1.Research progress on PD-1/PD-L1 inhibitors in neoadjuvant therapy for esophageal cancer

Esophageal cancer is one of the malignant tumors that poses a threat to human health, with both high incidence and malignancy. Currently, surgery following neoadjuvant chemoradiotherapy is the standard treatment for locally advanced esophageal cancer; however, the long-term prognosis remains unsatisfactory. In recent years, inhibitors of programmed death protein-1 (PD-1) and its ligand (programmed death ligand-1, PD-L1) have achieved breakthrough progress in other solid tumors, and research on esophageal cancer is gradually being conducted. With the demonstration of good efficacy of PD-1/PD-L1 inhibitors in the first-line and second-line treatment of advanced unresectable esophageal cancer, their incorporation into neoadjuvant treatment regimens has become a hot topic. Therefore, this article reviews the mechanism of action of PD-1/PD-L1 inhibitors and their application in the neoadjuvant treatment of esophageal cancer.
2.Significance of precise classification of sacral meningeal cysts by multiple dimensions radiographic reconstruction MRI in guiding operative strategy and rehabilitation.

OBJECTIVE:
To precise classify sacral meningeal cysts, effective guide minimally invasive neurosurgery and postoperative personalized rehabilitation by multiple dimensions radiographic reconstruction MRI.
METHODS:
From March to December 2021, based on the original 3D-fast imaging employing steadystate acquisition (FIESTA) scanning sequence, 92 patients with sacral meningeal cysts were pre-operatively evaluated by multiple dimensional reconstruction MRI. The shape of nerve root and the leakage of cyst were reconstructed according to the direction of nerve root or leakage track showed on original MRI scans. Sacral canal cysts were accurately classified as including nerve root and without nerve root, so as to accurately design the incision of skin and formulate corresponding open range of the posterior wall of the sacral canal. Under the microscope intraoperation, the shape of the nerve roots inside cysts or leakage track of the cysts without nerve roots were verified and explored. After the reinforcement and shaping operation, several reexaminations of multiple dimensional reconstruction MRI were performed to understand the deformation of the nerve root and hydrops in the operation cavity, so as to formulate a persona-lized rehabilitation plan for the patients.
RESULTS:
Among the 92 patients with sacral mengingeal cyst, 58 (63.0%) cysts with nerve root cyst, 29 (31.5%) cysts without nerve root cyst, and 5 (5.4%) cysts with mixed sacral canal cyst. In 58 patients with nerve root cysts, the accuracy of preoperative clinical classification on MRI image reached 96.6% (56/58) through confirmation by operating microscope. Only 2 cases of large single cyst with nerve root on the head of cyst were mistaken for without nerve root type. In 29 patients with sacral cyst without nerve root, the accuracy of preoperative image reached 100% through confirmation by operating microscope. The accuracy of judging the internal nerve root and leakage of 12 cases with recurrent sacral cyst was also 100%. Two cases of delayed postoperative hydrops were found one month after operation. After rehabilitation treatment by moxibustion and bathing, the hydrops disappeared 4-6 months after operation.
CONCLUSION
Multiple dimensional reconstruction MRI can precisely make clinical classification of sacral meningeal cysts before operation, guide minimally invasive neurosurgery effectively, and improve the rehabilitation effect.

3.Single-cell RNA sequencing reveals Shen-Bai-Jie-Du decoction retards colorectal tumorigenesis by regulating the TMEM131-TNF signaling pathway-mediated differentiation of immunosuppressive dendritic cells.

Colorectal tumorigenesis generally progresses from adenoma to adenocarcinoma, accompanied by dynamic changes in the tumor microenvironment (TME). A randomized controlled trial has confirmed the efficacy and safety of Shen-Bai-Jie-Du decoction (SBJDD) in preventing colorectal tumorigenesis. However, the mechanism remains unclear. In this study, we employed single-cell RNA sequencing (scRNA-seq) to investigate the dynamic evolution of the TME and validated cell infiltration with multiplex immunohistochemistry and flow cytometry. Bulk RNA sequencing was utilized to assess the underlying mechanisms. Our results constructed the mutually verifiable single-cell transcriptomic atlases in Apc Min/+ mice and clinical patients. There was a marked accumulation of CCL22+ dendritic cells (DCs) and an enhanced immunosuppressive action, which SBJDD and berberine reversed. Combined treatment with cholesterol and lipopolysaccharide induced characteristic gene expression of CCL22+ DCs, which may represent "exhausted DCs". Intraperitoneal injection of these DCs after SBJDD treatment eliminated its therapeutic effects. TMEM131 derived CCL22+ DCs generation by TNF signaling pathway and may be a potential target of berberine in retarding colorectal tumorigenesis. These findings emphasize the role of exhausted DCs and the regulatory mechanisms of SBJDD and berberine in colorectal cancer (CRC), suggesting that the multi-component properties of SBJDD may help restore TME homeostasis and offer novel cancer therapy.
4.IMM-H007 promotes hepatic cholesterol and triglyceride metabolism by activating AMPKα to attenuate hypercholesterolemia.

Hypercholesterolemia is a significant risk factor for the development of atherosclerosis. 2',3',5'-Tri-O-acetyl-N 6-(3-hydroxyphenyl) adenosine (IMM-H007), a novel AMPK agonist, has shown protective effects in metabolic diseases. However, its impact on cholesterol and triglyceride metabolism in hypercholesterolemia remains unclear. In this study, we aimed to elucidate the effects and specific mechanisms by which IMM-H007 regulates cholesterol and triglyceride metabolism. To achieve this goal, we used Apoe -/- and Ldlr -/- mice to establish a hypercholesterolemia/atherosclerosis model. Additionally, hepatocyte-specific Ampka1/2 knockout mice were subjected to a 5-week high-cholesterol diet to establish hypercholesterolemia, while atherosclerosis was induced via AAV-PCSK9 injection combined with a 16-week high-cholesterol diet. Our results demonstrated that IMM-H007 improved cholesterol and triglyceride metabolism in mice with hypercholesterolemia. Mechanistically, IMM-H007 modulated the AMPKα1/2-LDLR signaling pathway, increasing cholesterol uptake in the liver. Furthermore, IMM-H007 activated the AMPKα1-FXR pathway, promoting the conversion of hepatic cholesterol to bile acids. Additionally, IMM-H007 prevented hepatic steatosis by activating the AMPKα1/2-ATGL pathway. In conclusion, our study suggests that IMM-H007 is a promising therapeutic agent for improving hypercholesterolemia and atherosclerosis through the activation of AMPKα.
5.Preparation of tubeimoside Ⅲ nanoemulsion and evaluation of its adjuvant effect

Objective To prepare tubeimoside Ⅲ nanoemulsion(TBMⅢ-NE)and evaluate its adjuvant effect in vaccines.Methods TBMⅢ-NE was prepared using low-energy emulsification.Dynamic light scattering was used to characterize the particle size and polydispersity index of the obtained TBMⅢ-NE,and transmission electron microscopy(TEM)was employed to observe the morphology.CCK-8 assay was utilized to determine the cytotoxicity of TBMⅢ-NE on bone marrow-derived dendritic cells(BMDCs).The in vitro safety of TBMⅢ-NE was evaluated using a hemolysis assay.The ability of TBMⅢ-NE to promote the phagocytosis of antigens by DC2.4 cells was observed using confocal laser microscopy.After co-incubation of TBMⅢ-NE with BMDCs,the expression levels of CD40,CD86,MHC-Ⅰ,and CCR7 on the surface of BMDCs were detected using flow cytometry,and the levels of cytokines in the supernatant of BMDCs were measured using enzyme-linked immunosorbent assay(ELISA).After female BALB/c mice were immunized with the SARS-CoV-2 antigen RBD in combination with TBMⅢ-NE,ELISA was conducted to determine the serum levels of specific IgG,IgG2a,and IgG1 antibodies.The number of specific IFN-γ-secreting cells in mouse splenocytes was detected using enzyme-linked immunospot(ELISpot)assay.Results The prepared blank nanoemulsion(BNE)and TBMⅢ-NE were in a particle size of 25.46 and 25.89 nm,and a polydispersity index of 0.214 and 0.125,respectively.TEM displayed that TBMⅢ-NE was in uniform sphere and well dispersed.When the TBMⅢ-NE adjuvant was diluted by 400-fold,the survival rate of BMDCs was approximately 86%.Compared with free TBMⅢ,the hemolytic toxicity of TBMⅢ-NE was significantly reduced(P<0.01).TBMⅢ-NE promoted the phagocytosis of antigens by DC2.4 cells and significantly increased the expression of CCR7 on the surface of BMDCs(P<0.05),indicating its potential to promote more dendritic cells to effectively migrate to lymph nodes.TBMⅢ-NE also promoted the expression of IL-6 and IL-1β in the supernatant of BMDCs(P<0.05).When combined with RBD,TBMⅢ-NE significantly increased the levels of specific IgG,IgG2a,and IgG1 antibodies in mouse serum(P<0.01)and promoted the secretion of specific IFN-γ in splenocytes(P<0.01),indicating that TBM Ⅲ-NE could enhance specific cellular immune responses.Conclusion A stable and highly effective TBMⅢ-NE that can induce humoral and cellular immune responses is successfully prepared.
6.Application of long-read sequencing based haplotype construction in preimplantation genetic testing for a patient with Incontinentia pigmenti

Objective:To provide preimplantation genetic testing (PGT) for a patient with Incontinentia pigmenti (IP) due to IKBKG gene variant but without family samples through construction of single nucleotide polymorphism (SNP)-based haplotype by Long-read sequencing (LRS) technology. Methods:A female IP patient with a heterozygous IKBKG c. 1167dup variant but without family genetic data who sought genetic counseling at Women and Children′s Hospital of Ningbo University in November 2021 was selected as the study subject. The IKBKG gene has a highly homologous pseudogene IKBKGP1. Genomic DNA was extracted from peripheral blood samples from the couple, and LRS was used to obtain informative SNP loci flanking the variant locus, enabling the construction of SNP haplotype with a long segment spanning from the non-homologous region of IKBKG to the variant site. Trophoblast cells were biopsied from blastocysts fertilized through intracytoplasmic sperm injection, and next-generation sequencing (NGS) was used to determine the SNP information of the embryos. Linkage analysis with the parental SNP haplotypes was conducted to detect the carrier status of the embryos and exclude chromosomal aneuploidies. Sanger sequencing was carried out to validate the result. A euploid embryo without the pathogenic variant was selected for transfer. Prenatal diagnosis was carried out by amniocentesis at mid-trimester to verify the result of PGT, and follow-up was conducted after the baby was born. This study has been approved by the Ethics Committee of the Women and Children′s Hospital of Ningbo University (Ethics No. EC2023-094). Results:In total seven blastocysts were tested, and PGT results indicated that two embryos were euploid and did not carry the pathogenic variant. One euploid embryo was transferred, which resulted in a singleton pregnancy. Amniocentesis at 24 weeks of gestation confirmed that the status of fetal IKBKG gene, and its chromosomal status was consistent with the PGT results. A healthy male infant was born at 38 + 6 weeks of gestation. Conclusion:For IP patients with de novo mutation or without family samples, PGT with LRS can directly construct the SNP-based haplotype while avoiding interference from pseudogenes, providing an effective strategy for PGT.
7.Clinical features of dystonia in patients with different types of atypical Parkinson syndrome

Objective:To evaluate the clinical features of dystonia in patients with different types of atypical Parkinson syndrome (APS).Methods:A total of 104 patients with APS admitted in the Department of Neurology, Beijing Hospital from January 2015 to June 2023 were enrolled in the study, including 57 cases of multiple system atrophy (MSA), 38 cases of progressive supranuclear palsy (PSP) and 9 cases of corticobasal degeneration (CBD). Among 104 cases there were 63 males (60.6%), the mean age of patients was (62.3±8.9) years (54 to 73 years). The sex, age at onset, disease duration, first symptom, clinical features of dystonia and other neurological signs, response to levodopa therapy, numbers of Hoehn & Yahr scale≥3 after 3 years of disease, and MRI findings were documented in patients with different type APS.Results:The overall frequency of dystonia in this series was 45.2%(47/104), and 33.3% (19/57) for MSA group, 50.0% (19/38) for PSP group, 9/9 for CBD group. The types of dystonia were anterocollis, retrocollis, blepharospasm, oromandibular, foot/limb dystonia, Pisa syndrome and myoclonus. In all 47 cases presenting dydtonia, dystonia was not the first complaint and it did not respond to levodopa therapy.Conclusion:In this series of atypical Parkinson syndrome, dystonia is a common feature of the disease, while it is not the first symptom at disease onset, and usually does not respond to levodopa therapy.
8.Effect of Scutellariae Radix Combined with EGFR-TKIs on Non-small Cell Lung Cancer

ObjectiveTo investigate the effects of Scutellariae Radix combined with epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) on cell proliferation, apoptosis, cancer stem cell (CSC) marker expression, and metabolism in non-small cell lung cancer (NSCLC) cells. MethodsThe anti-tumor effects of Scutellariae Radix and EGFR-TKIs (gefitinib or osimertinib) in NSCLC cells were evaluated using the cell counting kit-8 (CCK-8) and Annexin V-FITC/propidium iodide (PI) double staining apoptosis assay. The activity of Scutellariae Radix and EGFR-TKIs in three-dimensional (3D) cultures of NSCLC cells was assessed using the CellTiter-Glo® 3D cell viability assay. The mRNA and protein expression levels of CSC markers, sex determining region y box protein 2 (SOX2) and aldehyde dehydrogenase 1 family member A1 (ALDH1A1), were detected by quantitative real-time polymerase chain reaction (Real-time PCR) and Western blot, respectively. Changes in intracellular reactive oxygen species (ROS) levels were detected by ROS staining, and the redox ratio was detected by femtosecond laser labeling free imaging (FLI). ResultsUnder both two-dimensional (2D) and 3D culture conditions, compared with the blank group and EGFR-TKI group, the combination group showed significantly reduced cell viability and increased apoptosis rate (P<0.05). Compared with the EGFR-TKI group, the mRNA and protein levels of CSC markers were significantly downregulated in the combination group (P<0.05). Additionally, the redox ratio was significantly elevated (P<0.05), and ROS levels were also increased in the combination group compared with the EGFR-TKI group. ConclusionIn NSCLC cells, Scutellariae Radix enhances the redox ratio and increases ROS levels, thereby inhibiting the expression of CSC markers and strengthening the anti-tumor effects of EGFR-TKIs. This provides a novel molecular mechanism by which Scutellariae Radix may enhance the sensitivity of targeted therapies.
9.Effects of electrophysiological combined therapy on pelvic floor function and electrophysiological indexes of pelvic floor in elderly patients with pelvic prolapse

Objective:To analyze the effects of electrophysiological combined therapy on pelvic floor function and electrophysiological indexes of pelvic floor in elderly patients with pelvic prolapse.Methods:A total of 46 elderly patients with pelvic prolapse admitted to Noncommissioned Officer School Hospital, Army Medical University from Jan. 2022 to Jul. 2023 were selected as subjects. All patients were divided into control group and study group by random number table method, with 23 cases in each group. The control group was treated with conventional operation, and the study group was treated with electrophysiological combination. The therapeutic effect, psychological state, pelvic floor electrophysiology and pelvic floor muscle function before and after treatment were compared between the two groups.Results:The treatment efficacy rate in the observation group was 95.65%, while that in the control group was 73.91%, with the observation group showing a significantly higher rate than the control group ( P<0.05). There were no statistically significant differences between the two groups in terms of psychological state scores, pelvic floor electrophysiological indicators, or pelvic floor muscle function before treatment ( P>0.05). After treatment, the SAS score, SDS score, fatigue degree of Type I muscle fibers, fatigue degree of Type II muscle fibers, and absolute values of gh scores in the observation group were lower than those in the control group ( P<0.05) ; The observation group had higher Type I muscle fiber strength, Type II muscle fiber strength, Ba point, Bp point, pb point, TVL point, pelvic floor contraction pressure, rapid contraction pressure, resting pressure, and number of contractions than the control group ( P<0.05) . Conclusion:Electrophysiological combined therapy is helpful to improve the therapeutic effect of elderly patients with pelvic prolapse, improve their psychological state, pelvic floor electrophysiological indexes and pelvic floor muscle function.
10.Anti-inflammatory mechanism of acupoint acupuncture combined with tetramethylpyrazine in regulating the NF-κB/NLRP3 signaling pathway in a rat model of Kashin-Beck disease induced by T-2 toxin

Objective:To study the anti-inflammatory mechanism of acupoint acupuncture combined with tetramethylpyrazine in regulating the nuclear factor kappa B (NF-κB)/NOD like receptor thermal protein domain associated protein 3 (NLRP3) signaling pathway in a T-2 toxin induced rat model of Kashin-Beck disease.Methods:A total of 40 clean-grade Sprague-Dawley (SD) rats (half male and half female), weighing 180 - 220 g, were divided using a random number table method into model group, tetramethylpyrazine group, acupoint acupuncture combined with tetramethylpyrazine group, and control group, with 10 rats in each group. Rats in the model group, the tetramethylpyrazine group, and the acupoint acupuncture combined with tetramethylpyrazine group were intraperitoneally injected with 1.5 mg/kg of 0.1% T-2 toxin five times per week for 4 consecutive weeks. Rats in both the tetramethylpyrazine group and the acupoint acupuncture combined with tetramethylpyrazine group were intraperitoneally injected with 100 mg·kg -1·d -1 tetramethylpyrazine solution. Meanwhile, rats in the model group and the control group were administered an equivalent volume of physiological saline for consecutive 4 weeks. Acupoint acupuncture combined with tetramethylpyrazine group was used to needle the bilateral "Qu Chi" and "Wei Zhong" of rats with a diameter of 3 - 5 mm, leaving the needle for 20 minutes, once a day, for 4 weeks. After the last injection, the rats were euthanized to collect blood samples and knee joint cartilage. Hematoxylin and eosin staining was performed to observe the pathological changes in the knee joint cartilage of rats. Enzyme-linked immunosorbent assay (ELISA) was used to measure serum levels of osteocalcin (OC), alkaline phosphatase (ALP), β-carboxy-terminal cross-linked telopeptide of type Ⅰ collagen (β-CTX), interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α). Western blot analysis was used to determine protein expression levels of NF-κB p65 and NLRP3. Results:In the control group, intact cartilage surface, middle, and deep structures were observed in the knee joint cartilage of rats. In the model group, pathological changes similar to adult Kashin-Beck disease were observed in the knee joint cartilage. The integrity of the knee joint cartilage of rats in the tetramethylpyrazine group and the acupoint acupuncture combined with tetramethylpyrazine group was significantly improved compared to the model group. Compared with the control group, the levels of OC and ALP in the serum of the model group rats were significantly reduced, while the level of β-CTX was significantly increased ( P < 0.05). Compared with the model group, the levels of OC and ALP in the serum of rats in the tetramethylpyrazine group and the acupoint acupuncture combined with tetramethylpyrazine group were significantly increased, while the levels of β-CTX were significantly decreased ( P < 0.05). The levels of OC and ALP in the serum of rats in the acupoint acupuncture combined with tetramethylpyrazine group were higher than those in the tetramethylpyrazine group, while the level of β-CTX was lower than that in the tetramethylpyrazine group ( P < 0.05). Compared with the control group, the levels of IL-1β, IL-6 and TNF-α in the serum of the model group rats were significantly increased ( P < 0.05). Compared with the model group, the levels of IL-1β, IL-6 and TNF-α in the serum of rats in the tetramethylpyrazine group and the acupoint acupuncture combined with tetramethylpyrazine group were significantly reduced ( P < 0.05). The levels of IL-1β, IL-6 and TNF-α in the serum of rats in the acupoint acupuncture combined with tetramethylpyrazine group were lower than those in the tetramethylpyrazine group ( P < 0.05). Compared with the control group, the expression levels of NF-κB p65 and NLRP3 proteins in the model group rats were significantly increased ( P < 0.05). Compared with the model group, the expression levels of NF-κB p65 and NLRP3 proteins were significantly reduced in the tetramethylpyrazine group and the acupoint acupuncture combined with tetramethylpyrazine group ( P < 0.05). The expression levels of NF-κB p65 and NLRP3 proteins in the acupoint acupuncture combined with tetramethylpyrazine group were significantly lower than those in the tetramethylpyrazine group ( P < 0.05). Conclusion:Acupoint acupuncture combined with tetramethylpyrazine has a good anti-inflammatory effect on T-2 toxin induced Kashin-Beck disease rats, and its mechanism may be related to the downregulation of NF-κB p65 and NLRP3 protein expression.
