1.Role of the iNOS/IRS1/AKT/GSK-3β signaling pathway in chronic intermittent hypoxia-induced insulin resistance

Objective :
To pathological changes and inducible nitric oxide synthase(iNOS), phosphorylated insulin receptor substrate 1 serine 307(p-IRS1ser 307), phosphorylated protein kinase B serine 473(p-AKTser 473), glycogen synthase kinase-3β(GSK-3β), and gluconeogenic synthase(GS) proteins were observed in the liver of rats under the condition of chronic intermittent hypoxia-replicated oxygen in control. And to explore the role of iNOS/IRS1/AKT/GSK-3β signaling pathway in chronic intermittent hypoxia-induced insulin resistance.
Methods :
Forty SD rats were randomly divided into a control group(NC group) and an experimental group(CIH group), with 20 rats in each group. The NC group was placed in a normoxic environment for 12 weeks, while the CIH group was first subjected to intermittent hypoxia for 8 weeks, and then resumed normoxic rearing until the 12th week. Fasting blood glucose(FBG) and fasting insulin(FINS) were measured at baseline, week 8 and week 12, and liver tissues were taken for pathology and measurement of iNOS, p-IRS1ser 307, p-AKTser 473, GSK3β and GS levels, to compare the differences between groups.
Results:
t baseline, there was no significant difference in liver pathology between the two groups, and the observed indexes were not statistically significant(P>0.05); at 8 weeks, compared with the NC group, liver pathology in the CIH group showed significant disorganization of hepatic blood sinusoids and hepatocyte cords, obvious hepatocyte edema, smaller nuclei, increased lymphocyte infiltration, and a small number of fat vacuoles, significantly higher levels of FBG, FINS, insulin resistance index(HOMA-IR), iNOS mRNA, p-IRS1ser 307 protein, GSK-3β protein levels, and decreased p-AKTser 473 protein and GS protein levels, all of which were statistically significant(allP<0.05). IRS1ser 307 protein, GSK-3β protein levels were increased, p-AKTser 473 protein and GS protein levels were decreased, and the differences were statistically significant(allP<0.05); at 12 weeks, no lymphocyte infiltration was seen in the CIH group compared with that of the NC group and fat vacuoles significantly increased, and there was no improvement in the other pathological damage that had already occurred, and the levels of p-AKTser 473 protein significantly increased. AKTser 473 protein level significantly increased, p-IRS1ser 307 protein and GS protein levels were significantly reduced, all of which were statistically significant(allP<0.05), and the rest of the observational indexes were not statistically significant. Pearson′s correlation analysis showed that HOMA-IR of CIH group was significantly positively correlated with the levels of iNOS mRNA, p-IRS1ser 307 protein, and GSK-3β protein at 8 weeks(r=0.874, 0.817,0.872;allP<0.05), and significantly negatively correlated with the levels of p-AKTser 473 protein and GS protein(r=-0.886,-0.879;allP<0.05).
Conclusion
Chronic intermittent hypoxia can lead to hepatic pathological damage that cannot be reversed even by reoxygenation interventions and may mediate the development of insulin resistance by upregulating the IRS1/AKT/GSK-3β signaling pathway through the upregulation of iNOS mRNA expression.
2.Significance of precise classification of sacral meningeal cysts by multiple dimensions radiographic reconstruction MRI in guiding operative strategy and rehabilitation.

OBJECTIVE:
To precise classify sacral meningeal cysts, effective guide minimally invasive neurosurgery and postoperative personalized rehabilitation by multiple dimensions radiographic reconstruction MRI.
METHODS:
From March to December 2021, based on the original 3D-fast imaging employing steadystate acquisition (FIESTA) scanning sequence, 92 patients with sacral meningeal cysts were pre-operatively evaluated by multiple dimensional reconstruction MRI. The shape of nerve root and the leakage of cyst were reconstructed according to the direction of nerve root or leakage track showed on original MRI scans. Sacral canal cysts were accurately classified as including nerve root and without nerve root, so as to accurately design the incision of skin and formulate corresponding open range of the posterior wall of the sacral canal. Under the microscope intraoperation, the shape of the nerve roots inside cysts or leakage track of the cysts without nerve roots were verified and explored. After the reinforcement and shaping operation, several reexaminations of multiple dimensional reconstruction MRI were performed to understand the deformation of the nerve root and hydrops in the operation cavity, so as to formulate a persona-lized rehabilitation plan for the patients.
RESULTS:
Among the 92 patients with sacral mengingeal cyst, 58 (63.0%) cysts with nerve root cyst, 29 (31.5%) cysts without nerve root cyst, and 5 (5.4%) cysts with mixed sacral canal cyst. In 58 patients with nerve root cysts, the accuracy of preoperative clinical classification on MRI image reached 96.6% (56/58) through confirmation by operating microscope. Only 2 cases of large single cyst with nerve root on the head of cyst were mistaken for without nerve root type. In 29 patients with sacral cyst without nerve root, the accuracy of preoperative image reached 100% through confirmation by operating microscope. The accuracy of judging the internal nerve root and leakage of 12 cases with recurrent sacral cyst was also 100%. Two cases of delayed postoperative hydrops were found one month after operation. After rehabilitation treatment by moxibustion and bathing, the hydrops disappeared 4-6 months after operation.
CONCLUSION
Multiple dimensional reconstruction MRI can precisely make clinical classification of sacral meningeal cysts before operation, guide minimally invasive neurosurgery effectively, and improve the rehabilitation effect.

3.Analysis of changes in cervical curvature and influencing factors after percutaneous posterior cervical endoscopic treatment for single segment cervical spondylotic radiculopathy

Objective To explore the changes in cervical curvature and their influencing factors after percutaneous posterior cervical endoscopic treatment for single segment cervical spondylotic radiculopathy.Methods 200 patients with single segment cervical spondylotic radiculopathy from January 2021 to March 2024 was selected,then divide them into control group and study group according to the random number table method.The control group received traditional open anterior cervical surgery,while the study group received percutaneous posterior cervical endoscopic treatment.Compare the treatment outcomes,surgical related conditions,cervical curvature and other sagittal parameters,pain levels,and cervical function between the two groups of patients.Using the generalized estimation equation(GEE)model to analyze the improvement degree of cervical curvature and other cervical sagittal parameters by two methods,and analyze the risk factors affecting changes in cervical curvature.Results The study group had significantly shorter surgical time,incision length,and hospitalization time compared to the control group.The intraoperative blood loss and hospitalization costs were also significantly lower than those of the control group,and the differences were statistically significant(P<0.05).The excellent rate of the study group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The C2-7 Cobb angle and T1 tilt angle in the study group were significantly higher than those in the control group,the intervertebral height was significantly higher than that in the control group,and the C2-7 sagittal vertical axis(C2-7 SVA)was significantly smaller than that in the control group,with statistically significant differences(P<0.05).At three months after surgery and the last follow-up,the visual analogue scale(VAS)scores and neck disability index(NDI)of the two groups of patients were significantly lower than those before treatment,and the study group was significantly lower than the control group.At three months after surgery and the last follow-up,the Japanese Orthopaedic Association(JOA)scores of the two groups of patients were significantly higher than those before treatment,and the study group was significantly higher than the control group,with statistically significant differences(P<0.05).Intervertebral height≤8 mm(OR^=1.627,95%CI:1.401~1.938,P=0.020),T1 tilt angle≤25°(OR^=1.598,95%CI:1.381~1.934,P=0.027),and no percutaneous cervical endoscopic treatment(OR^=2.591,95%CI:1.673~3.998,P=0.000)were independent risk factors affecting changes in cervical curvature.C2-7 SVA≤20 cm(OR^=0.748,95%CI:0.601~0.928,P=0.007)was a protective factor for changes in cervical curvature.According to GEE,the improvement of C2-7 Cobb angle,intervertebral height,T1 tilt angle,and C2-7 SVA in the study group was significantly better than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Percutaneous posterior cervical endoscopic treatment can significantly improve the cervical cervical curvature and cervical sagittal plane parameters of patients with single segment cervical spondylotic radiculopathy,and the treatment effect is better than traditional open anterior cervical surgery.The surgical trauma is smaller,the cost is lower,the hospital stay is shorter,and the postoperative recovery is faster.It is worthy for clinical application.

5.Anesthesia Management and Perioperative Outcome in Patients Receiving Left Ventricular Assisted Device Implantation

Objectives:Recently,domestical developed left ventricular assist devices(LVAD)have been frequently introduced into clinical practice.This study aims to report the anesthesia protocol and perioperative outcomes in Chinese patients receiving LVAD implantation surgery.Methods:This retrospective analysis included patients who underwent LVAD implantation at our center from June 2017 to November 2024.During and after separation from cardiopulmonary bypass,we optimized right heart function through careful adjustments in heart rate,rhythm,preload,myocardial contractility,and afterload.Vasoactive agents were administered as needed,and mechanical ventilation parameters were optimized.We implemented blood conservation strategies and established strict transfusion criteria to minimize allogenic blood transfusions.Results:A total of 100 patients were included in the analysis,with 54.0%classified as Interagency Registry for Mechanically Assisted Circulatory Support(INTERMACS)I or II.Before leaving the operating room,the mean arterial pressure(MAP),mean pulmonary arterial pressure(mPAP),central venous pressure(CVP),lactic acid levels,and urinary output after cardiopulmonary bypass were recorded as(74±7)mmHg,(25±7)mmHg,(7±3)mmHg,(2.3±1.9)mmol/L,and(8.2±5.4)ml/(kg·h),respectively.The transfusion rates for red blood cells and fresh frozen plasma were 20.0%and 28.0%.The in-hospital mortality rate was 3.0%,with a low incidence of severe complications including right heart failure(12%).Left ventricular ejection fraction increased from(23.7±4.8)%preoperatively to(25.3±10.5)%prior to discharge.Conclusions:Patients who received LVAD at our center exhibited low rates of postoperative mortality and complications and significant improvement in left heart function before discharge.
6.Fetal MRI in diagnosis of duodenal obstruction

Objective To observe the value of fetal MRI in diagnosis of duodenal obstruction(DO).Methods A total of 35 fetuses with suspected DO according to MRI were retrospectively included.The length and the maximum diameter of the dilated duodenum were measured,so as site(categorized in descending order from proximal to distal as descending segment,horizontal segment and ascending duodenum/proximal jejunum)and the degree of obstruction(complete or incomplete)were assessed.Taken findings of labor induction specimen,postnatal neonatal surgery or follow-up data as standards,the positive predictive value(PPV)of MRI for diagnosing fetal DO was calculated,while the correlations of the measured parameters of dilated duodenum and the confirmed obstruction site/degree were analyzed.Results Among 35 fetuses,DO was confirmed in 34 fetuses,yielding an overall PPV of 97.14%(34/35)for MRI.In 34 fetuses with confirmed DO,there were 23 with descending DO(DDO),4 with horizontal DO(HDO)and 7 with ascending DO/proximal jejunum obstruction(ADO/PJO),including 12 with complete DO and 22 with incomplete DO.PPV of MRI for diagnosing DDO,HDO and ADO/PJO was 87.50%(21/24),50.00%(2/4)and 100%(7/7),respectively,for diagnosing complete and incomplete DO was 90.00%(9/10)and 84.00%(21/25),respectively.Both the length and the maximum diameter of fetal proximal dilated duodenum showed on MRI were positively correlated with the actual obstruction site(from proximal to distal)(rs=0.736,P<0.001;rs=0.424,P=0.011,respectively),but had no significant rank correlation with the degree of obstruction(rs=-0.216,P=0.212;rs=-0.285,P=0.097,respectively).Conclusion Fetal MRI could effectively evaluate the length and the maximum diameter of dilated duodenum hence indicating the level and degree of DO.
7.Current status of implementation of infection control core elements in grass-roots medical institutions under background of construction of"compact county-level medical communities"

OBJECTIVE To explore the implementation and standardized management of infection control core ele-ments in grass-roots medical institutions within county-level medical communities.METHODS From Mar.2024 to Apr.2024,the current status of implementation of infection control core elements in the grass-roots medical institu-tions within county-level medical communities was investigated by means of questionnaire survey and qualita-tive interview,and the implementation strategies were further explored.RESULTS The infection management or-ganizational system and functions of the two county-level medical institutions within the county medical communi-ties were completed,there is no independent hospital infection management department in the primary medical in-stitutions.The infection management personnel in the 16 grass-roots medical institutions were part-time person-nel,the personnel with the educational background below junior college accounted for 84.21%,the personnel with the professional background of nursing accounted for 100.00%,the personnel with less than 5 years of working experience accounted for 78.95%,none of them had an on-the-job training certificate.The monitoring programs of the county-level medical institutions within the county medical communities were completed,there was no infec-tion management monitoring information platform in the grass-roots medical institutions.The infection cases,hand hygiene,environmental health and occupational exposures were monitored by people.The grass-roots medi-cal institutions had the highest requirements for various professional trainings and increase of training contents of prevention and control of public health infectious diseases(100.00%).The county-level medical institutions had inadequate capabilities of professional examination of medical equipment replacement and construction of medi-cal architecture.CONCLUSION It is necessary for the country and local levels of governments to attach great importance to the implementation of the infection control core elements in the grass-roots medical institutions within the county-lev-el medical communities,establish the county-level regional information platform,formulate the corresponding surveil-lance indexes and homogenized management systems,complete the cultivation of talents,and offer financial support.
8.Effects of electrophysiological combined therapy on pelvic floor function and electrophysiological indexes of pelvic floor in elderly patients with pelvic prolapse

Objective:To analyze the effects of electrophysiological combined therapy on pelvic floor function and electrophysiological indexes of pelvic floor in elderly patients with pelvic prolapse.Methods:A total of 46 elderly patients with pelvic prolapse admitted to Noncommissioned Officer School Hospital, Army Medical University from Jan. 2022 to Jul. 2023 were selected as subjects. All patients were divided into control group and study group by random number table method, with 23 cases in each group. The control group was treated with conventional operation, and the study group was treated with electrophysiological combination. The therapeutic effect, psychological state, pelvic floor electrophysiology and pelvic floor muscle function before and after treatment were compared between the two groups.Results:The treatment efficacy rate in the observation group was 95.65%, while that in the control group was 73.91%, with the observation group showing a significantly higher rate than the control group ( P<0.05). There were no statistically significant differences between the two groups in terms of psychological state scores, pelvic floor electrophysiological indicators, or pelvic floor muscle function before treatment ( P>0.05). After treatment, the SAS score, SDS score, fatigue degree of Type I muscle fibers, fatigue degree of Type II muscle fibers, and absolute values of gh scores in the observation group were lower than those in the control group ( P<0.05) ; The observation group had higher Type I muscle fiber strength, Type II muscle fiber strength, Ba point, Bp point, pb point, TVL point, pelvic floor contraction pressure, rapid contraction pressure, resting pressure, and number of contractions than the control group ( P<0.05) . Conclusion:Electrophysiological combined therapy is helpful to improve the therapeutic effect of elderly patients with pelvic prolapse, improve their psychological state, pelvic floor electrophysiological indexes and pelvic floor muscle function.
