institutions were selected, and a framework was established. Two rounds of expert questionnaires were conducted to determine the content of the index system based on experts′ evaluation, and each indicator′s relevance and importance were scored. Finally, two rounds of Delphi consultations were carried out, and the Analytic Hierarchy Process (AHP) was applied to calculate the weights of indicators.Results:The response rates for the total four rounds of questionnaire surveys were all 100%. The first two rounds focused on determining the framework, while the latter two focused on determining the weights for each indicator. The authority coefficients of the expert consultations for the two rounds of weights were 0.65 and 0.70, respectively, indicating the reliability of the research results. In the final round of survey, the Kendall′s coefficients of concordance at each level were all greater than 0.1. Through statistical testing, the P-values were all less than 0.05, indicating good coordination of expert opinions. Ultimately, we established an operational biosafety evaluation system for major emergency public health events in medical institutions, consisting of 4 primary indicators, 26 secondary indicators, and 119 tertiary indicators, with additional deduction items, bonus items, unacceptable items, and monitoring indicators.Conclusions:Based on scientific theory, a biosafety evaluation system for major emergency public health events in medical institutions was constructed, achieving the integration of routine and emergency measures. This system can be used for self-assessment of laboratory biosafety during emergency public health events, addressing the lack of unified standards in biosafety evaluation. Through regular self-assessment, it can enhance the level of biosafety management in medical institution laboratories, to realize the value of application and dissemination.
3.A study on the distribution patterns of traditional Chinese medicine syndrome types in antipsychotic-induced metabolic syndrome

Objective To explore the distribution patterns of traditional Chinese medicine(TCM)syndromes in patients with metabolic syndrome caused by antipsychotic drugs.Methods A standardized TCM syndrome survey was performed to collect diagnostic information from 160 patients diagnosed with metabolic syndrome due to antipsychotic drug use.Subsequent frequency analysis,cluster analysis,and Bayesian network analysis were carried out.The syndrome pattern distribution was ultimately determined through relevant literatures and expert opinions.Results Five TCM syndromes were identified through frequency,cluster,and Bayesian network analyses.The most common syndrome was qi deficiency with phlegm-dampness(30%),followed by spleen deficiency with phlegm-Heat(23.75%),qi and yin deficiency Pattern(21.88%),yin deficiency with damp-heat(17.50%),and stomach fire hyperactivity pattern(6.88%).Conclusion The pathogenesis of antipsychotics-induced metabolic syndrome involves a complex interplay of deficiency and excess factors.The primary disease is mainly located at the spleen and stomach,with involvement of the liver,kidney,and heart.Pathogenic factors include qi deficiency,yin deficiency,dampness,heat,pathogenic fire,and phlegm.
4.Potential profile analysis and influencing factors of social participation in colorectal cancer patients with stoma

Objective:To explore the potential category characteristics of social participation in colorectal cancer patients with stoma and analyze the influencing factors of different potential categories, so as to provide references for formulating and implementing targeted nursing measures.Methods:By using the convenience sampling method, colorectal cancer patients with stoma who were hospitalized or had outpatient follow-up visits at Guilin Hospital of Integrated Traditional Chinese and Western Medicine and the Second Affiliated Hospital of Guilin Medical University from January to December 2024 were selected as the survey subjects. A cross-sectional survey was conducted using the general information questionnaire, the voluntary participation Questionnaire, the Existential Anxiety Scale, the Family Support Scale and the Social Support Rating Scale. Potential profile analysis was used to identify the categories of social participation characteristics of colorectal cancer stoma patients, and multivariate Logistic regression was used to analyze the related influencing factors.Results:A total of 240 valid questionnaires were finally retrieved. Among the 240 colorectal cancer patients with stoma, 126 were male and 114 were female, aged 34-75 (55.63 ± 6.52) years. The social participation score of colorectal cancer patients with stoma was (55.84 ± 8.62) points. The results of potential profile analysis showed that the social participation of colorectal cancer patients with stoma could be divided into three potential categories: low participation-inefficient social life type accounted for 38.3% (92/240), medium participation-efficient indoor participation type accounted for 36.3% (87/240), and high participation-efficient outdoor participation type accounted for 25.4% (61/240). The results of multivariate Logistic regression analysis showed that, taking the high participation-efficient outdoor participation type as the reference, age, educational level, presence absence of stoma-related complications, existence of meaningless anxiety, family support, and social support were the influencing factors of low participation-inefficient social life type and medium participation-efficient indoor participation type ( OR values ranged from 0.163 to 1.879, all P<0.05). Conclusions:There are obvious classification characteristics in the social participation of colorectal cancer patients with stoma. Nursing staff can formulate and implement targeted intervention strategies based on different classification characteristics. Starting from the existence of meaningless anxiety in patients, an effective family and social support system should be established to improve their social participation.

8.PAI-1 genetic polymorphisms influence septic patients' outcomes by regulating neutrophil activity.

BACKGROUND:
Plasminogen activator inhibitor-1 (PAI-1) plays an important role in the pathophysiology of sepsis, but the exact mechanism remains debatable. In this study, we investigated the associations among the serum levels of PAI-1, the incidence of 4G/5G promoter PAI-1 gene polymorphisms, immunological indicators, and clinical outcomes in septic patients.
METHODS:
A total of 181 patients aged 18-80 years with sepsis between November 2016 and August 2018 in the intensive care unit in the Xinhua Hospital were recruited in this retrospective study, with 28-day mortality as the primary outcome. The initial serum level of PAI-1 and the presence of rs1799768 single nucleotide polymorphisms (SNPs) were examined. Univariate logistic regression and multivariate analyses were performed to determine the factors associated with different genotypes of PAI-1, serum level of PAI-1, and 28-day mortality.
RESULTS:
The logistic analysis suggested that a high serum level of PAI-1 was associated with the rs1799768 SNP of PAI-1 (4G/4G and 4G/5G) (Odds ratio [OR]: 2.49; 95% confidence interval [CI]: 1.09, 5.68). Furthermore, a high serum level of PAI-1 strongly influenced 28-day mortality (OR 3.36; 95% CI 1.51, 7.49). The expression and activation of neutrophils (OR 0.96; 95% CI 0.93, 0.99), as well as the changes in the expression patterns of cytokines and chemokine-associated neutrophils (OR: 1.00; 95% CI: 1.00, 1.00), were both regulated by the genotype of PAI-1.
CONCLUSIONS
Genetic polymorphisms of PAI-1 can influence the serum levels of PAI-1, which might contribute to mortality by affecting neutrophil activity. Thus, patients with severe sepsis might clinically benefit from enhanced neutrophil clearance and the resolution of inflammation via the regulation of PAI-1 expression and activity.

9.DeepCAPE: A Deep Convolutional Neural Network for the Accurate Prediction of Enhancers

The establishment of a landscape of enhancers across human cells is crucial to deciphering the mechanism of gene regulation, cell differentiation, and disease development. High-throughput experimental approaches, which contain successfully reported enhancers in typical cell lines, are still too costly and time-consuming to perform systematic identification of enhancers specific to different cell lines. Existing computational methods, capable of predicting regulatory elements purely relying on DNA sequences, lack the power of cell line-specific screening. Recent studies have suggested that chromatin accessibility of a DNA segment is closely related to its potential function in regulation, and thus may provide useful information in identifying regulatory elements. Motivated by the aforementioned understanding, we in-tegrate DNA sequences and chromatin accessibility data to accurately predict enhancers in a cell line-specific manner. We proposed DeepCAPE, a deep convolutional neural network to predict enhancers via the integration of DNA sequences and DNase-seq data. Benefitting from the well-designed feature extraction mechanism and skip connection strategy, our model not only consistently outperforms existing methods in the imbalanced classification of cell line-specific enhancers against background sequences, but also has the ability to self-adapt to different sizes of datasets. Besides, with the adoption of auto-encoder, our model is capable of making cross-cell line predictions. We further visualize kernels of the first convolutional layer and show the match of identified sequence signatures and known motifs. We finally demonstrate the potential ability of our model to explain functional implications of putative disease-associated genetic variants and discriminate disease-related enhancers. 10. The efficacy of gradeⅡ glioma with postoperative intensity modulated radiotherapy

Objective:
To retrospectively analyze the efficacy and prognostic factors of postoperative intensity modulated radiotherapy for grade Ⅱ gliomas.
Methods:
Retrospective analysis was conducted on patients with postoperative grade Ⅱ glioma in our hospital from Jan. 2010 to Dec. 2018. The primary endpoint was progression-free survival, and the secondary endpoint was overall survival. Correlative analyses of prognosis by age, gender, initial resection status, the maximum diameter of the lesions, bi-hemisphere, astrocytoma, chemoradiation, adjuvant chemotherapy were conducted.
Results:
A total of 109 cases with grade Ⅱ glioma were enrolled. The follow-up rate was 91.75%, including 10 cases dead and 27 relapsed. There were 24 cases (88.9%) of in-field failure, and 3 cases (11.1%) of out-field failure. 14 cases of recurrence occurred in 81 cases of total resection group, accounting for 17.3%, and 13 in 28 cases of subtotal resection group, accounting for 46.4%. The recurrence rate in the subtotal resection group was significantly higher than that in the total resection group (
