Viral myocarditis(VMC)is a type of myocarditis caused by Coxsackievirus B3(CVB3)infection.Its pathogenesis is complex that includes direct viral action, host immune and inflammatory response, myocardial structure and function remodeling. The therapeutic effect of single target drugs is limited, while traditional Chinese medicine not only has antiviral effects, but also demon-strates systematic advantages in intervening in VMC due to its"multi-component,multi-target,and multi-pathway"characteristics.This article reviews the pathogenic mechanisms of CVB3-induced VMC,clarifies the interaction between CVB3 and host cells, and ex-plores the intervention mechanisms and developments of traditional Chinese medicine interventions for CVB3-induced VMC.At the same time, the prospects of traditional Chinese medicine treatment for CVB3-induced VMC in this study suggest that future research should strengthen the analysis of multi-target synergistic mechanisms based on traditional Chinese medicine formulas, combine artifi-cial intelligence, genomics, and gene editing technologies to predict drug-target interaction networks, establish standardized and per-sonalized system for integrated traditional Chinese and Western medicine treatment plans, and provide more effective strategies for the prevention and treatment of VMC.

Objective:To determine the epidemiological characteristics of human bocavirus (HBoV) in children with acute respiratory infections (ARI) in Bengbu City, Anhui Province, in 2024.Methods:Nasopharyngeal swab samples were collected from 269 children with ARI in Bengbu City, Anhui Province, in 2024. Seventeen respiratory pathogens were screened using quantitative fluorescence PCR. For HBoV-positive samples, the VP1/VP2 structural gene fragments of HBoV were amplified and sequenced for genetic evolutionary analysis. Results:Among the 269 nasopharyngeal swab samples from children with ARI, the overall detection rate of respiratory pathogens was 48.33% (103/269). The top three pathogens with the highest detection rates were: Influenza A virus (FluA): 10.04% (27/269), Respiratory syncytial virus (RSV): 8.18% (22/269), Human bocavirus (HBoV): 7.43% (20/269). The age distribution of HBoV-infected children showed that the detection rate was highest in the 0-2 years age group (50%, 10/20), followed by the 3-5 years age group (25%, 5/20) and the over 6 years age group (25%, 5/20). However, there was no statistically significant difference in viral detection rates among the age groups. Genetic evolutionary analysis based on VP1/VP2 revealed that all 13 HBoV strains were of the HBoV-1 genotype. Conclusions:HBoV is one of the major pathogens causing ARI in children in Bengbu City, Anhui Province, in 2024, with HBoV-1 being the predominant genotype. Additionally, infants aged 0-2 years are the most susceptible population to HBoV infection.

Objective:To analyze the clinical characteristics and cytokines expression characteristics in infants with mild and severe respiratory syncytial virus (RSV) infection. Methods:From May 2023 to December 2023, plasma samples and clinical information were collected from 16 infants with RSV infection and 14 control infants. Cytek Aurora flow cytometry (Cytek, America) and Enzyme linked immunosorbent assay (ELISA) were used to detect the expression levels of 25 cytokines after mild and severe RSV infection. Results:Cough and nasal obstruction were the main clinical manifestations in infants with mild RSV infection, accompanied by polypnea, wheezing and other symptoms. The main symptoms of severe RSV infection were cough and rales, accompanied by fever and polypnea. In comparison with the control group, the expression levels of IL-2, IL-4, IL-5, IL-6, IL-9, IL-13, IL-22, TNF-α, IFN-α, IFN-β, MIP-1β, I-TAC, ENA-78, GROα, Eotaxin, and MCP-1 in the RSV infection group all exhibited an upregulation trend. Both IP-10 and MIP-3α demonstrated a downward trend in the RSV infection group; however, there was no statistically significant difference ( P>0.05). The levels of IL-10, IFN-γ, MIP-1α, and IL-8 in the RSV infection group were significantly higher than those in the control group, whereas the levels of MIG, TARC, and RANTES in the RSV infection group were significantly lower than those in the control group ( P<0.05). The levels of IL-2, IL-4, IL-5, IL-6, IL-9, IL-10, IL-13, IL-22, IFN-β, IFN-γ, TNF-α, IL-8, I-TAC, MIP-1β, Eotaxin, and MCP-1 in the mild RSV infection group were significantly higher than those in the severe RSV infection group ( P>0.05). Among these, the levels of MIG, RANTES, TARC, MIP-3α, and ENA-78 in the mild infection group were all lower than those in the severe infection group. The expressions of ENA-78 and MIP-1α in the severe infection group were significantly higher than those in the mild infection group and also higher than those in the control group. There was no significant difference in IP-10 and GROα between the mild and severe RSV infection groups ( P>0.05). Conclusions:The differences in clinical features and cytokines between infants with mild and severe RSV infection provide important data support for the prevention and treatment of RSV infection in infants.

Background and Aims:Iliac vein compression syndrome (IVCS) is a major cause of acute lower extremity deep vein thrombosis (DVT). Without timely treatment,it can lead to thrombus detachment, resulting in pulmonary embolism and increased mortality risk. Percutaneous mechanical thrombectomy, particularly using the AngioJet thrombectomy system,is a widely used thrombectomy method. The combination of AngioJet thrombectomy with iliac vein stent placement provides a novel treatment option for DVT associated with IVCS. This study was performed to evaluate the efficacy and safety of AngioJet thrombectomy combined with iliac vein stent placement in treating DVT with IVCS.Methods:The clinical data of 120 patients with DVT and IVCS treated between April 2019 and October 2022 were retrospectively analyzed. Sixty patients underwent AngioJet thrombectomy combined with iliac vein stent placement (observation group),while another 60 patients received catheter-directed thrombolysis (CDT) combined with stent placement (control group). The clinical efficacy, outcomes (thrombus clearance grade, thrombolysis duration, venous patency score, hospitalization duration, urokinase dosage, thigh circumference difference, and calf circumference difference),Villalta scores at various time points before and after treatment, and adverse events (chest tightness, mild pulmonary embolism, bleeding at the catheter insertion site, and mild renal function impairment) were compared between the two groups. Results:The overall efficacy rate in the observation group was significantly higher than that in the control group (98.33% vs. 86.67%,P=0.015). While there was no significant difference in thrombus clearance grade between the two groups (P>0.05),the observation group had significantly shorter thrombolysis duration, lower urokinase dosage, and shorter hospitalization time compared to the control group (all P<0.05). Before treatment, there were no significant differences in thigh circumference difference, calf circumference difference,or venous patency score between the two groups (all P>0.05). After treatment, the observation group had significantly better venous patency score and smaller thigh and calf circumference differences than the control group (all P<0.05). There was no significant difference in Villalta score between the two groups before treatment (P>0.05). Post-treatment Villalta scores showed a significant decreasing trend over time in both groups compared to pre-treatment scores (all P<0.05),but there were no statistically significant differences in Villalta scores between the two groups at 1,6,and 12 months after treatment (all P>0.05). The total incidence of adverse events was not significantly different between the two groups (10.00% vs. 3.33%,P=0.143).Conclusion:The use of AngioJet thrombectomy combined with iliac vein stent placement for the treatment of lower extremity DVT with IVCS can shorten thrombolysis and hospitalization duration, reduce thrombolytic drug dosage, and achieve better and safer outcomes, making it worthy of clinical application.

Objective To evaluate the predictive performance of C2 HEST score in the progression from paroxysmal to persistent atrial fibrillation(AF)in elderly patients. Methods A retrospective cohort trial was conducted on 515 patients>75 years old with paroxysmal AF admitted to the First and Second Centers of Chinese PLA General Hospital from January 2008 to December 2009.The occurrence of persistent AF(including permanent AF)or death was regarded as the end event.According to their C2 HSET score, the end population was divided into a progressive group(76 cases, progression to persistent AF)and a non-progressive group(439 cases, not progression to persistent AF).The risk factors for persistent AF were analyzed, and the survival rate was ana-lyzed using Kaplan-Meier survival curve.ROC curve was plotted to evaluate the performance of C2 HEST score in predicting the progression to persistent AF.Results Multivariate Cox regres-sion analysis showed that COPD(HR=2.018,95%CI:1.262-3.227,P=0.003),heart failure(HR=3.505,95%CI:1.5 9 8-7.688,P=0.002)and stroke or transient ischemic attack(HR=3.663,95%CI:2.293-5.852,P=0.000)were independent risk factors for progression to persis-tent AF.Kaplan-Meier survival curve analysis indicated that the survival rate was decreased with the increment of risk stratification(P<0.01).The median survival time of intermediate-risk patients was 124.81 months(95%CI:120.752-128.862)and that of high-risk patients was 97.04 months(95%CI:86.277-107.808).ROC curve analysis revealed that C2 HEST score(AUC value=0.649,95%CI:0.577-0.721,P<0.01)had good predictive value in the progression of paroxysmal AF to persistent AF.Conclusion In patients over 75 years of age,C2 HEST score has a good performance in evaluating the progression from paroxysmal to persistent AF.
