1.Spinal cord stimulation for spinal cord injury from 1999 to 2025: a bibliometric analysis

ObjectiveTo analyze the research hotspots and development trends in the field of spinal cord stimulation (SCS) for spinal cord injury (SCI). MethodsLiterature about SCS for SCI was retrieve from the Web of Science (WOS) Core Collection database, with a time range from January, 1999 to July, 2025. VOSviewer 1.6.20 and CiteSpace 6.4.R2 were used to analyze the annual publication volume, countries, authors, institutions, journals and keywords. ResultsA total of 636 literatures were included. From 1999 to 2025, the overall publication trend in this field showed an upward trajectory, with recent years fluctuating but tending to stabilize. The country with the most publications was the United States (429 papers), followed by Russia (98 papers) and China (70 papers). The institution with the highest number of publications was the University of California, Los Angeles (76 papers), the author with the most publications was V. Reggie Edgerton (70 papers), and the journal with the most publications was Journal of Clinical Medicine (31 papers). The most frequently cited study focused on exploring the combination of epidural spinal cord stimulation with task-specific training to restore motor function in patients with complete SCI. Keyword analysis showed that the research hotspots in this field were mainly focused on neuroregulation mechanisms, recovery of motor and autonomic nervous dysfunction, artificial intelligence, closed-loop stimulation and brain-computer interface technology innovations. In recent years, the research focus gradually shifted from basic mechanisms to personalized and precise multifunctional rehabilitation strategies. ConclusionThe field of SCS for SCI has undergone phases of basic mechanism exploration and clinical application expansion. Current research hotspots and future trends focus primarily on the development of new stimulation paradigms and combined innovative technologies.
2.Effect of Modified Shibaotang on Serum Sex Hormone Levels in Patients with Male Late-onset Hypogonadism of Kidney Essence Deficiency Syndrome Complicated with Diabetes Mellitus

ObjectiveTo observe the effect of modified Shibaotang on serum sex hormone levels in patients with male late-onset hypogonadism of kidney essence deficiency syndrome complicated with diabetes mellitus. MethodsA total of 60 patients with male late-onset hypogonadism of kidney essence deficiency syndrome complicated with diabetes mellitus,who met the inclusion criteria and were admitted to Yinchuan Hospital of Traditional Chinese Medicine from October 2022 to October 2023,were selected and randomly divided into an observation group and a control group,with 30 patients in each group. Both groups continued their original treatments,including blood glucose lowering and blood lipid regulation. The observation group was treated with modified Shibaotang,while the control group was treated with testosterone undecanoate capsules. The treatment lasted for 12 weeks. The changes in traditional Chinese medicine (TCM)syndrome scores,partial androgen deficiency in aging males (PADAM)symptom scores,glucose metabolism indexes [fasting plasma glucose (FPG),2-hour postprandial glucose (2 h PG),glycosylated hemoglobin (HbA1c)],and serum sex hormone indexes [sex hormone-binding globulin (SHBG),free testosterone (FT),total testosterone (TT),prolactin (PRL),luteinizing hormone (LH),follicle stimulating hormone (FSH),estrogen (E2)] were compared between the two groups before and after treatment. Safety was also evaluated. Results(1)Clinical efficacy comparison:After treatment,the clinical efficacy in both groups was similar,and there was no statistically significant difference between the two groups. (2)TCM syndrome score and PADAM symptom score comparison:After treatment,both groups showed a significant reduction in TCM syndrome scores and PADAM symptom scores (P<0.01),and the observation group showed a significantly greater reduction compared to the control group (P<0.05). (3)Glucose metabolism indexes comparison:After treatment,the levels of FPG,2 h PG,and HbA1c were significantly reduced in both groups (P<0.01),and there was no statistically significant difference between the two groups regarding FPG,2 h PG,and HbA1c levels after treatment. (4)Serum sex hormone indexes comparison:After treatment,the levels of FT,TT,PRL,LH,and FSH were significantly increased in both groups (P<0.01),while E2 levels were significantly decreased (P<0.01). There was no statistically significant difference between the two groups in the levels of FT,TT,PRL,LH,FSH,and E2 after treatment. There was also no significant difference in SHBG levels within or between the groups before and after treatment. During the clinical observation,neither group exhibited any obvious adverse reactions. ConclusionModified Shibaotang can significantly improve the clinical symptoms of male late-onset hypogonadism of kidney essence deficiency syndrome complicated with diabetes mellitus,reduce blood glucose,and increase sex hormone levels. The mechanism may involve the inhibition of aromatase transformation in adipocytes,promotion of GnRH production,and regulation of the hypothalamic-pituitary-gonadal axis function.
3.Hierarchical brain structural alterations in patients with white matter hyperintensity-related cognitive impairment based on morphometric similarity gradient

Objective To investigate the change in morphometric similarity gradient in patients with white matter hyperintensity (WMH)-related cognitive impairment and its influence on cognitive function. Methods Baseline and neuroimaging data were collected from 141 healthy controls (HC group), 66 patients with WMH and normal cognition (WMH-NC group), and 134 patients with WMH and cognitive impairment (WMH-CI group). The difference in gradient was compared between groups based on the morphometric similarity gradient analysis. The mediating effect model was used to investigate the association between WMH volume,plasma biomarkers for nerve injury, gradient, and cognitive function. Results The research findings showed significant aberrant alternations in morphometric similarity gradient, with a significant increase in variability in the second gradient (P=0.036, not surviving FDR correction). There was a significant negative correlation between the second gradient and geodesic distance (Pspin<0.001). In addition,visual cortex gradient played a key mediating role in the association between p-tau181/WMH volume and visuospatial function. Conclusion Patients with WMH-related cognitive impairment have aberrant macroscopic connectivity patterns of the cortex. The aberrant pattern of regional cortex mediates the process in which p-tau181 and WMH volume affect cognitive function, which provides a new perspective for understanding the potential structural mechanism of WMH-related cognitive impairment.
4.The relationship between EZH2 methylation and thyroid cancer differentiation-related markers and its therapeutic value

Objective :
To investigate the role of enhancer of zeste homolog 2(EZH2)-trimethylated lysine 27 of histone H3(H3K27me3) axis in the dedifferentiation of thyroid cancer and its clinical value as a potential target for the treatment of anaplastic thyroid cancer(ATC).
Methods :
Immunohistochemical SP method was used to detect the expression of EZH2, H3K27me3, paired box gene 8(PAX8), thyroglobulin(TG) and thyroid transcription factor 1(TTF1) in ATC and papillary thyroid carcinoma(PTC) and their adjacent tissues. The relationship between EZH2 and thyroid differentiation markers(PAX8, TTF1, TG) was further analyzed by gene expression omnibus(GEO) database. ATC cell lines 8305C and BHT-101 were culturedin vitro. Real-time reverse transcription PCR(RT-qPCR) was used to detect the expression of thyroid differentiation markers(TTF1, PAX8) mRNA in ATC cell lines treated with EZH2 inhibitor(GSK126), and evaluate the potential therapeutic effect of GSK126in vitro. The effects of GSK126 and BRAF inhibitor vemurafenib on the proliferation of ATC cell lines were observed by cell proliferation assay.
Results :
The expression of EZH2 in ATC tissues was significantly higher than that in papillary thyroid carcinoma and adjacent tissues(P<0.05). The expression of H3K37me3 in ATC tissues was significantly lower than that in PTC tissues(P<0.05). EZH2 was negatively correlated with PAX8 and TG expression levels, but not with TTF1 expression level.In vitroexperiments, GSK126 could reverse the expression of thyroid differentiation markers PAX8 and TTF1 in ATC cell lines. GSK126 combined with BRAF inhibitor vemurafenib could significantly inhibit the growth of ATC cell lines.
Conclusion
The EZH2-H3K27me3 axis plays an important role in regulating thyroid specific markers, and the inhibition of EZH2 by small molecular compounds is a promising target for ATC treatment in the future.

Metanephric adenoma is a rare renal epithelial tumor with a low incidence and lack of specific clinical manifestations,resulting in a lack of uniformity in clinical understanding and treatment. Its etiology and pathogenesis are still unclear,and it may be related to the abnormal number and structure of chromosomes 2,7,and 17,as well as mutations in genes such as BRAF V600E,NF1,and NOTCH1,etc. There may be a transformed relationship between this tumor and Wilms’ tumor and papillary renal cell carcinoma. For diagnosis,it has diverse but non-specific clinical manifestations,and it is difficult to accurately differentiate it from other tumors in the imaging examination,and the confirmation of diagnosis relies on pathological and immunohistochemical staining. Treatment is mainly based on surgery to preserve the renal unit,such as partial nephrectomy,etc.,but the difficulty of preoperative diagnosis often leads to over-treatment,and there is a lack of standardized treatment protocols for metastatic posterior renal adenoma. The aim of this article is to provide a reference for the in-depth understanding of posterior renal adenomas and to optimize the clinical diagnosis and treatment strategies.

Hand degloving injury represents one of the most severe forms of hand trauma, characterised by challenging treatment and a complex prognostic outcome. It is crucial to effectively utilise the degloved tissues in emergency or primary repair of a hand degloving injury. This consensus provides a comprehensive review of the existing literature on definition, classification, emergency assessment, debridement, judgment of skin viability, in situ repair of the degloved skin, and adjunctive treatment for degloving injury of hand. Based on conclusion of both domestic and international experiences, this expert consensus on the classification of hand degloving injury and the emergency repair with the avulsed skin is established, aiming to provide a guidance to surgeons on standardised treatment strategy and improve the management of hand degloving injury.
7.Effect of Qingfei-Jiedu-Huatan Formula on severe pneumonia in rats via mTOR-regulated alveolar macrophage autophagy

AIM:This study aims to investigate the mechanism by which Qingfei-Jiedu-Huatan Formula(QJHF)regulates autophagy in alveolar macrophages through mTOR in the treatment of severe pneumonia(SP)in rats.METHODS:Sixty SPF-grade male rats were randomly assigned to six groups:control,model,QJHF,moxifloxacin(MOX),rapamycin(RAPA),and QJHF+RAPA,with ten rats in each group.An SP rat model was established using Klebsiella pneumoniae.After seven days of treatment,changes in IL-33 and IFN-γ levels in bronchoalveolar lavage fluid(BALF)were measured using ELISA.Histopathological alterations in lung tissue were assessed via HE staining,and the autophagy of alveolar macrophages was detected using immunofluorescence co-localization methods.The expression levels of mTOR,beclin-1,and LC3 mRNA in lung tissue were analyzed using qPCR,while Western blot was employed to assess the protein levels of p-mTOR/mTOR,beclin-1,and LC3-II/LC3-I.RESULTS:Compared to the control group,the model group exhibited a deteriorated condition,characterized by alveolar wall rupture and thickening,significant inflammatory cell infiltration in the alveolar cavity,and extensive lung tissue damage(P<0.01).Elevated levels of IL-33 and IFN-γ in BALF were also observed(P<0.01),along with increased colocalization of CD68 and LC3 in immunofluorescence analy-sis.The mTOR mRNA expression in lung tissue decreased(P<0.01),while LC3 and beclin-1 mRNA expressions in-creased(P<0.01).Additionally,the protein expression ratio of p-mTOR/mTOR decreased(P<0.01),whereas LC3-II/LC3-I and beclin-1 protein levels increased(P<0.01).In comparison to the model group,significant improvements were noted after treatment with QJHF and MOX(P<0.01),while RAPA treatment led to a worsening of these indicators(P<0.05).A slight improvement was observed with the QJHF combined with RAPA intervention,though this was not statisti-cally significant.No significant differences were found between the MOX and QJHF groups.However,the QJHF+RAPA group displayed notable improvements in various indicators compared to the RAPA group(P<0.05).CONCLUSION:The QJHF can mitigate the inflammatory response associated with severe pneumonia,potentially by activating mTOR phos-phorylation activity,which in turn inhibits excessive autophagy in alveolar macrophages.
8.Exploration of Professor YAO Xinmiao's Experience in Treating Restless Legs Syndrome with Acupotomy from Four Sinews under the Theoretical Guidance of"Bones Can Stand Upright with Sinews"

[Objective]To summarize the clinical experience of Professor YAO Xinmiao's experience in treating restless legs syndrome(RLS)with acupotomy from four sinews separately under the theoretical guidance of"bones can stand upright with sinews",with a view to illustrate the theoretical basis and clinical application of treating RLS with acupotomy from four sinews.[Methods]Through the clinical study with Professor YAO,combined with the review of Chinese medical ancient Books and modern medical research literature on RLS,the overview of RLS,the theoretical overview of"bone can stand upright with sinews",and Professor YAO's opinions on the theoretical basis and clinical practice of treating RLS with acupotomy from four sinews separately are elaborated,and Professor YAO's idea of identification and treatment of RLS are analyzed.[Results]RLS is a central nervous system sensory dysfunction disease,and traditional treatment has problems such as significant side effects,drug tolerance and symptom rebound.The theory of"bones can stand upright with sinews"emphasizes the dominant position of tendons in the concept of balance between sinews and bones.Professor YAO believes that the onset of RLS is caused by the disruption of the balance between sinews and bones.Sinew of foot-Taiyang,sinew of foot-Shaoyang,sinew of foot-Jueyin,sinew of foot-Shaoyin are closely related to RLS.Doctors need to flexibly use acupuncture techniques such as perpendicular needling,Hegu needling and superficial needling to treat RLS based on characteristics of the four sinews separately.Meanwhile,doctors need to base themselves on the concept of muscle bone balance,focus on the training of core stability,reduce recurrence and improve long-term efficacy.[Conclusion]Professor YAO's emphasis on the concept of balance between sinews and bones based on the concept of"bone can stand upright with sinews"not only has important guiding significance in the treatment of musculoskeletal disorders with acupotomy,but also has high application value in the treatment of neuromuscular disorders with acupotomy,which is worth learning and promoting.
9.Developing a training curriculum for implementing the national initiative for promoting dementia care and prevention using the Delphi method

Objective:To develop a comprehensive training curriculum to enhance the effective implementation of the national initiative promoting dementia care and prevention.Methods:The Delphi method was utilized in an expert consultation that included 44 participants.The initial draft of the training curriculum was developed based on the current state of dementia care and prevention.This draft was subsequently evaluated for its importance, feasibility, and ease of dissemination.Experts offered targeted modifications and additional recommendations.Results:The recovery rate of the expert consultation questionnaire was 95.5%, with a recovery validity rate of 90.9%.The expert authority coefficient was 0.91, and the Kendall's coordination coefficient( W)for expert scoring was 0.316, with a significance level of P<0.001.Four course modules were ultimately identified: the foundation of memory clinic work, the complete management practice skills, group counseling techniques for caregivers, and practical skills for caregivers.The importance of these modules was rated with a mean of 4.92 to 4.95, and the coefficient of variation ranged from 0.044 to 0.063.Each module had a mean value of 4.92 to 4.95, with a coefficient of variation of 0.044 to 0.063; the mean value for practicality was between 4.78 and 4.92, with a coefficient of variation of 0.055 to 0.098; and the mean value for ease of generalization ranged from 4.28 to 4.65, with a coefficient of variation from 0.140 to 0.203.The four modules comprised a total of 55 specific course content items, with the mean value for each item ranging from 4.76 to 5.00 and a coefficient of variation from 0.000 to 0.121.The mean value of usefulness assigned to each entry ranged from 4.55 to 4.98, with a coefficient of variation from 0.031 to 0.150.Additionally, the mean value for ease of propagation assigned to each entry ranged from 4.00 to 4.83, with a coefficient of variation from 0.091 to 0.245. Conclusions:The developed training curriculum, which comprises four course modules and 55 items, demonstrated consistently high levels of importance, practicality, and ease of dissemination.These findings indicate that the curriculum is well-aligned with national initiatives aimed at enhancing dementia care and prevention.
10.Therapeutic effect of baicalin on chronic migraine rats by regulating MIP-1α/CCR1 signaling pathway

Objective:To explore the therapeutic effect and mechanism of Baicalin(BG)on chronic migraine(CM)rats.Methods:CM rat model was established by intermittent subcutaneous injection of nitroglycerin.Rats were randomly grouped into con-trol group,chronic migraine group(CM group),low-dose and high-dose Baicalin groups(BG-L group,BG-H group),Baicalin+MIP-1α recombinant protein group(BG+MIP-1α group)and CCR1 inhibitor ZK811752 group(CCR1-ZK group),with 10 rats in each group.General behavior of rats were observed and scored;von Frey fiber optic pain gauge was applied to measure the mechanical pain threshold of rats;Nissl staining was applied to observe the neuronal structure of ventrolateral periaqueductal gray(vlPAG);ELISA was applied to detect levels of TNF-α and IL-1β;immunohistochemical method was applied to detect expressions of pain related pro-teins c-Fos and CGRP;Western blot was applied to detect expressions of MIP-1α and CCR1 proteins.Results:Compared with Control group,the body mass,mechanical pain threshold,and the number of Nissl bodies in vlPAG area of rats in CM group were obviously reduced,the general behavioral score,levels of TNF-α,IL-1β,and expressions of c-Fos,CGRP,MIP-1α and CCR1 were obviously increased(P<0.05);compared with CM group,the body mass,mechanical pain threshold,the number of Nissl bodies in vlPAG area of rats in BG-L group,BG-H group and CCR1-ZK group were obviously increased,the general behavioral score,levels of TNF-α,IL-1β,and expressions of c-Fos,CGRP,MIP-1α and CCR1 were obviously reduced(P<0.05);compared with BG-H group,the body mass,mechanical pain threshold,and the number of Nissl bodies in vlPAG area of rats in BG+MIP-1α group were obviously reduced,general behavioral score,levels of TNF-α,IL-1β,and expressions of c-Fos,CGRP,MIP-1α and CCR1 were obviously increased(P<0.05).Conclusion:Baicalin has certain therapeutic effect on chronic migraine rats,and its mechanism may be related to inhibiting the activation of MIP-1α/CCR1 signaling pathway.
