1.Analysis on the operation effect of China’s drug complaints and reporting system

OBJECTIVE To evaluate the operation effect of the drug complaints and reporting system in China, and put forward the suggestions for improving the drug supervision system and enhancing the national drug scientific supervision capacity. METHODS The statistical data of drug supervision from 2012 to 2023 and the complaint data of various provinces and 7 third- party platforms of drug online sales from National 12315 Consumer Complaint Information Disclosure Platform from 2023 to 2024 were collected. The operation effect of drug complaints and reporting system was analyzed from the dimensions of public participation, public satisfaction, case handling efficiency and quality, social effect and risk early warning ability. RESULTS From 2012 to 2019, the acceptance of drug complaints and reporting fluctuated greatly, and the number of drug complaints and reporting cases showed a downward trend as a whole, but the public satisfaction was high and the closure rate continued to increase. From 2023 to 2024, the top five provinces in terms of the number of drug complaints were Guangdong, Henan, Zhejiang, Shandong and Shanghai. Drug complaints in the online platform mainly involved Chinese and Western medicines, traditional Chinese medicines and Chinese herbal medicines sold on the network platforms of Pinduoduo, Jingdong and Taobao. The complaints focused on quality and after-sales service, and there were significant differences in the proportion of mediation agreements in drug complaint cases. From 2021 to 2024, the drug regulatory authorities investigated and dealt with 11 typical cases of online sales violations through complaint and reporting clues. CONCLUSIONS The drug complaints and reporting system in China runs smoothly. It is suggested to further use the data of National 12315 Consumer Complaint Information Disclosure Platform to strengthen the monitoring of key areas and network platforms, and urge enterprises to implement the main responsibility, so as to improve the quality and safety of drugs and ensure the safety of public medication.
2.A randomized controlled study on the effect of intermittent theta burst stimulation on craving,mood,and cognitive function in alcohol-dependent patients during the withdrawal period

Objective·To explore the effect of intermittent theta burst stimulation(iTBS)targeting the left dorsolateral prefrontal cortex(DLPFC)on reducing craving in alcohol-dependent patients during the withdrawal period,as well as its impact on patients'emotions and cognitive functions.Methods·A total of 41 inpatients with alcohol dependence in the withdrawal period were recruited from the Addiction Department of Mental Health Center,Shanghai Jiao Tong University School of Medicine,and randomly assigned to the experimental group(20 patients)and the control group(21 patients).Both groups received routine inpatient treatment for alcohol-dependence.The experimental group received real iTBS stimulation targeting the left DLPFC on the basis of routine inpatient treatment,while the control group received sham stimulation with the same parameters.The intervention course lasted for 2 weeks,with a total of 10 sessions.The Visual Analogue Scale(VAS),Beck Depression Inventory(BDI),and Beck Anxiety Inventory(BAI)were used to measure the craving,anxiety,and depression of the patients before and after the intervention.The behavioral tasks of the detection task(DET),identification task(IDN),two back task(TWOB),the Groton maze learning task(GML),and international shopping list task(ISL)in the CogState software package were used to assess the cognitive processing speed,attention/vigilance,working memory,spatial problem-solving/error monitoring ability,and verbal learning and memory of the patients before and after the intervention.Results·Repeated measures ANOVA showed that the time effect[F=126.713,P<0.001,partial η2(ηp2)=0.765]and interaction effect(F=7.080.P=0.011,ηp2=0.154)of the VAS scores in the two groups of patients were statistically significant.The time effect(F=9.114,P=0.004,ηp2=0.189),group effect(F=5.557,P=0.024,ηp2=0.125),and interaction effect(F=4.977,P=0.032,η2=0.113)of the TWOB score were all statistically significant.Only the time effects of BDI(F=45.273,P<0.001,ηp2=0.578),BAI(F=31.432,P<0.001,ηp2=0.473),GML(F=8.993,P=0.005,ηp2=0.209),and ISL(F=26.657,P<0.001,ηp2=0.439)scores were statistically significant.There were no statistically significant effects of time,group,or interaction on the DET and IDN scores.Simple effect analysis showed that the VAS score of the real stimulation group was lower than that of the sham stimulation group after the intervention(F=8.805,P=0.005,ηp2=0.184),and the TWOB score of the real stimulation group was higher than that of the sham stimulation group(F=11.293,P=0.002,ηp2=0.225).Conclusion·Combining iTBS with routine inpatient treatment can enhance the efficacy of reducing alcohol craving in alcohol-dependent patients during the withdrawal period,and improve their working memory.
3.Prokaryotic expression,purification,and in vivo and in vitro interaction studies of the potential virulence factors EsxA and EsxB of Mycobacterium haemophilum

This studyobtained the proteins of virulence factors EsxA and EsxB from Mycobacterium haemophilum and explored their interactions both in vitro and in vivo.The aim was to lay a foundation for further analysis of the functional mechanisms of EsxA and EsxB,and to further the development of drugs targeting Mycobacterium haemophilum.On the basis of bioinformatics analysis of the virulence factors EsxA and EsxB from Mycobacterium haemophilum,we performed molecular cloning,using Mycobacterium haemophi-lum as a template to construct recombinant plasmids EsxA-pGl01,EsxB-pGl01,and EsxB-pET-29b.The proteins of EsxA,EsxB,and their complex were expressed with a prokaryotic system,then purified through nickel-affinity chromatography and gel filtration chromatography.Intracellular colocalization was determined through fluorescence colocalization.Prokaryotic expression systems for EsxA,EsxB,and their complex were successfully constructed,and purified proteins were obtained.Fluorescence colocalization indi-cated that EsxA and EsxB interacted in vivo.In vivo fluorescence colocalization and in vitro complex formation suggested that EsxA and EsxB interacted both intracellularly and extracellularly.Our findings lay a foundation for studying the pathogenic mechanisms of the virulence factors EsxA and EsxB in Mycobacterium haemophilum,and performing structural analysis of their complex.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
7.Diagnostic value of echocardiography combined with three-dimensional speckle tracking imaging for cardiac amyloidosis and hypertrophic cardiomyopathy

Objective To investigate the diagnostic value of echocardiography combined with three-dimensional speckle tracking imaging(3D-STI)for cardiac amyloidosis(CA)and hypertrophic cardiomyopathy(HCM).Methods Thirty-six patients with CA(CA group)and 36 patients with HCM(HCM group)who were treated at Zhengzhou Central Hospital affiliated to Zhengzhou University from September 2019 to September 2023 were selected as the research subjects.In addition,36 healthy individuals who did health check-ups at Zhengzhou Central Hospital affiliated to Zhengzhou University were selected as a control group.All patients in the three groups were tested using Doppler echocardiography to measure echocardiographic parameters[left ventricular ejection fraction(LVEF),left atrial volume index(LAVI),early diastolic mitral flow velocity/early diastolic mitral annulus velocity(E/e'),maximum left ventricular wall thickness(MLVWT),interventricular septum thickness(IVS),left ventricular end-systolic diameter(LVSED),left ventricular end-diastolic diameter(LVEDD)]and 3D-STI parameters[left ventricular three-dimensional global circumferential strain(GCS),global radial strain(GRS),global longitudinal strain(GLS),global area strain(GAS),and standard deviation of systolic peak time-GLS(Ts-SD-GLS)].The diagnostic value of each parameter for CA and HCM was analyzed using a receiver operating characteristic(ROC)curve.Results Compared with the control group,LVEF decreased,while LAVI,E/e',MLVWT,IVS,LVSED,and the proportion of patients with enlarged left atrium increased in the CA group and HCM group(P<0.05).The proportions of patients with enlarged left atrium,enlarged atria,and pericardial effusion in the CA group were significantly higher than those in the control group,while the LVEDD was significantly lower than that in the control group(P<0.05).LVEF,LAVI,MLVWT,IVS,LVEDD,and the proportion of patients with enlarged left atrium in the HCM group were significantly higher than those in the CA group,while E/e',LVSED,and the proportions of patients with enlarged atria and pericardial effusion were significantly lower than those in the CA group(P<0.05).The ROC curve showed that the area under the curve(AUC)for LAVI,E/e',MLVWT,IVS,left atrial enlargement,double atrial enlargement,and pericardial effusion in diagnosing CA was 0.88,0.972,0.946,0.995,0.694,0.792,and 0.903,respectively;the AUC for LAVI,E/e',MLVWT,IVS,and LVEDD in diagnosing HCM was 0.925,0.984,0.944,0.971,and 0.553,respectively.The absolute values of GCS,GRS,GLS,and GAS in the CA group and HCM group were significantly lower than those in the control group,while the Ts-SD-GLS level was significantly higher than that in the control group(P<0.05).The Ts-SD-GLS level in the CA group was significantly higher than that in the HCM group(P<0.05).The ROC curve revealed that the AUC for GCS,GRS,GLS,GAS,and Ts-SD-GLS in diagnosing CA was 0.828,1.000,0.993,0.838,and 1.000,respectively;and the AUC for GCS,GRS,GLS,GAS,and Ts-SD-GLS in diagnosing HCM was 0.826,0.919,0.854,0.829,and 0.970,respectively.The AUC for combined echocardiography and 3D-STI in diagnosing HCM and CA was 0.981 and 0.996,respectively.Conclusion Echocardiography combined with 3D-STI can be used for differential diagnosis of HCM and CA,with high effectiveness.
8.Correlation between type 2 diabetic kidney disease and trimethyl-amine-N-oxide

AIM:To explore the correlation between trimethylamine-N-oxide(TMAO)and type 2 diabetic kidney disease(DKD),and to provide new ideas for the early clinical diagnosis of DKD.METHODS:A total 246 patients with type 2 diabetes mellitus(T2DM)ad-mitted to the Department of Endocrinology of the First Hospital of Lanzhou University from January 1,2020 to May 31,2020 were divided into diabetic kidney disease group(DKD group)and simple diabetes mellitus group(NDKD group).According to urinary albumin/creatinine ratio(UACR),the patients were divided into A1,A2 and A3 subgroups.According to the estimated glomerular filtration rate(eGFR),the patients were divided into G1,G2,G3 and G4-5 sub-groups.According to the Kidney Disease:Improving Global Outcomes(KDIGO)guidelines,the risk of progression of DKD was assessed(low,medium,high or very high risk).General clinical data and laboratory indicators were collected.TMAO level was measured by euzymelinked immunosorbent assay.SPSS 25.0 software was used for statistical analysis.RESULTS:In T2DM patients,TMAO level was positively correlated with UACR(r=0.515,P<0.01)and negatively correlated with eGFR(r=-0.409,P<0.01).TMAO is an indepen-dent risk factor for the onset and progression of DKD.In diagnostic model,the AUROC was 0.745 with op-timal cut-off value was 5.37μmol/L.CONCLUSION:TMAO is closely related to the occurrence and de-velopment of DKD,and it has certain clinical predictive value for DKD.Therefore,TMAO may become a po-tential target for the early diagnosis and treatment of DKD.
9.Molecular mechanism whereby autophagy inhibits acute lung injury induced by acute kidney injury

Objective This study aimed to explore the regulatory role of autophagy in acute kidney injury(AKI)-induced acute liver injury(ALI).Methods Forty-eight male Sprague-Dawley rats were randomly divided into four groups:sham operation group,IRI group,3-MA group and RA group.Except for the sham operation group,a rat model of AKI induced by IRI was established in all groups.The AKI model was established by removing the right kidney,separating the left renal artery,and clamping the left renal artery,followed by reper-fusion for 12,24,48,or 72 h.The 3-MA and RA groups were intraperitoneally injected with 3-MA(15 mg/kg,1 mL)or RA(2 mg/kg,1 mL)12 h before and after IRI treatment.The structure and function of the rat lung and kidney tissues were evaluated,and the expression levels of autophagy-related proteins,oxidative stress,and apoptosis were measured.Results Renal IRI led to ALI after AKI,and the levels of blood urea nitrogen,creatinine,tumor necrosis factor-α,and interleukin-1βwere all significantly increased.In addition,compared to the IRI group,the expression levels of P62 and caspase-3 significantly decreased in the RA group,whereas the expression levels of LC3-Ⅱ/LC3-Ⅰ,Beclin-1,Bcl-2,and ULK1 increased.Autophagy reduced pathological damage to kidney and lung tissues by inhibiting inflammation and oxidative stress and effectively ameliorated AKI-induced ALI.Conclusion Autophagy plays an important role in the regulation of ALI induced by AKI and can be used as a new target for AKI treatment and to reduce complication-related mortality.
10.Analysis of the etiological features and clinical characteristics of herpes zoster cases under 20 years old in Beijing City from 2017 to 2021

Objective:To analyze the etiological features and clinical characteristics of herpes zoster cases under 20 years old in Beijing City from 2017 to 2021.Methods:Herpes zoster cases were collected from a surveillance system in Beijing City from December 2017 to April 2021. The cases included individuals under 20 years old from seven sentinel hospitals located in two districts (Miyun District and Changping District). The basic information, the rash date of rash onset and the location and number of lesions were investigated at the first visit to the hospital, and the lesion swab samples were collected for laboratory testing. A telephone follow-up was conducted 21 days after the onset of the rash to investigate the degree of pain, duration of the rash and duration of pain. The individuals who still experienced neuralgia were further investigated for their pain condition at 90 days after the onset of the rash, to discover cases with postherpetic neuralgia. DNA was extracted from the rash fluid, and the ORF62 gene region was amplified and sequenced to obtain the viral sequence. The wild-type strain or chickenpox vaccine strain was identified by using sequence alignment, and the clinical characteristics of cases with different varicella vaccinations were compared.Results:A total of 78 herpes zoster cases under 20 years old were investigated during 2017-2021 in Beijing City, and 61 cases completed the follow-up survey. The age range of 61 cases was 1.83 to 20.54 years with a median age of 17.50 years. There were 36 males (59.02%) and 25 females (40.98%). Among them, there were 29 cases with the chickenpox vaccine immunization history (18 cases with one dose, 5 cases with two doses and 6 cases with unknown doses), 13 cases with no vaccination history and 19 cases with unknown vaccination history. Among the 78 cases, the herpetic fluid samples of 64 cases were positive for VZV, including 62 cases identified as wild-type strains and two cases as vaccine strains. The two vaccine strain cases were both 2-year-old girls who had received one dose of varicella vaccine and developed herpes zoster 3 months and 13 months after vaccination. Among the 29 cases with chickenpox vaccine immunization history, the majority had 10 to 49 lesions, accounting for 58.62% (17 cases). The trunk was the most common site of lesions, accounting for 44.83% (13 cases). About 51.72% (15 cases) reported "no or mild" pain intensity. The median ( Q1, Q3) scores for the worst pain, duration of pain and the time to crusting of lesions in the herpes zoster cases were 3 (1.5, 5) points, 10 (1.5, 12.5) days and 10 (6.5, 13) days, respectively. There was no statistically significant difference in the constituent ratio of the location of lesions, number of lesions and pain degree among the cases with vaccination history, without vaccination history and with unknown vaccination history ( P>0.05). There was also no statistically significant difference in the distribution of pain score, duration of lesions and duration of pain across the three groups ( P>0.05). Conclusion:Wild strains are the predominant pathogens in herpes zoster cases under 20 years old in Beijing City during 2017-2021. The varicella vaccination has no significant impact on the clinical manifestations of herpes zoster cases.
