1.Prognostic Significance of Pretreatment 18FALF-NOTA-FAPI-04 PET/CT in Patients With Recurrent Gastric Cancer Undergoing Combined PD-1 Inhibitor and Chemotherapy

Objective:
This study aimed to evaluate the prognostic value of [ 18F]ALF-NOTA-FAPI-04 PET/CT-derived parameters, including the maximum standardized uptake value (SUVmax), FAPI-avid tumor volume (FTV), and total lesion FAP expression (TLF), in patients with recurrent gastric cancer undergoing combined chemoimmunotherapy. We sought to establish a noninvasive imaging biomarker framework to optimize patient stratification and therapeutic decision-making.
Materials and Methods:
This retrospective cohort study analyzed 51 patients with recurrent gastric cancer who received programmed cell death protein 1 (PD-1) inhibitors combined with chemotherapy after gastrectomy. All patients underwent [ 18F]ALF-NOTA-FAPI-04 PET/CT within 14 days of chemoimmunotherapy. Semi-quantitative parameters (SUVmax, FTV, and TLF) were derived using semi-automated tumor segmentation. The primary endpoint of this study was the assessment of the clinical efficacy of chemoimmunotherapy, categorized as a durable or nondurable clinical benefit. The secondary endpoints included progression-free survival (PFS) and overall survival (OS). In a subgroup of 16 patients who underwent sequential [ 18F]FDG PET/CT within seven days of [ 18F]ALF-NOTA-FAPI-04 PET/CT, tracer uptake values were compared between the two PET/CT examinations.
Results:
51 patients were included. The median PFS and OS were 7 and 10 months, respectively. Durable clinical benefit (DCB) was observed in 30 patients and showed significantly lower SUVmax, FTV, and TLF values than non-DCB. TLF demonstrated the highest diagnostic accuracy for DCB (area under the receiver operating characteristic curve [AUC] = 0.80). Multivariable analysis identified TLF ≥ 188.88 SUVbw·cm 3 as an independent factor associated with PFS (hazard ratio [HR] = 7.29, P = 0.001) and FTV ≥ 44.17 cm 3 as an independent factor associated with OS (HR = 5.16, P = 0.010). In the subgroup analysis of 16 patients, semi-quantitative analysis demonstrated consistently higher values of [ 18F]ALF-NOTA-FAPI-04-derived parameters than [ 18F]FDG-derived parameters (all P < 0.001).
Conclusion
[ 18F]ALF-NOTA-FAPI-04 PET/CT may be a useful imaging tool for predicting clinical outcomes in patients with recurrent gastric cancer undergoing treatment with PD-1 inhibitors and chemotherapy.
2.Abnormal Gait Recognition of Patients with Stroke Based on Deep Learning Fusion

Objective To address the personalized differences in motion gait between stroke patients and healthy older adults,as well as the issue of abnormal gait recognition,a deep learning fusion-based approach is proposed to effectively improve the accuracy of abnormal gait recognition.Methods A model fusing convolutional neural networks(CNN)and bidirectional long short-term memory networks(BiLSTM)was adopted,with the introduction of a residual network(ResNet).Unilateral ankle joint movement data at different walking speeds within a comfortable range were collected from healthy older adults and stroke patients.Signals from inertial sensors and electromyography sensors were used as inputs,while gait features were analyzed and gait differences between the two groups were compared.The effectiveness of the model was validated by comparing the classification performance of traditional deep learning models and CNN-ResNet-BiLSTM models with different layer combinations in terms of abnormal gait recognition accuracy.Results The CNN-ResNet-BiLSTM model,which introduced residual connectivity,performed excellently in abnormal gait recognition.Compared with traditional deep learning models such as the gated recurrent unit(GRU)and long short-term memory network(LSTM),its prediction accuracy was improved by 13.6%and 8.36%,respectively.Additionally,compared with other model combinations,this model achieved an overall accuracy of 97.78%.Conclusions The algorithm proposed in this study can be applied to stroke-related abnormal gait detection,providing technique support for the early diagnosis and precise monitoring of such diseases.
3.A randomized controlled study on the effect of intermittent theta burst stimulation on craving,mood,and cognitive function in alcohol-dependent patients during the withdrawal period

Objective·To explore the effect of intermittent theta burst stimulation(iTBS)targeting the left dorsolateral prefrontal cortex(DLPFC)on reducing craving in alcohol-dependent patients during the withdrawal period,as well as its impact on patients'emotions and cognitive functions.Methods·A total of 41 inpatients with alcohol dependence in the withdrawal period were recruited from the Addiction Department of Mental Health Center,Shanghai Jiao Tong University School of Medicine,and randomly assigned to the experimental group(20 patients)and the control group(21 patients).Both groups received routine inpatient treatment for alcohol-dependence.The experimental group received real iTBS stimulation targeting the left DLPFC on the basis of routine inpatient treatment,while the control group received sham stimulation with the same parameters.The intervention course lasted for 2 weeks,with a total of 10 sessions.The Visual Analogue Scale(VAS),Beck Depression Inventory(BDI),and Beck Anxiety Inventory(BAI)were used to measure the craving,anxiety,and depression of the patients before and after the intervention.The behavioral tasks of the detection task(DET),identification task(IDN),two back task(TWOB),the Groton maze learning task(GML),and international shopping list task(ISL)in the CogState software package were used to assess the cognitive processing speed,attention/vigilance,working memory,spatial problem-solving/error monitoring ability,and verbal learning and memory of the patients before and after the intervention.Results·Repeated measures ANOVA showed that the time effect[F=126.713,P<0.001,partial η2(ηp2)=0.765]and interaction effect(F=7.080.P=0.011,ηp2=0.154)of the VAS scores in the two groups of patients were statistically significant.The time effect(F=9.114,P=0.004,ηp2=0.189),group effect(F=5.557,P=0.024,ηp2=0.125),and interaction effect(F=4.977,P=0.032,η2=0.113)of the TWOB score were all statistically significant.Only the time effects of BDI(F=45.273,P<0.001,ηp2=0.578),BAI(F=31.432,P<0.001,ηp2=0.473),GML(F=8.993,P=0.005,ηp2=0.209),and ISL(F=26.657,P<0.001,ηp2=0.439)scores were statistically significant.There were no statistically significant effects of time,group,or interaction on the DET and IDN scores.Simple effect analysis showed that the VAS score of the real stimulation group was lower than that of the sham stimulation group after the intervention(F=8.805,P=0.005,ηp2=0.184),and the TWOB score of the real stimulation group was higher than that of the sham stimulation group(F=11.293,P=0.002,ηp2=0.225).Conclusion·Combining iTBS with routine inpatient treatment can enhance the efficacy of reducing alcohol craving in alcohol-dependent patients during the withdrawal period,and improve their working memory.
4.Evaluation of the reliability and validity of the Chinese listening self-efficacy questionnaire

Objective To develop and translate the Chinese listening self-efficacy questionnaire(C-LSEQ)and to test its reliability and validity.Methods A total of 172 subjects aged≥60 years with age-related hearing loss completed the C-LSEQ questionnaire via direct interviews.Pure tone hearing threshold test,Mandarin hearing in noise test,and the hearing handicap inventory for elderly-screening(HHIE-S)were evaluated.Twenty subjects were randomly selected from the original group to complete the C-LSEQ questionnaire in 2 weeks after the initial evaluation.Results ① Reliability:The Cronbach's a coefficients of the three sub-dimensions and the overall ques-tionnaire were all>0.8,and the test-retest reliability coefficients of the three sub-dimensions and the overall ques-tionnaire were all>0.9(P<0.001).② Validity:The experts collectively evaluated the representativeness of the statements with good content validity.The convergent validity test showed that the composite reliability(CR)of the 2 sub-dimensions and the overall questionnaire were all>0.7,and the average variance extracted(AVE)were all>0.5.The CR value of the complex listening dimension was 0.655,and the AVE value was 0.937.The criterion validity test showed that the overall questionnaire and the three sub-dimensions of the C-LSEQ were significantly correlated with the pure-tone average,speech recognition thresholds in noise,and HHIE-S(P<0.001).Conclusion The C-LSEQ exhibits stable structure,with good reliability and validity.It can be used to assess listen-ing self-efficacy in adults with hearing loss,especially in the elderly with age-related hearing loss.
5.Pregabalin abuse leading to addiction: a case report

Pregabalin, a structural analog of γ-aminobutyric acid (GABA), has been widely prescribed since its approval in 2004 for treating various neuropathic pain conditions. In Western countries, it is also approved for managing anxiety disorders. However, concerns about its potential for abuse and dependence have led to its reclassification as a second-line treatment in recent years. Although pregabalin addiction has been reported in international studies, there have been no such reports within China. This article presents a case of addiction stemming from pregabalin abuse to manage anxiety symptoms. The patient experienced withdrawal symptoms after discontinuing the drug, which were successfully treated. The development of pregabalin addiction, withdrawal symptoms, and the OD(overdose) pattern of drug abuse in this case should draw the attention of clinicians in China.

8.Discussion on the Differentiation and Treatment of Migraine and Acupuncture Treatment from"Spleen and Stomach Disorders"

[Objective]To propose syndrome differentiation and treatment approach of acupuncture for migraine based on the relationship between"spleen and stomach disorders"and the etiology and pathogenesis of migraine,providing theoretical and practical references for the clinical management.[Methods]The connotation of"spleen and stomach disorders"and its association with migraine were systematically reviewed.Based on this perspective,treatment strategies and acupuncture prescriptions for migraine were formulated,and supported by a representative clinical case study.[Results]"Spleen and stomach disorders"serves as a critical pathological foundation for migraine.Its pathogenesis can be summarized into four patterns"impaired spleen transport leading to malnourishment of the brain""phlegm-dampness obstructing the orifices and impairing mental clarity""spleen-stomach disharmony causing Qi stagnation"and"spleen deficiency with blood stasis resulting in orifice obstruction".Acupuncture treatment should prioritize regulating spleen-stomach function,supplemented by dredging local meridians and harmonizing Qi and blood.Key acupoints include Zhongwan(CV12),Xiawan(CV10),Qihai(CV6),and Guanyuan(CV4)for reinforcing primordial Qi,consolidating the foundation,and harmonizing Qi and blood;Zusanli(ST36),Shangjuxu(ST37)and Xiajuxu(ST39)for regulating Qi movement and balancing Yin and Yang;Sizhukong(TE23)and Fengchi(GB20)for clearing the head,improving vision,and relieving pain.In the presented case,the patient was diagnosed as dual deficiency of Qi and blood,acupuncture therapy focusing on replenishing Qi,nourishing blood and unblocking collaterals achieved significant clinical efficacy.[Conclusion]"Spleen and stomach disorders"is a pivotal mechanism underlying recurrent migraine.Acupuncture treatment for migraine should emphasize restoring spleen-stomach function,prioritizing the reinforcement of primordial Qi and harmonization of Qi and blood,thereby addressing the root cause to reduce migraine recurrence.
9.The relationship between EZH2 methylation and thyroid cancer differentiation-related markers and its therapeutic value

Objective :
To investigate the role of enhancer of zeste homolog 2(EZH2)-trimethylated lysine 27 of histone H3(H3K27me3) axis in the dedifferentiation of thyroid cancer and its clinical value as a potential target for the treatment of anaplastic thyroid cancer(ATC).
Methods :
Immunohistochemical SP method was used to detect the expression of EZH2, H3K27me3, paired box gene 8(PAX8), thyroglobulin(TG) and thyroid transcription factor 1(TTF1) in ATC and papillary thyroid carcinoma(PTC) and their adjacent tissues. The relationship between EZH2 and thyroid differentiation markers(PAX8, TTF1, TG) was further analyzed by gene expression omnibus(GEO) database. ATC cell lines 8305C and BHT-101 were culturedin vitro. Real-time reverse transcription PCR(RT-qPCR) was used to detect the expression of thyroid differentiation markers(TTF1, PAX8) mRNA in ATC cell lines treated with EZH2 inhibitor(GSK126), and evaluate the potential therapeutic effect of GSK126in vitro. The effects of GSK126 and BRAF inhibitor vemurafenib on the proliferation of ATC cell lines were observed by cell proliferation assay.
Results :
The expression of EZH2 in ATC tissues was significantly higher than that in papillary thyroid carcinoma and adjacent tissues(P<0.05). The expression of H3K37me3 in ATC tissues was significantly lower than that in PTC tissues(P<0.05). EZH2 was negatively correlated with PAX8 and TG expression levels, but not with TTF1 expression level.In vitroexperiments, GSK126 could reverse the expression of thyroid differentiation markers PAX8 and TTF1 in ATC cell lines. GSK126 combined with BRAF inhibitor vemurafenib could significantly inhibit the growth of ATC cell lines.
Conclusion
The EZH2-H3K27me3 axis plays an important role in regulating thyroid specific markers, and the inhibition of EZH2 by small molecular compounds is a promising target for ATC treatment in the future.
10.Intensive preoperative functional training can improve the balance and functional recovery of persons undergoing total knee arthroplasty

Objective:To explore the effect of preoperative intensive functional training on the balance and functional recovery of patients with knee osteoarthritis (KOA) receiving total knee arthroplasty (TKA).Methods:Sixty KOA patients were randomly divided into an outpatient group, a home-based group and a control group, each of 20. Before their TKAs, both the outpatient and home-based groups underwent intensive functional training for 4 weeks, while the control group did nothing special. After the TKA, all received 4 weeks of standardized postoperative rehabilitation training. Before any training, after the 4 weeks of preoperative training and 4 weeks after the TKAs, all of the subjects performed the timed up and go test (TUGT), and their joint range of motion (ROM) was recorded. They also completed the 30-second chair stand strength test (30sCST), and the 6-minute walk exercise endurance test (6MWT). KOA osteoarthritis indices (WOMACs) were also recorded.Results:After the 4 weeks of preoperative training, significant differences were observed in the trajectory length, elliptical area and TUGT times of both the outpatient and home-based groups. Four weeks after the TKAs, significant differences were observed in all of the measurements in all three groups, but the results of the outpatient and home-based groups were significantly better than those of the control group, on average. After the 4 weeks of postoperative training, there were significant differences between the outpatient and home-based groups in terms of the average knee flexion angle, knee extension angle, 30sCST and 6MWT results. There were significant differences among the 3 groups in all of the measurements 4 weeks after the TKAs, with those of the two training groups showing significantly better results than the control group. The pain scores, stiffness scores, function scores and total WOMAC scores had improved significantly compared with the control group, but the average function and total WOMAC scores of the outpatient group (24.25±2.38) and (35.41±3.02) were then significantly superior to the home-based group′s averages.Conclusions:Intensive preoperative functional training conducted in an outpatient clinic or at home can significantly improve the balance, lower limb strength, exercise endurance and symptoms of KOA patients after TKA.
